dinding uterus anterior dan posterior tidak dijahit menyatu. Dari sekitar 1300
kasus perdarahan postpartum yang ditatalaksana dengan B-Lynch, hanya 19
kasus yang gagal, rata-rata penyebabnya adalah terlambat penanganan, teknik
yang tidak sesuai prosedur, defibrinasi, dan pemilihan material yang tidak
tepat. Wolmuth, dkk. menyatakan bahwa tingkat kesuksesan B-Lynch
mencapai 91%.5,6
Beberapa komplikasi B-Lynch yang pernah dilaporkan antara lain nekrosis
iskemik parsial (1 hari setelah dilakukan B-Lynch)7, nekrosis uteri (12 hari
setelah dilakukan B-Lynch)8, pyometra yang akhirnya ditatalaksana dengan
histerektomi9, erosi SBR 6 minggu postpartum yang menyebabkan defek
dinding anterior uterus, sinekia dan perlengketan uterus10, fistula umbilikalis
dan nekrosis miometrium12, dan hematometra13. Sinekia yang terjadi
dihubungkan dengan penggunaan materi jahit yang lama diserap jaringan.
Ketika uterus mengalami involusi yang lebih cepat dibandingkan penurunan
tensile strength benang, maka kerusakan jaringan dapat terjadi. 10 Lynch
menggunakan jarum besar dan nontraumatik, serta benang chromic catgut no
2 yang terabsorbsi cepat sehingga tidak menimbulkan sinekia dan menjerat
usus.5 Penjahitan terlalu kuat akan menyebabkan pengurangan suplai darah
ekstrim ke uterus dan terhambatnya retraksi spontan uterus, sehingga dapat
terjadi nekrosis uterus. Sebaliknya, jahitan yang terlalu lemah akan
menyebabkan kompresi yang tidak adekuat. Meskipun demikian, komplikasi
akibat B-Lynch termasuk jarang terjadi dan Geznig, dkk. melaporkan hasil
histerosalpingogram dari 8 kasus B-Lynch memberikan hasil normal tanpa
defek pada kavum uteri.14

Metode Surabaya
Sulistyono, dkk. mempublikasi teknik Surabaya pada tahun 2010. Dari 12
kasus perdarahan postpartum antara Juli 2007- Agustus 2008 di RS Dr.
Soetomo, 4 kasus ditatalaksana dengan B-Lynch dan 8 kasus dengan metode
Surabaya. Teknik ini dilakukan dengan membuat 3 jahitan longitudinal
pararel menggunakan chromic catgut no 2 dan jarum lengkung yang dibuat
lurus secara manual. Langkah-langkah teknik ini yaitu: 28
I. RINGKASAN
1. Bila perdarahan postpartum disebabkan oleh perdarahan dari placental bed,
maka dapat dilakukan teknik kompresi Cho. Bila perdarahan disebabkan oleh
atonia uteri dan SBR tidak intak lagi, dapat dilakukan teknik B-Lynch. Bila
SBR masih intak, modifikasi B-Lynch dengan teknik Hayman atau metode
Surabaya dapat dilakukan.
2. Prevalensi infertilitas paling banyak ditemukan pada SWUD, diikuti
kemudian dengan B-Lynch. Prevalensi infertilitas paling sedikit ditemukan
pada BIL. Sehingga beberapa penelitian merekomendasikan BIL
dibandingkan SWUD dan B-Lynch sebagai pilihan pertama pada wanita muda
yang masih menginginkan fertilitas.
