Penyakit Fungal Superficial desease/ Mikosis
Superficial

Adalah penyakit kulit yang disebabkan jamur, yang

mengenai lapisan kulit paling atas (epidermis).
Penyakit ini dapat menyerang kulit, rambut, atau
kuku. Terbagi atas 2 kelompok :
-- Dermatofitosis
-- Non Dermatofitosis
Ada dua golongan jamur yang menyebabkan mikosis superfisialis
yaitu non dermatofita dan dermatofita

Trychophyton tonsurans rambut, kulit, buku

Non-Dermatofita    

Pityrosporum orbiculare kulit Tinea vesikolor

(Malasezia furfur)

 Dermatofitosis
Merupakan penyakit pada jaringan yang
mengandung zat tanduk, misalnya stratum
korneum pada epidermis, rambut, dan kuku yang
disebabkan oleh jamur golongan dermatofita.
 Identifikasi
Pada sediaan kulit dan kuku dengan 1 tetes larutan KOH 20 %
yang terlihat adalah hifa, sebagai dua garis sejajar, terbagi oleh
sekat dan bercabang, maupun spora berderet (artospora) pada
kelainan kulit lama dan/atau sudah diobati.

Pada sediaan rambut dengan 1 tetes larutan KOH 10 % yang

terlihat adalah spora kecil (mikrospora) atau besar
(makrospora). Spora dapat tersusun di luar rambut (ektotriks)
atau di dalam rambut (endotriks). Kadang-kadang dapat terlihat
juga hifa pada sediaan rambut.
 CARA PENULARAN
Penularan langsung dapat secara fomitis, epitel, rambut-rambut yang mengandung jamur baik dari manusia, binatang atau dari
tanah. Penularan tak langsung dapat melalui tanaman, kayu yang dihinggapi jamur, barang-barang atau pakaian, debu atau
air.Untuk timbulnya kelainan-kelainan di kulit tergantung dari beberapa faktor :
1. Faktor virulensi dari dermatofita
Virulensi ini tergantung pada afinitas jamur itu, apakah jamur Antropofilik, Zoofilik atau Geofilik. Selain afinitas ini masing-masing
jenis jamur ini berbeda pula satu dengan yang lain dalam afinitas terhadap manusia maupun bagian-bagian dari tubuh Misalnya :
Trikofiton rubrum jarang menyerang rambut, Epidermatofiton flokosum paling sering menyerang lipat pada bagian dalam.

2. Faktor trauma
Kulit yang utuh tanpa lesi-lesi kecil, lebih susah untuk terserang jamur.
 
3. Faktor-suhu dan kelembaban
Kedua faktor ini sangat jelas berpengaruh terhadap infeksi jamur, tampak pada lokalisasi atau lokal, di mana banyak keringat
seperti lipat paha dan sela-sela jari paling sering terserang penyakit jamur ini.
 
4. Keadaan sosial serta kurangnya kebersihan
Faktor ini memegang peranan penting pada infeksi jamur di mana terlihat insiden penyakit jamur pada golongan sosial dan
ekonomi yang lebih rendah, penyakit ini lebih sering ditemukan dibanding golongan sosial dan ekonomi yang lebih baik.
 
5. Faktor umur dan jenis kelamin
Penyakit Tinea kapitis lebih sering ditemukan pada anak-anak dibandingkan orang dewasa, dan pada wanita lebih sering
ditemukan infeksi jamur di sela-sela jari dibanding pria dan hal ini banyak berhubungan dengan pekerjaan. Di samping faktor-
faktor tadi masih ada faktor-faktor lain seperti faktor perlindungan tubuh (topi, sepatu dan sebagainya) , faktor transpirasi serta
pemakaian pakaian yang serba nilan, dapat mempermudah penyakit jamur ini.
 Etiologi
> Dermatofita ( gol. Jamur yang mencerna keratin ). 

> Terbagi atas 3 genus :

• 1. Microsporum 3. Epidemophyton
• 2. Trichophyton
> Berdasarkan tempat hidup terbagi 3 :
• 1. Geofilik ( mis : M. Gypseum)
• 2. Zoofilik ( mis : M. Canis )
• 3. Antropofilik ( mis : T. Rubrum )
 Spesies
1. Trichophyton Rubrum
Ciri :
- Hifa halus
- Membentuk banyak mikrokonidia
- Mikrokonidia kecil, berdinding
tipis dan bentuk lonjong.
- Mikrokonidia terletak pada
kondiofora pendek dam tersusu
satu persatu ( en thyrse) /
Makrokonidia seperti pensil tda
beberapa sel.
2. Tricophyton
Mentagrophytes
Ciri :-
- Mikrokonidia bentuk
bulat
- Memiliki banyak
bentuk hifa spiral
- Makrokonidia
berbentuk pensil
3. Microsporum Canis
Ciri :
- Makrokonidia berbentuk
kumparan yang berujung
runcing tda 6 sel atau lebih.
- Makrokonidia berdinding
tebal.
- Mikrokonidia bentuk
lonjong dan tidak khas.
- Beda dengan M. Gypseum,
tda 4-6 sel dan dinding lebih
tipis (makrokonidia ).
4. Epidermophyton
floccosum
Ciri :
- Hifanya lebar
- Makrokondia berbentuk
gada, berdinding tebal, tda
2- 4 sel.
- Beberapa makrokonidia
tersusun pada satu
konidiofora.
- Mikrokonidia biasanya
tidak ditemukan.
 Non Dermatofitosis
Merupakan penyakit Infeksi Jamur / Mikosis
Superfisial yang bukan merupakan golongan
Dermatofita.
 Pitriasis Versicolor Folikulitis Malassezia 

> Malassezia spp.

Memilik 7 Spesies dan yang
tersering menginfeksi : - M. Furfur,
M. Globosa, M. Sympodialis .
Morfologi :
-- Tampak jamur spt kelompok kecil,
sel ragi bentuk lonjong uniseluler
atau bertunas 4-8 um
- Hifa Pendek, berseptum dan
bercabang Spaghetti and Meat balls
 Etiologi
 Malassezia furfur (dahulu dikenal
sebagai Pityrosporum orbiculare,  vitro dan in vivo. Lebih lanjut, tahap
Pityrosporum ovale) merupakan jamur
lipofilik yang normalnya hidup di keratin
kulit dan folikel rambut manusia saat masa
pubertas dan di luar masa itu. Alasan
mengapa organisme ini menyebabkan
panu, pada beberapa orang sementara
tetap sebagai flora normal pada beberapa
orang lainnya, belumlah diketahui.
Beberapa faktor, seperti kebutuhan nutrisi
organisme dan respon kekebalan tubuh
inang (host's immune response) terhadap
organisme sangatlah signifikan.
 Sebagai organisme yang
lipofilik, Malassezia furfur memerlukan
lemak (lipid) untuk pertumbuhan in
miselium dapat dirangsang in vitro dengan
penambahan kolesterol dan ester
kolesterol pada medium yang tepat.
Karena organisme ini lebih cepat
berkoloni/mendiami kulit manusia saat
pubertas dimana lemak kulit meningkat
lebih banyak dibandingkan pada masa
remaja (adolescent) dan panu
bermanifestasi di area yang "kaya minyak"
atau sebum-rich areas (misalnya: di dada,
punggung), variasi lemak di permukaan
kulit individu dipercaya berperan utama
dalam patogenesis penyakit.
 Piedra

 Piedraia Hortae ( Hitam )

Ciri : tergolong kelas ascomycetes
dan membentuk spora seksual,
termasuk dematiaceae.
 Trichosporon beigelii (Putih)
Ciri : mempunyai gifa tidak
bewarna, termasuk moniliaceae.
Etiologi

Piedra Beigeli (Trikosporon beigeli) terutama terdapat didaerah

subtropis, daerah dingin, (di Indonesia belum ditemukan).

Trichosporon beigelii Merupakan jamur penyebab (infeksi pada

rambut berupa benjolan yang melekat erat pada rambut, berwarna
hitam). Penyakit ini umumnya terdapat di daerah-daerah tropis dan
subtropis. Terutama terdapat pada rambut kepala, kumis
ataujambang, dan dagu.
Biasanya penyakit ini dapat
timbul karena adanya kontak
langsung dari orang yang PATOGENESIS
sudah terkena infeksi.
 
 Tinea Nigra Palmaris

 Cladosporium wenecki/
Cladosporium mansoni
Morfologi :
-Jamur ini termasuk
demataceae yang membentuk
koloni coklat hitam.
-- Hifa berseptum dan bewarna
coklat hitam.
Tinea nigra ialah infeksi jamur superfisialis yang biasanya
menyerang kulit telapak kaki dan tangan dengan memberikan
warna hitam sampai coklat pada kulit yang terserang. Makula
yang terjadi tidak menonjol pada permukaan kulit, tidak terasa
sakit dan tidak ada tanda-tanda radang. Kadang-kadang
makula ini dapat meluas sampai ke punggung, kaki dan
punggung tangan, bahkan dapat menyebar sampai dileher,
dada dan muka.Gambaran efloresensi ini dapat berupa
polosiklis, arsiner dengan warna hitam atau coklat hampir sama
seperti setetes nitras argenti yang diteteskan pada kulit.
Etiologi

Penyebabnya adalah Kladosporium

wenecki dan jamur ini banyak menyerang
anak- anak dengan higiene kurang baik
dan orang-orang yang banyak berkeringat.
