• Bidirectional fashion:
• Slower upward pathway for chronic pain and cancer
pain,
• Faster downward direction for intense acute pain,
uncontrolled chronic pain, and breakthrough pain.
• Ascend slowly one step at a time in the case of chronic
pain
• Increase the rate of climb according to the PAIN
intensity
• Start directly at the fourth step, in extreme cases, to control
pain of high intensity
• PCA pumps for continuous intravenous, epidural, or

diberikan Non Steroidal Anti Inflammatory Drugs
(NSAID) atau Paracetamol sesuai dengan keadaan pasien
• Edukasi pasien dan keluarga pasien mengenai nyeri.
• Kaji ulang nyeri setiap 8 jam.
NYERI
SEDANG
(VAS 4-6, FLACC 4-6, BPS 6-8)
• Bila pasien adalah pasien dari Tim Tatalaksana Nyeri, perawat
melaporkan ke Tim Tatalaksana Nyeri
• Bila pasien bukan pasien dari Tim Tatalaksana Nyeri, perawat
melaporkan ke DPJP untuk tatalaksana nyeri.
• Sesuai keadaan pasien, pasien dapat diberikan NSAID,
Paracetamol, Opioid lemah (setelah persetujuan DPJP atau tim
tatalaksana nyeri)
• Bila nyeri masih ada, konsultasikan ke Tim tatalaksana Nyeri
• Terapi non farmakologis : Comforting, Distraction, Relaxation,
Massage dan sentuhan, Guided Imagery/Visualization
• Edukasi pasien dan keluarga pasien mengenai nyeri.
• Kaji ulang derajat nyeri setiap 2 jam, sampai nyeri teratasi
(VAS
<4) , pemantauan derajat nyeri kemudian setiap 8 jam
NYERI BERAT
(VAS >7/10, FLACC > 7/10, BPS > 9/12)
• Bila pasien adalah pasien dari Tim Tatalaksana Nyeri, perawat melaporkan ke
Tim Tatalaksana Nyeri
• Bila pasien bukan pasien dari Tim Tatalaksana Nyeri, perawat melaporkan ke
DPJP untuk tatalaksana nyeri.
• Sesuai dengan keadaan pasien, dengan persetujuan DPJP atau Tim
Tatalaksana Nyeri, pasien dapat diberikan:
– a.Opioid kuat
– b.Kombinasi opioid kuat dengan analgetik lain
– c.Tatalaksana nyeri intervensi
• Terapi non farmakologis : Comforting, Distraction, Relaxation, Massage dan
sentuhan, Guided Imagery/Visualization
• Edukasi pasien dan keluarga pasien mengenai nyeri.
• Kaji ulang derajat nyeri setiap 1 jam, sampai nyeri menjadi nyeri sedang
dikaji setiap 2 jam, dan bila nyeri telah teratasi setiap 8 jam

Weak opioids
• Morphine
• Pain is a physiological antagonist to the central depressant effect
of morphine.
• Rarely need naloxone :
– Have weak opioid for some time (not opioid naïve)
– Take medication by mouth (slower absorbtion, lower
peak concentration)
– Titrate the dose upward step by step
• Relationship of the therapeutic dose to the lethal dose is
greater
• Active Metabolite
• Addiction does not occur if morphine used correctly
• Morphine is not a panacea of cancer pain, particularly if the
psychosocial aspects of care are ignored
ORAL MORPHINE
• Tablets (10, 20mg), aqueous sol
• Initial doses depend upon current opioid requirements
• Patients previously receiving weak opioids
• begin 10 mg q4h (or m/r 30 mg q12h).
• Elderly : 5 mg q4h
• Additional rescue doses if pain not adequately relieved
• Adjust the dose upwards after 24 h, if the pain is not 90% relieved
• With ordinary tab or sol., a double dose at bedtime usually enables
a patient to go through the night without waking in pain

Fentanyl
• High fever and exposure to external heat sources (heat
blankets, electric blankets) may increase the rate of
delivery of fentanyl
• Equivalent rescue doses of a subcutaneous opioid should
be given to treat breakthrough pain
• After 24 h, the total number of rescue doses
given for breakthrough pain should be totalled
and administered via a CSCI (cont subcutaneous
infusions), in addition to the fentanyl patch

• with chronic opioids usage
