DEFINITION
▪ BNO  Blass Nier Overzicht (Blass = Kandung Kemih, Nier = Ginjal, Overzicht=
Penelitian) = KUB (Kidney, Ureter, Bladder)
▪ IVP  Intravenous Pyelography  radiographic procedure that provides
visualization of the entire urinary tract by administration of an intravenous injection
of a water-soluble iodine-containing contrast medium
▪ Often called Intravenous Urography, Excretory Urography
▪ Radiographic study of renal parenchyma, pelvicalyceal system, ureters and urinary
bladder
 ▪ A pair of fist-sized organs located at the
ANATOMY OF KIDNEY bottom of the rib cage
▪ Organs of urinary system:
▪ Kidneys
▪ Ureters
▪ Bladder
▪ Urethra
INDICATION CONTRAINDICATION
▪ General check-up  function of kidney ▪ Iodine sensitivity or previous reaction
 filtrasi, ekskresi to contrast media
▪ Congenital anomalies involving kidney ▪ Ureum > 60 mg, creatinine >2mg, if
BNO-IVP must be held  double
▪ Suspected chronic kidney infection contrast
▪ Suspected abdominal mass lesion arising ▪ Intractable cardial or renal failure
from kidney
▪ Multiple myeloma
▪ Blunt injuries of abdomen with
hematuria ▪ Renal insufficiency
▪ Suspected obstructive uropathies like ▪ Thyrotoxicosis
stone, etc
▪ Hepatorenal syndrome
PATIENT PREPARATION
▪ Tujuan: Untuk membersihkan usus halus dan usus besar dari feses dan gas
sehingga tidak menutupi kontur ginjal/kontras dalam traktus urinarius
▪ 18.00 = satu hari sebelum dilakukan pemeriksaan, harus sudah makan terakhir
dengan hanya makan makanan lunak saja
▪ 20.00 = Makan 2 tablet Dulcolax
▪ 21.00 = Makan 2 tablet Dulcolax. Batasi asupan cairan dalam 24 jam terakhir
(±360mL)
▪ 21.30 = Mulai puasa penuh sampai periksaan rontgen dilakukan
▪ 04.30 = Dulcolax sup dimasukkan ke dalam dubur, tahan, kemudian boleh BAB
▪ 08.00-08.30 = sudah ada di bagian radiologi dan masih dalam keadaan puasa/perut
kosong
▪ Cairan dibatasi dalam 24 jam terakhir sebelum periksaan (KECUALI: renal failure, bayi, DM,
multiple myeloma)
▪ Pasien mengosongkan kandung kencing sebelum pemeriksaan dimulai
▪ Dilakukan foto BNO terlebih dahulu
▪ Pemeriksaan Lab:
▪ Ureum : < 60 mg
▪ Creatinine : < 2 mg
▪ Skin test : (-)
▪ Kontras : Urografin 76%, Hypaque 50%
▪ Dewasa: 1-2 cc/KgBB (±20cc)
▪ Anak : 0.55 cc/KgBB (max. 20 cc)

Kontras masuk vena cubiti kanan  jantung  aorta abdominalis  a. renalis  a. lobularis  a.
interlobularis  a. arcuate  a. afferent  glomerulus
PROCEDURE
▪ Pasien diminta mengosongkan kandung kemih
▪ Dilakukan foto BNO
▪ Injeksi kontras IV, beberapa saat dapat terjadi flushing, rasa asin di lidah, sakit
kepala ringan, gatal, mual/muntah

Plain film shows a pre-vesical stone on the left.
13 minutes after infusion of contrast medium
there is contrast of the right pyelon and in the
bladder, but yet no contrast of the left
pyelon. There is also contrast outlining the left
kidney, whereas it has already cleared from the
right (delayed nephrogram).
Two hours after infusion you can appreciate a
distension of the left ureter and a
hydronephrosis of the left pyelon.

POSITIONING BNO
▪ Ada 3 macam posisi:
1) Supinasi/Antero-posterior/Telentang
Sentrasi: titik garis tengah midsagittal setinggi crista iliaca
2) Upright/Erect/AP berdiri
Untuk melihat free air di subdiafragma dan untuk melihat ketinggian dari air
fluid usus
3) Lateral Decubitus
Untuk melihat adanya free air intraperitoneum dan melihat air fluid level
pada usus
TECHNIQUE PROCEDURE
▪ Scout images
▪ (1-2 minutes) Nephrogram
▪ (>3 minutes) Early and late images of the upper collecting system (abdominal
compression then applied)
▪ Tomography may be obtained, if desired
▪ (10-15 minutes) Supine, after release of compression, images of the upper
collecting system and proximal ureters
▪ (20 minute) Supine image
▪ (20 minute) Prone image
