 STANDAR OPERASIONAL PROSEDUR (SOP) HUKNAH

A. Definisi

Memasukkan larutan kedalam rectum dan kolon sigmoid. Memasukkan cairan

kedalam rectum guna membuang feses ( gas) dari kolon dan rectum.

B. Tujuan

1. Huknah Rendah

a. Meningkatkan defekasi dengan merangsang peristaltic

b. Mengosongkan usus sebagai persiapan operasi, kolonskopi

c. Tindakanpengobatan

2. Huknah Tinggi

a. Membantu mengeluarkan feses akibat konstipasi atau impaksi fekal

b. Membantu defekasi normal sebagai bagian dari program latihan defekasi

( bowel training program)

c. Tindakan pengobatan / pemeriksaan diagnostic

C. Indikasi

a. Konstipasi

b. Defisit perawatan diri,toileting

c. Persiapan operasi

D. Persiapan pasien

Memberikan penjelasan tentang maksud dan tujuan tindakan

E. Persiapan Alat

1) Irrigator lengkap ( irrigator, selang, klem, selang rectal

2) Ukuran selang rektal untuk bayi dan anak – anak 10-12 G French, 22-26 G

Frenc Untuk dewasa

3) Cairan ( air hangat, NaCl 0,9% hangat, air kran, larutans abun)Bayi 150 - 250

cc

4) Toddler 250 - 350cc

5) Usia sekolah 300 - 500 cc

6) Remaja 500 - 750 cc

7) Dewasa 750 - 1000 cc

8) Bengkok, vaselin / jel

9) Beberapa potong kain kassa

10) Alas bokong

11) Selimut mandi

12) Pot / pispot dan tutupnya, air cebok dalam botol, kertas closet atau tissue

13) Bangku untuk pot

14) Standar untuk irrigator

15) Sarung tangan

16) Skort

17) Sampiran bila perlu

F.  Cara Kerja

1) Jelaskan prosedur kepada klien

2) Tutup ruamgan / pasang sampiran

3) Bantu klien pada posisi miring kekiri ( lateral kekiri) untuk huknah rendah

dan miring kekanan untuk huknah tinggi dengan lutut kanan fleksi,. Anak –

anak pada posisi dorsal rekumben

4) Letakkan perlak dibawah bokong

5) Pasang selimut mandi, area anal yang kelihatan

6) Pasang irrigator set ( irrigator, selang, klem, selang rectal)

7) Gantung di standar irrigator

8) Tututp klem pengatur

9) Masukan larutan ke irrigator sesuai indikasi ( jenis dan diklem lagi )

10) Letakkan standaran irrigator dekat klien ( dekat bokong sebelah kanan

untuk huknah tinggi dan sebelah kiri untuk huknah rendah)

11) Letakkan pispot dekat tempat tidur

12) Cuci tangan, pasang sarung tangan dan pasang skort

13) Beri pelumas / gel 3-4cm pada ujung selang rectal

14) Dengan perlahan regangkan bokong dn cari letak anus, instriksikan klien

rileks dengan menghembuskan napas perlahan melalui mulut

15) Masukkan selang rectal secara perlahan dengan mengarahkan kearah

umbilicus klien, panjang insersi 7,4-10 cn untuk dewasa,5-7,5 untuk anak-

anak 2,5-3,5 untuk bayi. Tarik selang dengan segera jika ditemukan

obstruksi

16) Naikan irrigator secara perlahan sampai pada keitnggian diatas anus (30-45

cm untuk hukanh tinggi dan 30 cm untuk hukanh rendah dan 7,5 cm untuk

bayi.

17) Buka klem dan alirkan secara perlahan. Waktu pengaliran sesuai dengan

pemberian volume cairan ( 1 liter dalam 10 menit)

18) Bila klien mengeluh kram, rendahkan irrigator atau klem selama 30 detik,

kemudian alirkan kembali secara lambat

19) Klem selang setelah larutan habis

20) Beritahu klien untuk menahan napas ketika selang ditarik

21) Letakkan tissue pada sekitar anus dan Tarik selang anus secara perlahan

22) Buang tissue pada bengkok

23) Jelaskan pada klien bahwa perasaan distensi adalah normal. Minta klien

untuk menahan larutan selama mungkin saat berbaring ditempat tidur, untuk

bayi dan anak – anak dengan perlahan pegang kedua bokong selama

beberapa menit

24) Singkirkan irrigator set dan selang tempat yang sudah disediakan
25) Atur posisi terlentang

26) Bantu klien ke kamar mandi atau pasang pispot di bawah bokong

27) Observasi karakter feses dan larutan ( beritahu klien jagan menyiram toilet

sebelum feses di observasi oleh perawat

28) Lepaskan perlak dari bawah bokong

29) Bantu / anjurkan klien untukb membersihkan daerah anal dengan air hangat

dan sabun

30) Lepaskan sarung tangan, cuci tangan dan lepaskan skort

31) Catat hasil enema pada lembar observasi

A. Definition

Insert the solution into the rectum and sigmoid colon. Inserting liquid into the

rectum to remove feces (gas) from the colon and rectum.

B. Purpose

1. Low Huknah

a. Increase defecation by stimulating peristaltic

b. Emptying the intestines in preparation for surgery, colonoscopy

c. Treatment

2. High Huknah

a. Helps remove faeces due to constipation or faecal impaction

b. Assist normal defecation as part of a bowel training program

c. Treatment measures / diagnostic checks

C. Indications

a. Constipation

b. Self-care deficit, toileting

c. Preparation for surgery

Provide an explanation of the intent and purpose of the action

E. Tool Preparation
 a.Complete irrigator (irrigator, hose, clamp, rectal hose

F. Procedure

a.Explain the procedure to the client

b. Cover the room / side sampiran

c.Help the client in the left tilt position (lateral left) for low huknah and right tilt

for high huknah with right knee flexion. Children in a recumbent dorsal position

d. Place the perlak under the buttocks

e.Install a shower blanket, visible anal area

f. Install the irrigator set (irrigator, hose, clamp, rectal hose)

g. Hang on a standard irrigator

h. Tututp regulating clamps

i. Put the solution into the irrigator as indicated (type and clamp again)

j. Place the irrigator stand near the client (near the right buttocks for high huknah

and the left for low huknah)

k. Place the potty near the bed

l. Wash hands, put gloves on and put a skort on

m. Apply 3-4cm lubricant / gel to the end of the rectal hose

n. Gently stretch your buttocks and find the location of the anus, instill in a

relaxed client by exhaling slowly through the mouth

o. Insert the rectal tube slowly by pointing towards the client umbilicus,

insertion length of 7.4-10 cn for adults, 5-7.5 for children 2.5-3.5 for infants. Pull

out the hose immediately if obstruction is found

p. Raise the irrigator slowly until the height is above the anus (30-45 cm for

high rain and 30 cm for low rain and 7.5 cm for infants.

q. Open the clamp and flow slowly. The drainage time is according to the

administration of liquid volume (1 liter in 10 minutes)

r. If the client complains of cramps, lower the irrigator or clamp for 30 seconds,

then slow it back down

s. Clamp the hose after the solution has run out

t. Tell the client to hold his breath when the hose is pulled

u. Place tissue around the anus and gently pull the anal tube

v. Discard the tissue in warping

w. Explain to the client that feelings of distention are normal. Ask the client to

hold the solution as long as possible while lying in bed, for infants and children -

slowly hold both buttocks for several minutes

x. Get rid of the irrigator set and the hose provided

y. Adjust the supine position

z.Help the client to the bathroom or place the potty under the buttocks

aa.Observation of faecal and solution characters (tell the client not to flush the

toilet before the faecal is observed by the nurse

bb. Remove the mustache from under the buttocks

cc. Help the client to clean the anal area with warm water and soap

dd. Remove gloves, wash hands and remove the skort

ee. Record the results of the enema on the observation sheet
