2.2 Etiologi
Coronavirus Disease (COVID-19) disebabkan oleh virus Severe Acute
Respiratory Syndrome Coronavirus-2 (SARS-CoV-2). Kata corona dalam bahasa
Latin mengandung arti crown atau mahkota (Barcena et al., 2009; Neuman et al.,
2006). Coronavirus merupakan virus RNA strain tunggal positif, berkapsul dan
tidak bersegmen. Coronavirus termasuk dalam genus betacoronavirus. Hasil
analisis filogenetik menunjukkan bahwa virus ini masuk dalam subgenus yang
sama dengan coronavirus yang menyebabkan wabah Severe Acute Respiratory
Illness (SARS) pada 2002-2004 silam, yaitu Sarbecovirus (Zhu N. et al., 2020).
Atas dasar ini, International Committee on Taxonomy of Viruses mengajukan
nama SARS-CoV-2 (Gorbalenya AE., et al., 2020).

2.3 Transmisi
Coronavirus disebut dengan virus zoonotik yaitu virus yang ditransmisikan
dari hewan ke manusia. Banyak hewan liar yang dapat membawa patogen dan
bertindak sebagai vector untuk penyakit menular tertentu. Kelelawar, tikus
bambu, unta, dan musang merupakan host yang biasa ditemukan untuk
coronavirus. Coronavirus pada kelelawar ,merupakan sumber utama untuk
kejadian Severe Acute Respiratory Illness (SARS) dan Middle East Respiratory
Syndrome (MERS) (PDPI,2020). Adapun hewan liar seperti cerpelai dan
trenggiling berpotensi menjadi inang perantara dalam proses penularan SARS-
CoV-2 dari hewan ke manusia (Lam et al., 2020).
Penularan antar manusia (human to human) dapat terjadi melalui kontak
langsung, kontak tidak langsung, atau kontak erat dengan orang yang terinfeksi
melalui sekresi seperti air liur dan sekresi saluran pernapasan atau droplet saluran
napas yang keluar saat orang yang terinfeksi batuk, bersin, berbicara, atau
menyanyi (Liu J et al.,2020 ; Chan et al, 2020 ; Huang C et al., 2020 ; Burke RM
et al., 2020; Hamner L, et al., 2020; Ghinai I et al., 2020; Pung R et al., 2020; Luo
L et al., 2020). Droplet saluran napas memiliki ukuran diameter > 5-10 μm
sedangkan droplet yang berukuran diameter ≤ 5 μm disebut sebagai droplet nuclei
atau aerosol (WHO, 2014)
Transmisi melalui udara didefinisikan sebagai penyebaran agen infeksius
yang diakibatkan oleh penyebaran droplet nuclei (aerosol) yang tetap infeksius
saat melayang di udara dan bergerak hingga jarak yang jauh (WHO, 2014).
Sekresi saluran pernapasan atau droplet yang dikeluarkan oleh orang yang
terinfeksi dapat mengontaminasi permukaan dan benda, sehingga terbentuk fomit
(permukaan yang terkontaminasi). Virus dan/atau SARS-CoV-2 yang hidup dan
terdeteksi melalui RTPCR dapat ditemui di permukaan-permukaan tersebut
selama berjam-jam hingga berhari-hari, tergantung lingkungan sekitarnya
(termasuk suhu dan kelembapan) dan jenis permukaan (Guo Z et al., 2020; Ong et
al., 2020; Pastorino B et al., 2020)
2.4 Patogenesis
Secara umum, virus masuk ke saluran pernapasan atas setelah terjadi
penularan, kemudian virus bereplikasi di sel epitel saluran pernapasan atas untuk
melakukan siklus hidupnya. Virus SARS-CoV menginfeksi sel inang melalui
perantaraan protein S yang ada di permukaan virus. Protein S pada permukaan
virus ini akan menempel pada reseptor sel inang yaitu ACE2. ACE2 dapat
ditemukan mukosa oral dan nasal, nasofaring, paru, lambung, usus halus, usus
besar, kulit, timus, sumsum tulang, limpa, hati, ginjal, otak, sel epitel alveolar
paru, sel enterosit usus halus, sel endotel arteri vena dan sel otot polos. Kemudian
ikatan ACE2-virus ditranslokasikan ke endosom tempat di mana protein S
dipotong oleh protease asam endosomal (cathepsin L) untuk mengaktifkan
aktivitas fusi. Genom virus keluar dari selubung virion dan ditranslasikan menjadi
poliprotein replikase virus pp1a dan 1ab, yang kemudian akan dipotong-potong
oleh enzim proteinase virus. Untai negatif RNA akan dibuat sebagai cetakan
untuk genom RNA. Penggabungan kompleks virus terjadi di dalam sitoplasma,
kemudian diikuti dengan proses budding ke lumen retikulum endoplasma. Virus
baru akan berfusi dengan membran plasma dan dikeluarkan dari sel inang
(Gambar 4, Du et al., 2009).

Masa inkubasi virus sampai muncul penyakit sekitar 3-7 hari. Tingkat
keparahan klinis berhubungan dengan usia (di atas 70 tahun), komorbiditas seperti
diabetes, penyakit paru obstruktif kronis (PPOK), hipertensi, dan obesitas
(Gennaro dkk., 2020; Susilo dkk., 2020). Pada infeksi akut terjadi peluruhan virus
dari saluran napas dan virus dapat berlanjut meluruh beberapa waktu di sel
gastrointestinal setelah penyembuhan. Penelitian membuktikan bahwa terdapat
ekspresi ACE2 yang berlimpah pada sel glandular gaster, duodenum, dan epitel
rektum, serta ditemukan protein nukleokapsid virus pada epitel gaster, duodenum,
dan rektum. Hal ini menunjukkan bahwa SARS-CoV-2 juga dapat menginfeksi
saluran pencernaan dan berkemungkinan untuk terjadi transmisi melalui fekal-oral
(Kumar dkk., 2020; Xiao dkk., 2020).
