YANG TIDAK BOLEH DIGUNAKAN

Menurut Lembaga Keamanan Praktik Kedokteran (Institute for Safe Medication Practices),
berikut adalah singkatan, symbol dan penulisan dosis yang seringkali disalah artikan dan dapat
meneyebabkan bahaya dalam praktik kedokteran. Dengan demikian , singkatan berikut
JANGAN digunakan dalam menjelaskan berbagai informasi kedokteran.

Singkata yang tercantum dalam Bab ini tidak boleh digunakan dalam bentuk huruf besar atau
kecil, dengan atau tanpa titik. Misalnya utuk singkatan QD, maka singkatan untuk qd, Q.D. juga
dilarang karena akan membingungkan dan dapat disalahartikan.
Daftar ini berlaku untuk seluruh instruksi dan seluruh dokumentasi terkait medikasi yang tertulis
(termasuk entry tulisan di komputer) atau pada formulir yang telah dicetak.
Pengecualian : Angka nol di belakang koma dapat digunakan untuk memastikan akurasi nilai
