ABSTRAK:
Pengantar:Perawat memiliki pendapat dan sikap yang berbeda tentang fungsi klinis Diagnosa
Keperawatan.
Tujuan:Untuk menggambarkan sikap profesional Keperawatan terhadap Diagnosa Keperawatan. Metode: Observasi,
deskriptif, studi potong lintang di sebuah rumah sakit di Kepulauan Canary (Spanyol). Jabatan Diagnosa Keperawatan
merupakan instrumen yang tervalidasi untuk mengukur sikap perawat terhadap Diagnosa Keperawatan dengan
reliabilitas yang sangat baik. Itu dibuat oleh skala kode dari 20 pasang kata sifat yang berlawanan di mana yang
diwawancarai harus memposisikan dirinya; skor ekstrim 1 (sikap paling negatif) dan 7 (sikap paling positif); 4 nilai
tengah; 140 skor maksimum dan 20 minimum satu. Analisis reliabilitas dilakukan dengan menggunakan alpha Cronbach
dan korelasi antar elemen. Distribusi frekuensi untuk variabel kualitatif dan proporsi untuk variabel kuantitatif. Analisis
asosiasi.
Hasil:Populasi penelitian adalah N=170. Skala telah menunjukkan keandalan yang tinggi (α = 0,955) serta
kekuatan yang memadai untuk menjelaskan varians total (66,13%). Skor rata-rata global adalah 74,41
(sd=23,53); sikap terburuk adalah Rutin (µ=2,91; sd=1,55) sedangkan sikap terbaik adalah Positif (µ=4,20;
sd=1,56). Kesimpulan:Sikap perawat di lingkungan kita mirip dengan konteks lain, dengan skor global lebih
rendah dari rata-rata. Peningkatan pelatihan akademik dan penggunaan Diagnosa Keperawatan di lingkungan
kerja tampaknya meningkatkan sikap para profesional ini, terutama berkaitan dengan penerimaan,
kenyamanan, kemudahan, relevansi, validitas, kreativitas dan kepuasan.

kenyamanan, kemudahan, relevansi, validitas, kreativitas, dan kepuasan.

PENGANTAR
Selain itu, model perawatan kesehatan dalam konteks kami terkait dengan penggunaan sistem
informasi yang semakin meningkat, melalui aplikasi yang menyediakan instrumen untuk
memfasilitasi Proses Keperawatan (NP), serta untuk mengevaluasi aktivitas profesional. Dalam
hal ini, ada hubungan erat antara penerapan aplikasi komputer ini dengan perkembangan
normatif yang mengatur isi Rekam Klinik pasien. Untuk proses standarisasi perawatan ini, perlu
menggunakan bahasa yang sama. Di bidang ini, Diagnosis Keperawatan (ND) adalah elemen
sentral; terutama karena 70% catatan perawat diselaraskan dengan bahasa yang dikendalikan,
melibatkan 40% waktu mereka ketika didokumentasikan di atas kertas dan 9% ketika dilakukan
dalam sistem elektronik
(3).
Kompetensi diagnostik melibatkan kualifikasi khusus dan atribut khusus yang terkait dengan keahlian
profesional melalui praktik reflektif, yang kondusif untuk otonomi Keperawatan. Dengan demikian,
Hasegawa et al.(4)mengevaluasi kerangka konseptual dari faktor-faktor yang mungkin mempengaruhi
kompetensi diagnostik ini; unsur-unsur ini termasuk pengalaman klinis, tanggung jawab pengambilan
keputusan, pengetahuan dan sikap terhadap ND.
Sikap merupakan posisi pribadi pada dimensi evaluatif atau afektif bipolar sehubungan dengan objek, tindakan
atau peristiwa; itu mewakili perasaan yang menyenangkan atau tidak menyenangkan(5)yang menghubungkan
pendapat dan perilaku individu-individu tersebut, yang melibatkan dan menunjukkan kesediaan mereka untuk
mempengaruhi dan bertindak(6).
Dalam pengertian ini, tahap diagnostik adalah kompleks, mempromosikan pendapat yang berbeda dan
tidak sesuai.(7)di antara para profesional yang mengekspresikan persepsi yang berlawanan tentang fungsi
klinis ND; perlu untuk membangun perdebatan tentang konsep dan tujuan mereka, tetapi juga nilai-nilai
dan paradigma mereka. Meskipun ND memberikan konsistensi, mereka dianggap kurang komprehensif
dan bermanfaat, yang mendorong ketidakakuratan(3)dan membatasi bukti(8). Sebaliknya, mereka dapat
berguna ketika mereka cocok dengan kemampuan kognitif para profesional yang menggunakannya. (9).
Mengetahui pendapat perawat tentang taksonomi diagnostik sangat penting untuk meningkatkannya (10).
Jadi, Krenz & Lunney(sebelas)mengembangkan Position on Nursing Diagnosis (PND), instrumen untuk
mengukur sikap terhadap ND(12).
Hasil dari pengalaman sebelumnya menunjukkan sikap yang sama dalam konteks yang berbeda yang
diamati. Jadi, di Brasil(13)mereka menerapkan PND ke populasi N=81 (n=26 siswa dan n=55 perawat),
61,8% perawat bekerja di lingkungan akademik dibandingkan dengan 34,6% di lingkungan perawatan
kesehatan. Rata-rata total rating yang dicapai dengan PND adalah
111,3 poin, yang merupakan peringkat keseluruhan yang positif. Sikap dengan skor tertinggi
adalahPenting (μ=6.3; sd=1,1) dan terendahSulit (μ=3.2; SD = 1.7). Lagi di Brasil (14), pada
sampel N=398 (n=184 siswa dan n=214 perawat), dimana 68,4% bekerja di tempat perawatan;
skor tertinggi adalahPenting (μ=6.3; sd=1,1) dan terendahSulit (μ=3.2; SD = 1.7). Aplikasi ketiga
dalam konteks Brasil(limabelas)untuk N=1605 profesional (n=632 perawat; n=973 asisten)
menunjukkan skor total rata-rata 114,34; pada kesempatan itu, sikap berperingkat terbaik
berhubungan denganPositif (µ=6.32) dan yang terburuk untuk rutin (μ=4.01).
Di spanyol(16)mereka menerapkan PND (α=0,96; r=0,90) pada N=621 perawat, di antaranya 76,5% adalah
wanita dan 72,9% bekerja di tempat perawatan. Sampel menunjukkan usia = 40,5 (sd = 10,1), tahun
pengalaman profesional = 17,1 (sd = 10,4). Pada kesempatan ini, rata-rata skor keseluruhan adalah 98,6
poin.
Di India(17)PND diterapkan melalui 2 pengukuran (pretest dan posttest) dalam studi dengan
intervensi pendidikan dan kelompok kontrol untuk N=101 perawat dari 2 rumah sakit. Hasil
mereka tidak menunjukkan signifikansi statistik pada pretest (μ=7.14; p=0.944); sebaliknya,
perbedaan rerata meningkat pada pengukuran kedua, baik pada kelompok kontrol (μ=8,38;
p≤0,05) dan pada kelompok intervensi (μ=25,8; p≤0,05), serta ketika membandingkan
pengukuran posttest kedua kelompok (μ=24,56; p≤0,05). Studi ini menyimpulkan bahwa seiring
dengan kemajuan keterampilan klinis perawat, sikap mereka terhadap ND berubah secara
positif.
Di sisi lain, di Italia(18)mereka menerapkan PND pada N=262 perawat, 60,7% wanita, usia
=37,7 (sd=8,18) tahun, pengalaman profesional =12,5 (sd=8.43) tahun; pada kesempatan ini,
sikap yang dinilai terbaik adalahPenting (μ=5.46; sd=1.61) dan yang terburuk Sulit (μ= 4.02;
SD=1.92).
Akhirnya, di Yordania(19)mereka menerapkan PND ke N=100 mahasiswa Keperawatan dengan skor skala
total rata-rata 100,50; sikap yang dinilai terbaik adalahPenting (μ=5,51; sd=1,59); yang terburuk
adalahTidak nyaman (μ=3.79; SD=1,61).
Mengingat hal di atas, tujuannya adalah untuk menggambarkan sikap profesional Keperawatan
terhadap ND dalam konteks kita.
METODE
Observasi, deskriptif, studi potong lintang menggunakan non-probabilistik sampling dengan kuota yang dilakukan selama bulan
Februari dan Maret 2018. Kami memasukkan populasi target perawat (N = 1032) dalamRumah Sakit Universitas Ibu dan Anak (CHUIMI,
di Kompleks Rumah Sakit Universitas Materno Infantil Insular Spanyol) (Kepulauan Canary, Spanyol) yang menjawab kuesioner secara
sukarela.
Tidak ada kriteria eksklusi yang ditetapkan. Sumber informasi dan alat ukur yang digunakan
adalah PND versi Spanyol (PND-SV®)(16), setelah mendapatkan izin penulis untuk
penggunaannya.
PND telah divalidasi dan diterapkan dalam berbagai bahasa dan konteks budaya, seperti Brasil,
India, Italia, dan Yordania(13-15,17-19). Ini juga telah dikembangkan dan diterapkan di Spanyol (16),
juga versi pengurangan kuesioner dengan 7 item telah diimplementasikan(12).
Instrumen PND terdiri dari 20 pasang kata sifat yang berlawanan (variabel independen) di
mana orang yang diwawancarai, menggunakan skala Likert yang dikodekan dalam 7 tingkat,
harus memposisikan diri dengan memilih salah satu yang sesuai dengan sikap mereka
terhadap ND; skor ekstrim menghindari bias persetujuan dengan nilai antara 1 dan 7; 4
adalah nilai pusat. Untuk analisis, pengkodean item respons telah ditetapkan di mana nilai 1
sesuai dengan sikap terburuk, sedangkan nilai 7 sesuai dengan yang terbaik. Skor total skala
PND bervariasi antara 20 dan 140, skor rata-rata 80; angka di bawah nilai ini akan
menunjukkan sikap negatif. Tujuh pertanyaan dilampirkan, menilai aspek sosio- demografis
populasi, seperti: jenis kelamin, usia, tahun pengalaman profesional, kualifikasi akademik,
bidang pekerjaan, penggunaan ND dalam praktik profesional, pelatihan di NP dalam 5 tahun
terakhir (variabel dependen). Anonimitas, kerahasiaan dan penggunaan eksklusif data untuk
penelitian dijamin, dan oleh karena itu izin diminta dan diperoleh dari komite etik kesehatan
provinsi Las Palmas. Paket perangkat lunak IBM® SPSS® Statistics (versi 21.0.0.0)
digunakan untuk analisis data. Analisis reliabilitas dilakukan dengan menggunakan koefisien
alpha Cronbach dan korelasi antar item. Analisis deskriptif menggunakan distribusi
frekuensi untuk variabel kualitatif dan proporsi untuk variabel kuantitatif, analisis asosiasi
menggunakan korelasi variabel kepentingan. Hasilnya telah dinyatakan dalam tabel dan
dijelaskan dalam bentuk narasi. Grafik telah diuraikan dengan bantuan Microsoft Excel
2011 untuk Mac, versi 14.0.0.
HASIL
N=170 tanggapan (16,47% dari populasi target). Wanita 79,4%, usia rata-rata 41,24 tahun
(Me=40; sd=7.15). Rata-rata pengalaman profesional 18,02 tahun (Saya=17,00;
SD=7.12). Wilayah kerja dibantu dalam 97,65% kasus. Mengenai pelatihan akademik,
31,8% sampel memiliki spesialisasi Keperawatan, 5,4% memiliki gelar Master resmi dan
hanya 0,6% memiliki PhD, sisa hasil sosiodemografi ditunjukkan pada Tabel 1.
Analisis deskriptif menunjukkan rerata skor global 74,41 (sd=23,53). Mayoritas item yang
disajikan skornya mendekati nilai sentral, sikap dengan skor terendah sesuai denganrutin
(=2,89; sd=1,55) sedangkan item dengan nilai terbaik adalahPositif (µ=4,22; sd=1,56).

eliminasi satu item ditunjukkan pada Tabel 2.

Dalam analisis asosiasi, Chi-square tidak menunjukkan signifikansi statistik untuk jenis kelamin, usia, pengalaman dan spesialisasi
dalam Keperawatan. Di sisi lain, ada signifikansi statistik (p<0,05) dalam asumsi gelar pascasarjana, resmi Master atau PhD,
lingkungan kerja, penggunaan pelatihan ND dan NP, seperti yang ditunjukkan pada Tabel 3.
DISKUSI
PND telah digunakan dengan mahasiswa dan profesional Keperawatan. Dalam hal ini, siswa menunjukkan sikap yang lebih positif
terhadap ND(13,19). Namun, minat kami terletak pada mencari tahu tentang sikap ini di kalangan profesional.
Sebelumnya, PND telah diterapkan pada 7 kesempatan dan di 5 negara berbeda antara 2005
dan 2017. Brasil, dengan 3 pengalaman, adalah konteks dengan jumlah kali tertinggi;
Sementara di Spanyol, penelitian ini merupakan aplikasi kedua dari versi 20 item, meskipun
versi pengurangan dari 7 item juga telah diterapkan, keduanya pada tahun 2013. Tidak ada
aplikasi PND yang telah diidentifikasi di negara lain, yang dapat menjadi sangat penting untuk
melengkapi peta dunia posisi perawat dalam kaitannya dengan ND di lingkungan yang jauh
seperti Amerika Utara, Asia, Anglo-Saxon dan bahkan Eropa Tengah, dengan peran penting
dalam panorama pengembangan disiplin Keperawatan.
Skor yang dicapai dalam berbagai aplikasi PND dapat diamati secara komparatif (Grafik 1), menunjukkan bahwa sikap terhadap ND
di antara individu-individu dalam penelitian kami (garis biru) lebih rendah daripada yang dicapai dalam aplikasi sebelumnya. Secara
umum, penyelidikan yang berbeda menunjukkan kecenderungan untuk mereproduksi nilai rata-rata untuk sebagian besar sikap,
terutama yang terburuk dan terbaik.
Dalam penelitian kami, dapat diamati bahwa kemajuan sikap positif terhadap ND terkait dengan
peningkatan pendidikan pascasarjana universitas dan penggunaan ND secara teratur dalam sistem
kesehatan oleh perawat, yang memberikan peningkatan keterampilan penalaran klinis di antara para
profesional ini.(dua puluh). Dalam hal ini, perawat meningkatkan terutama dalam kaitannya dengan
penerimaan, kenyamanan dan kemudahan ND, memberi mereka relevansi, validitas, kreativitas, dan
kepuasan yang lebih besar.
Di Spanyol, penerapan versi singkat(12)instrumen memungkinkan untuk melanjutkan penelitian di baris ini dengan cara yang
disederhanakan, dengan skala 7 item yang juga telah menunjukkan keandalan yang tinggi; namun, untuk penelitian kami, dianggap
lebih menarik untuk menerapkan versi luas PND untuk membuat perbandingan dengan konteks internasional yang dijelaskan di
atas.
Di antara keterbatasan penelitian, harus diperhitungkan bahwa pendapat atau sikap ini bervariasi sesuai dengan profil profesional
yang berbeda yang membentuk sampel; dalam urutan ini, sebuah studi internasional baru-baru ini (dua puluh satu)telah menyimpulkan
kebutuhan untuk mengkategorikan profesional untuk menetapkan, melalui penentuan cluster, strategi yang tepat untuk
meningkatkan hasil.
KESIMPULAN
Sikap yang ditunjukkan oleh perawat di lingkungan kita mirip dengan kenyataan lain, meskipun dengan skor keseluruhan di bawah
rata-rata di sebagian besar sikap. Generalisasi posisi internasional terhadap ND ini tampaknya menunjukkan bahwa kesulitan yang
berasal dari penggunaannya tidak hanya karena faktor pengkondisian dari lingkungan terdekat; ada kemungkinan bahwa ada
keadaan lain yang terkait dengan keistimewaan ND sebagai sebuah produk, tetapi juga sebagai sebuah proses. Peningkatan
pelatihan akademik dan penggunaannya di lingkungan kerja tampaknya meningkatkan sikap profesional terhadap ND, terutama
dalam hal penerimaan, kenyamanan, kemudahan, relevansi, validitas, kreativitas, dan kepuasan.
Studi mendalam diperlukan untuk mengidentifikasi kesulitan nyata yang menghasilkan sikap tersebut, serta untuk menentukan
strategi untuk perbaikan mereka. Dalam pengertian ini, diusulkan untuk mengoptimalkan keselarasan kognitif ND dengan
kompetensi profesional, meninjau kecukupan konseptual klasifikasi diagnostik dengan paradigma disiplin, mempromosikan proses
pelatihan yang mendukung keterampilan berpikir kritis dan mengevaluasi penggunaan ND dalam sistem kesehatan.

Diskusi
Studi ini menemukan bahwa sebagian besar pasien melaporkan kualitas
asuhan keperawatan yang diinginkan. Semua pasien memiliki tingkat
kecemasan dan depresi rumah sakit yang rendah. Temuan menunjukkan
bahwa ada hubungan terbalik antara kecemasan rumah sakit dan kualitas
asuhan keperawatan.
Temuan penelitian ini konsisten dengan penelitian lain
yang mengevaluasi kualitas asuhan keperawatan di rumah
sakit.18,29,30Namun, temuan kami tidak setuju dengan hasil
beberapa penelitian,19,20yang menunjukkan meningkatnya
kecemasan dan depresi pada pasien lama yang dirawat di Unit
Perawatan Koroner. Perbedaan ini mungkin karena unit di mana
mereka mengukur kecemasan dan depresi pasien tua di rumah
sakit tanpa tata kelola klinis. Ada peluang untuk meningkatkan
keamanan dan kualitas praktik klinis melalui tata kelola klinis di
rumah sakit.31Tata kelola klinis adalah salah satu perkembangan
kebijakan terpenting dalam sistem kesehatan. Tata kelola klinis

ABSTRACT:
Introduction: Nurses have different opinions and attitudes about the clinical functionality of Nursing Diagnosis.
Objective: To describe the attitudes of Nursing professionals towards the Nursing Diagnosis.
Method: observational, descriptive, cross-sectional study in a hospital in the Canary Islands (Spain). Position on
Nursing Diagnosis is a validated instrument to measure the attitudes of nurses towards the Nursing Diagnosis with
excellent reliability. It is made up by a coded scale of 20 pairs of opposite adjectives in which the interviewed
must position himself; extreme scores 1 (most negative attitude) and 7 (most positive attitude); 4 middle value;
140 maximum score and 20 minimum one. Reliability analysis was performed using Cronbach's alpha and inter-
element correlation. Frequency distribution for qualitative variables and proportion for quantitative variables.
Association analysis.
Results: The study population was N=170. The scale has shown high reliability (α=0.955) as well as adequate
power to explain the total variance (66.13%). The global mean score was 74.41 (sd=23.53); the worst attitude was
Routine (µ=2.91; sd=1.55) while the best attitude was Positive (µ=4.20; sd=1.56). Conclusions: The attitudes of
the nurses in our environment are similar to other contexts, with global scores lower than the average. The increase
in academic training and the use of the Nursing Diagnosis in the work environment seem to improve the attitudes
of this professionals, mainly with regard to acceptance, comfort, ease, relevance, validity, creativity and
gratification.
INTRODUCTION
The development of Nursing requires the creation of care models based on a disciplinary conception, in which the use of standardised
languages, symbols and common concepts for clinical practice reinforce the potential, commitment, professional identity and quality of care

provided to users and their families . In this sense, Nursing actions require lifelong and integrative learning in the face of highly flexible
and multidisciplinary situations with a high multidisciplinary content that requires critical thinking and analytical skills; but also a mastery of
very high level technical skills in which sensitivity must be maintained to provide care to individuals facing complex health-illness situations,
which are the driving force behind the work, the focus of attention and the object of study of Nursing.
In addition, the healthcare models in our context are linked to a growing use of information systems, by means of applications that provide
instruments to facilitate the Nursing Process (NP), as well as to evaluate professional activity. In this sense, there is a close relationship
between the implementation of these computer applications and the normative development that regulates the contents of the patients’ Clinical
Records. For this process of standardising care, it is necessary to use a common language. In this area, Nursing Diagnosis (ND) are central
elements; primarily because 70% of nurses' records are aligned with controlled languages, involving 40% of their time when they are

documented on paper and 9% when they are done in electronic systems .
Diagnostic competence involves a specific qualification and specific attributes linked to professional expertise through reflective practice,

which is conducive to Nursing autonomy. Thus, Hasegawa et al. evaluated the conceptual framework of possible factors influencing this
diagnostic competence; these elements included clinical experience, decision-making responsibility, knowledge and attitude towards ND.

In this sense, the diagnostic stage is complex, promoting divergent and discrepant opinions among professionals who express opposing
perceptions about the clinical functionality of ND; it is necessary to establish a debate about their concepts and objectives, but also their values
and paradigms. Although ND provide consistency, they have been considered to lack comprehensiveness and utility, which fosters

inaccuracies and restricts evidence . Instead, they can be useful when they match the cognitive abilities of the professionals who use

Knowing nurses' opinions on diagnostic taxonomies is essential to improve them . Thus, Krenz & Lunney developed Position on

Nursing Diagnosis (PND), an instrument to measure the attitudes towards ND .

Results from previous experiences suggest similar attitudes in the different contexts observed. Thus, in Brazil they applied PND to a
population N=81 (n=26 students and n=55 nurses), 61.8% of the nurses worked in the academic setting compared to 34.6% in the healthcare
setting. The average total rating achieved with the PND was
111.3 points, which was a positive overall rating. The attitude with the highest score was Important (μ=6.3; sd=1.1) and the lowest Difficult

(μ=3.2; sd=1.7). Again in Brazil , on a sample N=398 (n=184 students and n=214 nurses), of which 68.4% were working in the care setting;

the highest score was Important (μ=6.3; sd=1.1) and the lowest Difficult (μ=3.2; sd=1.7). A third application in the Brazilian context to
N=1605 professionals (n=632 nurses; n=973 assistants) showed mean total score 114.34; on that occasion, the best rated attitude corresponded
to Positive (μ=6.32) and the worst to Routine (μ=4.01).

In Spain they applied the PND (α=0.96; r=0.90) to N=621 nurses, of whom 76.5% were women and 72.9% worked in the care setting. The
sample indicated age μ=40.5 (sd=10.1), years of professional experience μ=17.1 (sd=10.4). In this occasion, the mean overall score was 98.6
points.

In India the PND was applied through 2 measurements (pretest and posttest) in a study with educational intervention and control group to
N=101 nurses from 2 hospitals. Their results showed no statistical significance in the pretest (μ=7.14; p=0.944); on the contrary, the differences
in the mean improved in the second measurement, both in the control group (μ=8.38; p≤0.05) and in the intervention group (μ=25.8; p≤0.05),
as well as when comparing the posttest measurements of both groups (μ=24.56; p≤0.05). This study concluded that as nurses' clinical skills
progressed, their attitudes towards ND changed positively.

On the other hand, in Italy they applied the PND to N=262 nurses, 60.7% women, age μ=37.7 (sd=8.18) years, professional experience
μ=12.5 (sd=8.43) years; on this occasion, the best rated attitude was Important (μ=5.46; sd=1.61) and the worst Difficult (μ=4.02; sd=1.92).
In view of the above, the objective is to describe the attitudes of Nursing professionals towards ND in our context.
METHOD
Observational, descriptive, cross-sectional study using non-probabilistic sampling by quotas carried out during the months of February and
March 2018. We included the target population of nurses (N=1032) in the Mother and Child University Hospital (CHUIMI, in Spanish
Complejo Hospitalario Universitario Insular Materno Infantil) (Canary Islands, Spain) who responded to the questionnaire voluntarily. No
exclusion criteria were established. The source of information and measurement instrument used was the Spanish version of the PND (PND-

SV®) , after obtaining the author's permission for its use.

The PND has been validated and applied in different languages and cultural contexts, such as Brazil, India, Italy and Jordan . It

has also been developed and applied in Spain , also a reduced version of the questionnaire with 7 items has been implemented .
The PND instrument is made up of 20 pairs of opposing adjectives (independent variables) in which the interviewee, using a Likert scale coded
in 7 levels, must position themselves by selecting the one that fits their attitude towards the ND; the extreme scores avoid acquiescence bias
with values between 1 and 7; 4 is the central value. For analysis, coding of the response items has been established in which the value 1
corresponds to the worst attitudes, while the value 7 corresponds to the best. The total scores of the PND scale vary between 20 and 140, mean
score 80; figures below this value would indicate negative attitudes. Seven questions were attached, assessing socio-demographic aspects of the
population, such as: gender, age, years of professional experience, academic qualifications, area of work, use of ND in professional practice,
training in the NP in the last 5 years (dependent variables). Anonymity, confidentiality and exclusive use of the data for the research were
guaranteed, and therefore permission was requested and obtained from the healthcare ethics committee of the province of Las Palmas. The
IBM® SPSS® Statistics software package (version 21.0.0.0) was used for data analysis. Reliability analysis was carried out using Cronbach's
alpha coefficient and inter-item correlation. Descriptive analysis using frequency distributions for qualitative variables and proportions for
quantitative variables, association analysis using correlation of variables of interest. The results have been expressed in tables and explained in
narrative form. The graphs have been elaborated with the help of Microsoft Excel 2011 for Mac, version 14.0.0. As a possible limitation to
external validity, we should point out that the instrument was self-administered.
RESULTS
N=170 responses (16.47% of the target population). Women 79.4%, mean age 41.24 years (Me=40; sd=7.15). Mean professional experience
18.02 years (Me=17.00;
The version of the PND applied showed high reliability (α=0.955) as well as adequate explanatory power of the total variance (66.13%) for the
sample. The Pearson correlation was adequate, with bilateral significance (p<0.05) for a range of values between 0.820-0.190. The intraclass
correlation coefficient performed with a two-factor mixed-effects model, in which the effects of the persons are random and the effects of the
measures are fixed, was for the individual measures r=0.515 (CI=0.458-0.576; p=0.000) and for the average measures r=0.955 (CI=0.955-
0.944; p=0.000).
Descriptive analysis showed mean global score 74.41 (sd=23.53). The majority of items presented scores close to the central value, the attitude
with the lowest score corresponded to Routine (µ=2.89; sd=1.55) while the best rated item was Positive (µ=4.22; sd=1.56). The total mean
scores, median and standard deviation, as well as Cronbach's Alpha assuming the elimination of one item are shown in Table 2.
Graphic 1 shows (blue line) the total scores after application of the PND in our setting, compared to previous applications in other contexts.

In the analysis of association, Chi-square showed no statistical significance for gender, age, experience and speciality in Nursing. On the other
hand, there was statistical significance (p<0.05) in the assumptions of postgraduate degree, official Master's or PhD, work environment, use of
the ND and NP training, as shown in Table 3.
DISCUSSION

PND has been used with both Nursing students and professionals. In this sense, students show more positive attitudes towards the ND .
However, our interest lies in finding out about these attitudes among professionals.
Previously, the PND has been applied on 7 occasions and in 5 different countries between 2005 and 2017. Brazil, with 3 experiences, is the
context with the highest number of times; while in Spain, the present study is the second application of the 20- item version, although a reduced
version of 7 items has also been applied, both in 2013. No applications of the PND have been identified in other countries, which could be of
vital importance to complete the world map of the positioning of nurses in relation to the ND in environments as distant as North America,
Asia, Anglo-Saxon and even Central Europe, with a prominent role in the panorama of the development of the Nursing discipline.
In our study, it can be observed that the progress of positive attitudes towards the ND is related to an increase in postgraduate university
education and the regular use of the ND in health systems by nurses, which provides improvements in clinical reasoning skills among these

professionals . In this sense, nurses improve mainly in relation to the acceptance, comfort and ease of the ND, giving them above all greater
relevance, validity, creativity and gratification.

In Spain, the application of an abridged version of the instrument makes it possible to continue research in this line in a simplified manner,
with a 7-item scale that has also demonstrated high reliability; however, for our study it was considered of greater interest to apply the broad
version of the PND in order to make comparisons with the international contexts described above.
Among the limitations of the study, it should be taken into account that these opinions or attitudes vary according to the different professional

profiles that make up the sample; in this order, a recent international study has concluded the need to categorise professionals in order to
establish, through the determination of clusters, appropriate strategies to improve results.
CONCLUSIONS
The attitudes shown by nurses in our environment are similar to other realities, although with overall scores below the average in most
attitudes. This generalisation of the international positioning towards the ND seems to indicate that the difficulties derived from its use are not
only due to conditioning factors of the immediate environment; it is possible that there are other circumstances linked to the idiosyncrasy of
the ND as a product, but also as a process. Increased academic training and its use in the work environment seem to improve professional
attitudes towards ND, mainly in terms of acceptance, comfort, ease, relevance, validity, creativity and gratification.
In-depth studies are needed to identify the real difficulties that generate these attitudes, as well as to determine strategies for their improvement.
In this sense, it is proposed to optimise the cognitive alignment of ND with professional competences, review the conceptual adequacy of
diagnostic classifications with the disciplinary metaparadigm, promote training processes that favour critical thinking skills and evaluate the
use made of ND in health systems.

Discussion
This study found that most patients reported the desired quality of nursing
care quality. All patients had low levels of hospital anxiety and depression.
The finding indicated that there was a reverse relationship between hospital
anxiety and nursing care quality.
The findings of this study were consistent with other studies
evaluating the nursing care quality in hospitals. 18,29,30 However, our findings
did not agree with the results of some studies, 19,20 that showed the rising
anxiety and depression in the old patients hospitalized in Coronary Care
Unit. These differences might be due to the units in which they measured
old patients’ anxiety and depression in hospitals with no clinical
governance. There are opportunities for improving the safety and quality of
clinical practices through clinical governance within hospitals.31 Clinical
governance is one of the most important policy developments in health
systems. Clinical governance is related to the performance of all health care
professionals to provide high-quality and cost-effective
