 DEFINISI
Pansitopenia adalah kondisi hematologi yang ditandai dengan penurunan ketiga sel darah tepi. Hal ini ditandai
dengan hemoglobin kurang dari 11,5 g/Dl pada wanita dan 13,5 g/dL pada pria, trombosit kurang dari 150.000 per
mcL, dan leukosit kurang dari 4000 per ml (atau jumlah neutrofil absolut kurang dari 1500 - 1800 per ml) (Chiravuri
& Jesus, 2021). Leukopenia terutama terlihat sebagai neutropenia karena neutrophil merupakan mayoritas leukosit.
Pansitopenia bukanlah penyakit, tetapi manifestasi dari kondisi lain yang mendasarinya. Hal ini terkait dengan
beberapa kondisi jinak dan ganas. Pansitopenia dapat disebabkan oleh penurunan produksi sel atau peningkatan
destruksi. Siapapun yang mengalami pansitopenia harus menjalani evaluasi menyeluruh untuk mengidentifikasi
etiologi yang mendasarinya (Chiravuri & Jesus, 2021). Ada beberapa penyebab pansitopenia yang sangat bervariasi
pada anakanak, mulai dari penekanan virus sumsum tulang sementara hingga infiltrasi sumsum tulang oleh keganasan
yang mengancam jiwa. Pansitopenia perifer memerlukan pemeriksaan mikroskopis dari biopsi sumsum tulang dan
aspirasi sumsum untuk menilai seluleritas dan morfologi secara keseluruhan.
Dampak dari pensitopenia bagi penderita adalah adanya gangguan sirkulasi darah dan gangguan pemenuhan
kebutuhan dasar. Pengobatan anemia aplastik dengan TST maupun dengan penggunaan imunosupresan menimbulkan
efek jangka panjang pada pasien. Pasien yang mampu bertahan hidup beresiko terkena keganasan. Pada pasien
dengan penurunan eritrosit atau sel darah merah dapat berakibat gagal jantung, pada pasien dengan penurunan
trombosit dapat berakibat perdarahan dan pada pasien dengan penurunan leukosit dapat terjadi penurunan imun yang
berakibat pada mudah terpapar infeksi.

mengancam nyawa pada pansitopenia berat. Pasien dapat datang dengan p

Berdasarkan teori beberapa

pansitopenia adalah gangguan

efektif, perfusi jaringan perifer tidak
efektif, intoleransi aktivitas, defisit
perawatan diri, resiko cedera /
jatuh, resiko infeksi. Namun tidak
semua diagnosa berdasarkan teori
muncul pada Ny. S. Diagnosa yang
muncul sesuai teori hanya
diagnosa perfusi perifer tidak
efektif. Diagnosa lainnya yang
muncul adalah resiko perdarahan
dan defisit pengetahuan. Hal itu
terjadi karena dalam penegakan
diagnosa berdasarkan keadaan
dan kondisi pasien.

3 Intervensi Keperawatan
Rencana intervensi yang akan dilakukan
sesuai dengan diagnosa perfusi perifer
tidak efektif adalah manajemen sirkulasi
dengan melakukan periksa sirkulasi perifer,
identifikasi risiko, gangguan sirkulasi,
anjurkan untuk selalu beraktivitas.
Selanjutnya rencana intervensi yang akan
dilakukan sesuai dengan diagnosa resiko
perdarahan adalah monitor tanda dan
gejala perdarahan, monitor ttv, jelaskan
tanda dan gejala perdarahan, anjurkan
meningkatkan asupan makanan vit.K,
berikan obat pengontrol perdarahan, jika
perlu.
5 Evaluasi Keperawatan
Setelah dilakukan implementasi maka dilanjutkan
mengevaluasi, dan didapatkan hasi evaluasi pada hari
selasa 1 november 2022 untuk diagnosa perfusi perifer
tidak efektif adalah S : Pasien mengatakan lemah, O :
TTV : TD: 120/80 mmHg, N : 74 x/menit, RR : 21 x/menit,
S : 36,4 oC, GDS: 165, A : perfusi perifer tidak efektif, P :
lanjutkan intervensi : memeriksa sirkulasi perifer,
mengidentifikasi risiko gangguan sirkulasi, menganjurkan

untuk selalu beraktivitas. Selanjutnya evaluasi pada
diagnosa resiko perdarahan adalah S : Pasien
mengatakan gusi berdarah ketika disikat, O: TTV: TD:
120/80 mmHg, N : 74 x/menit, RR : 21 x/menit, S :
36,4oC, A : Risiko perdarahan, P : Lanjutkan intervensi :
memonitor tanda dan gejala perdarahan, memonitor ttv,
menganjurkan meningkatkan asupan makanan vit.K,
berkolaborasi pemberian obat pengontrol perdarahan,
jika perlu.
