Pendahuluan
Human amniotic membrane (HAM) atau selaput amnion manusia adalah lapisan
terdalam plasenta yang mengelilingi embrio yang sedang berkembang dan membentuk
rongga amnion. HAM memiliki struktur dan karakteristik biologis yang khas sehingga
menjadi bahan yang sangat biokompatibel untuk berbagai aplikasi dalam rekayasa jaringan
dan pengobatan regeneratif (Hu et al., 2023; Ramuta & Kreft, 2018). HAM dan sel
turunannya memiliki banyak karakteristik yang dapat digunakan dalam pengobatan
regeneratif, seperti imunogenisitas rendah, meningkatkan epitelisasi, memiliki efek
antiinflamasi, angiogenik dan antiangiogenik, antifibrotik, antibakteri, dan antikanker
(Abbasi-Kangevari et al., 2019; Gholipourmalekabadi et al., 2016; Leal-Marin et al., 2021;
Mamede et al., 2012).
HAM memiliki berbagai kegunaan dalam rekayasa jaringan dan pengobatan
regeneratif, di antaranya sebagai scaffold untuk regenerasi kulit, tulang, tulang rawan, dan
saraf. Selain itu, HAM juga digunakan sebagai sumber sel punca dan faktor pertumbuhan
untuk regenerasi retina, serta sebagai cangkok pada kelainan permukaan mata (Elkhenany
et al., 2022; Hu et al., 2023; Orman et al., 2020; Ramuta & Kreft, 2018; Xiao et al., 2021).
HAM telah digunakan dalam bidang oftalmologi untuk regenerasi mata selama bertahun-
tahun. HAM digunakan dalam berbagai teknik untuk rekonstruksi permukaan mata,
termasuk pencangkokan, pembalutan, transplantasi inlay, rekonstruksi konjungtiva, dan
bedah symblepharon (Elkhenany et al., 2022; Fénelon et al., 2021; Malhotra & Jain, 2014;
Munoz-Torres et al., 2023; Murri et al., 2018)
Berdasarkan hasil penelitian terdahulu, HAM diketahui dapat meningkatkan
epitelisasi kornea (Baradaran-Rafii et al., 2018; L. Liang et al., 2009), mengurangi
inflamasi (J. S. Kim et al., 2000; J. Li et al., 2018; Meller et al., 2000; Shay et al., 2011; J.
L. Tan et al., 2015), dan mencegah terbentuknya jaringan parut (Goktas et al., 2017;

Palamar et al., 2014). HAM juga digunakan sebagai scaffold dalam transplantasi sel punca
limbal, pembawa faktor pertumbuhan, dan sel punca untuk regenerasi retina (Elkhenany et
al., 2022; Fénelon et al., 2021; Munoz-Torres et al., 2023; Murri et al., 2018). HAM
memiliki beberapa keunggulan dibandingkan bahan lain, beberapa di antaranya yaitu
biokompatibilitas yang baik, memiliki efek antiinflamasi, memiliki faktor pertumbuhan
regeneratif yang tinggi, serta memiliki sifat non-imunogenik dan non-tumorigenik (Insausti
et al., 2010; Malhotra & Jain, 2014; Miki, 2016; Munoz-Torres et al., 2023; Hassan
Niknejad et al., 2008; F. D. Tehrani et al., 2021). Akan tetapi, terdapat beberapa
keterbatasan dalam penggunaan HAM, yaitu ketersediaan yang terbatas dan variabilitas
dalam kualitas, masa simpan yang pendek, risiko kontaminasi, data jangka panjang yang
terbatas mengenai keamanan dan keberhasilan, serta biaya yang mahal.
Meskipun potensi HAM dalam regenerasi mata cukup menjanjikan, penelitian lebih
lanjut diperlukan untuk mengoptimalkan aplikasinya dalam penerapan klinis dan untuk
memastikan keamanan dan keberhasilan jangka panjang. Artikel review ini bertujuan
untuk merangkum karakteristik biologis HAM, teknik yang digunakan untuk transplantasi
HAM di bidang oftalmologi, dan pengaplikasian HAM dalam regenerasi mata. Dengan
memberikan gambaran yang komprehensif penelitian terkini mengenai HAM dalam
regenerasi mata, artikel review ini bertujuan untuk memberikan kontribusi dalam
pengembangan terapi berbasis HAM yang efektif dan aman untuk kelainan permukaan
mata dan penyakit pada retina.
HAM memiliki efek anti-inflamasi karena adanya hAEC (human amniotic epithelial
cell) dan hAMSC (human amniotic mesenchymal stromal cell), yang mengekspresikan
berbagai protein antiangiogenik dan anti-inflamasi (Hao et al., 2000). HAM dapat
menginduksi efek imunomodulator dengan menghambat sitokin pro-inflamasi, mengurangi
stres oksidatif, menghambat kemotaksis, dan meningkatkan fungsi fagositosis (J. Li et al.,
2018). Studi praklinis menunjukkan bahwa sel epitel amnion manusia (hAEC) yang berasal
dari amnion manusia berpotensi untuk mengobati sklerosis ganda sehubungan dengan
efeknya dalam mengurangi inflamasi (Xu et al., 2019). Perlu diketahui bahwa hAECs
dapat mensekresikan faktor neurotropik (NTF) dalam jumlah besar (Uchida et al., 2000),
yang dapat menstimulasi remyelinisasi dan melindungi oligodendrosit dari apoptosis untuk
memulihkan dan mempertahankan fungsi neurologis pada sklerosis ganda (Calzà et al.,
1997; Linker et al., 2010; Stankoff & Zalc, 2002). hAEC terbukti dapat menghasilkan
faktor anti-inflamasi dan faktor imunosupresif, dimana keduanya merupakan
imunomodulator yang kuat (Broughton et al., 2012; Evans et al., 2018; Hodge et al., 2014;
H. Li et al., 2005; H. Liang et al., 2014).
Studi penelitian terbaru menunjukkan bahwa fungsi angiogenik HAM dipengaruhi
oleh permukaannya, yang berarti dapat mendorong atau menghambat angiogenesis
(Munoz-Torres et al., 2023; H. Niknejad et al., 2013). Permukaan epitel amnion memiliki
efek inhibisi pada pembentukan pembuluh darah (Nogami et al., 2012) dan hAESC dapat
masuk ke dalam dinding arteri tanpa imunosupresi, namun tidak dapat meningkatkan
fungsi pembuluh darah (Vácz et al., 2016). Sedangkan pada sisi mesenkim amnion,
panjang pembuluh darah dapat meningkat (Ankrum et al., 2014). Prekursor embrio dari
selaput amnion mensekresikan berbagai faktor angiogenik dalam jumlah yang signifikan
(Jiang et al., 2015; Zhang et al., 2017) dan memiliki potensi angiogenik yang kuat (Jiang et
al., 2015; S. W. Kim, Zhang, Kim, et al., 2012; König et al., 2012; Wu et al., 2017).
Berdasarkan hasil penelitian pra-klinis, diketahui bahwa penggunaan hAMSC sebagai
terapi berbasis sel dapat memberikan efek terapeutik pada tungkai belakang iskemik (S. W.
Kim, Zhang, Kim, et al., 2012), memberikan efek terapeutik dalam terapi sklerosis ganda
(Abbasi-Kangevari et al., 2019), memiliki potensi terapeutik dalam penyembuhan luka
kronis (S. W. Kim, Zhang, Guo, et al., 2012), potensi terapeutik dalam regenerasi tulang
dan pembuluh darah akibat glukolipotoksisitas (Zhang et al., 2018), serta membantu proses
perbaikan setelah infark miokard akut (Danieli et al., 2015).

Mata kering adalah gangguan pada permukaan mata yang ditandai dengan
ketidakstabilan selaput air mata dan inflamasi yang mengganggu epitel dan saraf
kornea, yang mengakibatkan kerusakan permukaan mata dan siklus inflamasi yang
berlangsung terus-menerus (Stevenson et al., 2012). Siklus inflamasi kronis yang
terjadi pada pasien dengan penyakit mata kering dapat berkaitan dengan kondisi
mata lainnya atau berkaitan dengan kerusakan yang terjadi pada permukaan mata
melalui hiperosmolaritas lapisan air mata. Pelepasan mediator proinflamasi,
termasuk sitokin dan peningkatan ekspresi enzim matriks metaloproteinase dapat
meningkatkan pembentukan sel imun pada permukaan mata, yang selanjutnya
dapat mengganggu selaput air mata (Nebbioso et al., 2017).
Efek terapeutik CAM dalam pengobatan penyakit mata kering dapat
dikaitkan dengan beberapa mekanisme. Pertama, CAM bertindak sebagai perban
terapeutik yang menjaga kelembapan mata dengan menahan air mata dan
melindungi permukaan mata dari lingkungan sekitar. Mekanisme kedua adalah
dengan mengontrol inflamasi permukaan mata (Cheng et al., 2016) karena telah
diketahui bahwa inflamasi yang dipicu oleh respon imun bawaan dan adaptif sangat
penting dalam patogenesis dan kronisitas penyakit mata kering (Wei & Asbell,
2014; Yagci & Gurdal, 2014). Sebagai contoh fungsi anti inflamasi, CAM terbukti
dapat memicu apoptosis neutrofil (Park & Tseng, 2000; Wang et al., 2001),
monosit, makrofag (Shimmura et al., 2001), mengurangi infiltrasi neutrofil (Park &
Tseng, 2000; Wang et al., 2001), makrofag (Bauer et al., 2007; Heiligenhaus et al.,
2001), dan limfosit (Bauer et al., 2009), serta meningkatkan polarisasi makrofag
M2 (Bauer et al., 2012). Mekanisme ketiga adalah kemampuan CAM dalam
meregenerasi saraf kornea (John et al., 2017; Morkin & Hamrah, 2018).
Pterigium adalah pertumbuhan berlebih superfisial berbentuk sayap pada
permukaan mata yang ditandai dengan inflamasi, proliferasi konjungtiva
fibrovaskular, dan invasi pada permukaan kornea (Dushku, 2001; Dushku & Reid,
1994, 1997). Pterigium dapat disebabkan oleh kerusakan yang terjadi akibat
paparan sinar ultraviolet pada sel punca limbal (Dushku & Reid, 1997; D. T.H. Tan
et al., 1997) dan kemudian dianggap sebagai defisiensi sel punca limbal (Kwok &
Coroneo, 1994). Inflamasi permukaan mata sangat berperan dalam pertumbuhan
dan kekambuhan pterigium. Matriks stroma AM diketahui efektif dalam

Perforasi kornea adalah kondisi mata yang terjadi ketika terdapat lubang atau
ruptur pada kornea, yaitu bagian bening pada mata yang menutupi iris dan pupil.
Penyebab perforasi kornea dapat diklasifikasikan sebagai trauma atau nontrauma,
dengan penyebab perforasi nontrauma dibagi menjadi infeksi atau noninfeksi.
Perforasi infeksi dapat terjadi akibat infeksi bakteri, jamur, virus, atau parasit,
sedangkan noninfeksi meliputi gangguan pada permukaan mata atau penyakit
autoimun (Deshmukh et al., 2020). Berdasarkan hasil penelitian (Fan et al., 2016),
pada kondisi ketika transplantasi kornea tidak dapat dilakukan karena kurangnya
donor kornea segar, AMR yang dikombinasikan dengan metode multilayer
amniotic membrane cover merupakan alternatif yang efektif untuk transplantasi
konjungtiva atau operasi penutup flap konjungtiva dengan tingkat keberhasilan
100%.
Ocular graft versus host disease (oGVHD) adalah komplikasi yang
disebabkan oleh kekebalan tubuh dari transplantasi sel punca hematopoietik
alogenik (HSCT) yang muncul sebagai sindrom Stevens Johnson atau sindrom
Sjogren (Ogawa et al., 2013). CAM tanpa jahitan maupun CAM cangkok
mengandung kompleks HC-HA/PTX3 (rantai berat-hialuronat-pentraxin3) yang
dapat berfungsi sebagai anti inflamasi dan mencegah timbulnya jaringan parut (Liu
et al., 2010; Ogawa et al., 2017). Berdasarkan penelitian yang dilakukan oleh
(Ogawa et al., 2017), diketahui bahwa khusus untuk GVHD, injeksi subkonjungtiva
HC-HA/PTX3 yang dipurifikasi dari HAM terbukti dapat mencegah perkembangan
oGVHD ketika diberikan saat transplantasi sel induk hematopoietik alogenik pada
tikus dengan mempertahankan sekresi air mata, mempertahankan kepadatan sel
goblet konjungtiva, dan menghambat jaringan parut serta infiltrasi sel efektor CD4 +
Th 17 dan sel CD45+ dan MHC kelas II+ pada kelenjar air mata.
Sindrom Stevens-Johnson (SJS) dan nekrolisis epidermal toksik (TEN)
termasuk dalam spektrum penyakit yang ditandai dengan pengelupasan kulit dan
selaput lendir akut. SJS/TEN akut muncul di permukaan mata dengan peradangan
tepi kelopak mata, pembentukan pseudomembran konjungtiva, dan kerusakan epitel
pada kornea dan konjungtiva (Kohanim et al., 2016). AM yang diaplikasikan di
mata yang mengalami inflamasi fase akut pada pasien SJS/TEN akan memberikan
efek anti inflamasi dan mengurangi timbulnya jaringan parut (Tseng et al., 2004).

Kesimpulan
Selaput amnion manusia telah menjadi komponen penting dalam pengobatan
berbagai gangguan mata, terutama dalam kasus-kasus yang melibatkan peradangan,
luka, atau penyakit permukaan mata. Selaput amnion memiliki sifat antiinflamasi,
antimikroba, dan kemampuan penyembuhan yang baik, sehingga dapat membantu
memperbaiki kondisi permukaan mata yang rusak, meredakan peradangan, dan
meningkatkan kenyamanan pasien. Penggunaan selaput amnion dalam oftalmologi
masih merupakan area penelitian yang terus berkembang, dan pilihan
penggunaannya harus dipertimbangkan secara individual untuk setiap pasien oleh
profesional medis atau spesialis mata yang berpengalaman.
