TENTANG
KEBIJAKAN PENUNJANG PELAYANAN KLINIS PUSKESMAS

DENGAN RAHMAD TUHAN YANG MAHA ESA

KEPALA PUSKESMAS JABUNG,

Menimbang : a. bahwa pelayanan klinis Puskesmas dilaksanakan

sesuai kebutuhan pasien;
b. bahwa pelayanan klinis Puskesmas perlu
memperhatikan mutu dan keselamatan pasien;
c. bahwa untuk menjamin pelayanan klinis
dilaksanakan sesuai kebutuhan pasien, bermutu,
dan memperhatikan keselamatan pasien, maka
perlu disusun kebijakan penunjang pelayanan klinis
di Puskesmas Jabung.

Tahun 2009 tentang Kesehatan;

tentang Akreditasi Puskesmas;

5. Peraturan Menteri Kesehatan Republik Indonesia

Pelayanan Minimal Bidang Kesehatan di Kabupaten

Menetapkan : KEPUTUSAN KEPALA PUSKESMAS TENTANG

2. Permintaan Pemeriksaan Penerimaan Spesimen, Pengambilan dan
penyimpanan spesimen
Kebijakan Permintaan Pemeriksaan, Penerimaan Spesimen, Pengambilan
Dan Penyimpanan Spesimen :
a. Permintaan pemeriksaan laboratorium dengan membawa blanko
permintaan pemeriksaan dari unit pelayanan (poli umum, poli gigi, poli
kia, kb, kaber dan ugd) untuk pasien rawat inap permintaan
pemeriksaan di tulis di buku permintaan lab
b. Penerimaan spesimen harus diterima langsung oleh petugas
laboratorium, petugas laboratorium mencatat identitas pasien dan
meletakkan spesimen di meja spesimen sesuai nomor urut pasien untuk
menghindari kesalahan pengambilan
c. Pengambilan spesimen yang dilakukan di ruang sampling atau tempat
pengambilan darah laboratorium Puskesmas Jabung. Untuk pasien
rawat inap petugas laboratorium mengambil ke ruangan ruangan sesuai
dengan buku permintaan pemeriksaan laborat yang berada di ugd.
d. Pemeriksaan dilakukan dilaboratorium Puskesmas Jabung
e. Pemeriksaan dilakukan sesuai dengan SOP

3. Pelayanan diluar jam kerja

a. Petugas dapat melakukan pemeriksaan GDA (UGD, rwt inap)
b. Diluar jenis pemeriksaan tersebut ( GDA) Petugas memberikan KIE
keluarga pasien untuk dilakukan pemeriksaan laboratorium diluar
Puskesmas Jabung
c. Pengambilan sampel dilakukan di puskesmas Jabung (rawat inap dan
kaber) dan keluarga diminta mengantar sampel ke laboratorium rujukan
d. Setelah hasil keluar ,keluarga pasien diminta untuk membawa lagi ke
puskesmas dan memberikan hasil ke petugas piket ( perawat/bidan
yang saat itu jaga piket).

6. Peresepan, Pemesanan dan Pengelolaan Obat

Peresepan adalah proses pesanan atau permintaan obat tertulis dari dokter
dan dokter gigi, kepada pengelola obat di Puskesmas Tumpang untuk
menyediakan atau membuatkan obat dan menyerahkannya kepada pasien. Resep
merupakan sarana komunikasi professional antara dokter, penyedia obat dan
pasien (pengguna obat). Isi resep merupakan refleksi dari proses pengobatan.
Untuk itu, agar obat berhasil, resep harus rasional.
Kriteria resep yang tepat, aman dan rasional yaitu:
1. Tepat obat sesuai dengan diagnosis penyakitnya.
2. Tepat indikasi penyakit.
3. Tepat pemilihan obat.
4. Tepat dosis.
5. Tepat cara pemberian obat.
6. Tepat pasien.
Bahasa dalam penulisan resep menggunakan bahasa latin yang sudah
digunakan sebagai bahasa ilmu kesehatan karena bahasa latin tidak mengalami
perubahan (statis), sehingga resep obat yang ditulis dalam bahasa latin tidak akan
terjadi salah tafsir.
Penulisan resep yang baik harus lengkap dan jelas.Dalam resep untuk
pasien rawat jalan dan rawat inap di Puskesmas Tumpang harus tercantum:
1. Tanggal penulisan resep.
2. Nama pasien.
3. Umur pasien.
4. Alamat pasien.
5. Tanda R/ pada bagian kiri setiap penulisan obat.
6. Nama obat, jumlah dan dosis obat yang diberikan per oral.
7. Nama terang petugas penulis resep.
8. Kode Poli Umum, Poli Gigi, Poli KIA/Kaber, UGD/Rawat Inap
9. Kode pasien Umum, Askes atau JKM (BPJS)

A. PEMESANAN OBAT
Sumber penyediaan obat di Puskesmas Tumpang berasal dari Dinas
Kesehatan Kabupaten Malang.Obat yang diperkenankan untuk disediakan di
Puskesmas Tumpang adalah obat – obat yang tercantum dalam DOEN yang telah
ditetapkan oleh Menteri Kesehatan.
Permintaan obat untuk mendukung pelayanan obat di Puskesmas Tumpang
diajukan petugas pengelola obat yang telah ditunjuk (SK Petugas Penanggung
Jawab dan Pengelola Obat Puskesmas Tumpang) kepada Kepala Dinas
Kesehatan Kabupaten Malang dengan menggunakan format LPLPO, sedangkan
permintaan dari sub unit ke Petugas Pengelola Obat dilakukan secara periodic
menggunakan LPLPO sub unit.
Tujuan dari permintaan obat adalah untuk memenuhi kebutuhan obat di
Puskesmas Tumpang sesuai dengan pola penyakit yang ada di wilayah
Kecamatan Tumpang.
Kegiatan – kegiatan yang dilaksanakan dalam permintaan obat antara lain:
1. Menentukan jenis permintaan obat
a. Permintaan Rutin
Dilakukan sesuai dengan jadwal yang disusun oleh Dinas Kesehatan
Kabupaten Malang untuk Puskesmas Tumpang.
 b. Permintaan Khusus
Dilakukan di luar jadwal distribusi rutin apabila:
 Kebutuhan meningkat
 Terjadi kekosongan
 ada KLB atau Bencana
2. Menentukan jumlah permintaan obat
Data yang diperlukan antara lain:
a. Data pemakaian obat periode sebelumnya.
b. Jumlah kunjungan resep.
c. Jadwal distribusi obat dari UPTD Gudang Farmasi Kabupaten Malang.
d. Sisa Stok.
3. Menghitung kebutuhan obat dengan cara:

Q = SK + SP + (WT x D) – SS
Keterangan:
Q = jumlah obat yang dipesan
SK = stok kerja
SP = stok pengaman
WT = waktu tunggu
SS = sisa stok
D = pemakaian rata – rata per minggu/ per bulan

Stok Kerja Pemakaian rata – rata periode distribusi.

Waktu Tunggu Dihitung mulai dari permintaan obat oleh Puskesmas
Tumpang sampai dengan penerimaan obat di Puskesmas
Tumpang.
Stok Pengaman Persediaan obat untuk mengantisipasi terjadinya
peningkatan kunjungan, keterlambatan kedatangan obat.
Besarnya ditentukan berdasarkan kesepakatan antara
Puskesmas dan Gudang Farmasi Dinas Kesehatan
Kabupaten Malang.
Sisa Stok Sisa obat yang masih tersedia di Puskesmas Tumpang pada
akhir periode distribusi.

B. PENGELOLAAN OBAT
Obat dan perbekalan kesehatan hendaknya dikelola secara optimal untuk
menjamin tercapainya tepat jumlah, tepat jenis, tepat penyimpanan, tepat waktu
pendistribusian, tepat penggunaan dan tepat mutunya di tiap unit pelayanan
kesehatan.
Pengelolaan obat publik dan perbekalan kesehatan meliputi kegiatan:
1. Perencanaan dan permintaan,
2. penerimaan,
3. penyimpanan dan distribusi,
4. pencatatan dan pelaporan.
5. supervisi dan evaluasi pengelolaan obat.

7. Penggunaan Obat Yang Dibawa sendiri oleh Pasien / Keluarga

Untuk memberikan perlindungan kepada pasien dan masyarakat dalam
memperoleh sediaan farmasi dan jasa kefarmasian di Puskesmas Tumpang :
 1. Memberikan kebijakan dan prosedur penggunaan obat-obatan pasien
rawat inap yang resepnya dibawa dan diambil sendiri oleh
pasien/keluarga pasien;
2. Menunjuk Dokter dan petugas farmasi melakukan evaluasi dan
pengawasan kepada pasien dalam mengkonsumsi obat apabila terjadi
kejadian yang tidak diinginkan segera melapor ke petugas.

8. Penanganan Obat Kadaluarsa / Rusak

Tujuan dilaksanakannya penanganan obat rusak adalah untuk melindungi
pasien dari efek samping penggunaan obat rusak/kedaluwarsa.
Dalam menangani obat rusak/kedaluwarsa, maka langkah – langkah yang
harus dilakukan adalah:
a. Petugas pengelola obat mengumpulkan obat rusak dalam gudang
obat .
b. Obat yang rusak/kadaluwarsa dikurangkan dari catatan sisa stok
pada Kartu Stok oleh petugas pengelola obat.
c. Petugas pengelola obat melaporkan obat rusak/kadaluwarsa kepada
Kepala Puskesmas.
d. Kepala Puskesmas melaporkan dan mengirimkan kembali obat
rusak/kadaluwarsa kepada Kepala Dinas Kesehatan Kabupaten
Malang.

4. Pelayanan rekam medis dan metode identifikasi
Tata cara Identifikasi Pasien
a. Pasien Baru menuju loket 2
b. Petugas menanyakan kepada pasien, apakah dalam satu keluarga
sudah pernah berobat di Puskesmas Tumpang atau belum. Jika ada
yang pernah, petugas mencari nama Kepala Keluarga di Bank
Nomor Rekam Medis. Jika belum ada, petugas mendata pasien
sebagai pasien baru
c. Petugas memberikan nomor Rekam Medis sesuai urutan nomor
selanjutnya di Bank Nomor Rekam Medis
d. Petugas membuatkan Kartu Induk Berobat (KIB) yang tertulis nama
Kepala Keluarga
e. Petugas membuatkan berkas Rekam Medis
f. Petugas meregister data pasien di register Rawat Jalan
Rekam medis pasien diidentifikasi dengan cara penomoran sebagai
berikut:
a. Memakai Nomor Register / Nomor Rekam Medis
b. Satu KK satu Nomor ( Family Folder ) dengan Nomor Anggota dalam
keluarga 01 KK 02 Istri 03 Anak Pertama, dst.

dalam satu kesatuan baik catatan kunjungan poliklinik maupun catatan

1. Kondisi fisik bangunan dan lingkungan puskesmas wajib dipantau secara
rutin.
2. Prasarana puskesmas, yang meliputi air, listrik, genset, gas O2, gas LPG,
IPAL harus dipantau secara periodik, dipelihara, dan diperbaiki dan
dipastikan berfungsi.
3. Hasil pemantauan, pemeliharaan, dan perbaikan harus didokumentasikan.
4. Bahan dan limbah berbahaya harus diidentifikasi, disimpan dengan benar,
dimonitor penyimpanan dan penggunaannya, dan ditindak lanjuti.
5. Disusun program menjamin lingkungan puskesmas yang aman meliputi:
perencanaan, pelaksanaan, pendidikan dan pelatihan, pemantauan dan
evaluasi.
6. Disusun program pemeliharaan peralatan, meliputi perencanaan,

1. Pola ketenagaan sdm klinis harus disusun berdasar analisis kebutuhan

3. Evaluasi kinerja tenaga klinis harus dilakukan secara berkala paling lambat

5. Tiap tenaga klinis harus mempunyai uraian tugas dengan kejelasan
kewenangan klinis untuk masing-masing petugas.
