A. Epidemiologi DM di Indonesia
Di Indonesia, tidak begitu banyak data yang tersedia terkait prevalensi atau insidens DM.
Data-data yang ada justru berasal dari riset global atau riset regional (Wild, Roglic, Green,
Sicree, & King, 2004) (Craig, Jones, Silink, & Ping, 2007)(Ramachandran et al., 2010)(Wild et
al., 2004). Di Indonesia populasi DM tahun 2010 dilaporkan sebesar 7 juta orang dan akan
menjadi 12 juta orang di tahun 2030 dengan kata lain 6 % dari populasi DM di dunia ada di
Indonesia (Shaw et al., 2010). Prediksi ini menjadikan Indonesia sebagai salah satu dari 10
negara dengan populasi penderita DM terbesar di dunia (Ramachandran et al., 2010). Angka
prediksi prevalensi ini berpotensi jauh lebih besar, mengingat prevalensi DM yang tidak
terdiagnosa cukup tinggi sebesar 4.1 % (Pramono et al., 2010),(Mihardja, Delima, Siswoyo,
Ghani, & Soegondo, 2009).
Sebagai salah satu negara dengan penderita DM sepuluh besar di dunia, prevalensi luka
kaki diabetes (LKD) di Indonesia juga cukup tinggi. Besarnya persoalan DM dan LKD di
Indonesia berbanding terbalik dengan jumlah penelitian yang ada (Yusuf, 2015). Penelitian
kami melaporkan prevalensi LKD di unit perawatan home care 26% (Yusuf, Okuwa, et al.,
2016) dan rumah sakit regional sebesar 12% dengan prevalensi faktor resiko 55.4% (Yusuf,
Kasim, Okuwa, & Sugama, 2013). Sehingga modalitas pencegahan merupakan kunci penting
dalam manajemen LKD.
Ada banyak faktor resiko LKD namun dari sekian banyak faktor resiko tersebut,
setidaknya ada dua “pintu gerbang” terjadinya LKD, yaitu kerusakan saraf perifer
(neuropati) dan peripheral arterial desease (PAD) atau angiopati (International, 2013)(Noor,
Zubair, & Ahmad, 2015). Oleh karena itu langkah awal dalam konteks pencegahan adalah
deteksi dini adanya neuropati dan angiopati.
D. Pemeriksaan Neuropati

Ada banyak rekomendasi terkait titik pemeriksaan. Meskipun demikian tidak ada
perbedaan signifikan antara pemeriksaan 3-4 titik dengan 8-10 titik (Baraz, Zarea,
Shahbazian, & Latifi, 2014), dimana pemeriksaan pada 4 titik sudah bisa mendeteksi 90%
abnormalitas (Smieja et al., 1999).
E. Pemeriksaan Angiopati
Pemeriksaan kaki diabetik yang tidak kalah penting adalah Ankle Brachial Index (ABI)
atau ankle brachial blood pressure index (ABPI) atau ankle arm ratio, atau Winsor index.
Dari sekian banyak terminology American Heart Association (AHA) merekomendasikan
penggunaan ABI (Aboyans et al., 2012).
ABI diperkenalkan diakhir tahun 1960an dan merupakan pemeriksaan sederhana untuk
menentukan adanya PAD, dimana ABI merupakan rasio tekanan systolic antara ekstrimitas
bawah (ankle) dan ekstrimitas atas (lengan) (Potier, Abi Khalil, Mohammedi, & Roussel,
2011) (Aboyans et al., 2012).
American Diabetes Association merekomendasikan pemeriksaan ABI pada pasien
diabetes berumur 50 tahun lebih atau dengan gejala PAD atau resiko gangguan
cardiovascular lainnya (American Diabetes Association, 2003).
Tekhnik Pemeriksaan
1. Pasien
Nilai ABI lebih tinggi 0.35 saat duduk dibanding supinasi (karena ankle lebih rendah
dalam posisi duduk) oleh karena itu, pasien sebaiknya dalam posisi supinasi dimana kepala
atau kaki tidak dalam keadaan tergantung (sempurna diatas tempat tidur) (Aboyans et al.,
2012). Beberapa penelitian merekomendasikan pasien istirahat 5-10 menit sebelum
pemeriksaan dan tidak merokok sebelum pemeriksaan (Aboyans et al., 2012)(Potier et al.,
2011).
2. Manset.
Tekhnik pemasangan manset (spiral atau parelel) juga mempengaruhi nilai tekanan
darah sistolik brachial, dimana tekhnik parallel lebih direkomendasikan (Aboyans et al.,
2012). Meski pemeriksaan ABI adalah tindakan non invasive, aman dan relatif bisa
ditoleransi pasien, segera hentikan inflasi (pemompaan) manakala pasien mengeluh nyeri.
Dua hal yang perlu diperhatikan adalah jangan memasang manset langsung diatas luka
terbuka atau diatas bypass graft karena potensi resiko graft trombosis (Aboyans et al.,
2012).
Pneumatic manset ditempatkan melingkari ankle dan mengukur tekanan arteri dorsalis
pedis dan posterior tibialis dengan menggunakan Doppler probe (5-10 MHz), tekhnik yang
sama juga pada kedua lengan untuk mengukur tekanan arteri brachialis (Potier et al., 2011).
3. Interpretasi
ABPI < 0.9 atau > 1.3 mengindikasikan adanya PAD pada pasien diabetes mellitus (Faglia
et al., 2005)(Potier et al., 2011). Saat ini nilai ABI ≤ 0.90 sering dianggap sebagai cut off

point diagnosis PAD (Aboyans et al., 2012). Kategori PAD berdasarkan hasil ABI (Potier et al.,
2011):
Hasil Interpretasi
0.91-1.30 normal.
0.70-0.90 mild occlusion
0.40-0.69 moderate occlusion.
< 0.40 severe occlusion.
> 1.30 Poorly compressible vessels.
Pembacaan nilai ABI berdasarkan tekanan tertinggi diantara kedua kaki dibagi dengan
tekanan tertinggi pada arteri brachialis (Potier et al., 2011).
F. Pemeriksaan Klinis
1. Ipswich test.
Penelitian sebelumnya melaporkan Ipswich test memiliki sensitifitas 78.3% dan 81.2%
serta spesifitas 93.9% dan 96.4% di home care dan di klinik bila dibandingkan monofilement
test. Bahkan pemahaman pasien meningkat 20.4% terkait masalah kaki diabetes (Sharma,
Kerry, Atkins, & Rayman, 2014). Penelitian lainnya melaporkan reliability Ipswich test
dibandingkan monofilament test (k=0.88, P,0.0001) dengan interrater agreement (k=0.68)
(Rayman et al., 2011).
Pemeriksaan Ipswich test dilakukan dengan menyentuhkan (tanpa tekanan) ujung jari
pemeriksa pada ujung jari I, III dan V pasien.
Jumlah titik yang absen ≥ 2 titik yang absen dianggap adekuat untuk menyimpulkan
adanya neuropati, dengan sensitifitas 76% dan spesifisitas 90% (Rayman et al., 2011).

Abnormalitas nadi dorsalis pedis dan atau posterior tibialis memiliki sensitifitas 0.63-
.095 dan spesifitisitas 0.73-0.95 (LMcGee & Boyko, 2013). Akurasi palpasi ditentukan oleh
pengalaman pemeriksa sehingga perlu dikombinasikan dengan pemeriksaan obyektif
(Brearley & Frcs, 1992). Meskipun demikian, palpasi dorsalis pedis dan posterior tibialis bisa
dijadikan pemeriksaan subyektif manakala ketiadaan dukungan alat pemeriksaan obyektif.
H. Kesimpulan
Ada dua penyebab utama LKD; neuropati dan angiopati yang masing-masing bisa
dideteksi dengan pemeriksaan monofilament test dan ABI. Sayangnya kedua alat ini belum
tersedia dan merata di Indonesia. Oleh karena itu perawat hendaknya bisa terampil dalam
melakukan pemeriksaan Ipswich test dan palpasi nadi dorsalis dan posterior tibialis
sebagai pemeriksaan alternatif neuropati dan angiopati.
