PENDAHULUAN

dengan peningkatan risiko kejadian ACL pada wanita. Adanya cedera pada ACL
dapat ditandai dengan instabilitas lutut dan gangguan ambulasi (Neumann, 2002).
Diagnosis ruptur ACL dapat ditegakkan melalui anamnesis yang baik
mengenai mekanisme cedera dan pemeriksaan fisik yang benar termasuk beberapa
tes yang mendukung seperti “Lachman test”, “Pivot shift Test” dan “Drawer
Test”. Pemeriksaan penunjang radiologi juga dapat membantu (Frontera, 2019).
Penanganan cedera ACL tergantung pada sejumlah faktor, termasuk usia
pasien, tingkat aktivitas atau adanya cedera lain (Frontera, 2019). Penyembuhan
pada ruptur ACL sendiri tidak berlangsung secara spontan sehingga umumnya
ditangani dengan bedah rekonstruksi (Shea et al, 2015). Pembedahan adalah satu-
satunya pengobatan definitif ruptur ACL meskipun umumnya tidak diperlukan
pada individu usia tua yang tidak mengeluhkan ketidakstabilan lutut dengan
kegiatan rekreasi atau bekerja (Frontera, 2019). Rekonstruksi ACL bertujuan
untuk mengembalikan stabilitas lutut dan memaksimalkan kapasitas fungsional
agar memungkinkan individu kembali ke tingkat aktivitas fisik sebelum cedera
(CR Ardern, et al, 2013). Lutut dengan cedera ACL cenderung mengalami cedera
atau perburukan struktur lain di sekitarnya bahkan setelah rekonstruksi. Individu
dengan riwayat ruptur ACL mengalami peningkatan risiko untuk kejadian
osteoartritis lutut (Neumann, 2022).
Tatalaksana rehabilitasi perlu dilakukan sebelum maupun setelah operasi.
Tujuan rehabilitasi sebelum operasi ataupun sebagai tatalaksana konservatif
adalah untuk meningkatkan fungsi, khususnya pada ambulasi, fleksibilitas sendi
dan kekuatan otot. Pada tindakan pasca operatif, rehabilitasi medik berperan
penting untuk melatih pasien pada proses penyembuhan luka dan reconditioning
kekuatan dan kapasitas fungsional pasien.

mencegah tibia translasi ke arah depan femur (anterior tibial translation) dan
mencegah hiperekstensi. Fungsi lain ACL adalah mencegah varus, valgus dan
axial rotation yang berlebihan (Reyes, 1978).
ACL akan teregang selama gerakan ekstensi dan menjadi kendor selama
fleksi. Meskipun beberapa serat ACL relatif tetap teregang sepanjang luas gerak
sendi pada bidang sagital, terutama posterolateral band yang menegang saat lutut
hampir mencapai ekstensi penuh. Pada posisi 50-600 sebelum ekstensi penuh,
kontraksi otot-otot quadriceps akan menyebabkan gerakan translasi tibia ke
anterior sehingga ACL teregang dan membatasi pergeseran ke anterior
(Neumann, 2002).
Gerakan fleksi dan ekstensi sendi lutut memungkinkan terjadi gerakan
rotasi dari tibia. Pada posisi ekstensi penuh tibia mengalami sedikit rotasi internal.
Pada posisi fleksi 300 tibia dan femur berada pada posisi parallel dimana tidak
terjadi gerakan rotasi dari tibia. Pada posisi lutut fleksi 45 0 sampai penuh, tibia
mengalami gerakan rotasi eksternal. ACL berada dalam posisi istirahat pada 150
sampai 200 awal rotasi eksternal. Saat rotasi eksternal semakin jauh, ACL akan
semakin teregang dan berputar di sekitar bagian medial dari kondilus lateral
femur.

ACL membatasi gerakan translasi anterior tibia (A) dan rotasi eksternal tibia

ACL dibagi menjadi dua bundel serat yaitu anteromedial band (AMB) dan
posterolateral band (PLB) yang berjalan paralel saling menegang dan kendor satu
sama sepanjang gerakan lutut. Anteromedial band (AMB) menegang sepanjang
fleksi lutut 700 sampai penuh dan berperan 85% menahan translasi anterior saat
posisi fleksi penuh. Posterolateral band (PLB) menegang pada posisi ekstensi
penuh sampai sekitar 400-500 fleksi. Pada posisi fleksi lutut 400-500, kedua bundel
ACL berada dalam posisi kendor dan pergeseran fisiologis ke anterior terjadi
(Cailliet, 1973).
Hilangnya ACL akan mengarah pada suatu penurunan rotasi selama fleksi dan
instabilitas lutut. (Kweon,2013)

Saat posisi weight-bearing, posisi dari pinggul dan kaki akan sangat
mempengaruhi posisi tulang femur dan tibia. Gerakan adduksi dan rotasi internal
dari pinggul berkontribusi pada valgus yang berlebihan dan rotasi eksternal dari
lutut. Berkurangnya aktivasi otot-otot abduktor dan rotator eksternal dari lutut
berperan pada posisi pinggul. Gambar 2.5 menunjukkan posisi valgus yang
berlebihan; dengan rotasi internal yang berlebihan dari tulang femur. (Neumann,
2002).
Pada 1/3 pasien dengan mekanisme kontak yang menyebabkan cedera
ACL, seringkali didapatkan riwayat hiperekstensi atau stress valgus dari lutut
dengan popping yang dapat didengar atau diraba (Siegel, 2012).
Mekanisme cedera ACL lainnya saat hiperekstensi lutut yang berlebihan
pada kaki terfiksasi kuat ke tanah. Saat gerakan ekstensi normal melibatkan
pergeseran posterior yang berlebihan dari tulang paha terhadap tibia. Saat lutut
hiperekstensi, pergeseran femur ke arah posterior mungkin meregangkan dan
memutus ACL. Seringkali, cedera ACL terkait hiperekstensi berhubungan dengan
besar rotasi aksial atau gaya penghasil valgus yang selanjutnya meningkatkan
tegangan dari ACL.
Mekanisme loading pada ACL juga dapat terjadi karena gaya kompresif
sepanjang aksis longitudinal tibia yang mengakibatkan posterior tilted tibial
plateau. Dengan adanya slope posterior pada tibial plateau (posterior tibial

plateau slope), gaya kompresif dapat menghasilkan gaya shear anterior yang
mengakibatkan terjadinya translasi anterior tibia dan pembebanan pada ACL (Dai
et al., 2014).
Kolaps valgus lutut diduga sebagai salah satu mekanisme utama cedera
ACL terutama pada wanita. Hasil ini didapatkan dari studi analisa rekaman video
cedera ACL. Tetapi studi-studi kuantitatif tidak mendukung pendapat tersebut.
Walaupun momen valgus, varus dan rotasi internal mempengaruhi loading ACL,
pengaruhnya hanya memiliki efek bermakna jika disertai oleh gaya shear anterior
yang bekerja pada lutut (Dai et al., 2014).

giving away (rasa gamang) untuk melakukan aktivitas olahraga terutama
melompat dan berlari, kemampuan untuk menyangga berat badan, kemampuan
return to sport, adanya riwayat locking pada lutut, bengkak pada lutut, letak nyeri
dan riwayat trauma pada lutut sebelumnya.
Pada pemeriksaan fisik meliputi pemeriksaan alignment sendi, evaluasi
ada tidaknya efusi, evaluasi laxity varus-valgus, anterior-posterior dan rotasi lutut,
tes khusus ( Lachman test, pivot shift test dan anterior drawer test), tes khusus
lain yang perlu dilakukan untuk menyingkirkan diagnosis banding lain: PCL test,
MCL/LCL test, McMurray test, Appley’s compression and distraction test.
Lachman test merupakan pemeriksaan yang paling valid untuk menilai
stabilitas lutut dengan nilai sensitivitas 85% dan spesifisitas 95%. Tetapi dengan
melakukan pemeriksaan stabilitas yang lengkap (meliputi Lachman test, pivot
shift test dan anterior drawer test) akan meningkatkan sensitivitas dan spesifisitas
pemeriksaan dibandingkan pemeriksaan tunggal (Buckup, 2004; AAOS, 2015).

jika pada pemeriksaan fisik sudah membuktikan adanya instabilitas lutut yang
menunjukkan adanya cedera ACL. Namun MRI tetap memiliki kelebihan untuk
mendeteksi lesi intraartikuler lainnya seperti cedera ligamen lain, meniskus, dan
kartilago sendi.

Umur bukan merupakan faktor penting dalam mempertimbangkan apakah
seseorang dengan cedera ACL perlu dilakukan operasi rekonstruksi atau tidak.
Namun pasien muda lebih dipertimbangkan untuk dilakukan operasi rekonstruksi
lebih dini oleh karena karena aktifitas mereka yang tinggi. Penentuan waktu
operasi merupakan hal penting dalam penatalaksanaan cedera ACL. Rekonstruksi
sebaiknya dilakukan setelah fungsi optimal lutut tercapai dan reaksi sinovial
sudah berkurang. Tindakan operasi tidak selalu membawa keberhasilan. Beberapa
masalah yang dapat muncul dari kegagalan operasi ACL seperti graft
impingement pada intercondylar roof, graft tension, penempatan femoral dan
tibial tunnel yang tidak sesuai secara anatomis dan replikasi ACL intak yang tidak
lengkap, khususnya posterolateral bundle (Siegel, 2012).
Terdapat 2 macam graft yang paling sering digunakan pada operasi
rekonstruksi ACL yaitu tendon patella dan 4 strand hamstring tendon (tendon otot
gracilis dan semitendinosus). Kedua autograft tersebut menghasilkan stabilitas
lutut yang fungsional pada lebih dari 95% operasi dengan 3% perbedaan absolut
pada kegagalan graft yaitu 1,9% pada tendon patella dan 4,9% pada tendon
hamstring (Siegel, 2012).
Beberapa keuntungan penggunaan tendon patella antara lain mudah
diakses, memiliki properti struktur fiksasi yang baik dan potensial untuk
penyembuhan tendon to bone. Sedangkan kerugian penggunaan tendon patella
sebagai graft antara lain nyeri lutut anterior, hilangnya sensasi, fraktur patella, dan
kontraktur patella inferior. Penggunaan tendon patella sebagai graft juga
dihubungkan dengan kelemahan otot ekstensor quadrisep paska rekonstruksi ACL
(Siegel, 2012).
Penggunaan tendon hamstring sebagai graft dapat bertahan dengan tension
lebih besar dibandingkan dengan 10 mm tendon patella sebagai graft. Beberapa
peneliti menemukan bahwa penggunaan tendon hamstring sebagai graft ACL akan
mengurangi kekuatan dan endurance otot hamstring sampai 9 bulan paska operasi.
Graft hamstring juga susah untuk ditanam saat operasi oleh karena tendon

hamstring memiliki diameter dan panjang yang bervariasi (Siegel, 2012). Secara
keseluruhan, operasi rekonstruksi ACL dengan metode bone-patellar-tendon-bone
maupun metode rekonstruksi hamstring sama-sama memberikan hasil yang bagus,
stabil dan komplikasi yang rendah. Tidak ada kecenderungan dengan alasan
tertentu dalam memilih kedua teknik tersebut (Meuffels et al., 2012).

ektensi penuh lutut dan heel strike. Latihan penguatan, kontrol dan endurance otot
quadricep dan hamtsring, misalnya weight bearing/gravitasi, static cycle, leg
press, two leg quarter squats, hamstring curl. Latihan neuromuskuler dan
propriosepsi misalnya step lunges, half squat, lateral stepping, dan slide board.
Program latihan olahraga spesifik ditambahkan secara bertahap sejalan
dengan peningkatan fungsi, kemampuan dan kepercayaan diri pasien. Berbagai
program tersebut dapat diberikan dengan mengikuti protokol program
rehabilitasi akselerasi atau nonakselerasi (Tim Sport Clinic RSUD. Dr. Soetomo,
2009).

SUMMARY
Reporting a female, 41 years old, referred from orthopaedic outpatient with
Post Reconstruction ACL S. She came with chief complain cannot bend her left knee
to maximum flexion since 20 days ago (January 13th 2023) accompanied with pain
after surgery on her left knee around the surgical area, with swollen. The pain was
described as dull pain, not radiating, with no numbness nor tingling sensation, WBS
3-4. Aggravated mostly by strenuous activities, walking, standing on her left knee,
squatting, climbing stairs. The pain was relieved by rest or taking medication, but still
feels her knee not stable. She also complains not able to extend her knee and feel pain
when she tries to move her left knee. Never got rehabilitation management yet.
She started to feel pain on her left knee 8 years ago in December 2014 after
motorbike accident, but the pain only felt when she walked in long distance or run. In
September 2022, she jumped when doing aerobics training then the pain and unstable
sensation on her left knee became worse than before, WBS 6. On October 2022, she
went to Semen Gresik Hospital, got medication (Meloxicam) and bandaging. Then
she got Arthroscopy procedure, diagnosed with Left Knee Osteoarthritis and ACL
rupture, referred to Soetomo Hospital to get surgery. After the ACL Reconstruction
surgery 20 days before 1st visit, she uses double axillary crutch to move around her
house and wheel chair when going outside.
From Physical examination we found swelling at left knee , Circumference
difference 3,5cm (L<R), ROM limitation (+), leg length discrepancies ≤ 1cm,
Barthel Index 70/100 (Moderate dependent). Special test were positive for Lachman
test, Pivot test and valgus stress test. Left knee valgus and sensory deficit 50% on her
left tibialis anterior and 70% on ankle area medial side. We assess this patient with
Ambulation Disturbance ec Left Anterior Cruciate Ligament Rupture Post
Reconstruction (20 Days) + Left Knee Osteoarthritis Grade III
We planned to do Biofeedback when patient felt less pain. We give patient
with NMES, Laser and several therapeutic exercise such as AROM exercise on lower

extremities, Gentle stretching left knee to flexion and extension direction, Isometric
strengthening exercise for left quadriceps and hamstrings, Strengthening Left VMO
and Left Hip Abductors, Gait training, sensory resensitization, as well as Health
Education and Home Exercise Program. We also educate to the patient about her
condition and rehabilitation program, modified several activities, continue exercise at
home. After serial rehabilitation programs, follow up about her daily activity and
several examinations, there was a lot of improvement in ROM of knee, pain and
functional assesment. She hopes she can return to activity with no pain and stiffness.
