SAFETY PRECAUTION AND REQUIREMENTS / TINDAKAN PENCEGAHAN DAN PERSYARATAN KESELAMATAN

1) The equipment to be worked on should be positively isolated, both mechanically and electrically in accordance to equipment isolation and tagout procedure.
Peralatan yang akan dikerjakan harus benar-benar diisolasi / diputuskan, mesin dan listrik kedua-duanya yang berhubungan dengan pemutusan harus diberikan tag.
2) The equipment should be freed of liquid and gas by depressurizing, draining, venting, purging and flushing.
Peralatan harus benar-benar bebas dari liquid dan menurunkan tekanan, dikosongkan, purging dan pembersihan / pembilasan.
3) Any combustible materials located nearby should be cleaned up. Where it is not practical to remove combustible materials they should suitable covered or sprayed with water.
Setiap bahan mudah terbakar di dekatnya harus dibersihkan. Jika bahan tersebut tidak bisa dihilangkan, dipindahkan, mereka harus diatasi dengan water spray.
4) Drain covers and surface manholes covers within 15 m from the work site should be properly sealed throughout the work priod to prevent emission of hydrocarbon.
Tutup drain pembuang dan permukaan lobang masuk dalam jarak 15 m dari tempat berlangsungnya pekerjaan panas harus ditutup untuk menghindari gas hidrokarbon keluar.
5) If flammable liquid is present in any open drain or ditch which within 15 m from work site should be dammed at locations not less than 15 m from work site and pumped out.
Seandainya cairan yang mudah terbakar hadir di setiap drain yang terbuka atau parit yang mengalir dalam jarak 15 m dengan tempat kerja harus dibendung dalam jarak 15 m
dari tempat kerja dan dipompa.
6) Any potential source of flammable gas or vapour emission, such as sampling point, vent or bleeders, within 15 m of the work site should be rendered safe.
Setiap sumber uap yang mudah terbakar atau emisi uap,seperti tempat pengambilan sample,venting atau tempat lain di mana uap terus - menerus keluar dalam jarak 15 m dengan
tempat kerja harus dibuat aman.
7) Beware sluge, grease, scale and deposites, etc which could give rise to a hazardous condition.

8) Gas free condition must be maintained within 15 m from the work throughout the hot work period.
Keadaan bebas gas harus tetap terjaga seluruhnya tidak kurang 15 m dari tempat kerja selama berlangsungnya kerja panas.
9) During welding, cutting or grinding, the work site must be adequately surrounded by fire resistant or flame retarding terpaulin to contain sparks and molten metals.
Selama pengelasan, pemotongan atau gerinda berlangsung, tempat kerja harus dikelilingi dengan alat tahan api atau terpal tahan api seandainya terkena percikan panas atau
lelehan besi panas.
10) For hot work is done at altitude, immediately do a security with warning barricades or rope around and below the work area. Sparks that fell should be held with fire blankets.
Untuk kerja panas yang dilakukan di ketinggian, segera lakukan pengamanan dengan memasang barikade atau tali peringatan di sekeliling dan di bawah area kerja. Bunga api

11) Proper ventilation must be provided to ensure that hot work workers do not inhale the welding fumes.

12) All hot work site must be equiped with sufficient portable fire fighting appliances such as fire extinguishers of the approriate type, hose lines charged with water from the nearby
fire hydrants.

13) The vicinity of the hot work area where there is a potential of sparks or molten metals falling off the ground should be sprayed with water where applicable.

14) A Fire Watch from the Permit Applicant should be stationed at the work site to closely monitor the work for probable fires.

15) The sitting of mobile equipment such as welding sets and generators must be outside the hazardous area where possible.

General Guidelines / Penjelasan Umum :

1. Conduct Toolbox Safety Meeting to subordinates before the work begins and attach the required Maintenance Procedure.

2. Do your job in accordance with the Job Safety Analysis which have been made and approved.

3. Do the job in accordance with orders or suggestions from your supervisor.
Lakukanlah pekerjaan sesuai dengan perintah atau saran-saran dari pengawas anda.
4. Notify the area supervisor immediately if you find unsafe conditions in the workplace.
Beritahukan segera kepada pengawas area bila anda menemukan keadaan yang tidak aman di tempat kerja.
5. Clean your workplace when work is completed or not each day.
Bersihkan tempat kerja anda bila pekerjaan sudah selesai ataupun belum setiap harinya.
6. Return Safe Work Permit (SWP) to the Area Supervisor and inform the work already completed or still ongoing.
Kembalikan Surat Ijin Kerja Aman (SWP) kepada Pengawas Area dan beritahukan pekerjaan sudah selesai atau masih berlanjut.
7. Do not leave equipment / machinery alive in the work site without supervision.
Jangan tinggalkan peralatan / mesin dalam keadaan hidup di lokasi kerja tanpa pengawasan.
8. Ask your supervisor area, if you need a utility station at the work site.
Mintalah kepada pengawas area, jika anda membutuhkan stasiun utilitas di lokasi kerja.
