APA ITU SOAP?
Catatan yang bersifat sederhana, jelas,logis,dan tertulis. 4
metode Langkah dinamakan SOAP dipakai Untuk
mendokumentasikan asuhan pasien dan rekaman medis pasien
sebagai catatan kemajuan.

UMUMNYA
Digunakan untuk pengkajian awal pasien,dengan
penulisannya Sebagai berikut :
 1.SUBJECTIVE
Subjective dari metode soap adalah data-data yang dirasakan oleh pasien
yang bersifat subjektif misalnya sakit kepala,sesak nafas dan lain lain.
Juga data tentang apa yang dirasakan oleh pasien/ apa yang dapat
diamati tentang pasien tersebut merupakan gambaran apa adanya
dengan cara mengamati,berbicara,dan berespon dengan pasien
 2. OBJECTIVE
Objectif adalah data-data yang bersifat subjectif dan
bisa dibuktikan atau diukur dengan angka dan data
tertentu misalnya hasil pemeriksaan lab SGPT,
SGOT, tekanan darah, gula darah, respitory rate dan
lain sebagainya.
3.ASSESSMENT
Farmasis harus dapat menginterpretasikan data subyektif dan
obyektif untuk setiap problem untuk mengembangkan
rekomendasi terapi, mengikuti atau memonitor respon
terhadap suatu terapi, dan mendokumentasikan adanya
adverse drug reaction..
 4.PLAN

Plan adalah tindak lanjut dari assesment atau penilaian yang sudah kita
lakukan misalnya ada masalah di pasien gagal mendapatkan obat,
dosis berlebih, interaksi obat serta indikasi tanpa obat. Hal-hal yang
akan dilakukan terhadap pasien, meliputi treatment yang diberikan,
termasuk obat yang harus dihindari, parameter pemantauan (terapi dan
toksisitas) Dan Endpoint therapy informasi pada pasien
KASUS!
CONTOH

Seorang laki-laki usia yang berusia 70 tahun datang ke UGD dengan

keluhan sesak sejak 2 hari SMRS. Sesak berbunyi menciut yang tidak
dipengaruhi aktifitas dan posisi. Menurut kerabat, pasien sering
mengantuk sejak 1 hari ini. Batuk (+) sejak 2 minggu yang lalu dan
dahak berwarna hijau kental, demam (+). Pasien juga memiliki riwayat
merokok dan baru berhenti 1 tahun yang lalu, riwayat sesak menciut
sebelumnya (+). Tidak terdapat riwayat hipertensi, diabetes mellitus dan
alergi.
 CONTOH
1. Dari pemeriksaan didapatkan kesadaran GCS
14 (cendrung mengantuk), TD 100/60 mmHg,
Nadi 96x/menit, pernafasan 30x/menit,
saturasi 92, suhu 39.
Diagnosa : PPOK dan pneumonia 
2. Pemeriksaan torak tampak barel cest, gerakan

Diagnosa
PPOK dan Pneumonia
 Penyakit paru obstruktif kronis (PPOK) adalah peradangan
pada paru-paru yang berkembang dalam jangka panjang.
PPOK umumnya ditandai dengan sulit bernapas, batuk
berdahak, dan mengi (bengek).
 Pneumonia adalah peradangan paru-paru yang disebabkan
oleh infeksi. Pneumonia bisa menimbulkan gejala yang ringan
hingga berat. Beberapa gejala yang umumnya dialami
penderita pneumonia adalah batuk berdahak, demam, dan
sesak napas.
