Latar Belakang

- Rotator cuff : jaringan ikat fibrosa yang

mengelilingi bagian atas tulang humerus
- Fungsi sebagai stabilisator yang bersifat
dinamis untuk menjaga stabilitas
glenohumeral joint
- Rotator cuff = SITS

- Prevalensi cedera meningkat seiring

pertambahan usia
- 4% kelompok usia <40 tahun dan usia > 60
tahun 54%

Definisi

– Cedera rotator cuff adalah kelainan patologis yang ditandai dengan robekan sebagian
dan/atau total pada kelompok otot dan tendon di sendi bahu, tepatnya sendi glenohumeral
– Cedera pada rotator cuff atau rotator cuff injury menyebabkan terganggunya stabilitas
sendi bahu akibat kerusakan atau lesi dari Rotator Cuff

– 4% kelompok usia <40 tahun dan 54% berusia >60 tahun

– 9,7% berusia 20 tahun dan meningkat menjadi 62% pada pasien yang berusia >80
– 0% pada pasien usia 20 - 49 tahun; 10,7% pasien usia 50 - 59 tahun; 15,2% pasien usia 60

– Penelitian di Jerman terdapat sebesar 23% kasus bersifat asimtomatik dengan prevalensi

 Manifestasi Klinis

– Akut : nyeri tiba-tiba dan kelemahan pada bahu

– Kronik : keluhan nyeri timbul dgn onset yg lama, terutama aktivitas overhead
– Keluhan memburuk pada malam hari
– Sulit untuk aktivitas sehari-hari
Pemeriksaan fisik :
1. Inspeksi apakah ada atopi pada otot rotator
2. Palpasi ruang subacromial untuk mencari apakah ada nyeri tekan atau tidak
3. Rentang gerak aktif mungkin terbatas karena nyeri dan atau kelemahan
4. Menilai kelemahan saat abduksi (supraspinatus injury), external rotation (infraspinatus injury), atau
internal rotation (subscapularis injury)

Kesimpulan

– Rotator cuff bersifat dinamis dan berfungsi sebagai stabilisator untuk menstabilkan sendi glenohumeral
– Otot rotator cuff terdiri dari 4 otot yaitu; supraspinatus, infraspinatus, teres minor, dan subscapularis
yang biasanya disingkat menjadi “SITS”. Otot-otot tersebut memiliki fungsi yang berbeda; supraspinatus
berfungsi untuk gerakan absuksi dan fleksi, infraspinatus untuk gerakan rotasi eksternal, teres minor
untuk gerakan rotasi eksternal, dan subscapularis untuk adduksi dan rotasi internal.
– Gangguan yang sering terjadi pada rotator cuff adalah cedera. Rotator cuff injury menyebabkan
terganggunya stabilitas sendi bahu akibat kerusakan atau lesi dari rotator cuff.
– Cedera dapat menimbulkan gejala nyeri. Untuk mendiagnosis, selain dari anamnesis dan pemeriksaan
fisik, juga perlu dilakukan pemeriksaan penunjang berupa imaging/rontgen, MRI, CT-scan dan USG.
– Penanganan berupa tindakan nonoperatif dan operatif
