MORFOLOGI
Telur :
berbentuk oval atau bulat.
ukuran 47 x 37 mikron,
memiliki dinding berupa dua lapis membrane yang
melindungi embrio heksakan di dalamnya.
Membran sebelah dalam tipis berpori2/embriopore ,
terdapat dua buah penebalan dimana keluar 4-8 filamen
halus pada kedua kutubnya ( polar filamen ).

Dewasa
uk : p 25–40 mm , lebar 0,1- 0,5 mm,
proglotidnya mencapai 200 buah, uk strobila berbanding
terbalik dengan jumlah cacing didalam hospes.
Scolex : bulat, Ø 0,32 mm,memp 4 batil isap seperti mangkok,
memiliki rostelum yang pendek dan refraktil, dilengkapi
sebaris kait ( 20-30) .
Leher : panjang dan langsing.
Strobila : dimulai dari proglotid muda yang sangat pendek dan
sempit, belum terbentuk organ genital, kearah distal semakin lebar
dan pada ujung distal strobila membulat
Proglotid matur : bentuk trapezium dengan
lebar proglotid kira-kira 4 kali panjangnya,
dengan satu genital pore dilateral( kiri ) ,satu
ovarium berlobus, 3 testis ( satu didekat
genital pore dan dua lainnya di dipisahkan
oleh ovarium ), uterus berbentuk kantung yang
berisi 80 – 180 butir telur

• Habitat cacing ini adalah pada 2/3 bagian atas
dari ileum. Cacing ini dapat hidup sampai
beberapa minggu, sedangkan telur cacing ini
hanya dapat bertahan hidup selama 2 minggu
setelah dikeluarkan bersama feses hostnya.
Cacing ini di dalam siklus hidupnya tidak
memerlukan hospes perantara, kecuali
Hymenolepis nana var. fraterna yang hospes
alamiahnya adalah tikus dan menggunakan
flea serta kumbang sebagai hospes
perantaranya.
• Proglotid gravid pecah didalam usus host , telur keluar
bersama feses penderita termakan manusia. Di dalam
usus halus, telur akan menetas menjadi oncospher dan
menembus villi usus halus serta akan kehilangan
kaitnya. Selanjutnya dalam 4 hari kemudian akan
menjadi larva cysticercoid/cercocystis . Larva ini
terdapat pada tunica propria usus halus penderita.
Beberapa hari kemudian larva ini akan kembali ke
lumen usus penderita untuk menjadi dewasa dalam
waktu 2 minggu
• Kadang-kadang telur dapat menetas di dalam
lumen usus halus penderita kemudian oncospher
akan menembus villi usus dan siklus hidupnya
akan berulang kembali. Cara infeksi yang
demikian ini disebut sebagai autoinfeksi interna
yang dapat memperberat infeksi sehingga
memungkinkan terjadi reinfeksi pada individu
yang sama. Dalam 30 hari setelah infeksi, dapat
ditemukan telur dalam tinja hospes.
Pada rodents, flour beetle merupakan
intermediate host. Pada kasus ini telur H.nana
tertelan oleh insekta dan didalam tubuh insekta
telur akan menetas dan penetrasi menembus
dinding intestinal lalu akan masuk ke rongga
tubuh insekta /hemocoel dan akan tumbh
menjadi cysticercoid larva . Lalu insekta ini
akan tertelan oleh rodent atau oleh manusia
(accidentally host ) , cysticercoid larva akan
melekat pada tunica propria usus halus lalu
tumbuh menjadi dewasa.
PATOGENESA DAN MANIFESTASI KLINIS
tergantung pada intensitas infeksi, status imunologis hospes
dan penyakit penyerta .
tidak terjadi kerusakan pada mukosa usus tetapi dapat terjadi
desquamasi sel epithel dan nekrosis pada tempat perlekatan
cacing dewasa, sehingga dapat menimbulkan enteritis pada
infeksi yang berat.
Infeksi yang ringan asymptomatis / keluhan ringan
infeksi yang berat ( >1000 cacing, pada anak-anak o.k
autoinfeksi interna ): nafsu makan berkurang , BB menurun ,
nyeri epigastrium, nyeri perut dengan atau tanpa diare yang
disertai darah, mual, muntah, pusing, toxaemia, pruritus anal,
uticaria serta gangguan syaraf misalnya irritabilitas, konvulsi
dan kegelisahan, anemia sekunder dan eosinofilia.
DIAGNOSA
ditemukannya telur dalam feses penderita. Proglotid
biasanya tidak ditemukan di dalam feses, karena telah
mengalami desintegrasi di dalam usus sebelum
dikeluarkan.

Hymenolepis diminuta, while less frequent, has been
reported from various areas of the world.
MORPHOLOGY
Eggs
These eggs are round or slightly oval, size 70
- 85 µm X 60 - 80 µm, with a striated outer
membrane and a thin inner membrane.
The space between the membranes is smooth
or faintly granular. The oncosphere has six
hooks. There are no polar filaments extending
into the space between the oncosphere and the
outer shell.

Adult
The scolex in this species have 4 suckers
And unarmed Rostellum
the width of each proglottid is greater than its
length. The morphology of proglottids is
markedly similar to that of H. nana
Eggs of Hymenolepis diminuta are passed out in the feces of
the infected definitive host (rodents, man) . The mature eggs
are ingested by an intermediate host (various arthropod
adults or larvae) , and oncospheres are released from the eggs
and penetrate the intestinal wall of the host , which develop
into cysticercoid larvae. Species from the genus Tribolium
are common intermediate hosts for H. diminuta. The
cysticercoid larvae persist through the arthropod's
morphogenesis to adulthood. H. diminuta infection is
acquired by the mammalian host after ingestion of an
intermediate host carrying the cysticercoid larvae
• Humans can be accidentally infected through the ingestion of
insects in precooked cereals, or other food items, and directly from
the environment (e.g., oral exploration of the environment by
children). After ingestion, the tissue of the infected arthropod is
digested releasing the cysticercoid larvae in the stomach and small
intestine. Eversion of the scoleces occurs shortly after the
cysticercoid larvae are released. Using the four suckers on the
scolex, the parasite attaches to the small intestine wall. Maturation
of the parasites occurs within 20 days and the adult worms can
reach an average of 30 cm in length . Eggs are released in the
small intestine from gravid proglottids that disintegrate after
breaking off from the adult worms. The eggs are expelled to the
environment in the mammalian host's feces
Clinical Presentation
Hymenolepis nana and H. diminuta infections are
most often asymptomatic. Heavy infections with H.
nana can cause weakness, headaches, anorexia,
abdominal pain, and diarrhea.
Diagnostic Findings
The diagnosis depends on the demonstration of eggs
in stool specimens. Concentration techniques and
repeated examinations will increase the likelihood of
detecting light infections
Treatment Information
Praziquantel, adults and children, 25mg/kg in a
single-dose therapy.
Alternatives:
Niclosamide*: adults, 2 gm in a single dose for 7
days; children 11-34 kg, 1 gm in a single dose on day 1
then 500 mg per day orally for 6 days; children > 34
kg, 1.5 gm in a single dose on day 1 then 1 gm per day
orally for 6 days.
Nitazoxanide: adults, 500 mg orally twice daily for
3 days; children aged 12-47 months, 100 mg orally
twice daily for 3 days; children 4-11 years, 200 mg
orally twice daily for 3 days.
