DEFINISI
Sejarah
Fabricus ab Aquapendente (1537-1619)
CLP disebabkan oleh gangguan
embriologis
Pada akhir tahun 1950, Ralph millard
mengembangkan teknik yang dinamakan
cut as you go.

ETIOLOGI :
Bisa syndromik atau non syndromik.
Syndromik :
bila terdapat lebih dari satu malformasi dan
menyangkut lebih dari satu daerah perkembangan.
Non syndromik :
hanya terdapat satu malformasi atau terdapat
beberapa anomali yang berasal dari satu daerah
perkembangan.
Sebagian besar kasus celah bibir dan langit-langit adalah non
syndromik.
FAKTOR PENYEBAB :
A. Faktor HEREDITER
Sebagai faktor yang sudah dipastikan.
Gilarsi : 75% dari faktor keturunan resesif dan 25%
bersifat dominan.
1. Mutasi gen.
2. Kelainan kromosom
Faktor Genetik
Extra Cellular Molecules (ECM), yang berperan
sebagai faktor pertumbuhan dan induksi
terhadap molekul protein :
Bone Morphogenic Protein :
berperan membentuk palatum
Sonic Hedgehog:
membentuk primordial wajah dan epitel
Fibroblast Growth factor :
membentuk matrix ekstraseluler palatum
3. Nutrisi
Alkohol
5. Penyakit infeksi
Sifilis, virus rubella
6. Radiasi
7. Stres emosional
8. Trauma, (trimester pertama)
Embriology
Pembagian Lain :
OTTO-KRIENS dengan sistem LAHSHAL.
Keterangan :
Bibir (lips) : L
Gusi (Alveolus) : A
Langit-langit dibagi hard palate : H
soft palate : S
Bila tidak ada celah / normal : urutannya dicoret
Bila celah komplit (lengkap) : memakai huruf besar
Bila celah inkomplit (tidak lengkap) : memakai huruf kecil
Bila kelainan microform : memakai huruf kecil dalam
kurung.
Bilateral CLP
Celah yang terbentuk pada kedua sisi
lateral dari palatum dan bibir.
Menyebabkan gangguan struktur
wajah, gangguan pada 1/3 tengah
bagian wajah menyebabkan gangguan
struktur kosmetik wajah,
Masalah yang dapat muncul pada CLP
bilateral meliputi masalah anatomis
dan fungsional, masalah fungsional
meliputi potensi gangguan nutrisi,
fonasi, mendengar, serta bernafas.

Facial Cleft
No 3 Cleft
No 4 Cleft
Pre operation
Post operation
No 5 Cleft
Treatment :
Local transposition flaps
Bone Graft
No 6 Cleft
No 7 Cleft
No 8 Cleft
Extends from the lateral
canthus to the temporal
region.
A dermatocele often
occupies the coloboma of
the lateral commissure.
Reconstruction of the
lateral commissure is
achieved by local flap
No 9 Cleft
Is the rarest of the craniofacial
clefts.
The superior (cranial) hemisphere of
craniofacial clefts. The upper eyelid
is involved in its lateral third as is
the underlying lateral superior
orbital rim and roof.
Treatment consists of constructing a
superior orbital rim, orbital roof, and
forehead with split cranial bone
grafts.
Local flaps
No 10 Cleft
Located in the middle third of the eyelid
and eyebrow.
Cranial extension of the No. 4 cleft.
The soft tissue coloboma that disrupts the
midportion of the upper eyelid covers the
underlying defect of the central third of
the superior orbital rim and roof.
Encephalocele and hypertelorbitism.
Treatment : encephalocele repair and
correct the asymmetric hypertelorbitisms
and osteotomies
No 11 Cleft
Frequently occurs in combination with
the No. 3 facial cleft.
A small coloboma may be present in the
medial third of the upper eyelid.
The medial third of the eyebrow is also
disturbed, as the fault extends into the
frontal hairline.
The osseous path may pass lateral to the
ethmoid bone.
Treatment : local flaps, and periorbital
osteotomies are required to correct any
associated hypertelorbitism.
No 12 Cleft
No 13 Cleft
The No.13 cleft is the cranial
prolongation of the No. 1
cleft.
Paramedian location, it lies
medial to the eyebrow; an
omega-shaped distortion
marks its entrance into the
frontal hairline and
Hypertelorbitism.
The cribriform plate is
widened.
No 14 Cleft
