Manifestasi klinis atas lesi-lesi MLF tidak hanya terbatas berupa kelainan
horizontal saccades saja. MLF sendiri terbentuk dari berbeagai serabut-
serabut/jaras-jaras saraf yang memediasi berlangsungnya pergerakan mata ke arah
vertikal (pursuit, vestibular, and otolithic pathways); gangguan yang terjadi pada
pergerakan pandangan mata ke arah vertikal (vertical gaze) juga sering dijumpai
pada kasus-kasus INO.11,17,18 Gangguan vertical gaze yang terjadi seringkali
termanifestasi sebagai staircasing ductions yang mereda dengan dilakukannya
pergerakan horisontal. Pada pasien-pasien yang menderita INO bilateral dapat
dijumpai terjadinya gangguan vertical gaze holding yang signifikan, yang dapat
menyebabkan terjadinya primary-position atau gaze-evoked vertical nystagmus.
Berbeda halnya dengan abducting nystagmus dengan exponentially decaying slow
waveform, nistagmus vertikal pada INO memiliki pola gelombang yang khas
menyerupai gigi gergaji (typical saw-tooth waveform) yang ditimbulkan oleh
berlangsungnya mekanisme gaze holding yang tidak sufisien. Kerusakan yang
terjadi pada jaras-jaras utrikularis dalam MLF dapat menyebabkan terjadinya
ketidaksejajaran vertikal pada mata (vertical misalignment of the eyes), yang
termanifestasi dalam bentuk deviasi asimetris (skew deviation) atau
berlangsungnya full ocular tilt reaction (Gambar 5).
Pada kasus palsy nervus ke-3, keberadaan sebuah lesi fasikuler dapat
menyebabkan defisit parsial pada elevasi, depresi, aduksi, atau elevasi palpebra
pada mata ipsilateral.30 Dalam beberapa kasus yang jarang dijumpai, lesi-lesi
tersebut bersifat highly selective dan hanya menyebabkan terjadinya kelemahan
pada satu otot tunggal saja. Selain itu, terjadinya lesi diskrit (tak kentara) pada
fasikulus nervus ke-3 dapat menyebabkan kelainan yang menyerupai kasus
superior/inferior divisional nerve palsy (yang mana sering terlokalisasi pada sinus
kavernosa anterior atau orbita).32

SACCADIC ACCURACY
The accuracy of saccadic excursions is under the control of inputs from the
posterior fastigial nuclei and dorsal vermis in the cerebellum, which calibrate the
size of the saccadic pulse. Dysfunction of these pathways leads to saccadic
dysmetria; hypermetric saccades result from damage to the deep nuclei and
hypometric saccades result from damage to the vermis alone. Furthermore,
dysmetric saccades in one direction of gaze can occur from unilateral lesions.55
For example, a lesion of the inferior cerebellar peduncle (affecting the climbing
fibers) may cause contralateral hypometric saccades, which occurs because of
reduced stimulation of the ipsilateral fastigial nucleus, and consequently reduced
stimulation of the contralateral PPRF (Fig. 8). In contrast, a lesion of the Hook
bundle region near the superior cerebellar peduncle will cause contralateral
hypermetric saccades (Fig. 9).55 The reason for contralateral hypermetric
saccades is that fibers from the fastigial nucleus to th contralateral PPRF first
travel across the midline superiorly to circle the superior cerebellar peduncle
before descending to the PPRF.56
