Patient safety (keselamatan pasien) rumah sakit adalah suatu sistem dimana rumah sakit membuat asuhan pasien lebih aman.
Hal ini termasuk : assesment resiko, identifikasi dan pengelolaan hal yang berhubungan dengan risiko pasien, pelaporan dan
analisis insiden, kemampuan belajar dari insident dan tindak lanjutnya serta implementasi solusi untuk meminimalkan timbulnya
resiko. Sistem ini mencegah terjadinya cedera yang di sebabkan oleh kesalahan akibat melaksanakan suatu tindakan atau tidak
mengambil tindakan yang seharusnya dilakukan (DepKes RI, 2006).

Pasal 43 UU Kesehatan No. 36 tahun 2009 yang dimaksud dengan keselamatan pasien (patient safety) adalah proses dalam suatu
rumah sakit yang memberikan pelayanan kepada pasien secara aman termasuk didalamnya pengkajian mengenai resiko,
identifikasi, manajemen resiko terhadap pasien, pelaporan dan analisis insiden, kemampuan untuk belajar dan menindaklanjuti
insiden, dan menerapkan solusi untuk mengurangi serta meminimalisir timbulnya risiko. Yang dimaksud dengan insiden
keselamatan pasien adalah keselamatan medis (medical errors), kejadian yang tidak diharapkan (adverse event), dan nyaris
terjadi (near miss).

TAPS (Threats to Australian Patient Safety) dan penelitian lainnya telah mengidentifikasi dua jenis insiden keselamatan pasien
yang luas:
1. Insiden terkait dengan proses perawatan, termasuk proses administrasi, investigasi, perawatan, komunikasi dan pembayaran.
Ini adalah jenis kejadian umum yang dilaporkan (berkisar antara 70% -90% tergantung pada penelitian).
2. Insiden terkait dengan pengetahuan atau keterampilan praktisi, termasuk diagnosis yang tidak terjawab atau tertunda,

Adapun istilah insiden keselamatan pasien yang telah dikenal secara luas berikut definisinya yaitu:
1. Insiden Keselamatan Pasien (IKP) / Patient Safety Incident adalah setiap kejadian atau situasi yang dapat mengakibatkan atau
berpotensi mengakibatkan harm (penyakit, cedera, cacat, kematian dan lain-lain) yang tidak seharusnya terjadi.
2. Kejadian Tidak Diharapkan (KTD) / Adverse Event adalah suatu kejadian yang mengakibatkan cedera yang tidak diharapkan pada
pasien karena suatu tindakan (“commission”) atau karena tidak bertindak (“omission”), bukan karena “underlying disease” atau
kondisi pasien.
3. Kejadian Nyaris Cedera (KNC) / Near Miss adalah suatu insiden yang belum sampai terpapar ke pasien sehingga tidak
menyebabkan cedera pada pasien. Kejadian Tidak Cedera (KTC) adalah insiden yang sudah terpapar ke pasien, tetapi tidak
menimbulkan cedera, dapat terjadi karena “keberuntungan” (misal: pasien terima suatu obat kontra indikasi tetapi tidak timbul
reaksi obat), "pencegahan" (misal : suatu obat dengan overdosis lethal akan diberikan, tetapi staf lain mengetahui dan
membatalkannya sebelum obat tersebut diberikan), “peringanan” (suatu obat dengan reaksi alergi diberikan , diketahui secara dini
lalu diberikan antidotumnya).
Tujuan Patient Safety

 Terciptanya budaya keselamatan pasien di Rumah Sakit.

 Meningkatnya akuntabilitas Rumah Sakit terhadap pasien dan masyarakat.
 Menurunnya KTD di Rumah Sakit.
 Terlaksananya program-program pencegahan sehingga tidak terjadi penanggulangan KTDdiberikan), “peringanan”
(suatu obat dengan reaksi alergi diberikan , diketahui secara dini lalu diberikan antidotumnya).

• Budaya safety meningkat dan berkembang
• Komunikasi dengan pasien berkembang
• Kejadian tidak diharapkan (KTD) menurun
• Risiko klinis menurun
• Keluhan berkurang
• Mutu pelayan Rumah Sakit meningkat
• Citra Rumah Sakit dan kepercayaan masyarakat meningkat, diikuti dengan kepercayaan diri yang meningkat
 Komponen dan Sasaran Patient Safety

1. Ketepatan identifikasi pasien -> Hal ini untuk mengembangkan pola pendekatan agar bisa meningkatkan atau memperbaiki ketelitian
dalam identifikasi pasien. Aplikasinya seperti identifikasi sebelum pemberian atau pengambilan darah, konsumsi obat dan tindakan
lainnya. Salah satu pendukung poin ini adalah penggunaan gelang identitas pasien.
2. Peningkatan komunikasi efektif -> Cara ini untuk mengembangkan pola pendekatan agar komunikasi bisa berjalan dengan efektif. Hal ini
bertujuan agar komunikasi lisan terjadi dengan akurat, sehingga informasinya bisa diterapkan secara konsisten.
3. Peningkatan keamanan obat atau high alert yang harus diwaspadai -> Cara ini dilakukan agar memastikan obat tetap aman untuk
diberikan kepada pasien. Prosedur ini berkaitan dengan proses identifikasi, pemberian label, penetapan lokasi dan penyimpanannya.
4. Kepastian terhadap lokasi, prosedur dan pasien operasi -> Cara ini diaplikasikan agar pasien tercatat dengan valid sebelum mendapatkan
tindakan operasi.
5. Pengurangan terhadap risiko infeksi setelah menggunakan pelayanan kesehatan -> Hal ini adalah prosedur dalam pencegahan penyakit
menular dan infeksi sesuai dengan pedomannya.
6. Pengurangan risiko jatuh -> Setiap tenaga medis harus memahami dan mengaplikasikan sejumlah langkah untuk memastikan pasien tidak
mengalami risiko jatuh. Semua langkah akan diawasi untuk memastikan keberhasilannya. Dengan begitu segala risiko tersebut tidak akan
menimpa pasien yang tengah dirawatnya.
