Reaksi Anafilaktik
Diperantarai Ig E yang dikeluarkan sel mast/basophil
Terbagi 2, reaksi cepat (detik menit, contoh : syok anafilaktik, angioedema)
dan reaksi lambat (menit jam, max 24 jam, contoh : urtikaria ec drug
consumption, stomatitis alergika)
Mekanisme nya berupa, Kompleks alergen-IgE berikatan dengan sel mast
degranulasi sel mast pelepasan histamin dan mediator inflamasi lainnya.
Angioedema
Allergen reintroduced,
berikatan dengan IgE dan
Allergen masuk dalam mast cells Pelepasan mediator inflamasi
(calcium intraseluler, histamin,
tubuh, pembentukan
leukotriene, prostaglandin, dll)
antibody (IgE)
Mencari sebab allergen, stop penggunaan/konsumsi. Jika tidak memungkinkan desensitization therapy
List penggunaan obat pasien (terutama jika ada ACE1-Inhibitor)
Initial treatment antihistamine block aktifitas histamin 1&2 juga glucocorticoid
Penggunaan kortikosteroid mencegah konversi histidine menjadi histamine dengan menghambat
histidine decarboxylase enzyme
Pada kasus life threatening (edema laring), lakukan injeksi ephinephrine 0,5 mL (1:1000) intramuscular,
dapat diulang selama 10 menit sekali sampai recovery. Pasien diposisikan tredelenburg maneuver untuk
menjaga aliran darah ke otak.
Jika pasien tidak merespon pemberian ephinephrine setelah berulang kali, disarankan melakukan
prosedur tracheoctomy/cricothyroidotomy untuk membebaskan airway pasien
