Dry eye syndrome telah terbukti berdampak negatif terhadap kualitas hidup
seseorang meskipun mata kering biasanya dianggap sebagai keluhan ringan,
sedang sampai parah. Beberapa faktor risiko dry eye syndrome antara lain
bertambahnya umur, pemakaian lensa kontak, beberapa bulan setelah operasi
refraktif, iklim dan lingkungan, dan obat sistemik tertentu seperti beberapa
antihistamin dan antidepresan. Selective Serotonin Reuptake Inhibitors (SSRI)
adalah salah satu golongan obat antidepresan yang digunakan sebagai pengobatan
lini pertama pada pasien depresi. SSRI merupakan obat antidepresan yang paling
banyak digunakan di Indonesia karena memiliki efek samping yang lebih bisa
ditoleransi dan profil keamanan yang relatif lebih baik dari golongan antidepresan
lainnya. Beberapa penelitian melaporkan bahwa obat golongan SSRI memiliki
hubungan yang signifikan dengan kejadian sindroma mata kering. Karya tulis ini
bertujuan untuk mengkaji efek obat golongan selective serotonin reuptake
inhibitors (SSRI) terhadap patofisiologi dry eye syndrome. Terdapat dua efek obat
golongan SSRI terhadap patofisiologi dry eye syndrome, antara lain efek antikolinergik
dan efek serotonergik. Obat golongan SSRI berfungsi sebagai antagonis dari reseptor
muskarinik kolinergik pada saraf parasimpatis yang menginervasi kelenjar lakrimal
sehingga terjadi hiposekresi air mata. Selain itu, serotonin menginduksi respon inflamasi
dan apoptosis sel dalam sel epitel kornea dengan mengaktifkan pensinyalan NF-κB
sehingga mengakibatkan ketidakstabilan air mata dan menyebabkan hiposekresi air mata.

Dry eye syndrome has been proven to have a negative impact on a person's
quality of life even though dry eye is usually considered a mild, moderate to
severe symtomps. Some risk factors for dry eye syndrome include age, the use of
contact lenses, several months after refractive surgery, climate and environment,
and certain systemic drugs such as antihistamines and antidepressants. Selective
Serotonin Reuptake Inhibitors (SSRIs) are a class of antidepressant drugs that are
used as first-line treatment in depressed patients. SSRI is the most common used
antidepressant drug in Indonesia because the side effects that are more tolerable
and a relatively safe than other antidepressant groups. Some studies report that
SSRI drugs have a significant correlation with the incidence of dry eye syndrome.
This purpose of this study is to examine the effects of Selective Serotonin Reuptake
Inhibitors (SSRIs) as risk factors for Dry Eye Syndrome. There are two SSRI drug
effects on the pathophysiology of dry eye syndrome, including anticholinergic
effects and serotonergic effects. SSRI drugs function as antagonists of cholinergic
muscarinic receptors on the parasympathetic nerves that innervate the lacrimal
gland resulting in tear hyposecretion. In addition, serotonin induces inflammatory
responses and cell apoptosis in corneal epithelial cells by activating NF-κB
signaling resulting in tear film instability and causes tear hyposecretion.
