Abstrak: Pleurodesis adalah penyatuan pleura viseralis dengan parietalis baik secara kimiawi,
mineral ataupun mekanik, secara permanen untuk mencegah akumulasi cairan maupun udara
dalam rongga pleura. Tindakan tersebut umumnya diindikasikan untuk efusi pleura maligna
dan pneumotoraks spontan. Tidak ada kontraindikasi absolut untuk pleurodesis, namun perlu
dipertimbangkan kemungkinan tingkat keberhasilan prosedur serta risikonya agar pasien
mendapat manfaat optimal dari tindakan ini. Pemilihan teknik yang tepat, agen sklerosis,
kriteria pemilihan pasien merupakan hal yang sering diperdebatkan serta menentukan
keberhasilan tindakan. Telah dikenal banyak macam agen sklerosis seperti tetrasiklin,
doksisiklin, minosiklin, bleomisin, kuinakrin, dan darah pasien sendiri namun yang sering
digunakan adalah talk karena murah, cukup efektif, serta komplikasi yang minimal. Pleurodesis
menggunakan talk tidak membutuhkan anestesia umum maupun intubasi trakea. Sebelum
prosedur, perlu dilakukan evaluasi pasien meliputi foto toraks, bronkoskopi bila
memungkinkan, anamnesis dan pemeriksaan fisik ulang, menilai kembali hasil laboratorium,
serta insersi chest tube bila belum terpasang. Talk dimasukkan ke rongga pleura melalui chest
tube dan pasien diminta bernapas beberapa kali agar larutan talk tertarik ke rongga pleura.
Setelah prosedur, perlu dilakukan foto toraks dan pemantauan tanda vital, drainase chest tube
harian, kebocoran udara, serta kontrol nyeri. Komplikasi yang mungkin timbul meliputi nyeri,
takikardia, takipnea, pneumonitis, demam, ekspansi paru inkomplit, serta reaksi alergi.

Abstract: The aim of pleurodesis is to achieve symphysis between visceral and parietal pleural
layers, preventing accumulation of either air or fluid in pleural space. Its main indications are
malignant pleural effusions and spontaneous pneumothorax. There is no absolute
contraindica- tion for pleurodesis. However, several matters need to be taken in cosideration
to achieve optimal therapeutic effect. The right choice of technique, sclerosing agent, and
criteria for patients selec- tion are important and controversial issues. Many sclerosing agents
have been introduced, such as tetracyclin, doxycyclin, minocyclin, bleomycin, quinacrin, and
patient’s own blood, but talc is most often used because it is economis, effective, and cause
relatively minor complication. Talc pleurodesis does not require general anesthesia or
tracheal intubation. Before the procedure, patient evaluation such as chest x-ray,
bronchoscopy if possible, reviewing patient’s history of illness, and physical findings, is
evaluated. Talc is instilled into the pleural cavity through the chest tube and the patient is
asked to breath several times so that the talc slurry enters the cavity. Patient monitoring after
the procedur includes chest x-ray, vital signs monitoring, daily chest tube drainage, air
leakage, and pain control. Possible complication includes pain, tachycardia, tac- hypnea,
pneumonitis, fever, incomplete lung inflation, and allergic reaction.
