AT GALLAF PROJECT
The types of permits are cold work, hot work A, hot work B
and confined space entry.
COLD WORK
Cold Work permit required for work related to non-routine
construction, maintenance, and repair work provided that
in the work. Do not use equipment that can cause fire or
ignition sources. Examples are as follows:
 Scaffolding
 Pressure / pneumatic / hydro test
 Painting activity
 Routine, critical and heavy lift(Crane, Forklift, Man
lift, etc.)
 Any other hazardous work as decided by HSE

HOT WORK
A hot work permit is required for any type of work related
to the use of ignition sources that can ignite flammable
materials. This permit is granted for work that requires
open flames or sparks. For the Gallaf Batch 2 project, there
are several Hot Work Permits, including:
 Hot Work A (red - 2)
 Gas cutting activity (CNC, Burner, etc)
 Grinding, Welding, Fitting, Drilling and Gouging
 Any other hot work as decided by HSE Project
Manager/Project Manager
 Hot Work B (yellow - 3)
 Energizing/de-energizing of electrical system for
the commissioning or testing of electrical and
mechanical system
 Blasting
 Non-Destructive Examination (NDE)
 Any other hazardous work as decided by HSE

CONFINED SPACE ENTRY (Green - 4)
A work permit to enter this confined space is required if a
person, either all or part of his body, must enter a confined
space, this work includes; Entering tanks, vessels,
separators and entering sewers, tubs (pits), dug holes with

Jenis izin tersebut adalah pekerjaan dingin, pekerjaan panas
A, pekerjaan panas B dan masuk ruang terbatas.
COLD WORK / Pekerjaan Dingin (blue - 1)
Surat ijin kerja dingin diperlukan untuk pekerjaan yang
berhubungan dengan pekerjaan konstruksi, perawatan,
perbaikan yang sifatnya tidak rutin dengan ketentuan bahwa
dalam pekerjaan tsb. Tidak memakai peralatan yang dapat
menimbulkan api atau sumber nyala. Contohnya sebagai
berikut :
 Perancah
 Pressure / pneumatic / hydro test
 Kegiatan pengecatan
 Pengangkatan Rutin, Kritikal, dan berat (Crane,
Forklift, Manlift, etc.)
 Pekerjaan berbahaya lainnya seperti yang
diputuskan oleh Manajer Proyek / Manajer Proyek
HSE
HOT WORK / Pekerjaan Panas
Surat ijin kerja panas diperlukan untuk setiap jenis pekerjaan
yang berkaitan dengan penggunaan sumber penyalaan yang
dapat menyalakan bahan yang mudah terbakar. Surat ijin ini
diberikan untuk pekerjaan yang memerlukan api terbuka
atau bunga api. Untuk projek Gallaf Batch 2, ada beberapa
Hot Work Permit, diantaranya adalah :
 Hot Work A (red - 2)
 Gas cutting activity (CNC, Burner, etc)
 Grinding, Welding, Fitting, Drilling and Gouging
 Pekerjaan panas lainnya seperti yang diputuskan
oleh Manajer Proyek / Manajer Proyek HSE
 Hot Work B (yellow - 3)
 Pemberian energi / penghilangan energi pada sistem
kelistrikan untuk commissioning atau pengujian
sistem kelistrikan dan mekanis.
 Blasting
 Non-Destructive Examination (NDE)
 Pekerjaan berbahaya lainnya seperti yang
diputuskan oleh Manajer Proyek / Manajer Proyek
HSE.

apabila seseorang baik seluruh atau sebagian tubuhnya

a depth of more than 1.3 meters
 Hot work, whether carried out in a confined space
or otherwise
 Work involves entry into confined space -
 Spray painting and painting in confined space -
 Ballasting and de-ballasting of vessel/tank and
other confined space
 Any other hazardous work as decided by HSE

ISOLATION CERTIFICATE
Following the PTW process, the requestor applying for a
specific PTW shall assess the needs for Energy Isolation (i.e.
mechanical /electrical) in conjunction with the relevant
Authorizing Persons and functions involved for the specific
task.
Upon confirmation that Energy Isolation is required, the
Performing Authority shall seek the support of the
appointed Competent Person to prepare the relevant LOTO
Isolation Certificate which shall include the details for
proper isolation (i.e. de-energize) of the system, by tuning
off/locking switches or valves, securing the
machine/device/power lines in de-energized state.
The LOTO Isolation Certificate serial number shall be cross-
referenced to the relevant PTW.

diputuskan oleh Manajer Proyek / Manajer Proyek
HSE
SERTIFIKAT ISOLASI
Setelah proses PTW, pemohon yang mengajukan PTW
tertentu harus menilai kebutuhan Isolasi Energi (yaitu
mekanis / listrik) sehubungan dengan Pemberi Otorisasi
terkait dan fungsi yang terlibat untuk tugas tertentu.
Setelah konfirmasi bahwa Isolasi Energi diperlukan, Otoritas
Pelaksana harus mencari dukungan dari CPI yang ditunjuk
untuk menyiapkan Sertifikat Isolasi LOTO yang relevan yang
harus mencakup perincian untuk isolasi yang tepat (yaitu de-
energize) dari sistem, dengan mematikan / mengunci sakelar
atau katup, mengamankan mesin / perangkat / saluran listrik
dalam keadaan tidak berenergi.
Nomor seri Sertifikat Isolasi LOTO harus direferensikan ke
PTW terkait.
