Fraktur Scapular: Apa yang perlu diketahui
oleh ahli radiologi

OBJEKTIF. Tujuan dari artikel ini adalah untuk meninjau anatomi dan fungsi
scapular,menjelaskan gambaran dari trauma skapular, dan mendiskusikan peran
pencitraan diagnostik dalam pengambilan keputusan klinispada trauma bahu.

Fraktur skrapular jarang terjadi, terhitung hanya 3-5% dari fraktur sendi
bahu dan tidak sampai 1% dari seluruh fraktur [1].Trauma energi tinggi adalah
penyebab yang paling umum, fraktur skrapular sering dikaitkan dengan luka akut
lainnya, termasuk fraktur costa(53%), contusio paru (47%), trauma capitis(39%),
fraktur vertebra (29%), dan fraktur klavikula(25%) [2]. Diagnosis awal fraktur
skapular sering tertunda atau diabaikan, karena pada pasien dengan trauma berat,
perawatan klinisbiasanya difokuskan pada resusitasi keadaan yang mengancam
jiwa [3-5].
ANATOMI
BIOMEKANIKAL NORMAL
Latar Belakang
Fungsi skapula pada sendi bahu sebagai dasar dari pergerakan dan
stabilitasor dalam hubungannya dengan kompleks suspensori bahu superior, sendi
scapulothoracic, glenohumeral, dan acromioclavicular. Persendian ini
menghubungkan antara thoraks dan ekstremitas atas [7]. Kompleks suspensori
bahu superior terdiri dari tulang dan cincin ligamen yang dibentuk oleh skapula,

klavikula distal, sendi acromioclavicular, dan ligamen coracoclavicular. Glenoid,
coracoid, dan akromion merupakan bagiandari skapular[8].Kompleks suspensori
bahu superior, yang merupakan gabungan dari otot-otot scapulothoracic, bertindak
sebagai penopang ekstremitas atas dari toraks.Skapula, dalam hubungannya
dengan kompleks suspensori bahu superior, tergantung dari klavikula oleh sendi
acromioclavicular dan ligamentum corakoklavikular [9].

meningkatkan jangkauan gerak sendi bahu daripada yang disediakan oleh sendi
glenohumeral sendiri [6, 16].

teres major, dan otot utama pectoralis [10].

glenoid, dan scapular neck [28-31]. CT bahu khusus tidak diperlukan pada
keadaan trauma, karena reformat ulang gambar CT 2D dan 3D umumnya
diperoleh dari CT scan dada yang didapat pada pemeriksaan. Reformat ulang
gambar CT juga memungkinkan pembuatan gambar sketsa 3D yang sesuai dengan
tampilan radiografi skapula Y, Grashey, dan anteroposterior [28,32] (Gambar 3).
Selain itu, pemeriksaan CT 3D mempunyai kelebihan dapat mengurangi artefak
yang diproduksi oleh tubuh pasien, posisi pasien, dan teknik pencitraan [28].

Dislokasi bahu anterior adalah mekanisme tambahan yang berhubungan
dengan fraktur intraartikular glenoid anterior [40] (Gbr.5). Fraktur Ideberg tipe 1
dari glenoid ini adalah fraktur scapular yang paling sering ditemui bersamaan
dengan dislokasi bahu, dengan kejadian dua pertiga dari fraktur tipe 1 [33].

terjadi sendiri atau dalam kombinasi dengan cedera yang lainnya, baru-baru ini
menantang dogma tersebut (Lampiran 2). ORIF fraktur skrapular ekstraarticular
telah disebut-sebut dapat mengurangi nyeri jangka panjang, kelemahan, dan
kecacatan fungsional[4, 8, 9, 41-43]; Namun demikian, indikasi relatif untuk
fraktur skrapular ekstraarticular tetap kontroversial [1].
Prosesus Coracoid
Fraktur prosesus corasoid mewakili 2-13% dari fraktur skapular [37, 42,
44]. Fraktur ini paling sering terjadi di pangkal dengan dislokasi minimal [37, 45,
46]. Beberapa mekanisme dapat mengakibatkan fraktur prosesus coracoid
termasuk trauma tumpul langsung atau trauma tidak langsung dari dislokasi bahu
[47-49] (Gbr.6). Fraktur prosesus coracoid yang terjadi dengan dislokasi anterior
sering diabaikan pada radiografi [48]. Cedera tambahan meliputi fraktur klavikula
ipsilateral, avulsi pada perlekatan tendon, atau avulsi pada perlekatan ligamen
coracoclavicular ketika terjadi pemisahan sendi acromioclavicular [50-54].

Prosesus Acromion
Fraktur prosesus acromion mewaliki 8-16% dari fraktur skrapular [3, 44,
58]. Kecelakaan kendaraan bermotor adalah penyebab paling umum[37]. Selain
trauma tumpul langsung, mekanisme lainnya terjadi fraktur prosesus akromion
meliputi trauma tidak langsung setelah dislokasi bahu dan avulsio oleh otot
deltoid [37,46].

malalignment rotasi dari glenoid dengan sumbu anteroposterior tegak lurus
terhadap bidang skapula [4, 32]. Sudut glenopolar normal adalah 30-45 °, dan
sudut glenopolar 20-22 °, dengan atau tanpa kombinasi dengan cedera sendi bahu
yang lainnya, digunakan sebagai indikasi relatif untuk manajemen operasi untuk
menghindari rasa nyeri jangka panjang, kelemahan, dan pengurangan kapasitas
untuk aktivitas sehari-hari [1, 4, 38, 41]. Sudut glenopolar dibentuk oleh
persimpangan dua garis: baris pertama ditarik melalui kutub superior dan inferior
dari fosa glenoid, dan yang kedua ditarik melalui puncak kutub superior fosa
glenoid ke sudut inferior skapula.

akromion, atau proses coracoid atau gangguan sendi acromioclavicular dan
ligamentum coracoclavicular[7, 8] (Gambar 10 dan 11). Gangguan ganda
kompleks suspensori bahu superior adalah tidak terisolasi; Misalnya fraktur
klavikula atau pemisahan sendi acromioclavicular yang lengkap dapat
memperburuk fraktur dislokasi skapula, atau sebaliknya [8, 9]. Diskinesis
skapular terjadi pada disasosiasi clavicular dan skapular setelah fraktur klavikula
atau pemisahan sendi akromioklavikular [65, 66].

fisik [1]. Tujuan intervensi bedah pada floating shoulder injury adalah mengurangi
pola fraktur yang tidak stabil, mendukung program awal rehabilitasi fisik, dan
mencegah defisit fungsional jangka panjang[58, 69].
