- Gejala lokal : Batuk ≥ 2 minggu, hemoptisis, nyeri dada, dan sesak napas.
- Gejala sistemik : Malaise, demam, keringat malam, penurunan berat badan, dan
anoreksia.
- Pemeriksaan fisik : kelainan pada thorax, pembesaran KGB.

DIAGNOSIS KERJA:

Pasien 24 tahun, dengan TB Paru BTA (+) Primer

RENCANA TINDAK LANJUT

● Pengobatan TB Paru fase intensif diberikan selama 2 bulan.

● Selanjutnya akan difollow up untuk menilai efek terapi yang diberikan dengan cara dilakukan
pemeriksaan sputum BTA. Jika hasil BTA negatif, maka dilanjutkan pengobatan TB Paru fase
lanjutan selama 4 bulan menggunakan isoniazid dan rifampisin dalam kombinasi dosis tetap
(KDT) . Apabila hasil BTA masih positif, maka pasien melakukan terapi OAT-KDT kategori II
fase intesif.
● Evaluasi pada minggu ke-2 meliputi status gizi, pemeriksaan lab darah rutin, dan kepatuhan
konsumsi OAT-KDT.
PEMBAHASAN
ANALISIS DATA PRIBADI
Pasien merupakan seorang laki-laki berusia 24 tahun dan berstatus sebagai
mahasiswa.
TB lebih banyak diderita oleh laki-laki dibandingkan perempuan dan banyak diderita
pada usia dewasa (WHO, 2020).

Padat penduduk dan pasien cukup aktif di lingkungan kampung.

Faktor risiko penularan TB : Lingkungan padat penduduk, kontak dengan
sumber penularan (kontak dekat dengan penderita, lamanya waktu kontak dengan
penderita, dan konsentrasi kuman di udara) (CDC, 2016; Kemenkes, 2014).

Pasien tidak memiliki riwayat merokok dan penyakit lainnya. Pasien juga memiliki pola makan
yang normal (makan 3x sehari, nasi, sayur, lauk berganti-ganti, ayam/telur/daging/tempe/tahu)

Faktor risiko lain : Keadaan yang menyebabkan penurunan daya tahan tubuh, yaitu merokok,
diabetes mellitus, gizi buruk, HIV, dll (Pangaribuan, 2020).
 Riwayat Penyakit Sekarang
Tuberkulosis (IDI, 2014) :
• Gejala TB lokal diantaranya adalah batuk lama >2 minggu, sesak
nafas, dan nyeri dada.
• Gejala TB sistemik yaitu demam, gangguan nafsu makan, berat badan
menurun, dan keringat malam hari
Riwayat Penyakit Dahulu
• Batuk lama perlu diwaspadai karena dipengaruhi oleh banyak faktor
salah satunya karena infeksi paru-paru. Beberapa penyakit infeksi
paru-paru yang bisa menjadi penyebab batuk lama, antara lain
pneumonia, bronkiektasis, dan tuberkulosis (TB).
• Selain itu adanya faktor luar yang dapat memperburuk kejadian batuk
yaitu asap rokok karena zat-zat yang terkandung dalam asap rokok

Analisis dapat merusak fungsi paru-paru dan menekan kekebalan adaptif

individu.
Riwayat Penyakit Keluarga

Inspeksi: simetris kiri kanan normal. Pada penderita TB dengan fibrosis yang luas, dapat
ditemukan atrofi dan retraksi pada musculus intercostalis (Setiati
et al, 2014).

 KESIMPULAN SARAN
Kesimpulan
Berdasarkan hasil pengkajian yang dilakukan terhadap Tn. MU,
diagnosenya yaitu TB Paru BTA (+) Primer. Tatalaksana yang
diberikan untuk menghilangkan sumber infeksi dengan pengobatan
TB kategori 1. Evaluasi dilakukan setelah 2 bulan pengobatan untuk
menilai tingkat kemajuan pengobatan sebelumnya dengan tes sputum
BTA.

Saran

● Pada pemeriksaan penunjang, lebih baik dilakukan pemeriksaan laju endap darah (LED)
untuk memastikan tanda TB pada pasien, namun LED tidak digunakan untuk memantau
kemajuan pengobatan TB.
● Dalam pemantuan kemajuan pengobatan TB, pemeriksaan sputum secara mikroskopis
karena lebih baik dibandingan pemeriksaan radiologis.
● Pada pengobatan, pasien harus dilakukan penilaian apakah pengobatan teratur atau tidak
● Pemberian dosis tahap lanjutan harus dilakukan segera tanpa memberikan OAT sisipan,
meskipun hasil pemeriksaan sputum ulang adalah positif atau negatif.
