dapat menyelesaikan makalah ini yang bertujuan untuk memenuhi tugas mata
kuliah Kesehatan Mental.

PENDAHULUAN

menyenangkan. Distress, yaitu hasil dari respon terhadap stres yang bersifat tidak
sehat, negatif, dan destruktif (bersifat merusak). Hal tersebut termasuk
konsekuensi individu dan juga organisasi seperti penyakit kardiovaskular dan
tingkat ketidakhadiran (absenteeism) yang tinggi,yang diasosiasikan dengan
keadaan sakit, penurunan, dan kematian.
Stres dan depresi yang dibiarkan berlarut membebani pikiran dan dapat
mengganggu system kekebalan tubuh. Apabila kita berada dalam emosi yang
negatif seperti rasa sedih, benci, iri, putus asa, kecemasan, dan kurang bersyukur
dengan nikmat yang ada, maka system kekebalan kita menjadi lemah.Depresi

merupakan salah satu masalah kesehatan mental utama saat ini, yang mendapat
perhatian serius. Di negara-negara berkembang, WHO memprediksikan bahwa
pada tahun 2020 nanti depresi akan menjadi salah satu penyakit mental yang
banyak dialami dan depresi berat akan menjadi penyebab kedua terbesar kematian
setelah serangan jantung. Berdasarkan data WHO tahun 1980, hampir 20%-30%
dari pasien rumah sakit di Negara berkembang mengalami gangguan mental
emosional seperti depresi.

BAB II
PEMBAHASAN
Kata stres telah digunakan sejak awal tahun 1900-an untuk menggambarkan
situasi yang menimbulkan perubahan secara fisik dan psikis dalam diri kita. Sulit
untuk mengartikannya karena stres muncul dalam begitu banyak bentuk. Tiap
orang memandang stres secara berbeda-beda. Stres dapat menjadi berbahaya atau
malahan membantu, tergantung keadaan. Beberapa stres menguntungkan karena
memotivasi kita untuk meningkatkan kinerja dan membuat perubahan-perubahan
dalam hidup kita. Jika kita tidak memiliki stres, kita tidak akan melakukan fungsi
apapun.

Stress menurut Hans Selye merupakan respon tubuh yang bersifat tidak
spesifik terhadap setiap tuntutan atau beban atasnya. Berdasarkan pengertian
tersebut dapat dikatakan stress apabila seseorang mengalami beban atau tugas
yang berat tetapi orang tersebut tidak dapat mengatasi tugas yang dibebankan itu,
maka tubuh akan merespon dengan tidak mampu terhadap tugas tersebut,
sehingga orang tersebut dapat mengalami stress. Respons atau tindakan ini
termasuk respons fisiologis dan psikologis. Stres dapat menyebabkan perasaan
negatif atau yang berlawanan dengan apa yang diinginkan atau mengancam
kesejahteraan emosional. Stres dapat menggangu cara seseorang dalam menyerap
realitas, menyelesaikan masalah, berfikir secara umum dan hubungan seseorang dan rasa
memiliki. Terjadinya stres dapat disebabkan oleh sesuatu yang dinamakan stressor,

10. Membawa obat-obatan yang memang direkomendasikan dokter supaya
stres tidak berujung pada hal-hal yang fatal. Untuk beberapa orang, stres
bisa memicu penyakit dan dampak yang fatal. Misalnya, jika sudah
terdektesi menderita penyakit jantung, jangan lupa selalu membawa obat
yang direkomendasikan oleh dokter.

Kita harus tahu perasaan apa yang sedang kita alami, yang positif dan yang
negatif, dan dalam situasi yang bagaimana. Dan melalui refleksi lebih lanjut kita
akan menemukan kebutuhan apa yang belum atau sudah terpenuhi. Terutama
perasaan negatif yang merupakan lampu merah atau tanda bahaya perlu diketahui,
karena menunjukkan bahwa ada kebutuhan yang belum terpenuhi, supaya kita
dapat mengambil tindakan agar lain kali dapat terpenuhi dengan mengubah
perilaku kita sendiri, tidak menuntut dari orang lain. Kita menyusun suatu rencana
agar kita bertumbuh dan berkembang dalam hal ini. Bila perasaan negatif
dibiarkan terus, berarti ada kebutuhan yang terus-menerus tidak terpenuhi, maka
kita akan mencari kompensasi agar terpenuhi dengan perilaku yang tidak sehat
(Narkoba, judi, perilaku berbahaya lain, dan sebagainya) atau kita stres terus dan
mengalami gangguan jiwa.
Pasien perlu disadarkan bahwa kita bertanggung jawab agar kebutuhan
kita terpenuhi. Kita tidak dapat memenuhi sendiri kebutuhan-kebutuhan
psikologis kita, kebutuhan akan dicintai dan akan keterlibatan, serta kebutuhan
akan harga diri dan akan autonomi/kebebasan. Orang lain yang harus
memenuhinya, tetapi dalam suatu relasi antar manusia yang baik. Kalau kita
menuntut, maka relasi akan menjadi tegang sehingga kebutuhan lebih sukar atau
mungkin tidak dipenuhi oleh orang lain. Bila kita bertanggung jawab agar
kebutuhan kita terpenuhi, maka kita harus bertanggung jawab untuk menunjukkan
perilaku yang baik sehingga kita lebih layak dicintai dan dihargai.
Makin besar keterlibatan kita secara pribadi dalam suatu usaha untuk
mencapai kebutuhan atau tujuan kita, makin besar pula dorongan yang kita
rasakan. Misalnya, makin besar keterlibatan secara pribadi seorang mahasiswa
untuk mencapai tujuan studinya, makin besar pula motivasi dan kesediaannya
untuk belajar dibandingkan dengan kalau ia belajar hanya karena kemauan orang
tuanya. Atau karena keterlibatan pribadi biasanya besar sekali pada seorang yang
sakit, maka ia pun akan berusaha sungguh-sungguh mencari pertolongan atau
pengobatan supaya sembuh.

tugas dan ambisi, istri atau ibu, kesenangan sekarang atau ideologi, orang atau
panggilan (konflik pendekatan ganda).
Contoh lain lagi adalah konflik yang terjadi bila kita harus memilih antara
beberapa hal yang semuanya tidak kita inginkan, misalnya pekerjaan yang tidak
menarik atau menganggur, menikah dengan orang yang tidak simpatik atau
kemungkinan tidak menikah sama sekali, berbuat sesuatu yang berbahaya atau
dicap sebagai pengecut (konflik pengelakan ganda).
Tekanan juga dapat menimbulkan masalah penyesuaian, tekanan sehari hari
biarpun kecil, tetapi bila bertumpuk tupuk dan berlangsung lama (stresor jangka
panjang), dapat menimbulkan stres yang hebat. Tekanan seperti juga frustasi dan
dapat berasal dari dalam ataupun dari luar individu.
Tekanan dari dalam datang dari cita cita atau norma kita yang kita gantungkan
terlalu tinggi dan kita mengejarnya tanpa ampun, sehingga kita terus menerus
berada di bawah tekanan sebagai contoh : kita mau rajin, berani, bertanggung
jawab, tetapi semua itu kita lakukan secara berlebihan.
Contoh tekanan dari luar, misalnya orang tua menuntut dari anak prestasi sekolah
yang terlalu tinggi, istri tiap hari mengeluh kepada suaminya bahwa uang belanja
tidak cukup, di sekolah, dikantor, di organisasi atau di rumah tidak habis
habisnya masalah yang perlu dipecahkan dan keputusan yang perlu diambil,
sedangkan waktu sudah mendesak .
Tidak jarang suatu keadaan stres karena frustasi konflik dan tekanan sekaligus ,
misalnya kematian si pencari nafkah mengakibatkan seorang pemuda harus
bekerja untuk mendapatkan biaya sekolahnya, sehingga ia tidak lulus dalam
ujianya dan dianggap kurang pandai .
Krisis adalah keadaan karena stressors mendadak dan besar yang menimbulkan
stres pada seorang individu atau pun suatu kelompok, misalnya
kematian,

kali. Terdapat tempat tempat dengan banyak krisis (konsentrasi krisis), misalnya
ruang gawat darurat di rumah sakit, kamar bersalin, kamar bedah, taman kanak
kanak dan di tingkat pertama suatu fakultas pada minggu minggu pertama tahun
kuliah baru, desa yang terkena bencana alam dan kekurangan makanan
sesudahnya, atau bila kemudian bantuan makanan datang (krisis karena tidak ada
makanan, kemudian krisi karena tiba tiba ada makanan).
Dahulu dikira bahwa krisis selalu tidak baik bagi kesehatan jiwa, sekarang ini
ternyata tidak demikian setelah mengalami krisis, maka mungkin individu atau
kelompok menjadi :
1. Terganggu atau lebih mudah terganggu apabila stres lagi
2. Lebih matang atau lebih kuat dalam mengahadapi stres di hari kemudian.
Yang terakhir ini adalah penting, karena mengandung unsur pencegahan. Kita
dapat mempraktikan hal ini, misalnya orang tua atau guru sengaja menimbulkan
krisis pada anaknya, misalnya tergesagesa mau pergi, lalu mendapati ban sepeda
kempes.
2.5.Ciri-ciri Stres
Ciri-ciri stres yang baik:
1) Mengahadapi sesuatu dengan penuh harapan untuk melawan rasa takut dalam
diri.
2) Memiliki jadwal yang sangat padat, tetapi didalam sela-sela jadwal yang
padat itu adaaktivitas yang sangat diharapkandan sangat dinikmati.
3) Memiliki komitmen yang lebih terhadap apa yang Anda sayangi. Misalnya:
pernikahan, menjadi seorang ayah atau ibu, menjadi pekerja, atau menjadi
pegawai negeri.
4) Bekerja dengan tujuan tertentu dan Anda tahu kecepatan Anda saat bergerak
akan berkurang saat tujuan itu tercapai atau bahkan saat baru akan tercapai.

1) Menghadapi segala sesuatu dengan perasan takut, resah, gelisah dan khawatir.
2) Memiliki jadwal yang sangat padat, tetapi tak ada satupun yang dapat Anda
nikmati dan mau tidak mau, harus Anda penuhi kewajiban itu.
3) Merasa bahwa semua yang Anda lakukan tidaklah penting, tidak memenuhi
seluruh kebutuhan Anda, dan tak sebanding dengan tenaga, pikiran dan waktu
yang Anda curahkan.
4) Merasa tidak memegang kendali dan selalu merasa panik seakan-akan tidak
ada jalan keluar untuk menyelesaikan tugas, merasa tidak ada selesainya, dan
merasa tidak ada yang membantu menyelesaikannya.
5) Merasa lebih baik bekerja daripada berhenti atau istirahat sejenak saat jam
kerja.
6) Memiliki tidur yang tidak lelap, tidur yang resah, sering sakit maag, sakit
punggung dan mempunyai sakit yang sifatnya menahun.

Keadaan lingkungan yang tidak menentu akan dapat menyebabkan pengaruh
pembentukan struktur organisasi yang tidak sehat terhadap individu tersebut.
Dalam faktor lingkungan terdapat tiga hal yang dapat menimbulkan stress
bagi seseorang yaitu ekonomi, politik dan teknologi. Perubahan yang sangat
cepat karena adanya penyesuaian terhadap ketiga hal tersebut membuat
seseorang mengalami ancaman terkena stress.Hal ini dapat terjadi, misalnya
perubahan teknologi yang begitu cepat. Perubahan yang baru terhadap
teknologi akan membuat keahlian seseorang dan pengalamannya tidak
terpakai karena hampir semua pekerjaan dapat terselesaikan dengan cepat dan
dalam waktu yang singkat dengan adanya teknologi yang digunakannya.
2) Faktor Organisasi
Didalam organisasi terdapat beberapa faktor yang dapat menimbulkan stress
yaitu role demands, interpersonal demands, organizational structure dan
organizational leadership.
Pengertian dari masing-masing faktor organisasi tersebut adalah sebagai
berikut :
a. Role Demands
Peraturan dan tuntutan dalam pekerjaan yang tidak jelas dalam suatu
organisasi akan mempengaruhi peranan seseorang untuk memberikan
hasil akhir yang ingin dicapai bersama dalam suatu organisasi tersebut.
b. Interpersonal Demands
Mendefinisikan tekanan yang diciptakan oleh individu lainnya dalam
organisasi. Hubungan komunikasi yang tidak jelas antara individu satu
dengan individu lainnya akan dapat menyebabkan komunikasi yang tidak
sehat. Sehingga pemenuhan kebutuhan dalam organisasi terutama yang
berkaitan dengan kehidupan sosial akan menghambat perkembangan
sikap dan pemikiran antara inividu yang satu dengan individu lainnya.

Pada dasarnya, faktor yang terkait dalam hal ini muncul dari dalam keluarga,
masalah ekonomi pribadi dan karakteristik pribadi dari keturunan. Hubungan
pribadi antara keluarga yang kurang baik akan menimbulkan akibat pada
pekerjaan yang akan dilakukan karena akibat tersebut dapat terbawa dalam
pekerjaan seseorang. Sedangkan masalah ekonomi tergantung dari bagaimana
seseorang tersebut dapat menghasilkan penghasilan yang cukup bagi
kebutuhan keluarga serta dapat menjalankan keuangan tersebut dengan
seperlunya. Karakteristik pribadi dari keturunan bagi tiap individu yang dapat
menimbulkan stress terletak pada watak dasar alami yang dimiliki oleh
seseorang tersebut. Sehingga untuk itu, gejala stress yang timbul pada tiaptiap pekerjaan harus diatur dengan benar dalam kepribadian seseorang.

mengalami kecelakaan serius dalam waktu 6 bulan mendatang. Bila skor kurang
dari 300, maka kemungkinan mengalami ganggungan kesehatan yang serius
adalah 50% (menurut penelitian di Amerika Serikat).
Makin besar perubahan hidup dan beban stres, makin rendah daya tahan tubuh
terhadap penyakit dan makin berat penyakit yang timbul. Stres mungkin juga
timbul akibat stressors yang menyenangkan, seperti pernikahan, promosi, menarik
undian yang besar dan keberhasilan lain, sebagian besar karena perubahanperubahan yang terjadi akibat peristiwa itu.
Stres berhubngan erat dengan PJK (penyakit jantung koroner, pembuluh darah
jantung). Glazer menyusun sesuatu kuesioner untuk meramalkan besarnya resiko
terjadinya PJK
(Dinamakan: Self- Evaluation: The Glazer-Stresscontrol Life-Style Questionnair).
Dengan kuesioner ini dapat digali jenis kepribadian seseorang, A atau B.
Kepribadian jenis A ternyata mempunyai risiko lebih besar dari pada kepribadian
jenis B untuk mengalami PJK. Ciri-ciri pola perilaku kedua jenis kepribadian itu
adalah sebagai berikut.
Perilaku orang dengan kepribadian jenis A:
1. Berbicara cepat secara konstan.
2. Gerakan-gerakan dan cara makan yang cepat.
3. Menunjukkan ketidaksabaran secara terbuka bila sesuatu berjalan perlahan
menurut pendapatanya; terus-menurus merasa terburu-buru.
4. Berfikir tentang dan melekukan beberapa hal secara simultan.
5. Secara sengaja berusaha mengendalikan pembicaraan, menentukan bahan
pembicaraan dan sibuk dengan pikirannya sendiri sementara orang lain
berbicara.
6. Waktu istirahat atau waktu tidak melakukan apa-apa merasa bersalah
secara bersamar samar.
7. Kepribadian yang berlebihan untuk memperoleh sesuatu yang bernilai
tidak mempunyai waktu untuk menjadi orang yang bernilai.

pada hampir setiap orang. Termasuk disini adalah bencana alam misalnya bencana
bumi, bencana akibat ulah manusia seperti kebakaran, kecelakaan, dan
peperangan serta penyerangan fisik atau perkosaan.
Tidak semua yang terlibat dalam peristiwa itu mengalami reaksi yang
berkepanjangan, sebagian besar pulih dalam waktu satu bulan. Reaksi jangka
panjang yang paling sering terjadi adalah gangguan stres pasca trauma (GSPT)
atau post traumatic stress disorder (PTSD), gangguan fobik dan gangguan
depresif.
GSPT biasanya timbul dalam waktu enam bulan setelah terjadinya peristiwa
traumatik atau merupakan kelanjutan dari gangguann stres akut yang berlangsung
maksimal satu bulan. Alasan berubahnya diagnosis dari gangguan stres akut
menjadi gangguan stres pascatrauma setelah satu bulan adalah karena kasus yang
berlangsung lebih dari satu bulan biasanya menjadi kronis dan memerlukan
pendekatan dan pengobatan yang berbeda daripada gangguan stres akut.
Gejala utama GSPT adalah mengalami kembali secara involunter peristiwa
traumatik dalam bentuk mimpi atau banyangan yang intrusif, yang menerobos
masuk kedalam kesadaran secara tiba-tiba (kilas balik atau flashback). Hal ini
sering dipicu oleh hal-hal yang mengingatkan penderita akan peristiwa traumatik
yang pernah dialami.
Kelompok gejala yang lain adalah tanda-tanda meningkatnya keterjagaan
(arousal) berupa anxietas yang hebat, iritabilitas, insomnia. Dan konsentrasi yang
buruk. Anxietas akan bertambah parah pada saat terjadi kilas balik.
Gejala-gejala disosiatif merupakan kelompok gejala lainnya yang terdiri dari
peristiwa traumatik, perasaan bukan bagian dari peristiwa itu (dectachment)
ketidakmampuan untuk merasakan perasaan (emotional numbness). Kadangkadang terjadi depersonalisasi den derealisasi.

Sampai 50% kasus akan pulih dalam tahun pertama, namun sampai 30%
perjalanan penyakitnya akan kronis. Tampakya hasilnya bergantung pada
keparahan gejala awal. Pemulihan akan dibantu dengan: dukungan sosial yang
baik, tidak ada tanggapan negatif dari orang lain, tidak ada mekanisme coping
yang maladaptif, serta tidak adanya peristiwa traumatik berikutnya, termasuk
kesehatan fisik, disabilitas, kecacatan, relasi yang terputus, masalah keuangan dan
proses hukum.
3.4. Gangguan Penyesuaian Stres
Gangguan penyesuaian terjadi dalam satu bulan setelah stresor psikososial dan
berlangsung tidak lebih lama dari enam bulan setelah stresor tersebut (atau
akibatnya) menghilang, kecuali pada kasus reaksi depresif berkepanjangan.
Gejala-gejala menjadi klinis signifikan karena distress atau fungsi normal. Gejalagejala tersebut mungkin juga berupa manifestasi di bawah ambang (sub-threshold
manifestation) dari gangguan mood,gangguan anxietas, gangguan terkait stres,
gangguan somatoform atau gangguan tingkah laku.
Prevalensi gangguan penyesuaian pada pasien rawat inap atau rawat jalan
diperkirakan sekitar 5%. Pada rumah sakit umum prevalensinya bisa mencapai
20% dengan penyakit fisik merupakan stresor primer pada 70% kasus.
Presdisposisi individual sangat berpengaruh daripada kondisi lain, tetapi gejala
tidak akan muncul tanpa adanya stresor. Komorbiditas dengan penggunaan
alkohol berlebihan sering dijumpai dan menambah rumitnya gambaran kasus.

4. Sukar berkosentrasi.
5. Kehilangan minat terhadap reakreasi yang sebelumnya dapat dinikmati
dan sudah biasa dilakukan.
6. Menjadi khawatir mengenai hal hal yang sebenarnya tidak dapat
diselesaikan dengan perasaan khawatir saja.
7. Bekerja berlebihan, biarpun tidak seluruhnya efektif.
8. Makin lama makin banyak pekerjaan yang dibawa pulang ke rumah.
9. Makin banyak merokok atau makin banyak memakai minuman keras
dibandingkan dengan sebelumnya.
10. Berulang kali merasa kehilangan perspektif atau merasa masa depan suram
mengenai apa yang sebenarnya penting dalam pekerjaan dan keluarga atau
mungkin juga dalam hidup.
Untuk mencegah stres, yang paling baik adalah mengubah sikap terhadap stresor.
Makin penting stresor itu dianggap, makin besar stres yang timbul sebagai
akibatnya. Makin santai dan rileks stresor itu dihadapi, makin banyak alternatif
penyelesain yang dilihat, makin ringan stres itu. Berusahalah melihat peran dan
usaha kita dalam keadaan itu secara realistik dan dalam proporsi yang
sebenaranya, belajar mendelegasikan sebagian pekerjaan kepada orang lain dan
percayalah pada orang itu. Melakukan relaksasi (relaksasi ringan, relaksasi
progresif, meditasi, atau cara-cara relaksasi lain) dapat membantu mengurangi
stres atau pun mencegah timbulnya stres patologis.

Koping merupakan cara-cara yang digunakan oleh individu untuk
menghadapi situasi yang menekan.Oleh karena itu meskipun koping menjadi
bagian dari penyesuaian diri,namun koping merupakan istilahyang khusus

Berhenti merokok adalah bagian dari cara menanggulangi stres karena dapat
meningkatkan kesehatan dan mempertahankan ketahanan dan kekebalan
tubuh.
6. Pengaturan Waktu
Pengaturan waktu merupakan cara yang tepat dalam mengurangi dan
menanggulangi stres. Dengan pengaturan waktu segala pekerjaaan yang dapat
menimbulkan kelelahan fisik dapat dihindari. Pengaturan waktu dapat
dilakukan dengan cara menggunakan waktu secara efektif dan efisien serta
melihat aspek prokdutivitas waktu. Seperti menggunakan waktu untuk
menghasilkan sesuatu dan jangan biarkan waktu berlalu tanpa menghasilkan
sesuatu yang bermanfaat.
7. Terapi Psikofarmaka
Terapi ini dengan menggunakan obat-obatan dalam mengalami stres yang
dialami dengan cara memutuskan jaringan antara psiko neuro dan imunologi
sehingga stresor psikososial yang dialami tidak mempengaruhi fungsi kognitif
afektif atau psikomotor yang dapat mengganggu organ tubuh yang lain. Obat-

obatan yang digunakan biasanya digunakan adalah anti cemas dan anti
depresi.
8. Terapi Somatik
Terapi ini hanya dilakukan pada gejala yang ditimbulkan akibat stres yang
dialami sehingga diharapkan tidak dapat mengganggu sistem tubuh yang lain.
9. Psikoterapi
Terapi ini dengan menggunakan teknik psikologis yang disesuaikan dengan
kebutuhan seseorang. Terapi ini dapat meliputi psikoterapi suportif dan
psikoterapi redukatif di mana psikoterapi suportif memberikan motivasi atau
dukungan agar pasien mengalami percaya diri, sedangkan psikoterapi
redukatif dilakukan dengan memberikan pendidikan secara berulang. Selain
itu ada psikoterapi rekonstruktif, psikoterapi kognitif dan lain-lain.

4.1. Pengertian Depresi
Depresi adalah salah satu tahap paling buruk dalam gangguan saraf,
karena ini merampas begitu banyak keinginan untuk sembuh. Bagaimanapun juga
masih ada yang dapat mengalahkan perasaan itu dan masih punya harapan untuk
sembuh kembali. Saya yakin bahwa orang-orang semacam itu dapat sembuh,
betapapun berat stres yang mereka alami, dan bahwa mereka dapat melakukan itu
tanpa bantuan perawatan shock. Namun demikian, bagi yang sudah tidak lagi
berminat untuk sembuh, serig dianjurkan untuk mendapat perawatan shock. Obat

anti depresan sangat menolong, tetapi obat ini, sama halnya dengan pbat
penenang, hanya boleh dibeli dengan resep dokter.

Istilah depresi sudah begitu popular dalam masyarakat dan semua orang
sudah mengetahuinya, termasuk orang yang awamdalam bidang kedokteran dan
psikologi. Akan tetapi, arti sebenarnya dari depresi itu sukar didefinisikan secara
tepat. Istilah dan kata yang identik maknanya dengan depresi dalam bahasa
Indonesia sehari-hari tidak ada. Sedih tidak identik dengan depresi demikian
juga dengan putus asa, meski keduanya merupakan gejala penting dari depresi.
Orang awam menggunakan istilah depresi dengan sangat bebas dan umum
sehingga mengaburkan makna dari istilah itu sendiri. Ada yang beranggapan
bahwa depresi itu berarti suatu keadaan kesedihan dan ketidakbahagiaan.
Depresi adalah gangguan kesehatan mental, kondisi kejiwaan. Secara
khusus, itu adalah gangguan mood yang ditandai dengan suasana hati yang masih
rendah dimana ada perasaan sedih dan kehilangan minat. Depresi berbeda dari
fluktuasi suasana hati kita semua, pengalaman sebagai bagian dari kehidupan
yang normal dan sehat. Respons emosional sementara terhadap tantangan hidup
sehari-hari tidak merupakan depresi. Demikian juga, bahkan perasaan kesedihan
akibat kematian seseorang yang dekat atau jenis kerugian lainnya jika depresi
tidak stabil. Depresi bisa, bagaimanapun terkait dengan kematian ketika depresi
tersebut menjadi kerugian, psikolog menyebutnya berkabung rumit.

Menurut Rice PL, depresi adalah gangguan mood, kondisi emosional
berkepanjangan yang mewarnai seluruh proses mental (berpikir, berperasaan

b) Gejala Psikis
Adapun tanda-tanda gejala psikis sebagai berikut:
1. Kehilangan rasa percaya diri. Penyebabnya, orang yang mengalami depresi
cenderung memandang segala sesuatu dari sisi negatif, termasuk menilai diri
sendiri.
2. Sensitif. Orang yang mengalami depresi senang sekali mengaitkan segala
sesuatu dengan dirinya. Perasaannya sensitif sekali, sehingga sering peristiwa
yang netral jadi dipandang dari sudut pandang yang berbeda oleh mereka,
bahkan disalahartikan.
3. Merasa diri tidak berguna. Perasaan tidak berguna ini muncul karena mereka
merasa menjadi orang yang gagal terutama dibidang atau lingkungan yang
seharusnya mereka kuasai.
4. Perasaan bersalah. Perasaan bersalah terkadang timbul dalam pemikiran
orang yang mengalami depresi. Mereka memandang suatu kejadian yang
menimpa dirinya sebagai suatu hukuman atau akibat dari kegagalan mereka
melaksanakan tanggung jawab yang seharusnya dikerjakan.
5. Perasaan terbebani. Banyak orang yang menyalahkan orang lain atas
kesusahan yang dialaminya.
c)Gejala sosial

Jangan heran jika masalah depresi yang berawal dari diri sendiri pada
akhirnya memengaruhi lingkungan dan pekerjaan (aktivitas rutin lainnya).
Bagaimana tidak, lingkungan tentu akan bereaksi terhadap perilaku orang
yang depresi tersebutyang pada umumnya negatif (mudah marah, mudah
tersinggung, suka menyendiri, sensitif, mudah letih, mudah sakit).

pengobatan dapat lebih berbahaya daripada depresi.

Kebanyakan dari kita merasa lebih baik dibawah sinar matahari daripada

baik-baik saja ketika musim panas tetapi menjadi depresi ketika musim
dingin. Mereka disebut menderita seasonal affective disorder (SAD).
j. Kepribadian
Aspek-aspek kepribadian ikut pula mempengaruhi tinggi rendahnya depresi
yang dialami serta kerentanan terhadap depresi. Ada individu-individu yang
lebih rentan terhadap depresi, yaitu yang mempunyai konsep diri serta pola
pikir yang negatif, pesimis, juga tipe kepribadian.

Lithium. Terdapat dua jenis dari obat ini, yaitu lithium karbonat dan
lithium sitrat. Obat ini digunakan jika antidepresan tidak cukup kuat untuk
meredakan gejala depresi yang dirasakan. Lithium bisa berubah menjadi
racun jika kadarnya terlalu tinggi di dalam darah. Oleh karena itu,
penderita yang mengonsumsi lithium perlu melakukan tes secara teratur
untuk mengawasi tingkat lithium dalam darah. Konsumsi garam juga perlu
dikurangi karena dapat memicu efek keracunan akibat lithium.
Penyakit depresi yang parah dan tidak ditangani dapat menyebabkan penderita
kehilangan motivasi untuk hidup dan akhirnya memutuskan untuk bunuh diri.
Usahakan untuk membicarakan masalah apapun dengan orang-orang terdekat
Anda atau dengan dokter. Kenali gejala-gejala depresi jika terjadi pada orangorang di sekitar Anda. Makin cepat penanganan dan pengobatan yang dilakukan,
maka peluang kesembuhan secara menyeluruh menjadi lebih tinggi.
Depresi dapat disebabkan oleh banyak hal, seperti trauma, sedih berkepanjangan,
masalah keuangan, hingga tak memiliki pekerjaan. Namun ternyata tanpa
mengalami hal-hal tersebut seseorang bisa juga mengalami depresi. Faktanya, ada
hal-hal yang mungkin tidak disadari yang ikut berkontribusi menyebabkan depresi
yaitu:
1.Merokok
Merokok telah lama dikaitkan dengan depresi, meski belum jelas sebenarnya apa
yang jadi penyebabnya, merokok menyebabkan depresi atau depresi menyebabkan
orang cenderung untuk merokok. Namun demikian, zat yang ada dalam rokok
nikotin diketahui dapat mempengaruhi aktivitas saraf dalam otak sehingga
meningkatkan kadar dopamin dan serotonin yang berperan sebagai obat
antidepresan. Risikonya jika tidak merokok, tubuh akan kekurangan dopamin dan
serotonin yang mengakibatkan depresi. Inilah yang menjadikan rokok bersifat
adiktif.

2.Penyakit tiroid
Saat tiroid, kelenjar berbentuk kupu-kupu di dalam leher, tidak menghasilkan
cukup hormon tiroid (hipotiroidisme), maka seseorang bisa mengalami depresi.
Hormon ini sejatinya memiliki banyak fungsi, namun salah satu fungsi utamanya
yaitu sebagai penghantar rangsangan saraf dan mengatur kadar serotonin.
3.Kebiasaan kurang tidur
Tidur merupakan kebutuhan penting bukan hanya untuk menjauhkan tubuh dari
peradangan, tapi juga dari depresi. Sebuah studi menemukan, orang sehat yang
kurang tidur memiliki aktivitas otak lebih besar saat melihat gambar yang
membuat kesal dibandingkan mereka yang tidak. Aktivitas otak yang besar saat
kesal merupakan ciri dari orang yang depresi. Direktur Center for Circadian
Medicine dr Matthew Edlund mengatakan, jika tidak cukup tidur, maka otak tidak
memiliki waktu untuk memperbaiki sel-sel otak yang rusak sehingga mengganggu
kinerja otak.Kinerja otak merupakan salah satu faktor penyebab depresi.
4. Terlalu banyak menggunakan media sosial
Sejumlah studi menunjukkan, terlalu banyak menggunakan media sosial
berhubungan dengan depresi, terutama pada remaja.Hal ini terjadi karena
kurangnya interaksi antarmanusia dalam kehidupan nyata dan cenderung untuk
berpikir
tidak
realistis
pada
dunia.

tinggal di kota memiliki aktivitas otak lebih tinggi lantaran stres yang dialami.
Sehingga stres berkepanjangan akan menimbulkan depresi.
7. Terlalu banyak pilihan
Saat menghadapi banyak pilihan, orang mungkin saja mengalami depresi. Dari hal
sederhana seperti makanan, memilih yang terbaik dan sesuai dengan kebutuhan

akan keletihan atau bahkan capai mental. Pastikan bahwa Anda memanfaatkan

PENUTUP
5.1. Kesimpulan
Stress menurut Hans Selye merupakan respon tubuh yang bersifat tidak
spesifik terhadap setiap tuntutan atau beban atasnya.Faktor yang mempengaruhi
stress yaitu, faktor lingkungan, faktor organisasi, dan faktor individu. Koping
merupakan cara-cara yang digunakan oleh individu untuk menghadapi situasi
yang menekan. Meskipun koping menjadi bagian dari penyesuaian diri,namun
koping merupakan istilah yang khusus digunakan untuk menunjukkan reaksi
individu ketika menghadapi tekanan/stress.
Distress adalah jenis stres yang memiliki efek negatif pada kesehatan fisik
dan emosional. Distress sering menghasilkan emosi yang intens, seperti
kemarahan, rasa takut, dan kecemasan atau panik. Terkadang, tekanan juga dapat
terwujud dalam gejala fisik, seperti palpitasi, sesak napas, dan peningkatan
tekanan darah.
Depresi adalah gangguan kesehatan mental, kondisi kejiwaan. Secara
khusus, itu adalah gangguan mood yang ditandai dengan suasana hati yang masih
rendah dimana ada perasaan sedih dan kehilangan minat.

What is stress?
Stress is your bodys way of responding to any kind of demand or threat. When
you feel threatened, your nervous system responds by releasing a flood of stress
hormones, including adrenaline and cortisol, which rouse the body for emergency
action. Your heart pounds faster, muscles tighten, blood pressure rises, breath

quickens, and your senses become sharper. These physical changes increase your
strength and stamina, speed your reaction time, and enhance your focus.
This is known as the fight or flight stress response and is your bodys way of
protecting you. When working properly, stress helps you stay focused, energetic,
and alert. In emergency situations, stress can save your lifegiving you extra
strength to defend yourself, for example, or spurring you to slam on the brakes to
avoid an accident.
Stress can also help you rise to meet challenges. Stress is what keeps you on your
toes during a presentation at work, sharpens your concentration when youre
attempting the game-winning free throw, or drives you to study for an exam when
you'd rather be watching TV.
But beyond your comfort zone, stress stops being helpful and can start causing
major damage to your mind and body.
How do you respond to stress?
The latest research into the brain shows that we, as mammals, have three ways of
regulating our nervous systems and responding to stress:

argument with a friend, a traffic jam on your commute to work, or a mountain of
bills, for example, your body can still react as if youre facing a life-or-death
situation.
When you repeatedly experience the fight or flight stress response in your daily
life, it can raise blood pressure, suppress the immune system, increase the risk of

heart attack and stroke, speed up the aging process and leave you vulnerable to a
host of mental and emotional problems.

The causes of stress

Since social engagement appears to be our best defense against stress, isolation or
a lack of positive, consistent human interaction can be both a stressor in itself and
exacerbate other causes of stress.

The situations and pressures that cause stress are known as stressors. We usually
think of stressors as being negative, such as an exhausting work schedule or a
rocky relationship. However, anything that puts high demands on you or forces
you to adjust can be stressful. This includes positive events such as getting
married, buying a house, going to college, or receiving a promotion.
Of course, not all stress is caused by external factors. Stress can also be selfgenerated, for example, when you worry excessively about something that may or
may not happen, or have irrational, pessimistic thoughts about life.

What causes excessive stress depends, at least in part, on your perception of it.
Something that's stressful to you may not faze someone else; they may even enjoy
it. For example, your morning commute may make you anxious and tense because
you worry that traffic will make you late. Others, however, may find the trip

relaxing because they allow more than enough time and enjoy listening to music
while they drive.
Everyone experiences stress differently
Karen is terrified of getting up in front of people to perform or speak, while her
best friend, Nin, lives for the spotlight.
Phil thrives under pressure and performs best when he has a tight deadline, while
his co-worker, Matt, shuts down when work demands escalate.
Anita enjoys helping her elderly parents. Her sister, Constance, helps out as well
but finds the demands of caretaking very stressful.
Richard doesnt hesitate to send food back or complain about bad service when
eating out, while his wife, Miranda, finds it much too stressful to complain.
What determines your ability to manage stress?
We're all different. Some people seem to be able to roll with lifes punches, while
others tend to crumble in the face of far smaller obstacles or frustrations. Some
people even seem to thrive on the excitement and challenge of a high-stress
lifestyle.
Your ability to tolerate stress depends on many factors, including the quality of
your relationships and support network, your life experiences, your emotional
intelligence, and genetics.
Factors that influence your stress tolerance

Your exercise levels. Your physical and mental health are intrinsically
linked, so the better you take care of your body, the greater resilience
youll have against the symptoms of stress. Exercising regularly (for 30
minutes or more on most days) can lift your mood and help relieve stress,
anxiety, anger, and frustration. It can also serve as a distraction to your
worries, allowing you to find some quiet time and break out of the cycle of
negative thoughts that feed stress and anxiety.
Your diet. The food you eat can also have a profound effect on your mood
and how well you cope with lifes stressors. Eating a diet full of processed
and convenience food, refined carbohydrates, and sugary snacks can
worsen symptoms of stress while eating a diet rich in fresh fruit and
vegetables, high-quality protein, and healthy fats, especially omega-3 fatty
acids, can help you better cope with lifes ups and downs.
Your sense of control It may be easier to take stress in your stride if you
have confidence in yourself and your ability to influence events and
persevere through challenges. If you feel like things are out of your
control, youre likely to have less tolerance for stress.
Your attitude and outlook Optimistic people are often more stresshardy. They tend to embrace challenges, have a strong sense of humor,
and accept that change is a part of life.

Engage socially. The simple act of talking face to face with another
human being can release hormones that reduce stress even if youre still
unable to alter the stressful situation. Opening up to someone is not a sign
of weakness and it wont make you a burden to others. In fact, most
friends will be flattered that you trust them enough to confide in them, and
it will only strengthen your bond.

You may feel theres nothing you can do about stress. The bills wont stop
coming, there will never be more hours in the day, and your work and family
responsibilities will always be demanding. But you have more control over stress
than you might think. Stress management is all about taking charge: of your
lifestyle, thoughts, emotions, and the way you deal with problems. No matter how
stressful your life seems, there are steps you can take to relieve the pressure and
regain control.
What is stress management?
We all respond to stress differently so, theres no one size fits all solution to
managing stress. But if you feel like the stress in your life is out of control, its
time to take action. Stress management can teach you healthier ways to cope with
stress, help you reduce its harmful effects, and prevent stress from spiraling out of
control again in the future.
No matter how powerless you may feel in the face of stress, you still have control
over your lifestyle, thoughts, emotions, and the way you deal with problems.
Stress management involves changing the stressful situation when you can,
changing your reaction when you cant, taking care of yourself, and making time
for rest and relaxation. The first step is to recognize the true sources of stress in
your life.
What are the sources of stress in your life?
Start a stress journal

A stress journal can help you identify the regular stressors in your life and the way
you deal with them. Each time you feel stressed, keep track of it in your journal.
As you keep a daily log, you will begin to see patterns and common themes. Write
down:
Its easy to identify sources of stress following a major life event such as changing
jobs, moving home, or losing a loved one, but pinpointing the sources of everyday
stress can be more complicated. Its all too easy to overlook your own thoughts,
feelings, and behaviors that contribute to your stress levels. Sure, you may know
that youre constantly worried about work deadlines, but maybe its your
procrastination, rather than the actual job demands, that is causing the stress.
To identify your true sources of stress, look closely at your habits, attitude, and
excuses:
Do you define stress as an integral part of your work or home life (Things
are always crazy around here) or as a part of your personality (I have a
lot of nervous energy, thats all)?
Until you accept responsibility for the role you play in creating or maintaining it,
your stress level will remain outside your control.
Start a Stress Journal

A stress journal can help you identify the regular stressors in your life and the way

If your methods of coping with stress arent contributing to your greater emotional
and physical health, its time to find healthier ones. No single method works for
everyone or in every situation, so experiment with different techniques and
strategies. Focus on what makes you feel calm and in control.
Stress management strategy #1: Get moving
Physical activity plays a key role in reducing and preventing the effects of stress,
but you dont have to be an athlete or spend hours in a gym to experience the
benefits. Just about any form of physical activity can help relieve stress and burn
away anger, tension, and frustration. Exercise releases endorphins that boost your
mood and make you feel good, and it can also serve as a valuable distraction to
your daily worries.
While the maximum benefit comes from exercising for 30 minutes or more, you
can start small and build up your fitness level gradually. Short, 10-minute bursts
of activity that elevate your heart rate and make you break out into a sweat can
help to relieve stress and give you more energy and optimism. Even very small
activities can add up over the course of a day. The first step is to get yourself up
and moving. Here are a few easy ways:
Park your car in the farthest spot in the lot and walk the rest of the way
Pair up with an exercise partner and encourage each other as you workout
and require moving both your arms and your legsare especially effective at
relieving stress. Walking, running, swimming, dancing, cycling, tai chi, and
aerobic classes are good choices.
Pick an activity you enjoy, so youre more likely to stick with it. Instead of
continuing to focus on your thoughts while you exercise, make a conscious effort
to focus on your body and the physical (and sometimes emotional) sensations you
experience as youre moving. Adding this mindfulness element to your exercise
routine will help you break out of the cycle of negative thoughts that often
accompanies overwhelming stress. Focus on coordinating your breathing with
your movements, for example, or notice how the air or sunlight feels on your skin.
Getting out of your head and paying attention to how your body feels is also the
surest way to avoid picking up an injury.
When youve exercised, youll likely find it easier to put other stress management
techniques to use, including reaching out to others and engaging socially.
Stress management strategy #2: Engage socially
Reach out and build relationships

Social engagement is the quickest, most efficient way to rein in stress and avoid
overreacting to internal or external events that you perceive as threatening. There
is nothing more calming to your nervous system than communicating with another
human being who makes you feel safe and understood. This experience of
safetyas perceived by your nervous systemresults from nonverbal cues that
you hear, see and feel.
The inner ear, face, heart, and stomach are wired together in the brain, so socially
interacting with another person face-to-facemaking eye contact, listening in an
attentive way, talkingcan quickly calm you down and put the brakes on
defensive stress responses like fight-or-flight. It can also release hormones that
reduce stress, even if youre unable to alter the stressful situation itself. Of course,
its not always realistic to have a pal close by to lean on when you feel
overwhelmed by stress, but by building and maintaining a network of close
friends you can improve your resiliency to lifes stressors. On the flip side, the
more lonely and isolated you are, the greater your vulnerability to stress.
Reach out to family and friends and connect regularly in person. The people you
talk to dont have to be able to fix your stress; they just need to be good listeners.
Opening up is not a sign of weakness and it wont make you a burden to others. In
fact, most friends will be flattered that you trust them enough to confide in them,
and it will only strengthen your bond. And remember, its never too late to build
new friendships and improve your support network.
Reach out and build relationships

Learn how to say no Know your limits and stick to them. Whether in
your personal or professional life, taking on more than you can handle is a
surefire recipe for stress. Distinguish between the shoulds and the
musts and, when possible, say no to taking on too much.
Avoid people who stress you out If someone consistently causes stress
in your life, limit the amount of time you spend with that person, or end
the relationship.

If you cant avoid a stressful situation, try to alter it. Often, this involves changing
the way you communicate and operate in your daily life.
Manage your time better. Poor time management can cause a lot of
stress. But if you plan ahead and make sure you dont overextend yourself,
youll find it easier to stay calm and focused.
Look at the big picture. Take perspective of the stressful situation. Ask
yourself how important it will be in the long run. Will it matter in a
month? A year? Is it really worth getting upset over? If the answer is no,
focus your time and energy elsewhere.

Stress management strategy #6: Accept the things you cant change
Many sources of stress are unavoidable. You cant prevent or change stressors,
such as the death of a loved one, a serious illness, or a national recession. In such
cases, the best way to cope with stress is to accept things as they are. Acceptance
may be difficult, but in the long run, its easier than railing against a situation you
cant change.
Dont try to control the uncontrollable. Many things in life are beyond
our controlparticularly the behavior of other people. Rather than
stressing out over them, focus on the things you can control such as the
way you choose to react to problems.
Look for the upside. When facing major challenges, try to look at them as
opportunities for personal growth. If your own poor choices contributed to
a stressful situation, reflect on them and learn from your mistakes.
Learn to forgive. Accept the fact that we live in an imperfect world and
that people make mistakes. Let go of anger and resentments. Free yourself
from negative energy by forgiving and moving on.
Stress management strategy #7: Make time for fun and relaxation
Beyond a take-charge approach and a positive attitude, you can reduce stress in
your life by nurturing yourself. If you regularly make time for fun and relaxation,
youll be in a better place to handle lifes stressors.
Develop a "stress relief toolbox"

Come up with a list of healthy ways to relax and recharge. Try to implement one
or more of these ideas each day, even if you're feeling good.
Go for a walk
tennis or racquetball
Get a massage
Listen to music
Watch a comedy
Dont get so caught up in the hustle and bustle of life that you forget to take care
of your own needs. Nurturing yourself is a necessity, not a luxury.
Set aside relaxation time. Include rest and relaxation in your daily
schedule. Dont allow other obligations to encroach. This is your time to
take a break from all responsibilities and recharge your batteries.
Do something you enjoy every day. Make time for leisure activities that
bring you joy, whether it be stargazing, playing the piano, or working on
your bike.
Keep your sense of humor. This includes the ability to laugh at yourself.
The act of laughing helps your body fight stress in a number of ways.
Eat a healthy diet. Well-nourished bodies are better prepared to cope with
stress, so be mindful of what you eat. Start your day right with breakfast,

and keep your energy up and your mind clear with balanced, nutritious
meals throughout the day.
Reduce caffeine and sugar. The temporary "highs" caffeine and sugar
provide often end in with a crash in mood and energy. By reducing the
amount of coffee, soft drinks, chocolate, and sugar snacks in your diet,
youll feel more relaxed and youll sleep better.
Get enough sleep. Adequate sleep fuels your mind, as well as your body.
Feeling tired will increase your stress because it may cause you to think
irrationally.

New or repeated behavior which pushes the limits of decorum and which
interferes with effective management of the immediate environment.
Allow the student to speak freely about their current situation and
the variables that may be affecting their distress.
Refer the student to one of the University resources. See referral phone
numbers listed later in this article.
Severe Distress
Students in severe distress exhibit behaviors that signify an obvious crisis and that
necessitate emergency care. These problems are the easiest to identify. Examples
include:

Loss of contact with reality (seeing or hearing things which others cannot
see or hear; beliefs or actions greatly at odds with reality or probability).
Stalking behaviors.
Help the person recall constructive methods used in the past to cope; get
the person to agree to do something constructive to change things.
When called for, let the person know you are worried about their safety
and describe the behavior or situation that is worrisome to you.
After the student leaves your office, make some notes documenting your
interactions.

CAPS as a resource
At Counseling and Psychological Services (CAPS), we have a diverse staff of
mental health professionals providing confidential, solution-focused individual,
couples and group therapy to currently enrolled U of M students at no charge.
CAPSs main focus is providing college student mental health services. CAPS
regularly provides immediate advice to concerned faculty and staff through the
Counselor on Duty service.
Making a referral

While many students seek help on their own, your exposure to students increases
the likelihood you will identify signs or behaviors of distress in a student, or that a
student will ask you for help. What can you do?
You can help by telling the student that they can go to CAPS on the 3rd floor of
the Michigan Union. They will need to fill out intake information and then make
an appointment for as soon as possible. If the student is in crisis and needs to see
someone on the same day, they can indicate their need to see the Counselor on
Duty.
Other tips:
Dispute the myth that only "weak or crazy" people go for counseling or
use others for help.
Remind them that campus counseling resources are free and confidential
services.
Offer to help make the initial contact with the helping resource.

Having these problems persistently, such that they affect life for a period of time,
could signal the treatable condition of depression, rather than a passing episode of
"feeling down" - which most of us feel at some point.
According to the US Centers for Disease Prevention and Control (CDC), 8% of
people over the age of 12 years has depression in any two-week period.1
The World Health Organization (WHO) puts depression at the top of the list - it is
the most common illness worldwide and the leading cause of disability. The
organization estimates that 350 million people around the world are affected by
depression.2

Over the past month, have you often been bothered by feeling down,
depressed or hopeless?
Doctors may also follow the criteria set out by the Diagnostic and Statistical
Manual of Mental Disorders (DSM-5).4
Here, depression is diagnosed if five or more criteria are met, from a list of nine,
and represent a change from normal and have been present for the past two
weeks.11
At least one of the symptoms must be one of the two above - that is, either
depressed mood or loss of interest or pleasure. The full list of criteria are:3,11

Low mood for most of the day every day is one of the flags for depression.
The five or more symptoms must be severe enough to affect daily activities or
social interactions, or cause significant distress. It must not be possible to explain
them by another medical condition, substance use or other causes.3,11
For example, dementia may present as depression, and vice versa. However,
memory testing can distinguish the two, so people being investigated for
depression may be asked to answer questions that test their cognitive abilities.3
Other underlying causes of depression that should be diagnosed and treated
include:3,5
Hypothyroidism
Brain tumor
Parkinson's disease
Vitamin deficiency.

Psychotherapy
Psychological or talking therapies for depression include cognitive-behavioral
therapy (CBT), interpersonal psychotherapy and problem-solving treatment.2
In mild cases of depression psychotherapies are the first-line option for
treatment; in moderate and severe cases they may be an adjunctive therapy
with other treatment.2,5,6
CBT and interpersonal therapy are the two main types of psychotherapy used in
depression. Both talking therapies are present-focused and encourage the
regaining of control over mood and functioning:6,7

Interpersonal therapy helps patients to identify emotional problems that affect
relationships and communication, and how these in turn affect mood and can be
changed.14
Interpersonal therapy sessions - conducted over a limited number of weeks - will
involve the therapist gaining trust before asking about symptoms and
relationships, both through asking questions and providing questionnaires. 15
Antidepressant medications
Antidepressants are drugs available on prescription from a doctor, whether one
providing primary care treatment, or a psychiatrist.
In mild cases of depression, antidepressants are not generally the first-line
option for treatment - in which case, psychological therapy may be
recommended first.2
Drugs come into use for moderate to severe depression, but are not recommended
for children, and will be prescribed only with caution for adolescents.2
A choice of antidepressant medications is available - the individual selection is a
matter of personal preference, previous success or failure, adverse side-effects,
whether overdose is likely and could be a danger, and interaction with any other
treatments being used.3,5
A number of classes of medication are available in the treatment of depression:16
Selective serotonin and norepinephrine reuptake inhibitors (SNRI) desvenlafaxine (Pristiq), duloxetine (Cymbalta) venlafaxine (Effexor).

Electroconvulsive therapy
Severe cases of depression that have not responded to drug treatment may benefit
from electroconvulsive therapy (ECT), which is particularly effective for
psychotic depression.5 MNT has produced detailed information about the

Are talking therapies or antidepressants most effective for depression?
All medical treatments are prescribed against the individual suitability of the
treatment for the person, and the same is true in depression.
For some patients it may be a simple matter of choice - perhaps antidepressants
are more convenient for some than talking therapies. Another person, meanwhile,
may prefer to avoid drug treatment.
If there is a question of choosing one or other option based on effectiveness alone,
one study tested whether cognitive therapy had an enduring effect and compared
this against continued antidepressant medication.18
The conclusion of the 2005 report, published in the journal JAMA Psychiatry,
was:
"Cognitive therapy has an enduring effect that extends beyond the end of
treatment. It seems to be as effective as keeping patients on medication."
The study also cited evidence that CBT is associated with less relapse to
depression after treatment than medication.
Another comparison looked at mindfulness-based cognitive therapy and found
that it offered a level of protection similar to antidepressant drugs against relapses
of depression.19

The trial results were published in The Lancet in April 2015, as reported by MNT.
The conclusion was:
"Both treatments were associated with enduring positive outcomes in terms of
relapse or recurrence, residual depressive symptoms, and quality of life."
