Kesadaran adalah pengetahuan penuh atas diri, lokasi dan waktu.
(Corwin, 2001). Penurunan kesadaran adalah keadaan dimana penderita
tidak sadar dalam arti tidak terjaga/ tidak terbangun secara utuh sehingga
tidak mampu memberikan respons yang normal terhadap stimulus.
Kesadaran secara sederhana dapat dikatakan sebagai keadaan dimana
seseorang mengenal/ mengetahui tentang dirinya maupun lingkungannya.
(Padmosantjojo, 2000).

 Patofisiologi
Penurunan kesadaran pada pasien stroke
apabila yang diserang batang otak. Dia
akan mengalami gangguan pada fungsi
kesadaran, pernafasan dan aliran darah ke
otak menurun. Apabila yang mengalami
gangguan pada fungsi kesadarannya maka
akan terjadi penurunan tingkat kesadaran,
hal tersebut dapat mengakibatkan apatis
sampai dengan koma.

 Prognosis
Penurunan kesadaran ringan sampai dimana pasien
mengalami berkurangnya respons terhadap
lingkungannya dan lebih lambat bereaksi terhadap
rangsangan. Respon disertai tidak adanya gerakana
spontan, dapat dibedakan dari keadaan tidur dalam
yang masih dapat bangunkan dengan rangsangan yang
kuat dan berulang. Dalam kondisi ini pada keadaan
patologis dengan kondisi tidak bisa dibangunkan, mata
tertutup. (Supriyatno, 2018)
