BAB I
PENDAHULUAN
Pendidikan
dokter
adalah
pendidikan
yang
diselenggarakan untuk menghasilkan dokter yang memiliki
kompetensi untuk melaksanakan pelayanan kesehatan
primer. Pendidikan kedokteran terdiri dari dua tahap, yaitu,
tahap sarjana kedokteran dan tahap profesi dokter.
Dalam tahap profesi dokter, peserta didik melakukan
kepaniteraan klinik secara nyata di rumah sakit dan wahana
pendidikan lain yang bertindak sebagai dokter muda dengan
pengawasan bimbingan dokter spesialis di rumah sakit
(preceptor). Dalam menjalankan kepaniteraan klinik di
rumah
sakit
pendidikan,
dokter
muda
dapat
mengembangkan pembelajaran klinik secara nyata sesuai
kompetensi minimal yang harus dipenuhi oleh seorang
dokter umum.
Diberlakukannya
Standar
Pendidikan
Profesi
Dokter dan Standar Kompetensi Dokter oleh Konsil
Kedokteran Indonesia (KKI) akan memberi arahan
pendidikan profesi dokter yang lebih baik sesuai amanah
Undang-undang RI No. 29 Tahun 2004 tentang Praktik
Kedokteran.
Menyadari adanya perubahan dalam pendidikan profesi
dokter
saat
ini,
Fakultas
Kedokteran
Universitas
Muhammadiyah Palembang telah menetapkan kurikulum
dalam proses pendidikan terutama proses kepaniteraan
klinik di rumah sakit pendidikan Bagian Ilmu Penyakit Dalam
(IPD) yang akan dijalani selama 10 minggu (5 sks). Proses
pendidikan akan berlangsung di bangsal rawat inap, rawat
jalan dan instalasi gawat darurat (IGD).

Daftar Penyakit
Daftar penyakit merupakan penyakit-penyakit yang
dipilih menurut beban penyakit yang timbul berdasarkan
perkiraan data kesakitan, data kematian serta case fatality
rate di Indonesia pada tingkat pelayanan primer, tingkat
keseriusan problem yang ditimbulkan dan efeknya
terhadap individu, keluarga dan masyarakat. Lulusan
Dokter yang akan bekerja di tingkat pelayanan primer
harus mempunyai tingkat kemampuan yang memadai agar
mampu merujuk, membuat diagnosis yang tepat, memberi
penanganan awal atau memberi penanganan tuntas. Oleh
karena itu, pada setiap penyakit yang dipilih, ditetapkan
tingkat kemampuan yang diharapkan akan dicapai di akhir
pendidikan dokter berdasarkan perkiraan kewenangan
yang akan diberikan ketika bekerja di tingkat pelayanan
kesehatan primer, sesuai dengan kondisi rata-rata di
Indonesia.
Daftar penyakit dikelompokkan menurut sistem,
organ dan tahapan usia. Berikut ini tingkat kemampuan
yang diharapkan akan dicapai di akhir pendidikan
berdasarkan SKDI 2012
melakukan

Penilaian ranah keterampilan ditekankan pada penilaian
kemampuan keterampilan Shows how atau Does, yaitu
pernah melakukan atau pernah menerapkan di bawah
supervisi dan mampu melakukan secara mandiri.
Penilaian ini dilakukan dengan beberapa metode, berikut:

Komponen nilai akan diolah dalam rapat evaluasi akhir bagian, dilakukan
dalam koordinasi Bagian, Bakordik dan Koordinator Tahap Profesi UPK.
Penilaian dilakukan dengan menggunakan cara penilaian Acuan Patokan
(PAP).

TATA TERTIB PELAKSANAAN KEPANITERAAN KLINIK
A. Tata Tertib
1. Setelah melapor kepada ketua Bagian/Departemen dan
diberi penjelasan seperlunya, mahasiswa menghadap
preceptor untuk memperoleh petunjuk tentang tugas dan
kegiatan yang akan dilakukan selama menjalani
kepaniteraan

mahasiswa
menyerahkan surat pengantar dari Dekan
dengan
meyertakan paspoto ukuran 3x4 cm sebanyak 2 (dua)
buah kepada preceptor untuk dicatat namanya dan
ditempel pasfotonya di buku registrasi. Preceptor akan :
a. Menentukan judul referat yang akan disusun dan
dibacakan nanti pada waktunya
b. Memerintakan kepada ketua grup membuat daftar
giliran tugas presentasi kasus, pembacaan refrat,
diskusi kelompok dan tugas jaga untuk anggota
grupnya.
2. Mahasiswa melakukan kegiatan selama rotasi sesuai
dengan yang ditetapkan.
a. Jam Kerja : Senin Sabtu : 07.00-14.00 WIB.

DIAGNOSIS
In the majority of cases, the etiology is based on the case
history, dinical presentation, imaging techniques, and
examination of the pleural fluid.
Diagnostic approach
The presence of a pleural effusion is established only by
thoracentesis. If the effusion is small, thoracentesis can be
performed under ultrasound guidarice. Evaluation of the
pleural fluid yielda valuable diagnostic information or even
permits a dear diagnosis. The most important criteria are
appearance, protein content, and cellular components.
The appearance may be serous (light to dark, dear to
turbid), serosangulneous (blood tinged, in some cases
because of thoracentesis), hemorrhagic (bloody), purulent
(fetid odor in anaerobic effusions) or chylous (milky) bilious
(cholothorax), brown (perforated amebic abscess) black
(aspergillar infection), or yellowish green (rheumatoid
pleuritic, pancreatic effusion) appearances extremely rare.
The standard manner in which transudates have been
differentiated from exudates is by measuring the protein and
lactic dehydrogenate (LDH) levels in the pleural fluid and in
the serum. If one or more of the following three criteria are
satisfied, the patient has an exudative PE (light's criteria): 1)
Pleural fluid protein/serum protein > 0.50: 2) pleural fluid
LDH/serum LDH >0.60: 3) pleural fluid LDHAwo-third the
upper limit or normal.

Tuberculosis pleurisy
Tuberculous pleurisy mainly affects younger patients
and is twice as frequent In males than In females.
Most patients have a cough and pleuritic pain, which usually
precedes the cough. Previously, patients with a tuberculous
pleurisy were younger that those with a pulmonary tuberculosis,
but now it becomes more and more a disease of older
individuals.
Tuberculosis can be proved by granulomata in a pleural biopsy
specimen and from biopsy specimen culture. The effusion is
usually dear and serous, and seldom turbid and serous.
Predominantly, lymphocytes are present, but in the very early
stages, neutrophils may be prominent. Low glucose values are
found In only 20-50% of cases; however, the chances of a
positive Mycobacterium tuberculosis culture from the exudates
are higher in these cases.
History taking
Respiratory symptoms
Pleuritic pain, cough,, and dyspnoea are the three
cardinal symptoms in pleural disease.
A stick In the side constitutes the principal symptom of the
antients. Since the lung and the visceral pleura do not have pain
receptors, any pain witnessing a pleural involvement indicates
that the parietal pleura and/or the accompanying fascia is
Inflamed or infiltrated. The pleuritic pain does not indicate the
presence of a pleural effusion; on the contrary, it tenda to
subside when such an effusion appears.
Most frequenly a pleuritic pain has a sudden onset; it is usually
unilateral, fairly well localised, felt in the subaxillary region or
beneath the breasts. It is of a cutting, stabbing or tearing
character; it is also described as a knifelike or shooting pain. It is
definitely Intensified by deep breathing, coughing, laughing, or
sneezing, but relieved by anything that assists in immobilizing
the affected side, for instance by gentle pressure over the
affected side or by stopping breathing. The pain may be of such
a severity that it prevents sleep. In some cases (for instance in
mesothelioma), it is dull and dragging in character, or only some

Parapneumonic effusions
At the outset of pneumonia, the dinical presentation of a patient
with pneumonia alone does not differ from patients with
pneumonia and pleural effusion. Later some purulent or
haemoptoic sputum may appear, as well as general signs such a
fever, chills, headache. In case of anaerobic infection, a bad
breath or even a putrid smeel can be perceived from a distance.
The frequency of parapneumonic effusions varies with the
responsible
microorganism,
being
>50%
in
cases
if
streptococcus pneumonlae and <10% in cases of mycoplasma
pneumoniae.
Malignant effusions
If a patient is already known to suffer from a cancer, the history
will be relatively easy to take. Otherwise the available symptoms
should also be used to orientate the diagnosis. The age, sex, and
smoking history all of major importance, lung and breast cancer
being the two most frequent causes of malignant pleural
effusion
followed
by
lymphomas,
and
ovarian
and
gastrointestinal carcinomas. As the primary disease is already in
an advanced stage, weight loss, anorexia, and malaise are
commonly found.
Connective tissue diseases
Approximately 5% of patients with rheumatoid arthritis develop
pleurisy, usually several years a'l*,er the onset of articular
symptoms; it may mimic an acute or subacute disease, or be an
asymptomatic radiographical finding. Involvement of the pleura
(pleuritic pain or pleural effusion) occurs in more than one-half
of the patients with systemic lupus erythematosus during the
course of the disease (usually during an exacerbation), and is
the presenting manifestation in 5% of the cases.
