Anamnesis Pasien

Pasien datang mengeluh badan terasa lemas sejak 1 hari ini,

kepala terasa hoyong seperti berputar, mual dijumpai, muntah
tidak ada, nafsu makan berkurang, BAB dan BAK normal.
Nyeri dada tidak ada, sesak nafas dijumpai 1 hari ini. Batuk
tidak ada, demam tidak ada.
 Pemantauan Tanda-tanda Vital dan Skala Nyeri

oyong tidak ada. RR: 20 x/menit dengan menghambat Levofloksasin.
Suhu: 36.5°C penyerapan sistem
SpO2: 100% gastrointestinal atau
Sistem pencernaan
manusia.
Kesimpulan
Berdasarkan pengamatan yang telah dilakukan di RSUD H. Amri Tambunan pada bulan
November pasien Bernama Miss.S dengan diagnosa Cholecystitis dengan Sludge GB + Cystitis +
Krisis Hipertensi + DM Tipe 2 + Vertigo Berdasarkan pemantauan, didapatkan
katagori DRPs, yaitu :
1. adanya indikasi yang tidak diterapi untuk asam urat pada pasien. solusinya, direkomendasikan
pemberian obat asam urat kepada pasien.
2. adanya indikasi yang tidak diterapi untuk nafsu makan berkurang. solusinya, direkomendasikan
pemberian curcuma untuk suplemen penambah nafsu makan pasien.
3. Adalat Oros menurunkan efek Metformin. solusinya, disarankan kepada pasien saat
mengkonsumsi adalat oros dan metformin, diberi jarak waktu.
4. Sucralfate menurunkan kadar levofloksasin dengan menghambat penyerapan sistem
gastrointestinal atau Sistem pencernaan manusia. Disarankan Pantau pemberian Sucralfate dan
Levofloksasin.
