Carcino embryonic antigen

Pengertian cea
 Carcinoembryonic antigen (CEA) adalah protein yang
dihasilkan oleh epitel saluran cerna janin yang juga dapat
diekstraksi dari tumor saluran cerna orang dewasa.

 Ditemukan tahun 1965 oleh Gold & Freedman. Glikoprotein

dengan BM 180.000 dalton. CEA di bentuk di saluran gastro-
intertinal dan pancreas sebagai antigen pada permukaan sel
yang selanjutnya disekresikan ke dalam cairan tubuh. 
 CEA sebagai petanda tumor untuk kanker kolorektal,
oesofagus, pankreas, lambung, hati, payudara, ovarium dan
paru-paru 3

Tujuan Pemeriksaan cea

• untuk mengetahui adanya kanker usus besar,
khususnya ardenocarcinoma
• untuk mengidentifikasi kekambuhan setelah perawatan
atau pembedahan reseksi
• untuk membantu dalam pementasan dan penilaian
metastasis

Prinsip Pemeriksaan cea
ELISA SANDWICH :

Permukaan sumur dilapisi dg antibodi yg terikat dg jumlah yg diketahui untuk

menangkap antigen yg diinginkan. Setelah non specific binding sites diblok dg BSA,
sample yg mengandung antigen diaplikasikan pada plate. Antibodi primer kemudian
ditambahkan sehingga akan melapisi (sandwich) antigen tersebut.

Antibodi sekunder yg terikat enzim kemudian ditambahkan untuk mengikat antibodi

primer. Antibodi sekunder konjugat enzim yg tidak berikatan dg antibodi primer
kemudian tercuci.

Substrat ditambahkan dan secara enzimatik diubah menjadi substrat berwarna yg

dapat diukur. 5

Antibodi anti-CEA monoklonal terbiotinilasi (tikus /
manusia) 3,0 mg / L;buffer fosfat 100 mmol / L, pH 6,0;
pengawet.

Anti-CEA-Ab ~ Ru (bpy) (tutup hitam), 1 botol, 8 mL:
Antibodi anti-CEA monoklonal (tikus) berlabel ruthenium
kompleks 4,0 mg / L; buffer fosfat 100 mmol / L, pH 6,5;
pengawet.

 Prosedur Kerja
✘ Inkubasi pertama: 10 μL sampel, CEA monoklonal yang terbiotinilasi antibodi, dan
antibodi spesifik-CEA monoklonal berlabel kompleks ruthenium) bereaksi
membentuk kompleks sandwich.
✘ Inkubasi ke-2: Setelah penambahan mikropartikel berlapis streptavidin,kompleks
menjadi terikat pada fase padat melalui interaksi biotin dan streptavidin.
✘ Campuran reaksi disedot ke dalam sel pengukur dimana mikropartikel secara
magnetis ditangkap ke permukaan elektroda. Zat yang tidak terikat kemudian
dihilangkan dengan ProCell / ProCell M. Penerapan tegangan pada elektroda
kemudian diinduksi emisi chemiluminescent yang diukur dengan photomultiplier.
✘ Hasil ditentukan melalui kurva kalibrasi yang merupakan instrumen-khusus
dihasilkan oleh kalibrasi 2-titik dan kurva master disediakan melalui barcode reagen
atau e-barcode.
