Laserasi bronkus kanan setelah kecelakaan motor. Pada pemeriksaan radiografi paru
menunjukkan pneumothorak persisten yang luas pada paru kiri meskipun sudah
dilakukan drainase dengan chest tube yang adekuat. Tampak paru kiri kolaps ke inferior
menjauhi hilus (fallen lung sign).

Efusi pericardium merupakan hasil perjalanan klinis dari
penyakit disebabkan oleh infeksi, keganasan, maupun
trauma.

TRAUMA DIAFRAGMA
Berasal dari trauma tumpul atau trauma penetrasi ke
dada/ abdomen yang memerlukan tindakan bedah
Kadang tidak terdiagnosis kecil,tdk ada herniasi
Komplikasi seringkali timbul akibat herniasi isi abdomen
ke rongga thoraks.
Herniasi visceral dapat menyebabkan iskemia, obstruksi
perforasi, disertai kompresi/kolaps paru.
Robekan postero-lateral radial merupakan yang paling
umum ditemukan pada trauma tumpul.

Diagnosis banding
Patah tulang rusuk sederhana dan kontusio paru dengan
di bawah ventilasi sekunder untuk nyeri
Deformitas flail chest pada seorang laki-laki dengan dyspnea berat setelah mengalami
kecelakaan motor. Pada pemeriksaan radiografi dada menunjukkan mediastinum
abnormal, dan fraktur multiple iga kanan yang bersebelahan (panah). Catatan: fraktur
juga terjadi pada scapula dan clavicular.
EFUSI PLEURA
Terkumpulnya cairan di dalam kavum pleura
transudate, eksudat,darah maupun cairan limfe.
Efusi Pleura terjadi jika ada>> produksi cairan pleura
atau << resorbsi cairan di ruang pleura
Kelebihan produksi : tekanan hidrostatik, tekanan
osmotic, permeabilitas kapiler
berkurangnya resorbsi : absorbsi saluran limfatik
karena sumbatan (tumor) / tekanan vena yang
menurunkan transportasi cairan melalui duktus
torasikus, tekanan di rongga pleura, seperti atelectasis
akibat sumbatan bronkus.

cairan akan masuk ke dalam alveolar (fase alveolar).
PNEUMOMEDIASTINUM
Emfisema mediastinum udara ekstraluminal di mediastinum.
Pneumomediastinum dapat menyebabkan pneumotoraks,
pneumoperikardium, pneumoperitoneum, atau
pneumoretroperitoneum.
Penyebab :
- Sumber intrathoracic seperti asma, muntah, partus, dan trauma
tumpul dada.
- Sumber ektrathoraksis dapat dari kepala dan leher atau
retroperitoneum. i robekan jalan napas intrapulmonary kecil
(alveoli / bronkus), kebocoran udara melalui hilus paru-paru ke
mediastinum.

Udara pada pericardium
Disebabkan oleh trauma penetrasi langsung ke
pericardium:
-Iatrogenic (pada saat operasi jantung)
-Kecelakaan (dari trauma penetrasi).

yang mengelilingi jantung, terikat oleh lapisan parietal
pericardial, yang membentang tidak lebih tinggi dari akar
pembuluh darah besar (setinggi dengan arteri
pulmonary)
EMFISEMA SUBKUTIS
Udara dapat masuk kedalam jaringan lunak leher, dada
dan dinding abdomen dari trauma tembus pada dinding
dada.
Udara yang masuk sepanjang otot menghasilkan
gambaran lusensi pada soft tissue berbentuk gambaran
garis striata
Tidak berefek klinis,volume udara subkutan yang ada,
memerlukan beberapa hari / satu minggu untuk udara di
reabsorbsi
Emfisema subkutaneus. Udara dapat melalui jaringan subkutan leher, dada dan
dinding abdomen dari mediastinum, atau dapat mencapai jaringan lunak
melalui tindakan pembedahan thorakotomi drainage tube atau trauma penetrasi
pada dinding dada. Udara masuk sepanjang otot menghasilkan karakteristik
striated appearance (panah putih).
PENUTUP
Kasus kasus kegawatdaruratan thoraks saat ini
semakin sering dijumpai seiring meningkatnya kejadian
trauma
Para klinisi membutuhkan suatu penunjang diagnostic
untuk menegakkan diagnosa dengan cepat sehingga
menurunkan angka kesakitan dan kematian pada
pasien.
Salah satu pemeriksaan ataupun penunjang untuk
mendiagnosanya yaitu dengan pemeriksaan radiologi
konvensional
Selain sebagai penunjang diagnosis, pemeriksaan
tersebut bisa pula digunakan untuk evaluasi dilakukan
tindakan live saving

the visceral pleural line of a pneumothorax, this white line is convex away from the chest wall and
does not parallel the curve of the chest wall. This is the classical appearance of a bulla in a
patient
with emphysema. It is important to differentiate between a pneumothorax and a bulla, because
inadvertently placing a chest tube into a bulla will almost always produce a pneumothorax, which
may be difficult to reexpand. The walls of several bullae are visible in this patient (dotted white
arrows). On rare occasions, the bullae can grow large enough to render the hemithorax to be
seemingly devoid of visible lung tissue (vanishing lung syndrome).

Pneumothorax has also been classified based on the presence or absence of a shift of the
mobile mediastinal structures, such as the heart and trachea.
Simplethere is usually no shift of the mediastinal structures (Fig. 10-9).
Tensionthere is frequently a shift of the mediastinal structures away from the side of
the pneumothorax associated with cardiopulmonary compromise (Fig. 10-10).
Progressive loss of air into the pleural space through a one-way, check-valve mechanism may
cause a shift of the heart and mediastinal structures away from the side of the pneumothorax.
The air may be entering through a rent in
the parietal pleura or visceral pleura or from the tracheobronchial tree.
The continuously increasing intrathoracic pressure may lead to cardiopulmonary
compromise by impairing venous return to the heart.
Besides a shift of the mobile mediastinal structures away from the side of the pneumothorax,
there may be inversion of the hemidiaphragm (especially on the left side), and there may be
flattening of the heart contour on the side under tension.
Tension pneumothorax is associated with a shift of the mediastinal structures away from the
side of the pneumothorax; simple pneumothorax is not associated with any shift. There is never a
shift of the heart or mediastinal structures toward the side of a pneumothorax.
The question, How large is the pneumothorax? is common and oft-repeated, but the actual
issue is, Does this
patient require a chest tube to drain the pneumothorax? Box 10-2 summarizes the answer.

As little as 50 mL of pleural gas can be seen on chest film. A lateral chest film with a
1-cm intrapleural space corresponds to a 10% pneumothorax. The size of the
pneumothorax is accounted for by the collapsed lung and, to a lesser degree, the
expanding chest cage. The value of expiratory chest radiographs in
detecting pneumothoraces has been overstated. In
a study of 85 patients who had pneumothoraces and 93 controls, inspiratory and
expiratory upright chest radiographs had equal sensitivity for pneumothorax detection
[9]. Because expiratory films provide no added benefit, only inspiratory films
are recommended as the initial radiograph of choice for pneumothorax.
In the supine patient, approximately 500 mL of pleural air is needed for definitive
diagnosis of pneumothorax [10]. The pleural gas accumulates in the subpulmonic
location and outlines the anterior pleural reflection, the costophrenic sulcus, and
the anterolateral border of the mediastinum. The overall transradiancy of the entire
affected hemithorax can be increased on the side of a pneumothorax in the
recumbent patient. Small pneumothoraces can be visualized more easily in the lateral
decubitus view. In this position, as little as 5 mL of pleural gas is visible on the
nondependent side
The diagnostic tools are different in the acute and latent phase. In
the acute phase surgical procedures are often necessary and if the
patient has severe injuries bedside examinations, such as chest Xray and ultrasound, should be relied upon. In the latent phase
barium studies, spiral CT and MRI can give additional diagnostic
information. In the acute phase the chest X-ray is normal in about
one-quarter of cases. In some cases gas and fluid shadows
are seen in the thorax. Sometimes there is only a localised
density in close relationship to the diaphragm (or an alteration in the
diaphragmatic shape. The position of a nasogastric tube can help to
localise the gastric fundus, but does not tell anything about the
position of the diaphragm, which is essential in the diagnosis
of a diaphragmatic tear. A follow-up X-ray of an acutely injured
patient showing progressive opacification of one thorax side by a
gas-filled structure is strongly suggestive for diaphragmatic rupture
Barium studies can be very helpful in making the correct diagnosis, when an extrinsic
narrowing occurs on the border of the stomach or bowel at the point where
they pass the diaphragmatic tear. However, since barium studies cannot be used in
emergency situations, they are predominantly indicated in the latent phase and
eventually in the obstructive phase. Pneumoperitoneum can be established by
bringing a small amount of air into the abdominal cavity. If air shifts through the
diaphragmatic tear and a pneumothorax occurs, the test is diagnostic for
diaphragmatic rupture. However, no shift of air will occur when the tear is closed
by adhesions or by the herniated organs themselves. In this case the exact position
of the diaphragm can be visualised since it is delineated by the subdiaphragmatic air.
Ultrasound can be diagnostic if both the diaphragm and the herniated organs can be
visualised. Examination of the right hemidiaphragm is facilitated by the presence
of the liver, acting as an acoustic window. However, this technique is limited by the
often minimal visualization of the diaphragm itself, the tenderness over the upper
abdomen and the presence of gas in herniated bowel.
The MDCT diagnosis of diaphragmatic rupture is largely based on the fact that
abdominal organs are seen in the pleural space outside the diaphragm. However, the
identification of the diaphragm on standard CT images can be very difficult;
multiplanar CT reconstructions can help to show the defect directly. The more usual
CT signs of diaphragmatic rupture include3943 discontinuity of the diaphragm with
direct visualisation of the diaphragmatic injury (absent diaphragm sign) possibly with
inward curling of the torn hemidiaphragm (dangling diaphragm sign); herniation of
abdominal organs with liver, bowel or stomach in contact with the posterior ribs
(dependent viscera sign); thickening of the crus (thick crus sign); constriction of the
stomach or bowel (collar sign); mushroom-like mass in the right hemithorax
where the herniated liver is constricted by the tear (hump sign) (Fig. 10-29C), linear
lucency across the liver along the torn edges of the hemidiaphragm (band sign) (Fig.
10-29B); active arterial extravasation of contrast material near the diaphragm; and in
case of a penetrating diaphragmatic injury depiction of a missile or puncturing
instrument trajectory.
