ANATOMI
Terdiri atas 3 bagian:
1. Konjungtiva tarsal yang menutupi
tarsus, sukar digerakkan dari tarsus.
2. Konjungtiva bulbi menutupi sklera
dan mudah digerakkan dari sklera
di bawahnya.
3. Konjungtiva fornises/forniks yang
merupakan tempat peralihan
konjungtiva tarsal dengan
konjungtiva bulbi.
 KONJUNGTIVITIS

Konjungtivitis adalah peradangan pada konjungtiva

yang ditandai dengan adanya pelebaran pembuluh
darah, infiltrasi seluler dan eksudasi.

ETIOLOGI KONJUNGTIVITIS BAKTERI
1. Hiper Akut (purulent)
● Neisseria gonorrhoeae
● Neisseria meningitidis
● Neisseria kochii

2. Akut (mukopurulen)
● Pneumococcus (Streptococcus pneumoniae)
● Haemophilus aegyptius (Koch-Weeks baccilus)

3. Sub Akut
● Haemophilus influenzae

4. Menahun/Kronik, termasuk Blefarokonjungtivitis

● Staphylococcus aureus
● Moraxella lacunata (diplobacillus dari Morax-Axenfeld)

ETIOLOGI
Neisseria gonorrheae, N. Meningiditis, Hemophilus spesies, Streptococcus, dan Coryne bacterium
diphtheria.

GAMBARAN KLINIS
Konjungtivitis purulent hiperakut yang disebabkan ole N. gonorrheae (konjungtivitis gonore) pada bayi baru lahir usia 1-3
hari, memerlukan perhatian serius. Terdapat konjungtivitis hiperakut bilateral, eksudat (sekret) serosanguinolen lalu
menjadi purulent kental kekuningan. Edem palpebra yang berat, kemosis yang berat.

 Konjungtivitis Bakteri Hiperakut
KOMPLIKASI
Pada konjungtivitis gonorrheae, komplikasi yang dapat terjadi tukak kornea marginal terutama di bagian atas. Pada anak-
anak sering terjadi keratitis/tukak kornea sehingga sering terjadi perforasi kornea. Perforasi kornea dapat mengakibatkan
endoftamitis dan panoftalmitis sehingga terjadi kebutaan total

GAMBARAN KLINIS
Terapi konjungtivitis gonore pada bayi adalah
Irigasi konjungtiva dengan larutan salin tiap jam sampai sekret teratasi. Tetesi fluoroquinolone atau gentamisin. Dianjurkan
injeksi ceftriaxone IM atau IV dengan dosis tunggal 25-50 mg/kgBB atau cefotaxim dosis tunggal 100 mg/kgBB pada
kasus dimana terdapat kelainan lain selain konjungtivitisnya.

ETIOLOGI
Staphylococcus aureus, Staphylococcus epidermidis, streptococcus pneumoniae, Hemophylus influenzae.

GAMBARAN KLINIS
Injeksi konjungtiva yang hebat, mata merah (pink eye), secret mukopurulen, merasa ada benda asing, edema palpebral dan
saling melengket waktu bangun pagi, kadang ada lakrimasi, kemosis, membran dan perdarahan subkonjungtiva.
Konjungtivitis bakteri sering terdapat dalam bentuk epidemi.

KOMPLIKASI
Konjungtivitis yang disebabkan oleh H. Influenza pada anak kadang-kadang disertai otitis media.

GAMBARAN KLINIS
Terapi awal dengan antibiotik tetes mata atau salep mata seperti neomisin polymixin, chloramfenikol,
fluroquinolon/ciprofloxacin, levofloxacin, tobramisin, gentamisin.
Apabila terapi awal yang diberikan tidak memberikan respon setelah 3-5 hari, maka terapi diberikan
sesuai dengan hasil pemeriksaan mikrobiologi .

Konjungtivitis Bakteri Kronik
• Konjungtivitis bakterial menahun terjadi pada pasien dengan obstruksi
ductus nasolakrimalis dan dakriositisis menahun, yang biasanya
unilateral. Infeksi ini juga dapat menyertai blefaritis bacterial
menahun atau disfungsi kelenjar Meibom. Pasien dengan sindrom
palpebra lemas dan sktropion dapat menimbulkan konjungtivitis
bacterial sekunder.
 DASAR DIAGNOSIS
1. Anamnesa

● Rasa terbakar
● Gatal
● Fotofobia
● Kadang-kadang juga merasa sakit

2. Pemeriksaan Fisik
● Hiperemia
● Lakrimasi
● Eksudasi
● Edem palpebra
● Edem konjungtiva bulbi (kemosis)
● Folikel
● Membran, pseudomembran
● Papil hipertropi
● Pembesaran kelenjar preaurikule

 PROGNOSIS
• Konjungtivitis bakterial akut hampir selalu sembuh sendiri. Tanpa
diobati, infeksi dapat berlangsung selama 10-14 hari: jika diobati
dengan memadai, 1-3hari, kecuali konjungtivitis staphylococcus
(yang dapat berlanjut menjadi blefarokonjungtivitis dan memasuki
tahap menahun) dan konjungtivitis gonokokus (yang bila tidak diobati
dapat berakibat perforasi kornea dan endoftalmitis).
• Konjungtivitis bakterial menahun mungkin tidak dapat sembuh
sendiri dan menjadi masalah pengobatan yang menyulitkan.

 EDUKASI
• Pencegahan transmisi merupakan hal penting yaitu, dengan sering
melakukan cuci tangan, tidak menyentuh mata yang infeksi, tidak
berbagi handuk atau kosmetik. Pasien yag terinfeksi sebaiknya tidak
masuk kerja/sekolah dulu. Pemakaian lensa kontak harus berhenti
pemakaiannya sampai tanda dan gejala hilang.
