Pencarian artikel dilakukan pada tanggal 23 Juli 2022 pada tiga database yaitu
PubMed Clinical Queries dan Cochrane Library. Pencarian artikel pada kedua
database menggunakan 4 kata kunci yaitu Tuberculosis, Diabetes, Metformin, dan
Impact. Detail lebih lengkap mengenai kata kunci dan hasil pada masing-masing
database dapat dilihat pada Tabel 3.1.

Penelitian ini memenuhi semua syarat di kriteria kedua karena dapat ditemukan pada

Metformin dan konversi sputum TB
Comment:
Artikel ini menjelaskan adanya heterogenitas dengan penelitian lainnya. Hal ini
disebabkan karena adanya perbedaan metode diagnosis antar studi yang digunakan.
Analisis mengenai dampak metformin pada kekambuhan TB dan hasil konversi sputum
TB sebagai salah satu aspek penilaian keberhasilan pengobatan TB menunjukkan adanya
heterogenitas sedang secara statistik.
Laporan ini membahas tentang pasien TB dengan DM yang sedang menjalani
pengobatan anti tuberkulosis, sekaligus metformin sebagai antidiabetes, berdasarkan
metode pencarian literatur, didapatkan artikel dari penelitian Xinyu Yu, Ling Li,
Liangtao Xia, Xin Feng, Fan Chen, Shiyi Cao dan Xiang Wei dengan judul Impact of
metformin on the risk and treatment outcomes of tuberculosis in diabetics: a systematic
review. Penelitian ini merupakan sistematic review tahun 2019. Dari 12 studi yang termasuk
dalam systematic review ini, hanya 2 studi yang membahas tentang outcome tentang
keberhasilan pengobotan metformin berupa perubahan kultur sputum dan kejadian rekurensi.
Kedua studi tersebut merupakan studi cohort retrospektif yang dilakukan di korea dan cina.
Karakteristik dan hasil kedua penelitian tersebut dapat dilihat pada Tabel 1.

KESIMPULAN
Penggunaan metformin pada pasien TB dengan DM, tidak memberikan
outcome berupa pengurangan perubahan kultur positif dan kejadian relaps yang
signifikan, namun bisa menurunkan kemungkinan kematian selama pengobatan TB,
sehingga tetap berpotensi sebagai agen terapi hiperglikemik tambahan yang diarahkan
pada pasien TB.
SARAN
Pasien TB dengan DM yang mendapat tatalaksana berupa metformin sebagai
agen terapi hendaknya memberikan jarak waktu pemberian dengan obat anti
tuberkulosis untuk mencegah gejala gastrointestinal serta menggunakan
metoklopramide sebagai anti-emetik apabila keluhan gastrointestinal dirasakan, untuk
mengoptimalkan absorbsi pengobatan. Diperlukan identifikasi lebih lanjut mengenai
kombinasi antara insulin dan metformin untuk keberhasilan pengobatan TB.
