2.1 definisi
Istilah sarcopenia pertama kali diperkenalkan oleh Rosenberg pada tahun 1989
setelah memikirkan bahwa keadaan penurunan dan kehilangan massa otot pada lansia
tidak terlalu diperhatikan. Istilah ini berasal dari Bahasa Yunani, yaitu sarx (otot) dan
penia (kehilangan), sehingga sarcopenia berarti kehilangan massa otot (malafarina et
al., 2012)
Sarcopenia adalah penyakit dengan karakteristik penurunan massa otot disertai
dengan penurunan fungsi secara progresif yang terjadi seiring dengan pertambhan
usia (chen et al., 2014). Kecepatan penurunan massa otot yang terjadi pada sarcopenia
sesuai dengan proses penuaan. Pada usia 30-70 tahun, massa otot akan berkurang 6%
per decade dan proses akan lebih cepat saat memasuki usia 60 tahun ke atas, yaitu 1,4
– 2,5 % pertahun )lee et al., 2013). Kehilangan massa otot secara progresif tersebut
merupakan salah satu perubahan normal dari tubuh. Perubahan normal yang terjadi
adalah akibat dari kehilangan unit motoris otot, penurunan kaar hormone dan
peningkatan sensitivitas insulin yang berujung pada proteolysis, penurunan sintesis
protein otot dan pertambahan komponen lemak otot. Perubahan – perubahan tersebutt
merupakan proses fisiologis yang terjadi seiring pertambahan usia dan pada akhirnya
dapat berujung pada berkurangnya kemampuan fisik diantaranya adalah peningkatan
risiko jatuh dan fraktur, gangguan mobilitas dan kualitas hidup yang rendah disertai
juga dengan meningkatnya angkat mobiditas dan mortalitas lansia (smoliner., 2014)
Sarcopenia umumnya dihubungkan dengan penyakit penuaan pada kalangan
lansia, tetapi sebenarnya sarcopenia tidak hanya terbatas pada kalangan ini.
Pernyataan tersebut berdasarkan pada prevalensi sarcopenia yang lebih tinggi pada
populasi usia tua dan akan semakin meningkat seiring dengan bertambahnya usia
(smoliner., 2014)
2.2 prevalensi
Prevalensi sarcopenia meningkat secara dramatis dengan usia 4% laki - laki dan 3%
dari wanita berusia 70-75 sampai 16% pria dan 13% wanita berusia 85 atau lebih tua.
(DerSarkissian, 2020) Sarcopenia biasanya mempercepat sekitar usia 75 tahun
-meskipun mungkin terjadi pada orang usia 65 atau 80 - dan merupakan factor
terjadinya kelemahan dan kemungkinan jatuh dan patah tulang pada orang dewasa
yang lebih tua. Angka tersebut menunjukkan perbandingan yang lebih besar pada
jenis kelamin laki-laki dibanding wanita dan pada pasien sarcopenik yang disertai
obesitas ("kelebihan lemak") akan berdampak lebih buruk disbanding sarcopenik non-
obesitas. (Cruz-Jentoft AJ, Baeyens JP, Bauer JM, et al. 2010)
Ada variabilitas yang signifikan dalam prevalensi sarcopenia yang dilaporkan. Sebuah studi
baru-baru ini tentang orang dewasa yang tinggal di komunitas (usia rata-rata 67 tahun) di
Inggris ditemukan prevalensi Sarcopenia menjadi 4,6% pada pria dan 7,9% pada wanita yang

mengukur massa tubuh tanpa lemak), dan 7,1-98,0 % pada pria dan 19,8-88,0% pada wanita
(diukur dengan analisis impedansi bioelektrik) (9). pada penelitian kohort dari 2.867 orang
dewasa lanjut usia yang tinggal di komunitas (usia> 65 tahun) di Taiwan, prevalensi
sarcopenia bervariasi dari 3,9% menjadi 7,3% dengan prevalensi mencapai 13,6% di antara
pria yang lebih tua berusia 75 tahun ke atas (10). Banyak perbedaan dalam perkiraan ini
mungkin karena kurangnya kriteria yang seragam untuk mendiagnosis sarcopenia.
2.5 Klasifikasi
Categories of sarcopenia and sarcopenialike conditions
Primary and secondary sarcopenia
In some individuals, sarcopenia is largely attributable to ageing;
in many cases, other causes can be identified. Thus, the categories of primary sarcopenia and
secondary sarcopenia may be
useful in clinical practice (Figure 4) [1]. Sarcopenia is considered
‘primary’ (or age-related) when no other specific cause is evident, while sarcopenia is
considered ‘secondary’ when causal
factors other than (or in addition to) ageing are evident.
Sarcopenia can occur secondary to a systemic disease, especially
one that may invoke inflammatory processes, e.g. malignancy or
organ failure. Physical inactivity also contributes to development
of sarcopenia, whether due to a sedentary lifestyle or to diseaserelated immobility or
disability [132]. Further, sarcopenia can
develop as a result of inadequate intake of energy or protein,
which may be due to anorexia, malabsorption, limited access to
healthy foods or limited ability to eat.
Acute and chronic sarcopenia
EWGSOP2 newly identifies subcategories of sarcopenia as
acute and chronic. Sarcopenia that has lasted less than 6
months is considered an acute condition, while sarcopenia lasting ≥6 months is considered a
chronic condition. Acute sarcopenia is usually related to an acute illness or injury, while
chronic sarcopenia is likely to be associated with chronic and
progressive conditions and increases the risk of mortality. This
distinction is intended to underscore the need to conduct periodic sarcopenia assessments in
individuals who may be at risk
for sarcopenia in order to determine how quickly the condition
is developing or worsening. Such observations are expected to
facilitate early intervention with treatments that can help prevent
or delay sarcopenia progression and poor outcomes
Kategori Sarcopenia
European Working Group on Sarcopenia in Older People (EWGSOP) membagi
sarcopenia
menjadi presarcopenia, sarcopenia dan sarcopenia berat.3,6
Tahap presarcopenia
Ditandai dengan massa otot yang rendah tanpa dampak pada kekuatan otot atau
kinerja
fisik. Tahap ini hanya dapat diidentifikasi dengan teknik mengukur massa otot secara
akurat dan mengacu pada populasi standar.
Sarcopenia
Ditandai dengan rendahnya massa otot, ditambah kekuatan otot rendah atau kinerja
fisik
yang rendah.
Sarcopenia berat
Diidentifikasi ketika ketiga kriteria definisi terpenuhi (massa otot yang rendah,
kekuatan
otot yang rendah dan kinerja fisik yang rendah).

2.6 diagnostik ditambah punya Erika
2.6.1 pengukuran massa otot
Muscle quantity Muscle quantity or mass can be estimated
by a variety of techniques, and there are multiple methods
of adjusting the result for height or for BMI [46, 69, 70].
Muscle quantity can be reported as total body Skeletal
Muscle Mass (SMM), as Appendicular Skeletal Muscle Mass
(ASM), or as muscle cross-sectional area of specific muscle
groups or body locations.
Magnetic resonance imaging (MRI) and computed tomography (CT) are considered to be
gold standards for noninvasive assessment of muscle quantity/mass [64].
However, these tools are not commonly used in primary
care because of high equipment costs, lack of portability,
and the requirement for highly-trained personnel to use the
equipment [64]. Moreover, cut-off points for low muscle
mass are not yet well defined for these measurements.
Dual-energy X-ray absorptiometry (DXA) is a more
widely available instrument to determine muscle quantity
(total body lean tissue mass or appendicular skeletal muscle
mass) non-invasively, but different DXA instrument brands
do not give consistent results [31, 32, 71]. DXA is presently
favored by some clinicians and researchers for measuring
muscle mass [31]. Fundamentally, muscle mass is correlated
with body size; i.e. individuals with a larger body size normally have larger muscle mass.
Thus, when quantifying
muscle mass, the absolute level of SMM or ASM can be
adjusted for body size in different ways, namely using
height squared (ASM/height2), weight (ASM/weight) or
body mass index (ASM/BMI) [72]. There is an ongoing
debate about the preferred adjustment and whether the
same method can be used for all populations.
An advantage of DXA is that it can provide a reproducible estimate of ASM in a few minutes
when using the
same instrument and cut-off points. A disadvantage is that
the DXA instrument is not yet portable for use in the community, as needed for care in
countries that favor ageing-inplace. DXA measurements can also be influenced by the
hydration status of the patient.
Bioelectrical impedance analysis (BIA) [62] has been
explored for estimation of total or ASM. BIA equipment
does not measure muscle mass directly, but instead derives
an estimate of muscle mass based on whole-body electrical
conductivity. BIA uses a conversion equation that is calibrated with a reference of DXA-
measured lean mass in a
specific population [49, 73–75]. BIA equipment is affordable,
widely available and portable, especially single-frequency
instruments. Since estimates of muscle mass differ when different instrument brands and
reference populations are used,
we advise use of raw measures produced by the different
devices along with the cross-validated Sergi equation for
standardisation [74, 76]. BIA prediction models are most
relevant to the populations in which they have been derived,
and the Sergi equation is based on older European populations. Age, ethnicity and other
related discrepancies between
those populations and patients should be considered in the
clinic. In addition, BIA measurements can also be influenced
by hydration status of the patient. For affordability and portability, BIA-based determinations
of muscle mass may be
preferable to DXA; however, more study is necessary to validate prediction equations for
specific populations [75, 77].
As stated previously, muscle mass is correlated with
body size, so SMM or ASM can be adjusted for body size
in different ways, i.e. using height squared (ASM/height 2),

weight (ASM/weight) or body mass index (ASM/BMI)
[72]. The authors make no recommendation to adjust for
body size, but adjustment can be made if data are available
for a relevant normative population.
Although anthropometry is sometimes used to reflect
nutritional status in older adults, it is not a good measure of
muscle mass [78]. Calf circumference has been shown to
predict performance and survival in older people (cut-off
point <31 cm) [79]. As such, calf circumference measures
may be used as a diagnostic proxy for older adults in settings where no other muscle mass
diagnostic methods are
available.

Working Group on Sarcopenia in Older People merekomendasikan Dual energy
X-ray Absorptiometry (DXA), Computed Tomography (CT), Magnetic Resonance
Imaging (MRI), dan Bioimpedance Analysis (BIA) untuk penelitian
sarkopenia.16,21
Pemeriksaan BIA dapat memperkirakan massa lemak dan massa otot.
Pemeriksaan ini cukup murah, mudah dilakukan, pemprosesan cepat, dan non
invasif, sehingga penggunaan BIA cocok untuk penilaian sarkopenia komunitas.
Meskipun akurasi BIA dalam diagnosis sarkopenia telah divalidasi, namun itu
sangat tergantung pada suhu, kelembaban, kondisi kulit, dan lain-lain. Perkiraan
hasil yang didapat pada BIA sangat dekat dengan hasil MRI.4 Nilai cut-off yang
dianggap memiliki massa otot yang rendah sesuai rekomendasi EWGSOP2 adalah
< 7,0 kg/m2 untuk laki-laki dan <5,5 kg/m2 untuk perempuan.5
2.6.2 pengukuran kekuatan otot
Muscle strength Measuring grip strength is simple and inexpensive. Low grip strength is a
powerful predictor of poor
patient outcomes such as longer hospital stays, increased
functional limitations, poor health-related quality of life and
death [28, 29]. Accurate measurement of grip strength
requires use of a calibrated handheld dynamometer under
well-defined test conditions with interpretive data from
appropriate reference populations [41]. Grip strength correlates moderately with strength in
other body compartments,
so it serves as a reliable surrogate for more complicated
measures of arm and leg strength. Because of its ease of
use, grip strength is advised for routine use in hospital practice, in specialty clinical settings,
and in community healthcare [28, 29, 62–64]. The Jamar dynamometer is validated
and widely used for measuring grip strength, although use
of other brands is being explored [65]. When measurement
of grip is not possible due to hand disability (e.g. with
advanced arthritis or stroke), isometric torque methods can
be used to measure lower limb strength [66].
The chair stand test (also called chair rise test) can be
used as a proxy for strength of leg muscles (quadriceps muscle group). The chair stand test
measures the amount of time
needed for a patient to rise five times from a seated position
without using his or her arms; the timed chair stand test is a
variation that counts how many times a patient can rise and
sit in the chair over a 30-second interval [64, 67, 68]. Since
the chair stand test requires both strength and endurance,
this test is a qualified but convenient measure of strength

2.6.3 pengukuran kemampuan fungsional
Physical performance Physical performance has been defined
as an objectively measured whole-body function related to
locomotion. This is a multidimensional concept that not
only involves muscles but also central and peripheral nervous function, including balance
[80]. Physical performance
can be variously measured by gait speed, the Short Physical
Performance Battery (SPPB), and the Timed-Up and Go
test (TUG), among other tests. It is not always possible to
use certain physical performance measures, such as when a
patient’s test performance is impaired by dementia, gait disorder or a balance disorder.
Gait speed is considered a quick, safe and highly reliable
test for sarcopenia, and it is widely used in practice [81].
Gait speed has been shown to predict adverse outcomes
related to sarcopenia—disability, cognitive impairment,
need for institutionalisation, falls and mortality [82–85]. A
commonly used gait speed test is called the 4-m usual walking speed test, with speed
measured either manually with a
stopwatch or instrumentally with an electronic device to
measure gait timing [86, 87]. For simplicity, a single cut-off
speed ≤0.8 m/s is advised by EWGSOP2 as an indicator
of severe sarcopenia.
The SPPB is a composite test that includes assessment
of gait speed, a balance test, and a chair stand test [88]. The
maximum score is 12 points, and a score of ≤ 8 points indicates poor physical performance
[1, 64].
The TUG evaluates physical function. For the TUG
test, individuals are asked to rise from a standard chair,
walk to a marker 3 m away, turn around, walk back and sit
down again [89].
The 400-m walk test assesses walking ability and endurance. For this test, participants are
asked to complete 20
laps of 20 m, each lap as fast as possible, and are allowed
up to two rest stops during the test.
Each of these physical performance tests (gait speed,
SPPB, TUG, 400-m walk) can be performed in most clinical settings. In terms of its
convenience to use and ability
to predict sarcopenia-related outcomes, gait speed is
advised by EWGSOP2 for evaluation of physical performance [67]. The SPPB also predicts
outcomes [90], but it is
more often used in research than in clinical assessment
because the battery of tests takes at least 10 min to administer. Likewise, the 400-m walk test
predicts mortality but
requires a corridor more than 20 m long to set up the
testing course [91]. The TUG has also been found to predict mortality [92].
Alternative or new tests and tools
A variety of methods are being used or evaluated to determine the quantity and quality of
muscle and impact of sarcopenia on the patient’s QoL. These diagnostic measures
are being tested for validity, reliability and accuracy and may
play a relevant role in the future. For use in practice, tools
need to be cost-effective, standardised and repeatable by
practitioners in a variety of clinical settings and across different patient populations [78, 93].
Lumbar 3rd vertebra imaging by computed tomography
For patients with cancer, computed tomography (CT) has
been used to image tumors and their response to treatment,
and this technique has also been shown to give practical and
precise measures of body composition. In particular, CT
images of a specific lumbar vertebral landmark (L3) correlated significantly with whole-body
muscle [94, 95]. As a
result, this imaging method has been used to detect low muscle mass, even in patients with
normal or high body weights,
and it can also predict prognosis [96, 97]. L3-CT imaging is
not limited to patients with cancer; this parameter has been
used as a predictor of mortality and other outcomes in the
intensive care unit [98] and in those patients affected by liver
disease [99]. Quantification of lumbar L3 cross-sectional area
has also been done by MRI [42].
With ever-increasing needs to quantify muscle and
detect sarcopenia in early stages, high-resolution imaging is
expected to be more widely used in the future—initially in
research studies, and ultimately in clinical practice.
Mid-thigh muscle measurement
Mid-thigh imaging (by MRI or CT) has also been used in
research studies, as it is a good predictor of whole-body
skeletal muscle mass and very sensitive to change [50, 94,
96, 100]. Mid-thigh muscle area is more strongly correlated
with total body muscle volume than are lumbar muscle
areas L1–L5 [42].
Psoas muscle measurement with computed tomography
CT-based measurement of the psoas muscle has also been
reported as simple and predictive of morbidities in certain
conditions (cirrhosis, colorectal surgery) [101, 102]. However,
because psoas is a minor muscle, other experts argue that it
is not representative of overall sarcopenia [103, 104]. Further
studies are needed to verify or reject use of this method.
2.7 prognosa
Berdasarkan peningkatan kesadaran sarcopenia pada orang tua dan meluasnya
penggunaan alat
untuk skrining dan penilaian, dan tujuan utamanya adalah untuk latihan fisik,
mengidentifikasi
strategi diet, perubahan gaya hidup dan pengobatan dapat mencegah keparahan dari
sarkopenia
dan atau menunda timbulnya sarcopenia.
2.8. diagnosis banding
1. Cachexia
Cachexia dari Yunani Kakos "buruk" dan hexis "kondisi") atau wasting syndrome
adalah
penurunan berat badan yang dramatis dan otot atrofi terlihat pada pasien dengan
penyakit
kronis termasuk diabetes tipe I, multiple sclerosis, HIV, kanker, dan pada individu
dengan
'gagal tumbuh' terkait usia sindrom. Berat badan di penyakit berat itu tidak disengaja,
dan
merupakan proses patologis yang menyertai penyakit utama. 8,10
Cachexia menyebabkan kerugian dramatis otot rangka dan akhirnya menyebabkan
imobilitas.
Pada pasien kanker, cachexia dikaitkan dengan respons penurunan terapi dan
prognosis yang
buruk. Mekanisme yang tepat yang menyebabkan cachexia tidak diketahui, dan
meskipun
ada banyak obat dalam pengembangan untuk mengobati cachexia, mereka tidak
diarahkan
pada jalur umum. Pengobatan yang ditujukan untuk mengganggu atau membalikkan
cachexia
sangat dibutuhkan untuk meningkatkan kelangsungan hidup dan meningkatkan
kualitas
hidup pasien dengan penyakit kronis.8,10
Sekitar 50% dari semua pasien kanker menderita cachexia. Mereka yang pencernaan
bagian
atas dan kanker pankreas memiliki frekuensi tertinggi mengembangkan gejala
cachexic.
Angka ini meningkat sampai 80% pada pasien kanker terminal. Di samping
meningkatkan
morbiditas dan mortalitas, memperparah efek samping dari kemoterapi, dan
mengurangi
kualitas hidup, cachexia dianggap sebagai penyebab langsung kematian sebagian
besar
pasien kanker , mulai dari 22% sampai 40% dari pasien.9,10
Gejala cachexia kanker termasuk penurunan berat badan progresif dan menipisnya
cadangan
sejumlah jaringan adiposa dan otot rangka. Cachexia harus dicurigai jika berat badan
paksa
lebih dari 5% dari berat premorbid terjadi dalam jangka waktu enam bulan.8,9,10
2. Myopati
Secara sederhana miopati diartikan sebagai penyakit otot (dalam bahasa yunani
mio=otot,
sementara pati=menderita). Artinya kelainan primernya terjadi pada otot, bukan pada
saraf
(neuropati atau gangguan neurogenik) atau yang lain (otak dan sebagainya). Miopati
adalah
penyakit otot di mana serabut otot tidak dapat berfungsi normal, akibatnya otot
mengalami

kelemahan atau kelumpuhan, atau terjadi sebaliknya, otot mengalami kekakuan,
kram, atau
tegang. 11
Secara umum gejala miopati antara lain adalah otot mengalami kelumpuhan,
melemah,
mengecil, nyeri, bengkak, atau kram. Walaupun demikian, setiap penyebab
memberikan pola
gejala yang berbeda. Pada penyakit polio misalnya, gejalanya adalah lumpuh layu,
sedangkan
pada penyakit tetanus gejalanya kaku otot dan kejang-kejang. Jika tubuh kekurangan
cairan
atau dehidrasi, timbul gejala kram otot, dimana otot tegang, kaku, rasa tertarik, dan
nyeri.11
Selain itu, pada setiap orang gejala bisa berbeda walaupun penyebabnya sama.
Misalnya
pada penyakit stroke, ada yang mengalami gejala badan mati sebelah, ada yang bicara
pelo,
ada yang kehilangan penglihatan, ada yang terganggu daya pikirnya, dan sebagainya.
Biasanya, jika lumpuh berlangsung lama dan otot jarang digunakan, otot cenderung
mengecil. Keadaan ini disebut atrofi otot. Pengecilan otot juga dapat ditemukan pada
mereka
yang kurang gizi, terutama kekurangan zat protein.11
3. Neurovascular junction disorders : myasthenia gravis
Miastenia gravis adalah salah satu penyakit gangguan autoimun yang mengganggu
sistem
sambungan saraf (synaps). Pada penderita miastenia gravis, sel antibodi tubuh atau
kekebalan
akan menyerang sambungan saraf yang mengandung acetylcholine (ACh), yaitu
neurotransmiter yang mengantarkan rangsangan dari saraf satu ke saraf lainnya. Jika
reseptor
mengalami gangguan maka akan menyebabkan defisiensi, sehingga komunikasi
antara sel
saraf dan otot terganggu dan menyebabkan kelemahan otot.12
Penyebab pasti reaksi autoimun atau sel antibodi yang menyerang reseptor
acetylcholine
belum diketahui. Tapi pada sebagian besar pasien, kerusakan kelenjar thymus
menjadi
penyebabnya. Maka itu kebanyakan si penderita akan menjalani operasi thymus. Tapi
setelah
thymus diangkat juga belum ada jaminan penyakit autoimun ini akan sembuh.12
Thymus adalah organ khusus dalam sistem kekebalan yang memproduksi antibodi.
Organ ini
terus tumbuh pada saat kelahiran hingga pubertas, dan akan menghilang seiring
bertambahnya
usia. Tapi pada orang-orang tertentu, kelenjar thymus terus tumbuh dan membesar,
bahkan
bisa menjadi ganas dan menyebabkan tumor pada kelenjar thymus (thymoma). Pada
kelenjar

thymus, sel tertentu pada sistem kekebalan belajar membedakan antara tubuh dan zat
asing.
Kelenjar thymus juga berisi sel otot (myocytes) dengan reseptor acetylcholine.12
Gejala-gejala miastenia gravis pada pasein usia produktif antara lain : 12
Kelopak mata turun sebelah atau layu (asimetrik ptosis)
Penglihatan ganda
Kelemahan otot pada jari-jari, tangan dan kaki (seperti gejala stroke tapi tidak
disertai gejala
stroke lainnya)
Gangguan menelan
Gangguan bicara
Dan gejala berat berupa melemahnya otot pernapasan (respiratory paralysis), yang
biasanya
menyerang bayi yang baru lahir
Gejala-gejala ringan biasanya akan membaik setelah beristirahat, tetapi bisa muncul
kembali
bila otot kembali beraktifitas. Penyakit miastenia gravis ini bisa disembuhkan
tergantung
kerusakan sistem saraf yang dialami.
Bisa terjadi kesulitan dalam berbicara dan menelan serta kelemahan pada lengan
dan tungkai.
Kesulitan dalam menelan seringkali menyebabkan penderita tersedak.
Yang khas adalah otot menjadi semakin lemah. Penderita mengalami kesulitan
dalam menaiki
tangga, mengangkat benda dan bisa terjadi kelumpuhan.
Sekitar 10% penderita mengalami kelemahan otot yang diperlukan untuk
pernafasan (krisis
miastenik).12
