TENTANG
PEMBAKUAN SINGKATAN YANG DIGUNAKAN DALAM REKAM MEDIS

DENGAN RAHMAT TUHAN YANG MAHA ESA

KEPALA UPTD PUSKESMAS CIKARANG

Menimbang : a. bahwa dalam penyelenggaraaan pelayanan klinis yang

berkualitas di Puskesmas, diperlukan penulisan rekam
medis yang sesuai standar;
b. bahwa dalam rangka penulisan rekam medis yang sesuai
standar, perlu ditetapkan pembakuan singkatan yang
digunakan dalam rekam medis;\
c. bahwa berdasarkan pertimbangan pada huruf a dan b, perlu
menetapkan Keputusan Kepala UPTD Puskesmas Cikarang
tentang Pembakuan Singkatan Yang Digunakan Dalam
Rekam Medis.

Mengingat : 1. UU Nomor 29 Tahun 2004, tentang Praktik Kedokteran;

MEMUTUSKAN

Menetapkan : KEPUTUSAN KEPALA UPTD PUSKESMAS CIKARANG

TENTANG PEMBAKUAN SINGKATAN YANG DIGUNAKAN
DALAM REKAM MEDIS.

KESATU : Menentukan pembakuan singkatan yang digunakan dalam

 rekam medis sebagaimana terlampir dalam keputusan ini.

KEDUA : Keputusan ini berlaku sejak tanggal ditetapkan dan apabila

dikemudian hari terdapat kekeliruan dalam penetapannya,
maka akan diadakan pembetulan sebagaimana mestinya.
