ABSTRACT
This study aims to analyze nurse’s work load based on patient’s dependency
level, using time and motion study in the Inpatient Ward of Surabaya Haji General
Hospital (SHGH). This is an analytic observational research conducted cross
sectional from May until November 2008. The population was all nurses of
SHGH Inpatient Ward. Sample was taken by proportional random sampling
method amounting to 42 people. To find out the correlation of nurse’s work load
and patient’s dependency level, a Chi-square test was utilized.
Research result revealed SHGH policy was referring to 2002 Nurse Human
Resources Standard. Parties connected with Nursing Department were nurse’s
division, nurse’s committee and the Inpatient Ward Head. Respondents were
mostly female, aged 30-34 years old, most function was executing nurses, and
most seen at IIIA, IVA and IVC wards. Subjective work load was 81% (high).
Hand-over system at SHGH was good. The number of nurses for morning shift
was 35.29%, afternoon and evening shift was similiar 17.65%. Objective work
load on morning shift was 52.21% categorized low (<65% from total time), on
afternoon shift was 44.44% categorized low (<65% from total time), on evening
shift was 29.22% categorized low (<65% from total time). Self care patient had
17.04 minutes; partial care patient had 20.84 minutes, and total care patient had
25.74 minutes out of 24-hours nursing care. The statistical test revealed there was
no correlation between nurse’s work load and patient’s level of dependency (p=
>0.05).
Research conclusion showed no correlation between nurse’s work load and
patient’s level of dependency. Other dominant factors besides patient’s level of
depedency, are 1)provider [vision and mission, organization development, budget
allocation, hospital Bed Occupancy Rate (BOR), technology, human resources
planning, number of visitation, beds, length-of-stay, patient’s day and hour of
nursing care, hospital policy]; 2)nurse (characteristic, inpatient ward, total number
of patients, position/work status, achievement & satisfaction; and productivity);
3)environment (physical, biological, chemical, physiologic, sosio-physiolologic;
and distance); 4)patient (characteristic, case illustration; number of cases,
complaints, patient’s flow); 5)nursing care method; 6)non-nursing activity, and
7)Standard Operating Procedure (SOP).
