Anamnesis
Keluhan Utama Riwayat Penyakit Sekarang
Pasien datang ke poli kulit RSUD Dumai dengan keluhan adanya benjolan berawarna

kecoklatan pada hidung yang muncul sejak tahun 2015. Pasien mengatakan benjolan
pada hidung.
hanya terdapat pada daerah hidung dan tidak terdapat pada tempat lain serta benjolan
dirasakan semakin lama semakin membesar. Saat pasien menyadari adanya benjolan
pada hidung, awalnya benjolan terlihat sebesar wijen namun semakin lama semakin
membesar sampai sebesar biji kacang hijau. Pasien mengatakan awalnya benjolan tidak
terasa nyeri, namun sekitar kurang lebih 2 bulan yang lalu benjolan mulai terasa nyeri
terutama pada saat disentuh.
 Riwayat Penyakit Sekarang
Pasien sudah berobat ke dokter di salah satu klinik di dumai dan
diberikan obat salap, namun pasien lupa nama salap yang diberikan.
Setelah mendapatkan terapi, pasien mengatakan keluhan tidak
berkurang dan pasien mengatakan benjolan ini semakin nyeri serta
semakin mengganggu. Lalu dokter di klinik menyarankan kepada
pasien untuk datang ke poli klinik dokter spesialis kulit agar
mendapatkan terapi tindakan.

Riwayat Penyakit Dahulu

Sebelumnya pasien tidak pernah mengalami keluhan serupa seperti
sekarang.

 STATUS DERMATOLOGI

Distribusi regional pada regio nasalis, berjumlah multipel,

diskret, bentuk bulat ukuran miliar sampai lentikular
dengan ukuran 2 mm dan 0,5 cm, berbatas tegas lesi
timbul dan kering. Efloresensi terdapat papul verukosa
hiperpigmentasi.

PEMBAHASAN
Veruka ialah hiperplasia epidermis disebabkan oleh human
papiloma virus tipe tertentu. Virus ini bereplikasi pada sel-sel
epidermis dan ditularkan dari orang-orang. Penyakit ini juga
menular dari satu bagian tubuh ke bagian tubuh pasien yang
sama dengan cara autoinokulasi.

Veruka vulgaris / commmon warts / kutil adalah jenis kutil

(papul verukosa) yang banyak ditemukan dan disebabkan
terbanyak oleh HPV serotip tipe 2, 1 dan 4.
Veruka vulgaris terjadi terutama terdapat pada anak, tetapi juga
terdapat pada dewasa dan orang tua.3
Pada pemeriksaan klinis veruka vulgaris menunjukkan papul padat verukosa,
keratotik, dengan ukuran beberapa mm sampai dengan 1 cm, dan bila
berkonfluensi dapat menjadi lebih besar. Lokasi dapat dimana saja tetapi sering di
punggung, tangan dan jari tangan.4 Tempat predileksinya terutama di ekstremitas
bagian ekstensor, walaupun demikian penyebarannya dapat ke bagian tubuh lain
termasuk mukosa mulut dan hidung.

Penyakit ini bentuknya bulat berwarna abu-abu, besarnya lentikular atau kalau
berkonfluensi berbentuk plakat, permukaan kasar (verukosa), berwarna coklat,
kuning, abu-abu muda, atau abu-abu tua.3 Biasanya asimtomatik, tetapi beberapa
bisa nyeri jika disentuh. Nyeri dapat dirasakan bila tumbuh di palmar atau
plantar dan merusak kuku bila tumbuh pada lipatan atau bawah kuku. Pada
anak-anak bisa tumbuh di wajah dan leher.4
 Anamnesis

Dari hasil anamnesis yang didapatkan, pasien mengeluhkan adanya benjolan berwarna kecoklatan di daerah hidung
yang muncul sejak tahun 2015, benjolan dirasakan semakin lama semakin membesar, awalnya sebesar biji wijen sampai
sebesar biji kacang hijau. Awalnya benjolan tidak terasa nyeri namun sejak ± 2 bulan yang lalu benjolan terasa nyeri
terutama saat disentuh.

Dari hasil anamnesis diatas dapat disimpulkan bahwa beberapa tanda

dan gejala yang dikeluhkan pasien merupakan manifestasi klinis veruka
vulgaris.
 Pemeriksaan Fisik

Menurut teori pada veruka vulgaris akan tampak lesi dengan papul padat
verukosa, keratotik, dengan ukuran beberapa mm sampai dengan 1 cm, dan bila
berkonfluensi dapat menjadi lebih besar. Lokasi dapat dimana saja tetapi sering di
punggung, tangan dan jari tangan.4 pada pemeriksaan fisik pasien dilakukan
pemeriksaan pada wajah terutama pada daerah hidung, dilihat terdapat 2 benjolan
berwarna kecoklatan dengan ukuran 2 mm dan 0,5 cm. Pada saat diraba, teraba
benjolan padat, permukaan benjolan kasar dan nyeri saat disentuh. Distribusi regional
pada regio nasalis, berjumlah multipel, diskret, bentuk bulat ukuran miliar sampai
lentikular dengan ukuran 2 mm dan 0,5 cm, berbatas tegas lesi timbul dan kering.
Efloresensi terdapat papul verukosa hiperpigmentasi.

 Pemeriksaan penunjang

Histopatologi

Veruka terdiri dari epidermis yang akantotik dengan papillomatosis, hiperkeratosis, dan
parakeratosis.5 Rete ridges yang memanjang seringkali tertuju langsung pada pusat kutil. Pembuluh
darah kapiler dermis ialah prominen dan mungkin mengalami trombosis.

 Diagnosis Banding
Veruka Vulgaris
Definisi Veruka vulgaris adalah jenis kutil (papul
verukosa) yang banyak ditemukan dan
disebabkan terbanyak oleh HPV serotip tipe
2, 1 dan 4.1
Gambaran Pemeriksaan klinis veruka vulgaris
Klinis menunjukkan papul padat verukosa,
keratotik, dengan ukuran beberapa mm
sampai dengan 1 cm, dan bila berkonfluensi
dapat menjadi lebih besar. Lokasi dapat
dimana saja tetapi sering di punggung,
tangan dan jari tangan.4
Lesi bentuknya bulat berwarna abu-abu,
besarnya lentikular atau kalau
berkonfluensi berbentuk plakat, permukaan
kasar (verukosa), berwarna coklat, kuning,
abu-abu muda, atau abu-abu tua.3
Keratosis Seboroik
Keratosis seboroik merupakan tumor
jinak epidermal yang paling sering
terjadi. Lesi umumnya terjadi pada usia
pertengahan, namun dapat muncul pada
awal masa remaja.7
Terdapat banyak varian klinis. Pada lesi
awal terdapat batas yangtegas,
permukaan rata, kusam, dan berwarna
lebih kecoklatan. Seiring
perkembangannya, lesi menjadi
berbentuk papul, permukaannya
verukosa, stuck-on, mengkilat dan
terdapat kista pseudohorn.7-9
Nevus Verukosus
Tumor epidermal yang ditandai
dengan hiperkeratois dari keratinosit
dan permukaan verukosa, biasanya
tersusun linier dan ada sejak bayi.10
Nevus verukosus ditamdai dengan
papul verukosus terlokalisasi atau
difus, rapat, sewarna kulit, coklat atau
coklat keabuan, yang dapat menyatu
membentuk plak papilomatosa yang
berbatas tegas. Konfigurasi linier
sering ditemukan di ekstremitas
dengan distribusi pada garis
blaschko’s atau relaxed skin tension
lines.10
 Diagnosis Banding
Veruka Vulgaris Keratosis Seboroik Nevus Verukosus
Pemeriksaan Veruka terdiri dari epidermis yang Tampak akantosis, papillomatosis, kista Ditemukan gambaran akantosis,
Histopatologis akantotik dengan papillomatosis, pesudohorn, dan hiperkeratosis.8,9 papilomatosis dan hiperkeratosis, serta
hiperkeratosis, dan parakeratosis. 5 elongasi dari rete ridges.10
Rete ridges yang memanjang seringkali
tertuju langsung pada pusat kutil.
Pembuluh darah kapiler dermis ialah
prominen dan mungkin mengalami
trombosis.5
 Tatalaksana

Menjaga higiene perorangan supaya tidak tertular,
