GUIDELINES FOR MANAGEMENT OF SEVERE SEPSIS AND SEPTIC SHOCK Initial Rescucation
Tujuan resusitasi :
-
CVC 8-12 mmHg
-
MAP ≥ 65 mmHg
-
Produksi urin ≥ 0,5 ml/kg/hr
-
Ssaturasi O2 vena sentral ≥ 70%
Jika saturasi O2 pada vena sentral / mixed venous tidak mencapai 70% dengan CVP 8-12 mmHg maka
trensfusi dengan PRC untuk mencapai hematokrit ≥ 30% dan masukkan infuse Dobutamin dengan nilai
maksimal 20 μ/kg/menit
Diagnosis
Sebelum pemberian antibiotika, harus dilakukan kultur darah. Sampel darah diambil perkutaneus
atcaupun melalui tinadakan infasif (vascular assist device )yang terpasang pada pasien ± > 48 jam.
Sampel lani dapat diambil yaitu CSF, secret respiratori, urine dan cairan tubuh lainnya.
Terapi Antibiotika
-
Jika dikeketahu pasien mengalami sepsia berikan antibiotika IV dalam 1 jam pertama.
-
Berikan satu atau lebih jenis obat yang dapat mengatasi pathogen bakteri atau fungi.
Perimbangan pada kerentaan mikroorganisme dalam komunitas dan RS
-
Evaluasi kembali setelah 48-72 jam pemberian anitibiotika denga tujuan penggunaan anitibiotika
denga spectrum tang sangat sempit.
-
Pertimbangkan terapi kombinasi pada pasien neutropenia dan pada pasien dengan infeksi
pseudomonas.
-
Stop pemberian antibiotika sesegerah mungkin jika kondisi pasien dinyatakan penyebabnya buka
agen infeksius.
Source control
Terapi cairan
-
Perhantikan rekomendasi resusitasi cairan awal, penggunaan kristaliod / koloid.
-
Berikan cairan ( fluid challenge ) pada pasien yang dicurigain dengan perfusi jaringan yang tidak
cukup yaitu 500-1000 ml kristaloid atau 300-500 cc koloid dalam waqktu 30 menit. Ulangi lagi
pemberian cairan jika tekanan darah dan produksi urin tidak meningkat serta tidak ada tanda-
tanda overload volume intravascular.
Vasopressor
-
Mulai memberikan vasopressor apabila terapi cairan (fluid challenge ) gagal mengembalikan
tekanan darah dan perfusi organ yang cukup atau juga dapat diberikan sementara menunggu
sampai resusitasi cairan mengembalikan perfusi jaringan .
-
Pilihan vasopressor yang utama yaitu norepinefrin dan dopamine.
-
Tidak menggunakan dopamine dengan dosis renal untuk proteksi ginjal.
-
Dilakukan pemasangna kateter arteri sesegera mungkin.
-
Pertimbangkan penggunanna vasopressin pada pasien yang mengalami syok berulang walaupun
telah dilakukan resusitasi cairan yang cukup dan telah diberikan vasopressor konvensional dosis
tinggi.
-
Vasopresin tidak direkomendasikan untuk menggantikan norepinefrine / dopamine sebagai first-
line agent.
-
Diberikan infuse vasopresin 0,01- 0,04 unit/mnt pada orang dewasa.
Terapi inotropik
-
Perimbangkan pemberian dobutamine pada pasien dengan COP yang rendah walaupun telah
dilakukan resusitasi cairan lanjutkan titrasi vasopesor sampai MAP≥ 65 mmHg.
-
Do not incrase cardiax index to achieve an arbitrarily predecfined elaveted level of O 2 delevery.
Terapi Steroid
-
Berikan hydrocortisone 200-300 mh/hr selam 7 hari dengan dosis 3-4 x/hari atau dengan infuse
yang berkelanjutan walaupun telah diberikan terapi pengganti cairan dan pasien masih
memerlukan vasopressor.
-
Optimal :
Lakukan tes stimulasi ACTH dan hentikan pemberian steroid pada pasien yang kadar
kortisol > 9 μg/dl.
Turunkan dosis steroid jika sepsis teratasi
Turunkan secara bertahap dosis steroid pada akhir terapi
Tambahkan fludrocortison 50 μg/hr
Kontrol Glukosa
-
Pertahankan glukosa darah < 150 mg/dl setelah dilakukan stabilisasi pasien
-
Berikan insulin dan infuse glukosa
-
Monitoring BS setiap 30-60 menit sampai terstabilisasi kemudian monitor setiap 4 jam
Renal Replacement
-
Hemodialisasi intermittent dan continues veno-venous Hemoflitration ( CVVH) dapat
diperimbangkan.
-
CVVH lebih sederhana dan dapat dilakukan pada pasien dengan hemodinamik tidak stabil.
Terapi Bicaronate
-
Jangan menggunakan terapi bikarbonat dengan tujuan untuk meningkatkan hemodinamik atau
untuk menurunkan kebutuhan vasopressor ketika menterapi academia laktat yang disebabkan
oleh hipoperfusi (pH ≥ 7,15 )
Diagnosis
● Obtain appropriate cultures before starting antibiotics provided this
does not significantly delay
antimicrobial administration
Obtain two or more BCs
One or more BCs should be percutaneous
One BC from each vascular access device in place > 48 hrs
Culture other sites as clinically indicated
● Perform imaging studies promptly to confirm and sample any source of
infection, if safe to do so
Antibiotic therapy
● Begin intravenous antibiotics as early as possible and always within the
first hour of recognizing severe sepsis ) and septic shock
● Broad-spectrum: one or more agents active against likely
bacterial/fungal pathogens and with
good penetration into presumed source
Fluid therapy
Vasopressors
● Maintain MAP ≥ 65 mm Hg
● Norepinephrine and dopamine centrally administered are the initial
vasopressors of choice
_ Epinephrine, phenylephrine, or vasopressin should not be administered
as the initial
vasopressor in septic shock . Vasopressin 0.03 units/min may be
subsequently added to
norepinephrine with anticipation of an effect equivalent to
norepinephrine alone
_ Use epinephrine as the first alternative agent in septic shock when blood
pressure is poorly
responsive to norepinephrine or dopamine
● Do not use low-dose dopamine for renal protection
● In patients requiring vasopressors, insert an arterial catheter as soon as
practical
Inotropic therapy
● Use dobutamine in patients with myocardial dysfunction as supported
by elevated cardiac
filling pressures and low cardiac output
● Do not increase cardiac index to predetermined supranormal levels
Steroids
_ Consider intravenous hydrocortisone for adult septic shock when
hypotension responds poorly
to adequate fluid resuscitation and vasopressors
_ ACTH stimulation test is not recommended to identify the subset of
adults with septic shock
who should receive hydrocortisone
_ Hydrocortisone is preferred to dexamethasone
_ Fludrocortisone (50 μg orally once a day) may be included if an
alternative to hydrocortisone
is being used that lacks significant mineralocorticoid activity.
Glucose control
● Use intravenous insulin to control hyperglycemia in patients with
severe sepsis following
stabilization in the ICU
● Aim to keep blood glucose < 150 mg/dL (8.3 mmol/L) using a
validated protocol for i
insulin dose adjustment
● Provide a glucose calorie source and monitor blood glucose values
every 1–2 hrs (4 hrs
when stable) in patients receiving intravenous insulin
● Interpret with caution low glucose levels obtained with point of
care testing, as these
techniques may overestimate arterial blood or plasma glucose
values
Renal replacement
_ Intermittent hemodialysis and CVVH are considered equivalent
_ CVVH offers easier management in hemodynamically unstable
patients
Bicarbonate therapy
● Do not use bicarbonate therapy for the purpose of improving
hemodynamics or reducing
vasopressor requirements when treating hypoperfusioninduced
lactic acidemia with pH
