ABSTRAK
Penelitian ini dilakukan untuk mengetahui karakteristik pasien covid-19
dan profil penggunaan vitamin dan suplemen pada pasien covid-19 rawat inap
selama masa pandemi di Rumah Sakit Bhayangkara Tingkat III Banjarmasin.
Tujuan dari penelitian ini ialah mengetahui profil penggunaan vitamin dan
suplemen pada pasien Covid-19 rawat inap di Rumah Sakit Bhayangkara
Tingkat III Banjarmasin pada Tahun 2020. Sumber data pada penelitian ini
yaitu data sekunder yang diambil dari hasil rekam medik tahun 2020. Hasil dari
penelitian yang telah dilakukan persentase Pasien Laki-Laki lebih banyak
terinfeksi Covid-19 yaitu dengan jumlah 69 orang (71%) dan kelompok Usia
diatas ≥30 tahun yaitu sebanyak 76 orang (78.35%). Sedangkan Pekerjaan yaitu
Polisi dengan jumlah 48 orang (49%). Sedangkan Persentase Penggunaan
Vitamin dan Suplemen pada Pasien Covid-19 Rawat inap di Rumah Sakit
Bhayankara Banjarmasin tahun 2020 yaitu Vitamin D (61%), dan Vitamin C
(39%). Adapun untuk Suplemen yaitu Curcuma (63%), Becom C (26%),
Imboost Force sirup (4%), Hepa – Q (2%), Cibexon sirup ( 1%), Becom zet
(2%), Curvit (2%) dan Curbexon sirup (1%), dalam 97 sampel.

ABSTRACT
This study is conducted to determine the characteristics of covid-19
patients and the profile of the use of vitamins and supplements in hospitalized
covid-19 patients during the pandemic at Bhayangkara Hospital Level III
Banjarmasin. The purpose of this study is determine the profile of the use of
vitamins and supplements in hospitalized Covid-19 patients at Bhayangkara
Hospital Level III Banjarmasin in 2020. The source of data in this study is
secondary data taken from the results of medical records in 2020. The results of
the research conducted were that the percentage of male patients infected with
Covid- 19 was more than 69 people (71%) and the age group above 30 years
was 76 people (78.35%). While the occupation is the police with a total of 48
people (49%). While the percentage of use of vitamins and supplements in
Covid-19 patients hospitalized at Bhayankara Hospital Banjarmasin in 2020 is
Vitamin D (61%), and Vitamin C (39%). As for supplements, namely Curcuma
(63%), Becom C (26%), Imboost Force syrup (4%), Hepa-Q (2%), Cibexon

Abstrak
Coronavirus Disease 2019 (Covid-19) adalah penyakit jenis baru yang disebabkan oleh virus yang
dinamakan Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Jumlah kasus positif
terkonfirmasi di Indonesia hingga 30 April 2021 berjumlah 1.668.368 orang dengan total kematian
45.521. Sedangkan untuk data pasien Covid- 19 di Provinsi Sulawesi Utara khususnya di Kota Bitung
data pasien positif per tanggal 30 April 2021 berjumlah 1.236 orang dan pasien meninggal 41 orang.
Tujuan penelitian ini adalah untuk mengetahui tatalaksana terapi dan tingkat kesesuaian pemberian
terapi pada pasien Covid-19 di Rumah Sakit Umum Bitung, Provinsi Sulawesi Utara. Sumber data yang
digunakan dalam penelitian ini berupa data rekam medik pasien Covid-19 selama 6 bulan dimulai dari
bulan November 2020 sampai April 2021. Pengambilan data dilakukan dengan menggunakan metode
purposive sampling dan teknik random sampling pada setiap data pasien yang memenuhi kriteria
inklusi. Sesuai rekomendasi Persatuan Dokter Paru Indonesia, Rumah Sakit Daerah Umum Bitung
menggunakan antivirus dan vitamin pada terapi pasien Covid- 19, hasil yang diperoleh menunjukkan
kesembuhan 100% dan dari data yang diperoleh dan menunjukan pencapaian presentase yaitu 82,4%
dalam jangka waktu 6 bulan serta bisa dikatakan dalam kategori baik dan sesuai dengan pedoman yang
dikeluarkan oleh Persatuan Dokter Paru Indonesia.
Abstract
Coronavirus Disease 2019 (Covid-19) is a new type of disease caused by a virus called SARS-CoV-2.
The number of confirmed cases in Indonesia as of April 30, 2021 is 1,668,368 people with 45,521 cases
of death. Data on Covid-19 patients in North Sulawesi Province, especially in Bitung City, positive
patient data as of April 30, 2021 totaled 1,236 people and 41 patients died. The purpose of this study
was to determine the therapeutic management and the level of suitability of giving therapy to Covid-19
patients at Bitung Hospital, North Sulawesi Province. The source of data used in this study was in the
form of medical record data for Covid-19 patients for 6 months starting from November 2020 until
April 2021. Data collection was carried out using purposive sampling and random sampling techniques
on each patient data that met the inclusion criteria. In accordance with Indonesian Lung Doctors
Association recommendations, hospitals use antivirals and vitamins in the therapy of Covid-19 patients,
the results obtained show 100% recovery and from the data obtained, the percentage achievement is
82,4% in a period of 6 months and which can be said to be in the good and appropriate category with
the compass published by the Indonesian Lung Doctors Association.

Abstract
Corona virus or COVID 19 is a virus that causes disease in animals or humans. COVID-19 more often
attacks the respiratory tract. Improving the immunity of COVID-19 patients is very much needed
because the treatment for COVID-19 has not been found. Vitamin C is a vitamin that can prevent and
treat respiratory infections by enhancing various immune cell functions. Vitamin C acts as a powerful
antioxidant and helps replace damaged cells so it can help with COVID-19. This literature study aims to
determine the use of vitamin C as an immunomodulator in adult COVID-19 patients. The data collection
method was obtained by reviewing 10 journals using Google Scholar and Pubmed using English-
language journals. The keywords used are vitamin C, COVID-19, treatment. The results obtained in the
form of intravenous vitamin C with a dose of can improve the lives of COVID-19 patients by increasing
the immunity of infected patients. This can be seen from the inhibition of the development of cytokines
and an increase in lymphocytes and CD4+ T cells. In addition, it improves the patient's respiratory
function with an increase in PiO2/FiO2.

ABSTRAK
Latar belakang: Penyakit Coronavirus 2019 (COVID-19) merupakan penyakit yang disebabkan
oleh Coronavirus jenis baru yang kemudian diberi nama Severe Acute Respiratory Syndrome
Coronavirus 2 (SARS-CoV-2). Virus ini terutama menyerang reseptor enzim-2 pengubah angiotensin
(ACE2), terutama menyerang paru-paru dengan gejala utama pernapasan seperti demam, batuk, flu, dan
dyspnea. Penularan terjadi melalui aerosol, droplet dan penularan melalui fecal-oral. Pada kasus yang
berat dapat menyebabkan pneumonia, sindrom pernapasan akut, dan bahkan kematian. Sistem kekebalan
memiliki peran utama dalam melawan berbagai jenis infeksi tetapi untuk membuat kekebalan sel yang
berfungsi dengan baik diperlukan beberapa suplemen seperti vitamin C. Kajian ini ditulis untuk melihat
secara khusus pengaruh dari vitamin C terhadap respon tubuh sebagai proses pencegahan dan
pengobatan COVID-19. Metode: Penelitian ini menggunakan metode studi systematic review yang
menggunakan database berbasis online meliputi PubMed, Science Direct, dan Cochrane antara tahun
2010 sampai 2020. Telaah jurnal dilakukan pada 334 jurnal ilmiah dan didapatkan 10 jurnal yang sesuai
dengan kriteria restriksi dan PICO. Hasil: Hasil telaah 10 jurnal didapatkan bahwa vitamin C mampu
mempercepat proses penyembuhan, memodulasi sitokin inflamasi, dan menurunkan kerusakan organ
pada objek penelitian dan berpotensi meningkatkan kesembuhan terhadap pasien terinfeksi COVID-19.
Kesimpulan: Penggunaan vitamin C pada penderita penyakit COVID-19 mungkin layak secara klinis
dalam pengobatan maupun pencegahannya.

ABSTRACT
Severe acute respiratory syndrome coronavirus-2 (SARS-nCoV-2) the cause of Coronavirus Disease 2019
(COVID-19) was first discovered in Wuhan in December 2019, has spread throughout the world. Each country
tries to develop treatment protocol guidelines following the latest research developments to increase the cure
rate due to the absence of standard guidelines. The purpose of this research is to know the profile of prescribing
COVID-19 drug therapy in patients at Jakarta haji hospital the period September 2020-February 2021. This
research is non-experimental with quantitative descriptive study design was taken retrospectively using
secondary data in the form of prescription data history of COVID-19 patient visits recorded on the system with
sampling techniques that is total sampling. The results showed that in total 387 COVID-19 patients the most
were 46-65 years old with 198 patients and the mostly male with 212 patients. The longest duration of treatment
was 3-10 days as many as 246 patients and the highest degree of disease was mild to moderate as many as 363
patients. The most widely prescribed drugs were paracetamol (84.37%), dexamethasone (78.4%), oseltamivir
(74.1%), heparin (67.81%), alprazolam (60.27%), acetylcysteine (58.12%), insulin (54.93%), CTM (53.13%),
amlodipine 50.43%, vitamin C (48.04%), levofloxacin (35.48%), and omeprazole (31.98 %). COVID-19
patients at the Jakarta Haji Hospital receive the main therapies, namely vitamins, antibiotics, antivirals,
respiratory drugs, gastrointestinal drugs. The suitability of prescribing drug therapy classes based on the
guidelines for the management of COVID-19 is 83% and those that are not in accordance with the guidelines
are 27%.

POLA PERESEPAN TERAPI OBAT COVID-19
DI RUMAH SAKIT HAJI JAKARTA
ABSTRAK
Severe acute respiratory syndrome coronavirus-2 (SARS-nCoV-2) penyebab penyakit Coronavirus Disease
2019 (COVID-19) yang pertama kali ditemukan di Wuhan pada bulan Desember 2019 dan menyebar ke
seluruh dunia. Setiap negara mencoba menyusun pedoman protokol pengobatan mengikuti perkembangan
penelitian terkini untuk meningkatkan angka kesembuhan dikarenakan belum adanya pedoman standar. Tujuan
penelitian untuk mengetahui pola peresepan terapi obat COVID-19 pada pasien rawat inap di Rumah Sakit
Haji Jakarta Periode September 2020-Februari 2021. Penelitian ini termasuk penelitian jenis non eksperimental
dengan desain penelitian deskriptif kuantitatif yang diambil secara retrospektif menggunakan data sekunder
berupa data resep histori kunjungan pasien COVID-19 yang terdata pada sistem dengan teknik pengambilan
sampel yaitu total sampling. Hasil penelitian menunjukkan sejumlah 387 pasien COVID-19 terbanyak adalah
usia 46-65 tahun sebanyak 198 pasien dan jenis kelamin laki-laki sebanyak 212 pasien. Lamanya perawatan
terbanyak 3-10 hari sebanyak 246 pasien dan derajat penyakit terbanyak yaitu ringan hingga sedang sebanyak
363 pasien. Jenis obat yang paling banyak diresepkan adalah parasetamol (84,37%), deksametasone (78,4%),
oseltamivir (74,1%), heparin (67,81%), alprazolam (60,27%), asetilsistein (58,12%), insulin (54,93%), CTM
(53,13%), amlodipin 50,43%, vitamin C (48,04%), levofloksasin (35,48%), dan omeprazole (31,98%). Pasien
COVID-19 di Rumah Sakit Haji Jakarta menerima terapi yang utama yaitu vitamin, antibiotik, antivirus, obat
saluran napas, dan obat saluran cerna. Kesesuaian peresepan kelas terapi obat berdasarkan guideline
penatalaksanaan COVID-19 yaitu 83% dan yang tidak sesuai dalam guideline sebesar 27%
PENDAHULUAN
SARS-CoV-2, penyebab Penyakit Coronavirus 2019 (COVID-19), ditemukan di Wuhan,
Cina, pada bulan Desember 2020.(1) Lebih dari 121.000 orang di seluruh dunia telah
terinfeksi oleh virus ini, dan Organisasi Kesehatan Dunia (WHO) telah menyatakannya
sebagai pandemi global.(2) Pada 2 Maret 2020, kasus pertama di Indonesia diumumkan
secara resmi. Pandemi Covid-19 telah memberikan dampak sosial dan ekonomi yang
signifikan terhadap kehidupan masyarakat.(3)(4)
SARS-CoV-2 dapat ditularkan antar manusia melalui batuk atau bersin, aerosol, udara,
atau muntahan.(5)(6) Gejala yang ditimbulkan dapat berupa batuk, pilek, demam,
konjungtivitis, sakit tenggorokan dan diare.(7)(8) Ginjal, hati, sistem saraf pusat,
kardiovaskular, dan sistem pencernaan merupakan organ target utama infeksi virus corona
dalam tubuh manusia, sehingga perlu pemantauan yang tepat.(9) Kasus positif COVID-19 di
Indonesia mencapai 1.217.468 pada 14 Februari 2021, menurut data Gugus Tugas Percepatan
Penanganan COVID-19. Ada 159.012 kasus aktif, 1.025.273 kasus dikonfirmasi, dan 33.183
kematian dikonfirmasi.(10) Di Indonesia, kasus positif terbanyak terdapat di wilayah DKI
Jakarta dengan jumlah sebanyak 303.715 dengan kasus dalam perawatan/isolasi mandiri
sebanyak 24.258, kasus sembuh 274.740, dan kasus meninggal 4.717.(10)

Penggunaan obat pasien COVID-19 harus dipantau secara ketat karena penelitian obat
uji COVID-19 terbaru terus mengubah informasi tentang khasiat dan keamanan obat yang
digunakan untuk mengobati COVID-19. Protokol pengobatan yang dapat digunakan sebagai
dasar pengobatan untuk meningkatkan angka kesembuhan sedang dikembangkan di negara-
negara di dunia, namun belum seragam.(11)
Informasi obat COVID-19 dari Badan Pengawas Obat dan Makanan Republik Indonesia
dapat dijadikan standar penanganan pengobatan COVID-19 bagi tenaga kesehatan di seluruh
rumah sakit rujukan dan fasilitas lainnya di Indonesia.(12) Rumah Sakit Haji Jakarta
merupakan rumah sakit swasta besar yang terletak di Jakarta Timur, dan daerah tersebut
memiliki pasien terkonfirmasi positif COVID-19 terbanyak di DKI Jakarta per 15 Februari
2021, sehingga pola peresepan yang digunakan harus disesuaikan dengan Informatorium
Pedoman Penanganan COVID-19 untuk mendapatkan pengobatan yang tepat.(13)(14)
Berdasarkan uraian tersebut, peneliti tertarik untuk mempelajari Pola peresepan COVID-19
untuk Pasien Rawat Inap di Rumah Sakit Haji Jakarta dari September 2020 hingga Februari
2021.
METODE PENELITIAN
Penelitian ini bersifat deskriptif kuantitatif dan menggunakan data retrospektif. Sampel
diambil dari data pasien COVID-19 RS Haji Jakarta yang memenuhi kriteria tertentu, dengan
menggunakan teknik total sampling. Data sekunder berupa data resep histori kunjungan
pasien yang terdata pada sistem THC (Telkomedica Health Center), meliputi usia pasien,
jenis kelamin pasien, lama perawatan, derajat penyakit pasien, obat yang diberikan kepada
pasien COVID-19 berdasarkan kelas terapi dan jenis obat yang banyak diresepkan serta
kesesuaian peresepan terapi COVID-19 dengan guideline penatalaksanaan COVID-19
berdasarkan buku Informatorium Badan POM dan Pedoman Tatalaksana COVID-19 dan
disajikan dalam bentuk tabel distribusi frekuensi.

Sebanyak 212 pasien (54,78%) adalah laki-laki, dan usia rata-rata berkisar antara 46
hingga 65 tahun. Akibat faktor biologis dan gaya hidup, pria lebih rentan terkena penyakit
saluran pernapasan dibandingkan wanita menurut beberapa pakar kesehatan. Ini karena
tingkat kekebalan pria lebih rendah daripada wanita dan pria lebih sering merokok.(14)
Sistem kekebalan tubuh mulai menurun dengan bertambahnya usia, kelompok usia 46-65
tahun memiliki persentase pasien yang positif COVID-19 lebih tinggi.(15) Sistem kekebalan
yang melemah ditambah dengan penyakit kronis dapat meningkatkan risiko COVID-19.(16)
Hal ini sejalan dengan penelitian Tiodora (2020) yang menunjukkan kelompok usia 45-65
tahun memiliki risiko lebih tinggi terinfeksi Virus Corona.(17) Rentang hari lama perawatan
selama 3-10 hari sebanyak 246 pasien (63,57%) dan derajat keparahan penyakit terbanyak
adalah derajat penyakit ringan-sedang sebanyak 363 pasien (93,80%). Derajat penyakit
menunjukkan suatu tingkatan atau keparahan gejala penyakit yang dialami pasien berbeda-
beda tergantung dari kondisi tubuhnya atau adanya penyakit bawaan yang dapat
memperburuk keadaan. Mengetahui berapa lama pasien dirawat di rumah sakit sangat
penting untuk perencanaan dan memprediksi ketersediaan hunian tempat tidur, petugas. dan
kebutuhan peralatan.(18)

Pada tabel 2 jumlah dan persentase peresepan obat berdasarkan kelas terapi yang banyak
diresepkan adalah kelas terapi antibiotik 1023 R/ (20,53%). Pada penelitian F.Moretto,dkk
(2021) menunjukkan bahwa antibiotik lebih sering diresepkan pada pasien lanjut usia yang
sebelumnya memiliki penyakit bawaan dan pasien yang mengalami demam, sesak serta
kebutuhan oksigen yang tinggi.(19) Hal ini berkaitan dengan jumlah dan persentase
berdasarkan usia di Rumah Sakit Haji Jakarta, dimana pada kelompok usia lansia merupakan
jumlah pasien COVID-19 terbanyak. Jika pasien dengan COVID-19 mengalami infeksi
bakteri sekunder, antibiotik akan diberikan untuk mencegah pertumbuhan bakteri patogen.
Antibiotik Levofloksasin adalah fluoroquinolone yang efektif melawan berbagai bakteri
patogen yang dapat menyebabkan pneumonia.(20) Sebuah studi in silico menunjukkan
bahwa fluoroquinolon dapat menghambat SARS-CoV-2, bersama dengan sifat
immunomodulator mengusulkan penggunaan sebagai tambahan dalam merawat pasien
COVID-19.(21) Hal ini juga sejalan dengan penelitian Marie Chedid, dkk antibiotik yang
sering digunakan untuk pengobatan pasien COVID-19 adalah Levofloksasin dan diikuti oleh
Seftriaksone dan Azitromisin.(22)

Vitamin C diresepkan untuk pasien COVID-19 pada tingkat tertinggi (hingga 477 R/)
(48,04 %). Sifat antioksidan vitamin C mencegah kerusakan sel yang disebabkan oleh ROS
dan nitrogen. Pasien COVID-19 yang diobati dengan vitamin C meningkatkan kesehatan
daripada pasien yang tidak diberikan.(8) Vitamin C mengurangi lama tinggal di ICU sebesar
8% dalam meta-analisis dari 12 uji coba terkontrol yang melibatkan 1.766 pasien. Obat
oseltamivir, hanya dapat digunakan pada pasien yang menunjukkan gejala infeksi antara
COVID-19 dan flu. Pasien dengan influenza yang positif virus corona (tidak termasuk
SARS-CoV-2) yang menerima oseltamivir selain pengobatan standar, pulih lebih cepat
daripada mereka yang menerima pengobatan standar saja. Persatuan Dokter Paru Indonesia
(PDPI) merekomendasikan oseltamivir untuk pengobatan COVID-19 karena
ketersediaannya dan produksi dalam negeri.(23)(24)
Pada masa pandemi COVID-19, parasetamol atau asetaminofen merupakan terapi
suportif pilihan untuk mengatasi demam pada penderita COVID-19. Selain itu NSAID dapat
digunakan dengan pertimbangan komorbiditas dan faktor risiko setiap pasien.(8)(25) Namun,
beberapa laporan penelitian dan Otoritas Nasional mempermasalahkan keamanan NSAID
karena dugaan adanya induksi tingkat enzim pengubah angiotensin 2 (ACE2) (reseptor yang
digunakan oleh SARS-CoV-2 untuk memasuki sel saluran udara inang), peningkatan risiko
superinfeksi bakteri, dan penyamaran dari gejala penyakit. Sebagai akibatnya, penggunaan
NSAID masih tidak dianjurkan sementara parasetamol masih lebih disukai.(26)
Pasien COVID-19 yang dirawat di rumah sakit yang diobati dengan Deksametason
memiliki tingkat kematian yang lebih rendah daripada mereka yang menerima perawatan
biasa dalam uji coba label terbuka, multicenter, dan acak. WHO (World Health Organization)
dan EMA (European Medicines Agency) merekomendasikan penggunaan sistemik
kortikosteroid untuk pasien COVID-19 dengan gejala parah hingga kritis, tidak
merekomendasikan untuk gejala ringan sampai sedang, kecuali pasien tersebut sudah
menggunakan obat ini untuk kondisi lain.(27)(28)(29)
Pasien dengan COVID-19 terkonfirmasi parah hingga kritis diberi resep terapi
antikoagulan dengan heparin Na 5000 IU/mL (67,81% R/) untuk mengobati pembekuan
darah yang tidak terkontrol dan mengurangi pembentukan mikrotrombus, yang merupakan
penyebab mendasar. Heparin, selain sebagai antikoagulan, telah terbukti memiliki sifat anti-
inflamasi tambahan.(29)(30)

Asetilsistein 200mg (58,12%) mengatasi ketidakseimbangan oksidan yang dapat
menyebabkan peradangan dan kerusakan jaringan, sehingga N-acetylcysteine dapat
digunakan sebagai antioksidan, Asetilsistein memiliki potensi sebagai terapi tambahan untuk
penyakit COVID-19.(8)(31)
Peresepan obat saluran cerna pada pasien COVID-19 yaitu Omeprazole sebanyak 245
peresepan (31,98%). Penelitian sebelumnya telah menunjukkan bahwa omeprazole dapat
menghambat replikasi virus dengan mengganggu pengasaman lisosom. Sebuah penelitian
obat baru-baru ini di Jerman telah menunjukkan bahwa Omeprazole mengganggu
pembentukan virus SARS-CoV-2 di luar konsentrasi plasma terapeutik pada 8 µM.(32)
Skrining in-vitro dari 60 obat yang disetujui FDA mengungkapkan potensi antivirus
omeprazole, mendukung penggunaan ulangnya terhadap COVID-19.(33)
CTM atau Chlorpheniramine maleat 4 mg sebanyak 17 peresepan (53,13%) merupakan
antihistamin yang banyak diresepkan. CTM antihistamin efektif dengan aktivitas antivirus
yang kuat terhadap berbagai jenis influenza A / B. Pada penelitian sebelumnya menunjukkan
bahwa CTM tidak hanya efektif dalam mengobati anafilaksis tetapi juga memiliki aktivitas
antivirus dan anti-inflamasi yang kuat. Pada penelitian sebelumnya dilaporkan bahwa
penggunaan CTM pada pasien COVID-19 dapat menurunkan tingkat mortalitas pasien.(34)
Tabel 2 menunjukkan peresepan obat Psikofarmaka pada pasien COVID-19 paling banyak
adalah Alprazolam sebanyak 44 peresepan (60,27%). Pada Buku Pedoman Dukungan
Kesehatan Jiwa dan Psikososial pada Pandemik COVID-19, penatalaksanaan pemilihan obat
untuk memperbaiki kecemasan dan kualitas tidur yaitu benzodiazepin seperti estazolam,
alprazolam, dll. Ketakutan pasien dan kekhawatiran dapat mengganggu pola tidur pasien
yang akan memicu serangkaian peristiwa fisiologis sehingga menyebabkan turunnya tingkat
kekebalan.(35)(36)
Obat antihipertensi Amlodipin yang diresepkan sebanyak 117 R/ (50,43%). Amlodipine
merupakan obat antihipertensi Calcium Channel Blockers (CCB) yang secara signifikan
dapat menghambat kejadian replikasi SARS-CoV-2 pasca-masuk secara in vitro
dibandingkan Angiotensin Converting Enzyme Inhibitors (ACEI) dan Angiotensin Receptor
Blocker (ARB). Investigasi klinis retrospektif pasien COVID-19 mengungkapkan bahwa
pemberian CCB amlodipine besylate dikaitkan dengan penurunan tingkat kematian pasien
dengan hipertensi.(37)

Diabetes mellitus tipe 1 dapat diobati pada pasien COVID-19 dengan menggunakan
pompa insulin atau bolus insulin basal, yang merupakan regimen terbaik. Menurut temuan,
39 resep ditulis untuk insulin (54,93 persen). Obat antidiabetes non-insulin dapat digunakan
untuk mengobati diabetes mellitus tipe 2 pada pasien COVID-19 ringan hingga sedang
dengan kadar glukosa ringan-sedang, tetapi analog insulin adalah pengobatan lini pertama
dan terapi insulin harus disesuaikan untuk setiap pasien. Jika pasien mengalami demam atau
sedang menjalani terapi glukokortikoid, insulin harus menjadi pengobatan lini pertama.

Pada gambar 1 didapatkan hasil dari 12 kelas terapi obat sebanyak 83% peresepan telah
sesuai dengan yang tercantum pada guideline tatalaksana COVID-19. Ini menunjukkan
bahwa profesi kesehatan yang terkait di RS Haji telah menggunakan acuan resmi dalam
mempertimbangkan penatalaksanaan atau manajemen terapi COVID-19 untuk mencapai
terapi pengobatan yang tepat. Peresepan obat sebanyak 27% tidak sesuai dengan yang
tercantum pada guideline, kemungkinan terdapat indikasi lain dari pasien yang
membutuhkan obat di luar guideline, seperti antibiotik Metronidazole yang tidak tercantum
pada guideline tetapi digunakan bagi pasien COVID-19 yang mengalami infeksi bakteri pada
lambung dan usus.

SIMPULAN
Berdasarkan hasil penelitian, dari 387 pasien COVID-19 dengan usia 46-65 tahun
sebanyak 198 pasien (51,16%), jenis kelamin terbanyak laki-laki sebanyak 212 pasien
(54,78%). Lamanya perawatan sekitar 3-10 hari sebanyak 246 pasien (63,57%) dan derajat
keparahan penyakit yaitu ringan hingga sedang (93,80%). Peresepan obat berdasarkan kelas
terapi yang banyak diresepkan adalah kelas terapi antibiotik sebanyak 1023 R/ (20,53%)
dengan jenis obat Levofloksasin 363 R/ (35,48%). Semua pasien COVID-19 di RS Haji
Jakarta menerima terapi yang utama yaitu vitamin, antibiotik, antivirus, obat saluran napas,
dan obat saluran cerna. Untuk kelas terapi lainnya diberikan sesuai komorbid pasien.
Peresepan berdasarkan kelas terapi obat sebanyak 83% sesuai dengan yang tercantum pada
guideline tatalaksana COVID-19 dan yang tidak sesuai guideline sebanyak 27%.

Abstrak
Pada tanggal 31 Desember 2019, Tiongkok melaporkan kasus pneumonia misterius yang tidak diketahui
penyebabnya. Dalam 3 hari, pasien dengan kasus tersebut berjumlah 44 pasien dan terus bertambah hingga saat
ini berjumlah ribuan kasus. Pada awalnya data epidemiologi menunjukkan 66% pasien berkaitan atau terpajan
dengan satu pasar seafood atau live market di Wuhan, Provinsi Hubei Tiongkok. Sampel isolat dari pasien diteliti
dengan hasil menunjukkan adanya infeksi coronavirus, jenis betacoronavirus tipe baru, diberi nama 2019 novel
Coronavirus (2019-nCoV). Pada tanggal 30 Januari 2020 WHO telah menetapkan sebagai Kedaruratan Kesehatan
Masyarakat Yang Meresahkan Dunia/ Public Health Emergency of International Concern (KKMMD/PHEIC).
Penelitian ini menggunakan metode critical review. Sumber data penelitian ini berasal dari literatur Internasional
dan literatur Indonesia yang berupa hasil penelitian dari publikasi jurnal. Dari hasil kajian analisis sistematis dari
jurnal internasional yang sudah didapatkan, maka penggunaan Vitamin C sebagai penatalaksanaan dalam COVID-
19 sangat penting, dimana pemberian vitamin C dapat mempercepat perbaikan pada kasus COVID-19 yang
kerjanya pada plasma dan netrofil, selain itu Vitamin C juga dapat menangkal radikal bebas dan mencegah stress
oksidatif oleh coronavirus yang berikatan di heme.
Abstract
On December 31, 2019, China reported a mysterious case of pneumonia with no known cause. In 3 days, there
were 44 patients with such cases and it continues to grow until now there are thousands of cases. Initially
epidemiological data showed 66% of patients were related or exposed to a seafood market or live market in
Wuhan, Hubei Province, China. Samples of isolates from patients were examined with the results showing the
presence of coronavirus infection, a new type of betacoronavirus, named 2019 novel Coronavirus (2019-nCoV).
On 30 January 2020 the WHO had designated the Emergency Health Public Disaster / Public Health Emergency
of International Concern (KKMMD / PHEIC). This research uses the critical review method. The data source of
this research comes from international literature and Indonesian literature in the form of research results from
journal publications. From the results of systematic analysis studies from international journals that have been
obtained, the use of Vitamin C as a management in COVID-19 is very important, where the administration of
vitamin C can accelerate the improvement in COVID-19 cases that work on plasma and neutrophils, besides that

Abstract: Background and Objectives: Starting in early December 2019, the novel Coronavirus Disease
(COVID-19) from infection with COVID-19 has caused a global pandemic. Many aspects of its patho-
genesis and related clinical consequences are still unclear. Early diagnosis and dynamic monitoring
of prognostic factors are essential to improve the ability to manage COVID-19 infection. This study
aimed to provide an account of the role played by vitamins C and D on the onset, progression and
severity of COVID-19. Clinical features and infection-related risk factors are also briefly discussed.
Material and Methods: In March 2022, the main online databases were accessed. All the articles that

foundation to transpose the findings to patients with severe COVID-19 [49]. Patients with
pneumonia, sepsis and multiple organ failure have low vitamin C status and high oxida-
tive stress [50–52]. These critically ill patients have a higher requirement for vitamin C,
requiring gram doses to normalize their blood levels [53,54], 20–30 times greater than what
is required for the general population. Intravenous administration of vitamin C to patients
with pneumonia to normalize plasma levels is an intervention which, as suggested by
some studies, is able to reduce the severity (days spent in ICU) and duration (hospital stay)
of the disease [55–60]. Despite these findings, critically ill patients with sepsis continue
to receive milligram amounts of vitamin C, which is insufficient to replenish their vita-
min C status [61]. Vitamin C has pleiotropic physiological functions, many of which are
relevant to COVID-19. These include antioxidant, anti-inflammatory, antithrombotic and
immunomodulatory functions [62]. Many of the functions of vitamin C seem relevant for
the cytokine storm, sepsis and ARDS related to COVID-19 [63–68].

quality of the clinical trials based on randomization, blinding and withdrawals. The scale
ranged from zero to five points. A final score ≥3 stars indicate low risk of bias.
3. Results
3.1. Search Results
The literature search resulted in 1314 articles. Of them, 878 were excluded, as they were
duplicates. A further 413 were excluded with reason: not clinical investigation (n = 174), not
focusing on Vitamin D or C (n = 201), not focusing on COVID (n = 30), language limitation
(n = 8). This left 23 studies for the present investigation (Figure 1).

Nine studies compared Vit C supplementation in patients with COVID (Table 2).
Overall, 1488 patients were included: 605 in the treatment group and 883 in the control
group. Men made up 52% (774 of 1488). The mean age of the patients was 59.8 ± 7.4 years
old. High-dose vitamin C may reduce the mortality [85] and the rate of thrombosis [86],
and improve oxygenation [85,87], in patients with COVID-2019. Other studies did not find
benefits for vitamin C in addition to the main treatment regimen in inpatient and outpatient
regimes [86,88–93], with no impact on mortality, need for mechanical ventilation [89], nor
vasopressor requirements or Sequential Organ Failure Assessment (SOFA) scores [90].

4. Discussion
According to the main findings of the present study, our hypothesis that the administra-
tion of vitamins C and D in COVID-19 patients impacts positively on disease susceptibility,
severity and progression was not supported by the current evidence. The current evidence
on vitamin C and D supplementation in patients with COVID-19 infection is heterogeneous.
Although most studies focused on the same endpoints, such as infection progression, hos-
pitalisation length, oxygenation, need of ICU and mechanical ventilation, and mortality,
the results of these studies are controversial, and no consistent recommendation can be
inferred. The current clinical investigations evidenced high variability in eligibility criteria,
patient comorbidities and age, and associated therapies. In some studies, vitamins were
used as coadjuvant of a formal experimental therapy, while in others as main treatment.
Ethnicity and hospital setting (inpatient/outpatient) were also variable. Moreover, there
was no consensus between studies in administration protocol: high heterogeneity in dosage,
administration and duration of the treatment were evident. Finally, some studies adminis-
tered vitamins pre- and/or during COVID infection, in patients with different risk factors
and infection severity.
The improved metabolism of vitamins in diseases and inflammatory processes may
justify vitamin C and D supplementation during sepsis or pneumonia. However, the
possibility of decreasing the incidence of viral diseases in a population using dietary sup-
plements of vitamin C and D is not supported by the present published peer reviewed
literature. This is in accordance with previous evidence, which concluded that vitamin
administration at high doses does not contribute to reducing the severity and progression of
COVID-19 [49,94,95]. Among other things, the basal state of vitamin C probably influences
the response to its administration. As recently highlighted, most patients who were enrolled
in the clinical studies suffered from hypovitaminosis, and such hypovitaminosic patients
are more likely to respond to its therapeutic supplementation [49,94,96]. We were unable
to identify studies which investigated the efficacy of vitamin C and D supplementation in
the prevention of COVID-19 in healthy patients. In this context, the effects of vitamin C
and D application are not fully generalisable. While waiting to develop a targeted, safe
and effective therapy, it is important to investigate individual predisposition and proper
disease management [97]. Available data on the use of nutraceuticals in COVID-19 are
inconsistent. Irrespective of COVID-19 infection, vitamins have beneficial properties with
practically no side effects. Prevention of malnutrition by providing adequate amounts of
macronutrients to maintain energy needs is highly recommended [98]. Integration with
micronutrients is equally important to prevent viral infections: low levels of vitamins
have been associated with adverse clinical outcomes [99]. Given the good safety profile,
low cost, and the potential for rapid increase in their production, the administration of
vitamins C and D to patients with infectious diseases may be reasonable. However, there is
a lack of evidence-based guidelines which recommend vitamin C and D supplementation
in patients with COVID-19, and results from high quality randomised controlled trials
(RCTs) are inconsistent. Current investigations so far are mostly observational, and include
a relatively small sample size which can lead to biased results. Large-scale multicenter

early and at the hyperinflammatory stages [77,102,128]. Despite these positive results, the
efficacy of Vitamin D application in patients with COVID-19 still remains controversial,
and future studies are required [129]. A phase 3 RCT, the CORONAVIT study is currently
ongoing (NCT04579640). In this study the authors determined whether population-level
implementation of a test-and-treat approach to correction of sub-optimal vitamin D status
influences the risk of acute respiratory infection or COVID-19 disease. In adult sub-optimal
vitamin D status, vitamin D implementation has no impact on the risk of acute respiratory
infection or COVID-19 disease.
Regarding vitamin C, many observational studies indicated a low state of vitamin C
in critically ill patients with COVID-19 [130,131]. Currently, several RCTs are evaluating
intravenous administration of vitamin C monotherapy [132–135]. Given the pharmacoki-
netics of vitamin C and the increased requirement during pulmonary infections, oral
vitamin C might not be as effective as intravenous administration [136], significantly reduc-
ing levels of IL-6 by the first week [87]. Coagulopathy and development of microthrombi
represent further common complications of COVID-19 [63], which probably are important
components of lung problems [137]. Early vitamin C injections prevent microthrombi
formation and blockage of capillaries [138]. Indeed, previous studies have also shown
that the level of D-dimer in COVID-19 patients decreased if intravenous vitamin C was
administered [139]. Another possible complication of COVID-19 is the extracellular neu-
trophil traps, which may predispose to the coagulopathy related to COVID-19 [140,141]. In
sepsis models, administration of vitamin C may decrease the extracellular neutrophil trap
formation [142]. Neutrophil-derived oxidative stress is thought to induce tissue damage in
COVID-19 [143,144]. Patients with respiratory infection and sepsis demonstrated a consid-
erable elevation of the oxidative stress markers compared to other critical patients [145,146].
In these patients, administration of vitamin C stabilized markers of oxidative stress, im-
proving the survivorship [56,147]. Moreover, the administration of vitamin C in critical
patients might have shortened the mechanical ventilation and the length the intensive
care stay [148,149]. The above findings may be particularly important in countries with
limited capacity of ICUs beds and more generally in contexts with limited resources, such
as low-middle-income countries [150]. Most of the top 10 countries with the highest
number of cases for COVID-19 have low-medium income [151], a known risk factor for
hypovitaminosis C [152].
5. Conclusions
According to the main findings of the present study, our hypothesis that the adminis-

severity and progression was not supported by the current evidence. While waiting to

and Treatment of COVID-19

Abstract: Investigation into the role of vitamin C in the prevention and treatment of pneumonia and
sepsis has been underway for many decades. This research has laid a strong foundation for translation
of these findings into patients with severe coronavirus disease (COVID-19). Research has indicated
that patients with pneumonia and sepsis have low vitamin C status and elevated oxidative stress.
Administration of vitamin C to patients with pneumonia can decrease the severity and duration of
the disease. Critically ill patients with sepsis require intravenous administration of gram amounts of
the vitamin to normalize plasma levels, an intervention that some studies suggest reduces mortality.
The vitamin has pleiotropic physiological functions, many of which are relevant to COVID-19.
These include its antioxidant, anti-inflammatory, antithrombotic and immuno-modulatory functions.
Preliminary observational studies indicate low vitamin C status in critically ill patients with COVID-19.
There are currently a number of randomized controlled trials (RCTs) registered globally that are
assessing intravenous vitamin C monotherapy in patients with COVID-19. Since hypovitaminosis C
and deficiency are common in low–middle-income settings, and many of the risk factors for vitamin
C deficiency overlap with COVID-19 risk factors, it is possible that trials carried out in populations
with chronic hypovitaminosis C may show greater efficacy. This is particularly relevant for the
global research effort since COVID-19 is disproportionately affecting low–middle-income countries
and low-income groups globally. One small trial from China has finished early and the findings
are currently under peer review. There was significantly decreased mortality in the more severely
ill patients who received vitamin C intervention. The upcoming findings from the larger RCTs
currently underway will provide more definitive evidence. Optimization of the intervention protocols
in future trials, e.g., earlier and sustained administration, is warranted to potentially improve its
efficacy. Due to the excellent safety profile, low cost, and potential for rapid upscaling of production,
administration of vitamin C to patients with hypovitaminosis C and severe respiratory infections,
e.g., COVID-19, appears warranted.

For a quarter of a century, it has been known that critically ill patients, including those with
sepsis and multiple organ failure, have very low vitamin C status [1–3]. It has also been demonstrated
that these critically ill patients have higher requirements for vitamin C, with gram doses required
to normalize their blood levels [4,5], 20–30 times more than is required for the general population.
Despite these findings, critically ill patients with sepsis continue to be administered milligram amounts
of vitamin C, which is insufficient to replete their vitamin C status [6]. In 2014, Dr Fowler and colleagues
published the findings of a small clinical trial which indicated that intravenous administration of
gram amounts of vitamin C to patients with sepsis could improve organ failure scores and decrease
markers of inflammation (C-reactive protein) and tissue damage (thrombomodulin) [7]. In a larger
randomized controlled trial (RCT) of septic patients with acute respiratory distress syndrome (ARDS),
the CITRIS-ALI trial, intravenous administration of 200 mg/kg/d of vitamin C for 4 days resulted in a
28 day mortality of 30% relative to 46% in the placebo group (p = 0.03) and a hazard ratio (HR) of 0.55
(95% CI, 0.33–0.90, p = 0.01) [8]. The number of intensive care unit (ICU)- and hospital-free days was
also significantly higher in the vitamin C group.
Administration of vitamin C late in the disease process, e.g., when ARDS has developed,
likely attenuates its effectiveness. Earlier clinical trials have indicated that administration of vitamin C
to patients with pneumonia can decrease the severity of the respiratory symptoms, particularly of the
most severely ill patients, and the duration of hospital stay [9,10]. Thus, administration of vitamin C
earlier in the respiratory infection process may prevent its progression to sepsis [11]. Survival data
from the CITRIS-ALI trial has indicated that the effect of vitamin C on survival is most apparent
during the 4 day infusion period [12]. Furthermore, pharmacokinetic research has indicated that upon
cessation of vitamin C infusion, the vitamin C status of some patients returns to their low pre-infusion
levels [5]. These findings call to sustained administration of the vitamin in the ICU. This will likely also
improve the long-term outcomes of the patients, particularly if they continue to take the vitamin orally
following discharge from ICU, due to its important roles in immunological function and in multiple
organ systems [11].
Earlier this year, Dr Fowler and colleagues published a review on the emerging role of vitamin C
as a treatment for sepsis [13]. In this review, they summarised the current state of knowledge around
its pleiotropic physiological functions in sepsis. These include its roles as an antioxidant, a cofactor for
the synthesis of vasopressors (norepinephrine and vasopressin), and roles in leukocyte and platelet
functions, and endothelial and epithelial cell integrity. In the face of the current severe acute respiratory
syndrome coronavirus (SARS-CoV-2) pandemic, this review has been very timely, with sepsis being a
significant complication of severe coronavirus disease (COVID-19) [14].
Many of the functions of vitamin C appear relevant to COVID-19-related sepsis and ARDS.
For example, recent research has uncovered a connection between SARS-CoV-2 infection and depleted
levels of the antiviral cytokine interferon [15], and a negative association between interferon levels and
disease severity [16,17]. Of note, vitamin C has been shown to augment interferon levels in animal
models of viral infection [18,19]. Another characteristic of severe COVID-19 is elevated inflammatory
markers and this can present as a ‘cytokine storm’ in some cases [14]. Vitamin C has anti-inflammatory
and antioxidant activities which can potentially counteract this phenomenon [13]. Preliminary evidence
from a small COVID-19 trial indicates that administration of intravenous vitamin C can significantly
decrease IL-6 levels by day 7 of infusion [20].
Other common complications of COVID-19 are coagulopathy and microthrombi formation [14],
which is likely a major component of COVID-19 lung pathology [21]. Early injection of vitamin C
has been shown to prevent microthrombi formation and capillary plugging [22], and a case series
has shown decreased D-dimer levels in COVID-19 patients who were administered intravenous
vitamin C [23]. Neutrophil extracellular traps (NETs) have been implicated in COVID-19-related
thrombotic complications [24,25]. Previous research has indicated that vitamin C administration can
attenuate NETs in sepsis models [26], and post-hoc analysis of the CITRIS-ALI trial indicated decreased
circulating cell-free DNA 48 h after administration of intravenous vitamin C [27]. Neutrophil-derived
oxidative stress is believed to induce tissue damage in COVID-19 [28,29]. Patients with pneumonia and
sepsis have significantly elevated oxidative stress markers relative to other critically ill patients [30,31],
and early studies indicated that administration of vitamin C and other antioxidants to patients with
septic shock and ARDS stabilized oxidative stress markers and improved cardiovascular parameters
and survival [32,33].

veracity of the methodology used to assess the vitamin C status of the patients was not clear, so the
values could be artifactually low [45].

of the methodology used to assess the vitamin C status of the patients was not clear, so the values
could be artifactually low [45].
There are a number of clinical trials registered globally assessing intravenous vitamin
C monotherapy in COVID-19 patients (Table 1). The first trial off the ground was in
Hubei, China (NCT04264533) [46]. In this trial the investigators planned to treat 140 patients with a
placebo control or intravenous vitamin C at a relatively high dose of 24 g/day for 7 days, and assess
requirements for mechanical ventilation and vasopressor drugs, organ failure scores, ICU length of
stay and mortality. A preprint of the findings of 54 patients from this trial is currently under peer
review [15]. Although the investigators reported no differences between the treatment and placebo
groups for the above outcomes, when a subgroup of the most severely ill patients (SOFA scores ≥ 3)
was assessed, a significant decrease in ICU and hospital mortality was observed in the vitamin C
group—18% vs. 50% in the placebo group (HR 0.2, 95% CI 0.1–0.9, p = 0.03). Unfortunately, the baseline
vitamin C status of the patients was not reported.
The USA currently has the highest number of COVID-19 cases globally, however despite this,
there are only a couple of small intravenous vitamin C monotherapy and COVID-19 trials registered in
the USA (NCT04344184 and NCT04363216). A trial at the Cleveland Clinic is assessing oral vitamin C
with and without zinc (NCT04342728). However, due to the pharmacokinetics of enteral and parenteral
vitamin C differing dramatically, and the higher requirement for vitamin C during respiratory infections,
oral vitamin C may not be as efficacious as intravenous vitamin C [42]. As such, future meta-analyses
of vitamin C and COVID-19 clinical trials should include subgroup analyses of studies comprising
intravenous vs. oral vitamin C. Furthermore, subgroup analysis of low vitamin C populations could
also be carried out as RCT findings will likely vary depending on the country and hence the baseline
vitamin C status of the population in which the study was carried out [47]. In populations with a high
prevalence of chronic hypovitaminosis C, vitamin C intervention may show greater efficacy.
Overall, vitamin C exhibits plausible mechanisms of action that are of relevance to severe respiratory
infection, including antioxidant, anti-inflammatory, antithrombotic, and immuno-modulatory functions.
Based on the findings from clinical trials of patients with pneumonia and sepsis, and preliminary
observational and interventional studies of COVID-19 patients, it is likely that vitamin C administration
will improve outcomes in COVID-19. The upcoming findings from the larger RCTs currently underway
(e.g., LOVIT-COVID), including introduction of intravenous vitamin C arms to large adaptive trials
(e.g., REMAP-CAP; NCT02735707), will provide more definitive evidence. Some of these trials
(e.g., LOVIT-COVID) are also examining the longer-term quality of life effects of short-term vitamin C
administration. Optimization of the intervention protocols in future trials, e.g., earlier and sustained
administration, is warranted to potentially improve its efficacy. Due to the excellent safety profile,
low cost, and potential for rapid upscaling of production, administration of vitamin C to patients with
hypovitaminosis C and severe respiratory infections, e.g., COVID-19, appears warranted.

Abstrak
Vitamin C merupakan salah satu nutrisi yang tidak bisa disintesis oleh manusia. Fungsi Vitamin
C dalam tubuh sangat banyak, salah satunya sebagai antioksidan dan efek pada sistem imun.
Vitamin C terbukti dapat melawan virus karena sebagai imunomodulasi, sehingga dapat
meningkatkan produksi interferon dan mengatur sintesis sitokin proinflamasi. Vitamin C berhasil
mencegah dan mengurangi pasien ICU pada ISPA. Vitamin C juga mepercepat kesembuhan
pada penyakit flu. Pemberian terapi vitamin C diharapkan membantu dalam menurunkan
morbiditas dan mortalitas pada COVID-19. Vitamin C pada tubuh pasien COVID-19 juga
menurun, sehingga sangat dibutuhkan terapi vitamin C. Vitamin C dapat sebagai
imunosupresan yang akan menghambat cytokin storm pada pasien COVID-19. Vitamin C
dalam pasien yang terinfeksi virus dapat menghambat ROS. Pemberian vitamin C dengan dosis
tinggi dapat membantu penyembuhan dalam dengan sepsis. Dokter di China melaporkan 50
pasien COVID-19 dengan pemberian vitamin C berhasil sembuh. Hal ini dapat membuktikan
vitamin C dapat dijadikan terapi pada pasien COVID-19. Tujuan pembuatan literature review ini
untuk mengatahui efektivitas vitamin C pada pasien COVID-19.

Abstrak
Peran media sosial sebagai sarana penyebaran informasi kesehatan makin meningkat selama pandemi
COVID-19. Meskipun akses informasi semakin mudah, tetap harus dilakukan pemilahan info karena dapat
tercampur hoax. Salah satu contoh informasi salah yang berkembang adalah melebih-lebihkan peran vitamin
C terhadap pencegahan COVID-19. Hal ini menyebabkan kepanikan, kenaikan harga, dan kelangkaan produk
di pasaran, dan penggunaan melebihi dosis. Oleh sebab itu, dilakukan edukasi untuk meluruskan
pemahaman ini. Edukasi dilakukan melalui media sosial Instagram dan website Makanmu.com, dengan
partisipan berusia di atas 17 tahun, berjumlah 80 orang. Evaluasi kegiatan dilakukan melalui pretest dan
posttest. Terdapat peningkatan pengetahuan para peserta mengenai peran vitamin C terhadap pencegahan
COVID-19. Edukasi melalui media sosial bermanfaat untuk meningkatkan pengetahuan masyarakat dewasa
muda selama pembatasan sosial akibat pandemic COVID-19.
Abstract
Information spreading about the role of vitamin C in preventing COVID-19 through social media. During
the COVID-19 epidemic, the use of social media to disseminate health information has grown. Even while information is
becoming more accessible, it is still needed to filter it properly because it might be mixed in with hoaxes. Overestimating
the significance of vitamin C in preventing COVID-19 is one example of spreading disinformation. This results in panic,
price hikes, product shortages on the market, and overdosing. As a result, education is carried out in order to replace
incorrect understanding. With 80 participants aged 17 and above, education was provided via social media Instagram,
and the Makanmu.com website. The pretest and posttest are used to evaluate the activities. The importance of vitamin C
in preventing COVID-19 was better understood by the participants. Social media education is important for enhancing
young adults' understanding during social limitations caused by the COVID-19 epidemic.
1. PENDAHULUAN
Akses untuk mendapatkan informasi di masa kini semakin terbuka lebar. Platform media
sosial menjadi pintu yang paling banyak digunakan dalam penyebaran informasi. Pada
masa pandemi, penggunaan media sosial sebagai sumber informasi utama semakin
penyuluhan kesehatan juga dialihkan menjadi metode daring. Selain menggunakan metode
seminar, media sosial jejaring pertemanan, menjadi sarana yang disukai dalam

pada rentang 18-44 tahun. Facebook menjadi platform dengan jumlah pengguna terbanyak
yaitu 130 juta pengguna, disusul instagram dengan 62 juta pengguna. Pada tahun 2018,

hewan seperti hati dan ginjal. Vitamin C berperan penting dalam banyak proses fisiologis
pada manusia. Vitamin C dibutuhkan untuk perbaikan jaringan di semua bagian tubuh.
Fungsi penting vitamin C termasuk pembentukan protein yang digunakan untuk sintesis
kulit, tendon, ligamen, dan pembuluh darah; untuk penyembuhan luka dan membentuk
jaringan parut; untuk memperbaiki dan memelihara tulang rawan, tulang, dan gigi dan

adaptif. Vitamin C terakumulasi dalam sel fagosit, seperti neutrofil, dan dapat

Absorpsi vitamin C terjadi di saluran cerna, terutama pada bagian atas usus halus.
Arbsorpsi ini terjadi secara difusi terfasilitasi kemudian masuk ke peredaran darah dan
dimetabolisme di hati dengan hasil berupa asam oksalat dan diekskresikan oleh ginjal.
Ambang ginjal untuk ekskresi vitamin C adalah 1,4 mg/dL. Ketika konsentrasi plasma

menimbulkan efek samping berupa gangguan pencernaan seperti mual, muntah dan diare.
Efek samping lainnya yaitu kelebihan zat besi dan jika konsumsi berlebihan dalam jangka
panjang yang dapat terjadi yaitu pembentukan batu oksalat pada ginjal.12
Vitamin C adalah salah satu zat gizi yang berperan sebagai antioksidan dan efektif
mengatasi radikal bebas yang dapat merusak sel atau jaringan, termasuk melindungi lensa
dari kerusakan oksidatif yang ditimbulkan oleh radiasi. Diantara berbagai fungsi vitamin
C, fungsi yang relevan dengan kasus COVID-19 diantaranya meningkatkan sistem imun
dan sebagai antioksidan. Defisiensi vitamin C dapat menyebabkan gangguan imunitas dan
meningkatkan diferensiasi, proliferasi, dan memodulasi fungsi sel T, Sel B, dan sel natural

dan menyebabkan terjadinya kerusakan pada jaringan. Sel yang rusak menginduksi
inflamasi di paru yang sebagian besar dimediasi oleh makrofag proinflamasi dan
granulosit. Pada sepsis, terjadi stres oksidatif dan terbentuk mediator pro-inflamasi secara
berlebihan. Hal ini dapat menyebabkan peningkatan permeabilitas endotel sehingga terjadi
oleh ROS (reactive oxygen species) tersebut. Vitamin C yang digunakan secara intravena
dengan dosis tinggi dapat bekerja secara pleiotropik sebagai prooksidan yang membantu
meningkatkan bersihan cairan alveolar dan sebagai antioksidan yang memperbaiki fungsi
epitel.13,15-18
Vitamin C memiliki potensi untuk digunakan pada terapi COVID-19 karena dapat
meningkatkan sistem imun dan berfungsi sebagai antioksidan. Dari beberapa penelitian
juga didapatkan efek positif pemberian vitamin C pada kondisi kritis dan sepsis, sehingga
diharapkan bermanfaat pada pasien COVID-19 dengan kondisi tersebut. Salah satu
penelitian awal mengenai penggunaan vitamin C pada sepsis dilakukan oleh Marik et al.
Pada penelitian dengan desain before-after study tersebut disimpulkan bahwa pemberian
vitamin C, bersama dengan hidrokortison dan tiamin dapat mencegah progresi disfungsi
organ dan mengurangi mortalitas pada pasien sepsis berat dan syok septik. Akan tetapi
terdapat beberapa kelemahan dari penelitian tersebut diantaranya tidak ada penyamaran,
adanya 3 intervensi sekaligus, dan besar sampel relatif kecil sehingga dapat membatasi
Sampai saat ini, belum ada hasil uji klinik terkait penggunaan vitamin C pada kasus
COVID-19. Penggunaan vitamin C untuk saat ini adalah sebagai terapi empiris berdasarkan
pengalaman para klinisi di Cina yang memberikan vitamin C intravena dengan dosis
berkisar antara 50–200 mg/kgBB sesuai tingkat keparahan penyakit. Selain itu, pemberian
vitamin C telah terbukti mencegah tertularnya infeksi saluran pernapasan akut pada
beberapa populasi khusus seperti marathon, atlit ski, dan anggota militer, namun tidak
pada populasi umum. Dari penelitian didapatkan bahwa suplementasi reguler dengan
dosis minimal 200 mg/hari dapat mengurangi durasi ISPA sebanyak 8% pada dewasa.21
Meskipun belum ada hasil uji klinik terkait vitamin C terhadap COVID-19, namun
untuk terapi pneumonia COVID-19. Untuk pneumonia COVID-19 tanpa gejala dan gejala
ringan diberikan vitamin C oral dengan dosis 100-200 mg sebanyak 3x/hari. Untuk

ginjal akut (oxalate nephropathy) pada pasien yang mendapat vitamin C dosis tinggi.
Oxalate nephropathy terjadi akibat akumulasi kalsium oksalat. Pada beberapa kasus,
kerusakan ginjal bersifat reversibel tetapi ada beberapa kasus yang terlambat ditangani
berakibat gagal ginjal kronik dan berakhir dengan kematian. Hal ini bisa disebabkan karena
tingginya konsumsi prekursor oksalat, salah satunya adalah vitamin C. Risiko lain yang
harus diwaspadai pada penggunaan vitamin C dosis tinggi adalah terjadinya hemolisis
pada pasien dengan defisiensi enzim G6PD dan iron overload pada pasien
haemochromatosis. Maka dari itu, konsumsi vitamin C pada dosisi tinggi harus
dipertimbangkan.19-24

3. METODE
Sasaran kegiatan ini adalah masyarakat pengguna Instagram, berusia> 17 tahun, dengan
jumlah target 60 orang. Kegiatan edukasi utama dilakukan secara daring melalui publikasi
artikel di website Makanmu.com. Promosi artikel edukasi ditautkan yang di profil akun
(Gambar 1).
Sebelum membaca artikel, calon pembaca diarahkan untuk mengisi tautan kuesioner
identitas dan pretest untuk mengetahui gambaran tingkat pengetahuan khalayak sasaran.
Setelah selesai mengisi pretest, partisipan akan diarahkan untuk membaca artikel edukasi
mengenai COVID-19 dan peran vitamin C pada pencegahan COVID-19 (Gambar 2).

Begitu juga pada soal nomor 9 mengenai perlu atau tidak konsumsi vitamin C pada
masa pandemi Covid-19. Pada pretest dapat dilihat paling banyak partisipan menjawab “ya,
perlu” (66 orang). Sedangkan, pada posttest didapatkan hasil berbalik, 66 orang menjawab
“tidak”. Hal ini diduga disebabkan partisipan telah membaca seksama artikel yang telah
disebarkan, dimana dijelaskan mengenai kebutuhan harian, dosis maksimal, dan sumber
utama vitamin C berasal dari buah dan sayur. Soal no 10 kuisioner pretest didapatkan 44

5. SIMPULAN
