 ANAMNESIS

 Keluhan Utama
Nyeri tenggorokan sejak 2 hari yang lalu

 Riwayat Penyakit Sekarang

Pasien datang berobat ke Puskemas Klangenan dengan keluhan

nyeri pada tenggorokan yang memberat sejak 2 hari yang lalu. Keluhan
dirasakan pertama kali 2 minggu yang lalu awalnya pasien merasa
tenggorokannya kering dan panas kemudian disertai batuk namun tidak
berdahak. Kemudian 2 hari sebelum ke Puskesmas keluhan makin
memberat dan pasien mengeluhkan nyeri menelan terutama jika
menelan makanan yang padat sehingga pasien lebih nyaman untuk
makan makanan yang lembek seperti bubur. Keluhan batuk juga masih
dirasakan dan pasien juga mengeluhkan deman sejak 2 hari. Keluhan
demam tidak disertai dengan menggigil dan keluar keringat yang
berlebih. Pasien juga mengeluhkan tidur sering menggorok. Keluhan
nyeri pada telinga disangkal.
Riwayat Penyakit Dahulu :
 Pasien pernah mengalami keluhan serupa
saat SMP dan membaik setelah berobat ke
Puskesmas.
 Riwayat alergi obat/makanan disangkal Riwayat Kebiasaan :
 Riwayat keluhan menderita batuk lama lebih Pasien mengaku sering jajan sembarangan
dari 3 bulan disangkal oleh pasien sejak masih sekolah dan sering memakan
makanan pedas serta mengkonsumsi

pabrik rokok dan pendapatan ayah pasien sebagai
buruh bangunan. Keluarga pasien termasuk dalam
keluarga dengan ekonomi rendah.

tonsil
 Keluarga pasien sangat memperhatikan
kondisi penyakit pasien dan keluarga
pasien peduli dan akan melakukan yang
terbaik demi kesembuhan penyakit yang
diderita pasien

 Pola makan : Pasien sering untuk jajan
sembarangan
 Pekerjaan : Pekerjaan pasien sebagai
buruh pabrik rokok dapat
menyebabkan pasien merasa
kelelahan dan dapat berpengaruh
terhadap sistem imun pasien.

Kunjungan rumah 30 Pada Kunjungan 3 hari setelah ke puskesmas dilakukan edukasi
Agustus 2021 dan evaluasi:
 Edukasi untuk menerapkan pola hidup bersih dan sehat serta
menerapkan cuci tangan menggunakan sabun.
 Edukasi dan konseling mengenai tonsillitis, faktor penyebab
tonsillitis serta pencegahannya.
 Edukasi dan konseling untuk menjaga pola makan dan mejaga
higinitas makanan.
Evaluasi yang saya lakukan
 Obat yang diberikan teratur dikonsumsi
 Keluhan pasien membaik

FARMAKOTERAPI
 Pengobatan tonsilitis meliputi
medikamentosa dan pembedahan.
Analgetik oral efektif untuk mengurangi
nyeri. Terapi antibiotik dikaitkan dengan
biakan dan sensitivitas yang tepat. Penisilin
masih merupakan obat pilihan, kecuali jika
terdapat resistensi atau penderita sensitif
terhadap penisilin. Pada kasus tersebut,
eritromisin atau antibiotik spesifik yang
efektif melawan organisme sebaiknya
digunakan.
 Farmakokinetik Penisillin

Mekanisme Kerja:  Penisilin didistribusikan secara luas

paruh 1-2 jam. Dosis amoksisilin adalah 250-500 mg
yang diberikan 3 kali sehari

arakhidonat menjadi prostaglandin 4-6 jam. Maks 4 g/ hari.

Efek Samping
Efek ini menyerupai gejala dari suatu
gangguan yang disebabkan oleh
produksi kortisol faal berlebihan, yakni
sindroma Cushing

Metformin mempunyai efek utama
