DEFINISI / DEFINITION
Yang dimaksud dengan Pemeriksaan Kesehatan di sini adalah pemeriksaan kesehatan karyawan
atau calon karyawan dimana parameter pemeriksaannya disesuaikan dengan jenis pekerjaan dan
pajanan bahaya kesehatan di area kerja karyawan atau calon karyawan tersebut. / Medical Check
Up is medical examination for employees or prospective employees based on parameters tailored to
the types of work and exposure to health hazards in the work areas of the employees or prospective
employees.
1.0 KONTEKS / CONTEXT
KPC beroperasi dalam lingkungan yang dinamis. Untuk meminimalisir risiko kesehatan secara
efektif, dibutuhkan pemantauan kondisi kesehatan karyawan yang diyakini memiliki potensi untuk
berubah akibat aktivitas pekerjaan sehari-hari. / KPC operates in a dynamic environment. To
minimize health risks effectively, it requires monitoring of employees' health conditions which are
believed to have the potential to change due to daily work activities.
2.0 TUJUAN / PURPOSE

1. Memberikan panduan proses pemeriksaan kesehatan bagi calon karyawan dan pemeriksaan
kesehatan rutin bagi karyawan; baik karyawan KPC maupun karyawan kontraktor.
To provide guidelines for pre-employment and regular medical check-up for both KPC and
contractor employees.
2. Untuk memantau kondisi kesehatan karyawan sebelum, selama (setiap tahun), dan sesudah
beraktivitas di tempat kerja. Hasil pemantauan digunakan sebagai bahan pertimbangan untuk
menentukan apakah karyawan mampu beraktivitas di lingkungan kerja yang akan dihadapinya
serta memantau dampak dari aktivitas pekerjaan terhadap derajat kesehatan karyawan.
To monitor the health conditions of employees before, during (every year), and after they
perform activities in the work place. Monitoring results are used when considering whether the
employees are capable of performing activities in the designated work environment as well as
monitoring impacts of work activities on their health status.
3.0 RUANG LINGKUP / SCOPE

Ruang lingkup SOP ini mencakup pengaturan proses pemeriksaan kesehatan bagi calon karyawan,
pemeriksaan kesehatan rutin karyawan dan sebelum berakhirnya hubungan kerja dengan
perusahaan (PHK), baik karyawan KPC, kontraktor, konsultan dan peserta magang.
The scope of this Standard Operating Procedure of Medical Check-up Process applied for both KPC
and Contractor Employees, which includes arrangement of: pre-employment, periodic, and prior
termination of employment.

4.0 Ketentuan Umum / General Requirements
Setiap orang harus diperiksa kesehatannya sebelum dipekerjakan dan setelah itu setiap tahun.
Dokter yang memeriksa dapat merekomendasikan untuk menambah frekuensi pemeriksaan
periodik bagi seorang karyawan jika terindikasi secara klinis (karyawan dengan masalah kesehatan
kronis tetapi tidak menyebabkan pembatalan status medisnya) atau jika ada perubahan yang
signifikan dalam paparan bahaya di tempat kerja. / Every person should be examined prior to
employment and thereafter every year. The assessing physician may recommend adding frequency
of periodic assessments for an employee if clinical indication occurs (worker with chronic diseases
but not lead to medical status annulment) or any significant changes in exposing hazard at work.
Karyawan yang telah berulang kali absen (tidak masuk kerja) singkat atau sekali absen jangka
panjang karena sakit atau cedera, harus ditinjau ulang kondisi kesehatannya sebelum kembali
bekerja. / Employees who have had repeated short term absences or had a long term absence due
to illness or injury should be medically reviewed prior to work.
Pemeriksaan kesehatan bagi calon karyawan dan pemeriksaan kesehatan rutin karyawan baik
karyawan KPC maupun kontraktornya hanya boleh dilakukan di: / Pre-employment and regular
medical check up for both KPC and contractor employees may only be performed:
- Di klinik perusahaan (KPC) yang saat ini dikelola oleh International SOS. / At company clinic
(KPC) which is currently operated by International SOS.
- Di beberapa klinik dan rumah sakit di Sangatta berikut ini: RSUD Kudungga Sangatta, Rumah
Sakit SOHC/Medika Sangatta, Rumah Sakit Prima Sangatta, RSIA Cahaya, Rumah Sakit Meloy
dan Klinik Pelita Kasih. / At the following clinic and hospital di Sangatta: RSUD Kudungga
Sangatta (Government Regional Hospital), SOHC Hospital, Sangatta Prima Hospital, Cahaya
Hospital, Meloy Hospital and Pelita Kasih Clinic.
- Untuk di kota-kota lain minimal dilakukan di rumah sakit–rumah sakit ternama, terutama yang
menjadi rujukan klinik KPC, atau minimal Rumah Sakit Umum Daerah (RSUD) atau Rumah
Sakit Umum Pusat (RSUP). Contoh beberapa rumah sakit yang saat ini menjadi rujukan klinik
KPC antara lain: RS PKT Bontang, RSUD Bontang, RSUD Wahab Syahrani Samarinda, RS
Pertamina Balikpapan, RS Restu Ibu Balikpapan dan Prodia OHI (Occupational Health Institute).
/ For other cities, pre-employment and regular medical check-up for both KPC and contractor
employees may be performed at well-known / credible hospitals, especially those used for
referral by KPC clinic, or as minimum Regional Government Hospital (RSUD) or Government
Central Hospital (RSUP). Some examples of hospital used as referral by KPC clinics are PKT
Hospital Bontang, RSUD Bontang, RSUD Wahab Syahrani Samarinda, Pertamina Hospital
Balikpapan, and Restu Ibu Hospital Balikpapan and Prodia OHI (Occupational Health Institute).
- Pemeriksaan kesehatan bagi pekerja bawah air (penyelam) dan penerbangan, dilakukan di
fasilitas kesehatan khusus yang telah ditentukan oleh pemerintah. / Health examination of
commercial diver and air aviation worker should be done in specific medical facilities determined
by government.
- Semua provider MCU tersebut telah memenuhi persyaratan perundangan yang berlaku dan
dapat diaudit oleh departemen OHS KPC dan CMO. / All medical providers which conduct
health examination already complied with related government regulation and audible by KPC
OHS department and CMO.

Pemeriksaan di klinik atau rumah sakit di luar ketentuan tersebut hanya boleh dilakukan setelah
mendapat persetujuan dari Manager OHS dan Chief Medical Officer (CMO). / Medical check-up at
any other clinic or hospital may only be performed with the approval from Manager OHS and Chief
Medical Officer (CMO).
Ketentuan teknis pelaksanaan pemeriksaan kesehatan dan parameter kesehatan yang harus
diperiksa harus merujuk pada Matriks Rangkuman Pemeriksaan Kesehatan KPC. Penentuan
seseorang dinyatakan FIT atau TIDAK FIT sesuai pekerjaannya dilakukan oleh dokter yang ditunjuk
oleh perusahaan (KPC) setelah melakukan evaluasi terhadap dokumen hasil pemeriksaan
kesehatan. / For implementation of technical medical examination and parameters for medical
assessment should refer to the KPC Matrix Medical Check-Up (MCU) Summary. Decision to be
made by company (KPC) appointed doctor whether a person is FIT or UNFIT for work after
evaluation on the medical assessment result document.
5.0 Pertanggungjawaban / Accountability
Pertanggungjawaban untuk peninjauan dan pemutakhiran SOP ini ada di tangan Manager
Occupational Health and Safety (OHS). / The accountability for reviewing and updating of this
Standard Operating Procedure resides with Manager Occupational Health and Safety.
6.0 Prosedur / Procedures Penanggungjawab
Langkah 1 / Step 1
Penetapan rumah sakit dan klinik yang boleh digunakan. / Determination Manager OHS
of hospitals and clinics that can be used.
Penentuan rumah sakit dan klinik yang boleh dijadikan tempat
pelaksanaan pemeriksaan kesehatan calon karyawan maupun
pemeriksaan kesehatan rutin karyawan dilakukan oleh Manajer OHS
setelah berkonsultasi dengan Chief Medical Officer (CMO) dan
Manajemen KPC. / Determination of hospitals and clinics that may be
used as the venue for pre-employment medical check-up for candidates
and regular medical check-up for employees shall be conducted by
Manager OHS after consulting with the Chief Medical Officer (CMO) and
KPC Management.

Penjadwalan Pemeriksaan Kesehatan / Scheduling of medical check up
Karyawan atau Calon Karyawan KPC / KPC Employees or

Candidates:
Karyawan atau calon karyawan datang ke klinik perusahaan, sesuai

dengan jadwal yang telah ditentukan / Employees or candidates must
visit company clinic as per their schedule.
Jika Pemeriksaan Kesehatan sudah dilakukan lebih dari tiga (3)
bulan, maka calon karyawan perlu menjalani pemeriksaan
kesehatan ulang sebelum bekerja (Permenakertrans 02/1980 pasal
2.7) / If Medical Check Up has been done more than 3 (three)
months, candidate should undergo re-medical check up.
 Untuk pemeriksaan calon karyawan KPC / For KPC employee Recruitment HRD KPC
candidates MCU
Jadwal pemeriksaan diajukan oleh Tim Rekrutmen.
Hasil pemeriksaan menjadi milik Tim Rekrutmen dan keputusan FIT
atau UNFIT secara medis ditetapkan oleh Dokter yang ditunjuk oleh
perusahaan (KPC). Kandidat dengan hasil MCU UNFIT “tetapi masih
memungkinkan untuk sembuh setelah diberikan pengobatan” harus
didiskusikan dengan Tim Rekrutmen mengenai kemungkinan
pemeriksaan ulang ataupun pengobatan”, keputusan terkait
kelanjutan proses perekrutan diserahkan sepenuhnya kepada Tim
Rekrutmen. / Schedule to be proposed by Recruitment Team. The
results of medical check-up belong to Recruitment and decision
whether the candidate is medically FIT or UNFIT shall be made by
appointed company doctor (KPC). Candidate with UNFIT results, but
there is still possibility for recovery after medical treatment, need to
be discussed with Recruitment Team, the decision regarding the
further selection stage are made by Recruitment Team.
 Untuk pemeriksaan berkala bagi karyawan KPC / For KPC Admin Dept.
employees regular MCU
Petugas administrasi yang ditunjuk di departemen/divisi
mendaftarkan karyawan untuk MCU kepada petugas adminsitrasi di
klinik 4 – 6 minggu sebelum masa berlaku MCU sebelumnya
berakhir. Jika tidak ada petugas administrasi yang ditunjuk,
karyawan boleh mendaftar MCU langsung ke petugas administrasi
klinik perusahaan. / Designated Admin Officer within the department
or division shall submit nomination for MCU to the clinic Admin
Officer 4-6 weeks prior to the expiry date of the current ID/KIMPER. If
there is no designated admin office, employee/staff may contact the
clinic admin officer directly for MCU registration.

Pemeriksaan kesehatan dilakukan sebelum ID/KIMPER kadaluarsa
(expired). Namun, apabila jadwal yang diberikan pihak klinik
perusahaan telah melewati masa berlaku ID/KIMPER, maka
karyawan yang bersangkutan harus menghubungi petugas
ID/KIMPER di Departemen OHS Tango Delta untuk mendapatkan
perpanjangan sementara ID/KIMPER-nya. / MCU shall be conducted
prior to the ID/KIMPER expiry date. However, if schedule provided by
the company clinic is after the ID/KIMPER expiry date, then the
employee shall contact ID/KIMPER Officer at OHS Department,
Tango Delta, to get temporary ID/KIMPER extension.
 Pemeriksaan Kesehatan Terminasi diberikan kepada karyawan yang

dalam periode lebih dari 6 bulan setelah MCU terakhirnya akan
meninggalkan pekerjaannya atau pensiun. Bagi karyawan yang
akan meninggalkan pekerjaannya atau pensiun dalam periode 6
bulan setelah MCU terakhirnya, tidak diwajibkan untuk
menjalani MCU terminasi, dalam hal ini MCU terakhir dianggap
sebagai MCU terminasi. / MCU termination is given to employees
who in a period of more than 6 months after the last MCU will leave
his job or retire. For those employees who will be leaving job or retire
within a period of 6 months after previous MCU, are not required to
undergo a MCU termination MCU, in this case the latter is regarded
as MCU termination.
MCU terminasi sebagai pegangan untuk penentuan adanya PAK selama

bekerja dan sebagai data pendukung untuk menghadapi klaim PAK di
kemudian hari / MCU termination as a tool to detect if there’s
Occupational Illness or not during their service with the company and as
a supporting data if there’s claims in the future.

Karyawan atau Calon Karyawan Kontraktor / Contractor Employees Manager Kontraktor

or Candidates:
Karyawan atau calon karyawan datang ke klinik atau rumah sakit yang
ditunjuk, sesuai dengan jadwal yang telah ditentukan / Employees or
candidates must visit appointed clinic or hospital as per their schedule.
 Untuk pemeriksaan calon karyawan kontraktor / For contractor

employee candidates MCU
Jadwal pemeriksaan dibuat oleh Tim Recruitment Kontraktor.
Hasil pemeriksaan menjadi milik Recruitment Kontraktor dan
keputusan FIT atau TIDAK FIT secara medis ditetapkan oleh dokter
yang ditunjuk oleh KPC. / Schedule to be proposed by the Contractor
Recruitment Team. Results of medical check-up belong to Contractor
Recruitment and decision whether the candidate is medically FIT or
UNFIT shall be made by KPC appointed doctor.
 Untuk pemeriksaan berkala bagi karyawan Kontraktor / For

Contractor employees regular MCU
Petugas administrasi kontraktor membuat jadwal dan persetujuan
dengan klinik/RS minimal 1 bulan sebelum masa berlaku MCU
berakhir. Pemeriksaan kesehatan harus dilakukan sebelum masa
berlaku ID/KIMPER berakhir. / The contractor admin officer shall
make schedule and agreement with the clinic/hospital minimum 1
month prior to the ID/KIMPER expiry date.
Langkah 3 / Step 3
Pemeriksaan kesehatan di klinik atau rumah sakit / Medical check-
Up at clinic or hospital
Pemeriksaan kesehatan hanya boleh dilakukan oleh dokter yang CMO/Kepala RS atau
memiliki sertifikat hiperkes. / MCU may only be performed by medical Klinik
doctor with Government’s Hyperkes Certificate.
Sebelum dilakukan pemeriksaan kesehatan: / Prior to MCU: Karyawan

 Pastikan karyawan atau calon karyawan berpuasa 8-10 jam sebelum
melakukan pemeriksaan kesehatan. / Ensure the employee /
candidate is fasting 8-10 hours prior to the medical check up.
 Pastikan karyawan atau calon karyawan tidak terpapar bising
selama minimal 14 jam jam sebelum melakukan tes audiometri. /
Ensure employees or candidate has not been exposed to noisy
environment during at least the last 14 hours prior to audiometric
test.

Pemeriksaan terhadap karyawan atau calon karyawan dilakukan sesuai CMO/ Kepala RS atau
dengan kategori sbb / Employees or candidates go through medical Klinik
check up as per the following category:
Group A : Karyawan berisiko rendah / Employees at Low Risk

Group B : Karyawan berisiko tinggi (tergantung SEG (Group Pajanan
Serupa), termasuk Operator HE, Mekanik, Pekerja Lapangan,
Pelaut, Pekerja Ruang Terbatas) / Employees at High Risk
(Depend on SEGs (Similar Exposure Groups), including HE
Operator, Mechanic, Field worker, Seafarer, Confined Space
worker)
Group C : Petugas Katering dan penjamah makanan / Caterer and Food
Handler
Group D : Petugas pestisida dan pembasmi serangga / Pesticide or
Insect Control Officer
Group E : Petugas pengolah limbah cair domestik / Domestic sewage
handler
Group F : Petugas Tanggap Darurat, Pekerja Bawah Air (Penyelam) dan
petugas udara dan darat penerbangan / Emergency
Response, Commercial Divers and Air and Ground Crew of
Aviation worker.
Group G: Resiko rendah dengan masa kerja singkat (<3 bulan) / Low
risk-Short Term Worker (work assignment less than 3 month).
Parameter pemeriksaan kesehatan dari masing-masing kategori dapat

dilihat pada Lampiran Matriks Rangkuman Pemeriksaan Kesehatan
KPC. / Parameters used for medical check-up of each employee group
or category can be found at Attachment “KPC MCU Matrix Summary”.
Khusus untuk konsultasi dan pemeriksaan (VCT) HIV & AIDS berlaku
ketentuan dibawah ini: / Especially for Voluntary Counselling & Testing
(VCT) for HIV&AIDS, the following requirement apply:
a. Tes HIV ditawarkan kepada karyawan/calon karyawan. / HIV Test
is offered to employee/candidate.
b. Pernyataan kesediaan tertulis didokumentasikan. / Inform concern
should be documented.
c. Hasil tes HIV dijamin kerahasiaannya. / HIV Test result must be
treated as confidential.
d. Hasil tes HIV tidak berpengaruh terhadap penilaian kinerja
karyawan atau menjadi penentu penerimaan bagi calon karyawan.
/ HIV Test result will not affect individual performance appraisal or
decision in accepting candidate.
e. Hasil tes HIV disampaikan oleh konselor HIV terlatih. / HIV Test
result must be delivered by trained HIV counsellor.
f. Hasil tes HIV tidak dicantumkan dalam dokumen MCU. / HIV test
result must not be included on MCU document.

Karyawan KPC / KPC Employees
Karyawan menunggu hasil pemeriksaan dan konsultasi dengan dokter
yang berwenang. Employees shall wait for the result of medical check up
and consult with the authorized doctor
Adapun hasil detail dari pemeriksaan MCU adalah sebagai berikut :

The detailed results of the examination MCU are as follows:
1. FIT-1 / FIT-1
Fit boleh bekerja dan tidak perlu memakai kacamata dan ID/KIMPER
dicetak tanpa tanda kacamata. / Fit may work and not have to wear
glasses and ID/KIMPER issued without glasses.
2. FIT-2 / FIT-2
Fit boleh bekerja dan harus memakai kacamata dalam aktivitas
pekerjaan dan ID/KIMPER dapat dicetak dengan tanda kacamata. /
Fit may work and have to wear glasses in work activities and ID/
KIMPER can be issued with sign glasses.
3. FIT-3 / FIT-3
Belum FIT untuk bekerja mengoperasikan kendaraan dan harus
dirujuk ke dokter spesialis mata karena hasil tes mata tidak sesuai
standar MCU-KPC. Karyawan tersebut tidak boleh mengoperasikan
kendaraan. ID sementara dapat dicetak sampai maksimal satu bulan
(tanpa kacamata). / Temporary not fit to operate vehicle and should
be referred to ophthalmologist due to substandard previous result
from MCU-KPC’s standard. Employees are not allowed to operate
vehicle. Temporary ID Only can be issued for maximum up to one
month (without glasses).
4. Review-1 / Review-1
FIT boleh bekerja tetapi dirujuk ke dokter spesialis penyakit tertentu
dan ID/KIMPER dapat dicetak sampai maksimal satu bulan. / FIT
may work but referred to certain specialist and ID/KIMPER can be
issued for maximum up to one month.
5. Review-2 / Review-2
Belum FIT bekerja untuk sementara waktu karena penyakit tertentu,
perlu pengobatan untuk menjadi FIT bekerja kembali. / Temporary
not fit for work due to certain disease, need medication to be FIT.
6. Long Sick Leave / Long Sick Leave
ID/KIMPER tidak dapat diberikan oleh HSES tetapi akan mendapat
ID khusus dari HR Industrial Relations. / ID/KIMPER cannot be
issued by the HSES, certain ID will be issued by HR Industrial
Relations.

7. Hasil evaluasi kesehatan karyawan dalam program Rehabilitasi
Kerja/ MCU result for worker during Work Rehabilitation Program
Khusus bagi karyawan yang menjalani WRP. ID/KIMPER dicetak
menyesuaikan restriksi yang ditetapkan oleh kepala dokter
perusahaan (CMO). / For employees who undergo WRP. ID/KIMPER
issued according to restriction made by CMO.
8. Hasil evaluasi kesehatan karyawan dalam masa Pembebasan Tugas

Sementara terkait tindakan disiplin / MCU result for worker during Off
Duty disciplinary action period
ID/KIMPER tidak dapat diberikan oleh HSES tetapi akan mendapat
ID khusus dari HR Industrial Relations. / ID/KIMPER cannot be
issued by the HSES, certain ID will be issued by HR Industrial
Relations.
Jika hasil pemeriksaan menyatakan BOLEH BEKERJA (FIT-1, FIT-2,

FIT-3 dan Review-1), maka: / If the result is MAY WORK (FIT-1, FIT-2,
FIT-3 dan Review-1), then:
 Petugas klinik International SOS akan memperbaharui status CMO Klinik
medical karyawan ke dalam data base Alesco. / Clinic admin officer
from International SOS will update the employee’s medical status in
Alesco data base.
 Karyawan atau petugas admnistrasi departemen/divisi menghubungi Admin Dept.
Petugas ID/KIMPER untuk menerbitkan ID/KIMPER baru. /
Employee or admin officer from department or division shall contact
ID/KIMPER Officer for issuing new ID/KIMPER.

Jika hasil pemeriksaan BELUM BOLEH BEKERJA (Review-2), maka / If CMO Klinik
the MCU result is NOT FIT TO WORK (Review-2), then:
 Karyawan yang bersangkutan harus melakukan pemeriksaan ulang /
Employee shall undergo re-medical check-up.
 Selama karyawan tersebut belum mendapatkan hasil pemeriksaan
FIT, karyawan harus mengikuti perawatan kesehatan yang
dianjurkan oleh dokter dan hanya mendapatkan ID/KIMPER
perpanjangan sementara. / Unless employee received MCU result
FIT, s/he must follow medical treatment advised by the doctor and
s/he will only receive temporary ID/KIMPER extension.
 Pemeriksaan ulang akan dilakukan selama periode tertentu hingga
hasil pemeriksaan kesehatan karyawan tersebut dinyatakan FIT
untuk bekerja. / Re-medical check-up will be conducted during
certain period until s/he is stated as medically FIT for work.
Jika hasil MCU FIT dengan Batasan, maka ID/KIMPER dicetak

disesuaikan dengan restriksinya. / If MCU result is Medically Fit With
Restriction (MFR), then ID/KIMPER will be printed with the limitation
(restriction).

Kondisi kesehatan yang membutuhkan partisipasi aktif dari karyawan

dalam menjaga derajat kesehatannya: / Health Condition which requires
active participation from employee in maintaining a healthy condition:
 Penyakit tertentu dapat dibawa atau diperburuk oleh perubahan gaya
hidup. karyawan diminta bekerjasama dengan mengikuti nasihat
medis dari dokter (termasuk perawatan medis) terutama terkait
penyakit seperti diabetes, hipertensi dan dislipidemia. Semua
karyawan dengan penyakit tersebut di atas harus membuat
komitmen bahwa mereka bersedia melaksanakan nasihat medis
dalam mengubah gaya hidup, misalnya: berhenti merokok,
menghentikan konsumsi alkohol dan menurunkan berat badan bagi
mereka yang mengalami obesitas (tapi tidak terdiskualifikasi dari
pekerjaan). / Certain diseases can be brought on or exacerbated by
a change in life-style. Cooperation is requested from the employee in
following medical advice from physician (including medical treatment)
especially with diseases such as diabetes, hypertension and
dyslipidaemia. All employees with the above diseases shall make a
commitment that they are willing to follow medical advice for
changing their life style, such as: stop smoking, stop alcohol
consumption and reduce body weight for obese worker (but not
disqualified from work).

mengabaikan nasihat medis sebagai bagian dari komitmen tersebut,
dapat diberikan tindakan disiplin sesuai dengan PKB. / If there is
clear evidence that the employee is failing to follow or neglecting
medical advice, disciplinary action can be taken as per the CEA
(Collective Employment Agreement).
 Karyawan yang memiliki indikator “tingkat kebugaran” buruk, merujuk
kepada parameter kebugaran: indeks masa tubuh, kadar gula darah,
tekanan darah, tingkat lemak darah, dan kebiasaan merokok, wajib
mengikuti program peningkatan kebugaran perusahaan (merujuk
kepada kebijakan perusahaan tentang Program Kebugaran KPC). /
Employees who have poor "fitness" indicator, refers to wellness
parameters such as: body mass index, blood sugar levels, blood
pressure, blood fat levels, and smoking habit, shall follow the
company's wellness program to improve their fitness level (referring
to KPC Wellness Program).
Karyawan Kontraktor / Contractor Employees
Dokumen hasil pemeriksaan kesehatan harus diserahkan ke Bagian Manager Kontraktor

Kesehatan Kerja Departemen OHS KPC untuk dilakukan evaluasi oleh
dokter yang ditunjuk oleh perusahaan (KPC). Jika hasil pemeriksaan
FIT, maka: / MCU document must be submitted to Occupational Health
Section of OHS Department for evaluation by company (KPC) apponted
doctor. If the result is FIT, then:

 Salinan laporan hasil pemeriksaan kesehatan (MCU) yang sudah
dievaluasi dan dinyatakan FIT harus diserahkan ke Bagian
Penerbitan ID/KIMPER atau Bagian Pendaftaran Induction
Departemen OHS untuk proses permbaruan ID/Kimper atau
mengikuti Induction Safety. / Copy of MCU report which has been
evaluated and stated as FIT for work shall be submitted to
ID/KIMPER Issuance Section or Induction Registration Section of
OHS Department for ID/KIMPER extension or Safety Induction
process.
 Karyawan baru boleh mengikuti Induction Safety KPC. / New
employees may follow KPC Safety Induction.
 Petugas ID/KIMPER akan mencetak ID/KIMPER baru. / ID/KIMPER
Officer will issue new ID/KIMPER.
 ID/KIMPER baru diserahkan kepada requestor. / New ID/KIMPER
will be handed over to the requestor.
Jika hasil pemeriksaan selain FIT, maka / If the result is other than FIT,
then:
 Karyawan tersebut harus melakukan pemeriksaan ulang. / The
employee shall go through re-medical check-up.
 Selama karyawan tersebut belum mendapatkan hasil pemeriksaan
FIT, ia harus mengikuti perawatan kesehatan yang dianjurkan oleh
dokter dan ID/KIMPER tidak akan diterbitkan/diperpanjangan. / The
employee must follow medical treatment advised by the doctor until
he is stated as FIT and ID/KIMPER will not be issued / extended.
 Pemeriksaan ulang akan dilakukan selama periode tertentu hingga
hasil pemeriksaan kesehatan karyawan ybs dinyatakan FIT untuk
bekerja. / Re-medical check-up will be conducted during certain
period until s/he is stated as medically FIT for work.
