WRONG BLIND INSERTING
1. PURPOSE
 
Untuk membuat standar yang sistematik dari Lock Out dan Tag Out (LOTO)
yang melibatkan sumber energy bahaya, seperti suhu, tekanan, zat berbahaya,
dll., sebelum bekerja, saat bekerja, dan setelah selesai bekerja dengan
peralatan terkait dan sumber bahaya lain. Memastikan pekerjaan aman, tidak
menyebabkan accident, incident, dan nearmiss, termasuk cedera, kehilangan
properti, dan kerusakan lingkungan ketika bekerja di fasilitas CAP.

2. SCOPE
 
LOTO digunakan untuk semua pekerjaan di area PT. Chandra Asri
Petrochemical, Tbk. oleh pekerja yang dilibatkan dalam pekerjaan dengan
sumber energi dan/atau melepas alat atau pipa yang berbahaya di proses
produksi atau dalam sumber energi yang berbahaya, seperti tekanan, suhu,
zat berbahaya, dll
2. SCOPE CONT.
 
Setiap orang yang dilibatkan harus memenuhi standar, yang terdiri dari
karyawan CAP, kontraktor, dan kontraktor temporary dan individu lain yang
dapat dicederai oleh pekerjaan, kecuali ada kontrol khusus yang mempunyai
instruksi kerja dan isi yang sesuai prosedur operasi standar untuk isolasi
sumber listrik dan/atau Line Break.

Ketika proyek baru atau harus memodifikasi sistem lebih lanjut, perhatikan
bahwa sistem dapat diisolasi dengan aman, dan juga memperhatikan
pemasangan Valve, Drain-point-Vent, dan sebagainya.
 LOTO TRACKING CARD AND LOTO TAGS
1.LOTO Tracking Card or Drawing shall be supplied by Area Authority.
All parties shall use the same LOTO Tracking Card or Drawing as used
by Area Authority.

2.There are four colors of LOTO Tagging Cards, Yellow for Area
Authority, Dark Blue for Work Execution Authority, Pink for Electrical
Section and Turquoise for Instrument Section.

 General LOTO Step of Work
* AA menyiapkan paket isolasi untuk kegiatan yang terdiri dari prosedur
isolasi, pembaharuan P&ID, LOTO Tracking Card dan proses persetujuan.

* AA harus membuat rencana isolasi dan identifikasi lokasi untuk mengunci di

P&ID dan form LOTO Tracking Card.

* Field Operator AA harus menjadi yang pertama untuk mengetahui bahwa

inlet dan outlet valve isolasi telah pada kondisi yang diinginkan (tutup/buka)
dan di tag. Semua valve yang dikunci dan tag harus dicatat di LOTO Tracking
Card dan P&ID terbaru. Pihak lain juga akan menggunakan Tracking Card
atau P&ID yang digunakan oleh Area Authority.

* Kunci milik AA harus ditempatkan di CCR/PCR.

* Field Operator AA melepaskan tekanan dan residu chemical dan kemudian

verifikasi bahwa sistem isolasi telah aman dengan memilih metode tes
sekurangnya dua cara seperti pembacaan pressure gauge, membuka drain,
knocking sound, check temperature, check gas, test run, machinery, dll.
* WEA harus lock dan tag valve yang sama dan mencatatnya sebelum
memulai pekerjaan ( merujuk ke Standard OSHA 29 CFR 1910.147)

Jika power perlu untuk diisolasi, pada MCC wajib dikunci dan ditag untuk
semua voltase. Perhatikan hal berikut:
* Untuk isolasi elektrikal kurang dari 380 kV dapat dilakukan oleh
AA dan untuk isolasi elektrik lebih dari 380 kV harus dilakukan oleh
Electrical Section dan AA.
* Electrical Section wajib rack out di MCC kemudian lock dan tag
pada switch gear di switch house dan juga mencatat semua lock dan
tag yang digunakan.
* Isolasi elektrik pada tegangan 6 kV atau diatasnya, dilarang
menggunakan cincin dan jam tangan dari metal untuk mencegah
sengatan listrik
* Di CSS/LOP/COS, AA dan Electrical wajib meletakkan tag dan
lock (tergantung dari fasilitasnya). Jika tidak ada fasilitas untuk
lock, mereka tidak perlu lock dan hanya tag (mandatory).
*Jika sebuah instrumentasi dilibatkan, Instrument Section wajib lock dan tag
energi yang terisolasi, seperti udara atau suplai listrik ke instrumen dan
mencatatnya.

*Jika dibutuhkan untuk mengetes peralatan atau solo run tes, seluruh
aktifitas harus dicatat dan meminta persetujuan kepada AA sebelum
melalukannya.

*Ketika pekerjaan telah selesai, Work Execution Authority harus melepaskan

semua lock dan tag yang diletakkan olehnya, catat dan tanda tangan pada
LOTO Tracking Card removed column dan kembalikan Master Work Permit.

*Sebelum commissioning, AA harus melepaskan semua lock dan tag dan

menandatangani LOTO removed column dari LOTO Tracking Card.
 Tagging Procedure for Inserting and Removing
Blind
• 1. PURPOSE
• To provide a systematic work procedure for tagging,
inserting and removal of blind, especially during
TAM/SDM, when thousands of blinds will be inserted
and removed, thus, enhancing safety, job planning and
productivity in CA.

• 2. SCOPE
• This procedure shall be used whenever blind has to be
installed for positive isolation before work is to be
carried out safely and for removing blind before a
system is commissioned. This procedure shall be
practiced in all Red Areas of CA.
• 4. DEFINITION
Blinds shall include:
• Spectacle blind that is permanently installed in a pair of
flanges.
• Round blind cut from steel plate with a handle for
positive isolation of a line or equipment.
• End blank to blank off a disconnected line.
• End blank to blank off a valve when a R/V or equipment
is removed.
• 90 degree elbowed blanks to blank off both flare and
product sides after a R/V or equipment is removed.
• Slide plates for isolation of furnace ducting.
• Others as specified by Area Authority or Work
Execution Authority.

• The above references explain clearly the tagging procedure for inserting or removing
blinds. This practice is part of the Lock Out and Tags Out (LOTO) system and it will
enhance the following safety management on blinds:
• Work Execution Authority has been shown by Area Authority where he shall insert the
blinds.
• With the tags in place he will not forget where are the flanges he needs to insert blinds.
• Work Execution will be able to show the fitters where to insert the blinds.
• Fitters will not make mistake because he has cards which bears the same number as the
ones hung on the flanges.
• Fitter will inform Work Execution Authority that had inserted the blind by returning 2
cards.
• Work Execution Authority will check that the blinds are inserted properly when he
retrieves a card from the flanges and return all cards to Area Authority.
• The procedures are the same for removing of blinds.
• All blinds inserted and removed shall be recorded in “LOTO for Record and Control of
Blind Work” with cross-reference to the Drawing for Blind Work.

TEST RUN AND TROUBLESHOOTING :

This is allowed under the following conditions :

WEA request the test run to AA.

1. WEA remove the Lock and Tag.

2. AA removes his Lock and Tag from the equipment. Only
equipment related to the test is de isolated.
3. WEA proceeds with the equipment test.
4. Once the test is completed, WEA inform the AA.
5. AA will then ensures the equipment isolation return to previous
condition and secures the group lock keys.
