dan proses hendaklah didasarkan pada penilaian risiko yang
dijustifikasi dan didokumentasikan.
 PENGORGANISASIAN DAN PERENCANAAN KUALIFIKASI DAN
VALIDASI

Direncanakan dengan Hanya dilakukan oleh personel yang

mempertimbangkan siklus hidup telah mendapat pelatihan yang
fasilitas, peralatan, sarana memadai dan mengikuti prosedur
penunjang, proses dan produk. yang telah disetujui

Elemen kunci program kualifikasi dan

Personel yang diberi tanggung validasi hendaklah ditetapkan secara
jawab untuk kualifikasi/validasi jelas dan didokumentasikan dalam

Pengendalian perubahan dan pengembangan kriteria validasi, termasuk
penanganan penyimpangan keberterimaan; rekualifikasi
 DOKUMENTASI, TERMASUK RIV

Cara dokumentasi yang baik penting untuk mendukung pengelolaan pengetahuan (knowledge
management) sepanjang siklus hidup produk. Semua dokumen yang dihasilkan selama kualifikasi
dan validasi hendaklah disetujui dan disahkan oleh personel yang diberi wewenang
sebagaimana ditetapkan dalam SMIOT

Hubungan antar dokumen dalam proyek validasi yang kompleks hendaklah ditetapkan secara
jelas. Protokol validasi hendaklah disiapkan dengan menetapkan sistem kritis, atribut dan
parameter, serta kriteria keberterimaan. Jika sesuai, dokumen kualifikasi dapat digabungkan
bersama, misal Kualifikasi Instalasi (KI) dan Kualifikasi Operasional (KO).

Bila protokol validasi dan dokumentasi lain disediakan oleh pihak ketiga yang menyediakan jasa
validasi, personel yang diberi wewenang di lokasi pabrik terkait hendaklah memastikan
kesesuaian dan kepatuhan terhadap prosedur internal sebelum disetujui.
Setiap perubahan signifikan terhadap protokol yang disetujui selama pelaksanaan validasi, misal
kriteria keberterimaan, parameter operasional, dan lain-lain, hendaklah didokumentasikan sebagai
penyimpangan dan dijustifikasi secara ilmiah.

Hasil yang tidak memenuhi kriteria keberterimaan yang telah ditentukan hendaklah dicatat
sebagai penyimpangan dan diinvestigasi secara menyeluruh sesuai prosedur internal. Setiap
implikasi terhadap validasi hendaklah dibahas dalam laporan

Pelulusan formal untuk tahap berikutnya dalam kualifikasi dan validasi proses hendaklah
disahkan oleh personel yang bertanggung jawab baik sebagai bagian dari persetujuan laporan
validasi atau sebagai dokumen ringkasan terpisah.
TAHAP KUALIFIKASI UNTUK PERALATAN, FASILITAS, SARANA PENUNJANG
DAN SISTEM
mempertimbangkan semua tahap mulai dari pengembangan awal sesuai spesifikasi
kebutuhan pengguna sampai pada akhir penggunaan peralatan, fasilitas, sarana
penunjang, atau sistem.

Spesifikasi Kebutuhan Pengguna (SKP)/User Requirement Specification (URS)

Menjadi dasar acuan selama siklus hidup validasi

Kualifikasi Desain (KD)/Design Qualification (DQ)

kepatuhan desain pada CPOTB hendaklah dibuktikan dan didokumentasikan.
Verifikasi terhadap persyaratan SKP hendaklah dilakukan selama kualifikasi desain.
 Factory Acceptance Testing (FAT)/Site Acceptance Testing (SAT)

Pengkajian dokumentasi dan

beberapa pengujian dapat dilakukan
saat FAT atau tahap lain tanpa perlu
Evaluasi terhadap peralatan di mengulangi kembali di lokasi pabrik

 Kualifikasi Instalasi (KI)/Installation Qualification (IQ)

• KI hendaklah dilakukan terhadap peralatan, fasilitas, sarana penunjang atau sistem.

• KI hendaklah mencakup, namun tidak terbatas pada hal-hal berikut:
a. Verifikasi kebenaran instalasi komponen, instrumentasi, peralatan, pemipaan, dan peralatan
sarana penunjang sesuai dengan gambar teknis dan spesifikasi;
b. verifikasi kebenaran instalasi terhadap kriteria yang telah ditentukan;
c. pengumpulan dan pemeriksaan dokumen instruksi kerja dan instruksi pengoperasian serta
instruksi perawatan peralatan dari pemasok;
d. kalibrasi instrumen; dan verifikasi bahan konstruksi.
 Kualifikasi Operasional (KO)/Operational Qualification (OQ)

• KO umumnya dilakukan setelah KI, namun bergantung pada kompleksitas peralatan, bisa saja
dilakukan sebagai kombinasi Kualifikasi Instalasi/Operasional (KIO).
• KO hendaklah mencakup, namun tidak terbatas pada hal-hal berikut:
a. pengujian yang dikembangkan berdasarkan pemahaman proses, sistem, dan peralatan untuk
memastikan sistem beroperasi sesuai desain; dan
b. pengujian untuk mengonfirmasi batas operasi atas dan batas operasi bawah, dan/atau kondisi
"terburuk".
• Penyelesaian KO yang berhasil hendaklah digunakan untuk memfinalisasi prosedur operasional
dan prosedur pembersihan, pelatihan operator, dan persyaratan perawatan berkala
 Kualifikasi Kinerja (KK) / Performance Qualification (PQ)

• KK umumnya dilakukan setelah KI dan KO berhasil. Namun, mungkin dalam beberapa kasus,
pelaksanaannya bersamaan dengan KO atau Validasi Proses.
• KK hendaklah mencakup, namun tidak terbatas pada hal-hal berikut:
a. Pengujian dengan menggunakan bahan yang dipakai di produksi, bahan pengganti yang
memenuhi spesifikasi, atau produk simulasi yang terbukti mempunyai sifat yang setara pada
kondisi operasional normal dengan ukuran bets pada kondisi terburuk.
b. Pengujian hendaklah mencakup rentang operasional proses yang diinginkan, kecuali jika
tersedia bukti terdokumentasi dari tahap pengembangan yang telah mengonfirmasikan rentang
operasional.
 KUALIFIKASI ULANG

• Hendaklah dilakukan evaluasi terhadap peralatan, fasilitas, sarana penunjang, dan sistem secara
berkala untuk memastikan bahwa status kualifikasi tetap terkendali.
• Bila diperlukan kualifikasi ulang dan dilakukan pada periode waktu tertentu, periode hendaklah
dijustifikasi dan kriteria untuk evaluasi ditetapkan. Selanjutnya, kemungkinan perubahan minor
dari waktu ke waktu hendaklah dinilai.
 VALIDASI PROSES
Umum

validasi awal dari proses baru, validasi bila terjadi perubahan proses, transfer lokasi pembuatan, dan
verifikasi proses on-going.

• Validasi proses dapat diterapkan menggunakan pedoman Validasi Proses lain yang relevan.
a) Pedoman tentang Validasi Proses dimaksudkan untuk memberikan panduan mengenai informasi
dan data yang diperlukan dalam pengajuan izin ke regulator. Namun, persyaratan CPOTB untuk
validasi proses berlanjut sepanjang siklus hidup produk.
b) Pendekatan ini hendaklah diterapkan untuk menautkan pengembangan produk dan proses. Hal
ini akan memastikan proses pembuatan skala komersial tervalidasi dan dijaga agar proses
produksi skala komersial rutin selalu dalam keadaan terkendali.
