PSORIASIS VULGARIS

Keluhan Utama

Terdapat bercak kemerahan dan bersisik tebal berlapis pada

kulit yang disertai rasa gatal pada punggung, lengan, dan siku
sejak ± 3 bulan yang lalu SMRS.
1 Riwayat Penyakit Sekarang

± 3 bulan yang lalu pasien mengeluhkan bercak kemerahan sebesar uang

koin 100 rupiah yang terdapat pada kedua lengannya yang lama kelamaan
bercak tersebut menjadi semakin gatal

Lama kelamaan bercak tersebut juga semakin membesar sehingga

membentuk bercak – bercak kemerahan yang meninggi, bersisik tebal dan
berlapis berwarna putih dan tidak berminyak.

Jika bercak – bercak kemerahan terasa gatal pasien mengaruknya dan

mengakibatkan jadi mengelupas dan bercak tersebut juga meluas ke bagian
tubuh lainnya.
 Riwayat Penyakit Dahulu Riwayat Penyakit Keluarga

Pasien pernah merasakan gejala seperti Pasien mengatakan bahwa di keluarga

ini sejak 5 tahun yang lalu. pasien tidak ada yang memiliki keluhan

 Riwayat Pengobatan Riwayat Kontak dan Alergi

Pasien sebelumnya telah menggunakan Alergi makanan (-), debu (-), obat (-),
obat lotasbat salep dan insidal namun maupun cuaca (-).

tidak membaik.

Riwayat Psikososial

 Pasien tinggal dirumah bersama istri dan anaknya

 Pasien juga merupakan perokok aktif yang mengkonsumsi
rokok 1 bungkus sehari.
 Pasien mandi 2 kali sehari untuk menjaga kebersihan
tubuhnya.
 Pasien mengaku 3 bulan terakhir ini sedang banyak pikiran
tentang pekerjaannya.

Abdomen Inspeksi : Perut datar, soepel, scar (-), lesi kulit (+) (lihat status dermatologis)
Auskultasi : Bising usus (+) normal
Palpasi : Hepar dan lien tidak teraba
Perkusi : Timpani pada keempat kuadran

Ekstremitas Akral hangat, udem (-/-), pucat (-), RCT < 2 detik, lesi kulit (+) (lihat status dermatologi)
 PEMERIKSAAN FISIK

Distribusi Generalisata

A/R Dada sampai perut, punggung, kedua ekstremitas atas dan bawah

Multiple, berbentuk bulat sirkumskrip, sebagian diskret, sebagian

konfluens, sebagian menimbul di permukaan kulit dengan ukuran
Lesi terbesar 3 x 2 x 0,3 cm sampai ukuran terkecil 0,5 x 0,5 x 0,3 cm, sebagian
tidak menimbul di permukaan kulit dengan ukuran terbesar 5 x 1 cm dan
ukuran terkecil 0,3 x 0,2 cm, lesi kering.

Makula eritermatosa, plak eritematosa, erosi, skuama halus, skuama tebal,

krusta hemoragik.
 Pemeriksaan Penunjang

 Pemeriksaan fenomena titisan lilin

Dengan menggunakan benda yang yang ujungnya agak tajam seperti

kaca preparat. Hasilnya : (+) skuama berubah warna menjadi putih
pada goresan seperti tetesan lilin.
 Pemeriksaan Penunjang

 Pemeriksaan auspitz sign

Dengan mengerok skuama. Hasilnya : (+) tampak titik-titik

perdarahan pada permukaan kulit yang skuamanya terkelupas.

Resume
Pasien Tn. K berusia 53 tahun datang ke Poli Kulit RSUD banjar dengan keluhan
terdapat bercak kemerahan dan bersisik tebal berlapis pada kulit yang disertai rasa gatal pada
punggung, lengan, siku , dan paha sejak ± 3 bulan yang lalu SMRS. Pasien mengaku awalnya
bercak kemerahan sebesar uang koin yang terdapat pada kedua lengannya yang lama
kelamaan bercak tersebut menjadi semakin gatal, lama kelamaan bercak tersebut juga semakin
membesar sehingga membentuk bercak – bercak kemerahan yang meninggi, bersisik tebal
dan berlapis berwarna putih dan tidak berminyak. Jika bercak – bercak kemerahan terasa gatal
pasien mengaruknya dan mengakibatkan jadi mengelupas dan bercak tersebut meluas ke
bagian tubuh lainnya. Keluhan ini sering timbul kembali 1 tahun sekali atau 6 bulan sekali
bahkan 3 bulan sekali. Pasien terakhir mengalami keluhan seperti ini sekitar 1 tahun yang lalu.
Pasien mengaku 3 bulan terakhir ini sedang banyak pikiran tentang pekerjaannya. Pasien juga
merupakan perokok aktif yang mengkonsumsi rokok 1 bungkus sehari. Pasien mandi 2 kali
sehari untuk menjaga kebersihan tubuhnya.

Status dermatologikus ditemukan : Makula eritermatosa, plak eritematosa, erosi, skuama halus,
skuama tebal, krusta hemoragik.
 Diagnosis Psoriasis
Non - Medikamentosa
• Memberikan edukasi kepada pasien
mengenai penyakitnya, faktor
pencetus yang paling mungkin
• Menghindari faktor pencetus
• Menghindari garukan untuk
mencegah timbulnya lesi baru
Medikamentosa
• Sistemik
methotrexate tablet 3 x 2,5 mg yang
diminum setiap 12 jam, dan hanya
berlaku untuk 3 kali pengulangan setiap
minggunya.
• Topikal
Clobetasol propionate 0,05 % krim
 PROGNOSIS

 Quo ad vitam : ad bonam

 Quo ad sanationam : dubia ad bonam

Non-Medikamentosa Medikamentosa

• Memberikan edukasi kepada Metrotrexat adalah obat

pasien mengenai penyakitnya, derivat aminopetrin analog asam

faktor pencetus yang paling folat yang memiliki anti-inflamasi,
mungkin antiproliferasi dan imunospuresi.

• Menghindari faktor pencetus Dosis MTX 2,5 – 25 mg

• Menghindari garukan untuk perminggu dengan interval 12 jam,
mencegah timbulnya lesi baru dosis dapat ditingkatkan jika tidak
mengalami perbaikan
