andHCW.Indirectly, noninvasive respiratorytherapiesmaybeofparticular benefit inreducing
theriskstohealthcareworkersbyobviatingtheneedforintubation,apotentially highly
infectiousprocedure.

important. Indeed some authors that NIV has a role
in self inflicted lung injury (SILI) and the risk of
impacting in a change in ARDS phenotype (Paolo
Pelosi, personal communication, Webinar ESICM).
The extended effect of heated and humidified
oxygen in HFNC to avoid mucosal injury, improve
secretion clearance, reduce transpulmonary
driving pressure, should also be looked at in this
setting.68 Its role in preventing injury and
accelerate recovery if initiated early in the clinical
courseshouldbealsoanalyzed.

PEMBAHASAN
D. Metode Penelitian
1. Jenis Penelitian
Penelitian ini menggunakan metode Tinjauan Literatur/Litelatur Riview.
Tinjauan literatur tersebut dilakukan dengan pencarian database literature.
Pencarian dilakukan di MEDLINE,EMBASE,Cochrane Central Registar of
Controlled Trials dari 2010 hingga 15 april 2020 dan meninjau referensi artikel
yang diambil untuk study tambahan. Istilah utama dalam pencarian artikel yang
digunakan adalah Covid-19 dan Sindrome gangguan pernafasan akut (ARDS),
SARS,MERS, ventilasi non invasif (NIV), Saluran hidung aliran tinggi
(HFNC), Pronasi, petugas kesehatan, sindrom pernapasan akut berat (SARS),
influenza H1N1, sindrom pernapasan timur tengah (MERS).
Tinjauan Literatur adalah kajian kritis yang dilakukan terhadap suatu topik yang
sudah ditulis oleh ilmuwan atau pakar dibidangnya. Tulisan yang ditelaah
adalah tulisan yang ditulis pada berbagai literatur seperti buktu teks yang
tersimpan di perpustakaan, publikasi hasil penelitian pada jurnal ilmiah atau
jurnal online, laporan penelitian (skripsi, tesis), pencarian literatur dari internet
dan rangkuman hasil suatu seminar bertaraf nasional atau internasional
(Dharma, 2011).
2. Populasi, sampel dan Teknik Sampling
a. Populasi adalah wilayah generalisasi yang terdiri atas : objek/subyek yang
mempunyai kualitas dan karakreristik tertentu yang ditetapkan oleh
peneliti untuk dipelajari dan kemudian ditarik kesimpulannya (Priyono,
2016). Populasi pada jurnal ini tidak dijelaskan secara jelas terkait, berapa
jumlah artikel yang teridentifikasi atau digunakan dalam penelitian ini.
b. Sampel adalah bagian dari jumlah dan karakteristik yang dimilki oleh
populasi tersebut dan peneliti tidak mungkin mempelajari semua yang ada
pada populasi, misalnya karena keterbatasan dana, tenaga dan waktu
(Dahlan, 2016). Pada penelitian ini tidak terdapat sampel penelitian,
karena metode yang dilakukan adalah Tinjauan Literatur atau literature
review.
c. Teknik sampling
Teknik pengambilan data atau teknik sampling yang digunakan pada jurnal
ini tidak dijelaskan. Dalam jurnal ini memiliki kekurangan karena tidak
menjelaskan secara rinci terkait populsi, dan teknik samplingnya.
3. Instrument Penelitian
Instrumen penelitian merupakan alat-alat yang digunakan untuk mengukur,
menilai, atau mengobservasi suatu fenomena dapat berupa kuesioner, formulir
observasi, dan formulir lainnya (Notoatmodjo 2014).
Pada penelitian ini instrumen yang digunakan tidak di jelaskan secara rinci
namun dijelaskan jurnal ini menggunakan metode literatur untuk mencari dan
membandingkan tentang NIV dan HFNC berdasarkan FaO2 dan FiO2.
Pada penelitian atau jurnal ini, instrumen yang digunakan tidak di jelaskan
secara rinci alangkah lebih baik instrumen yang di gunakan lebih di jelakan
secara spesifik lagi untuk mengetahui tentang NIV dan HFNC berdasarkan
FaO2 dan FiO2. pada pasien dengan gangguan pernapasan.
E. Hasil Penelitian
Ventilasi non Invasif (NIV) dan saluran hidung aliran tinggi (HNFC), Dalam
uji coba terkontrol secara acak baru-baru ini, Patel et al, menemukan bahwa NIV yang
diberikan melalui helm mengurangi tingkat intubasi pada pasien dengan ARDS lebih
signifikan, dibandingkan dengan NIV yang diberikan melalui masker wajah (masing-
masing dari 61% menjadi 18%). Karena helm tampak lebih efektif dan antarmuka
yang dapat ditoleransi dalam pengaturan ini, masuk akal untuk mengevaluasi
bagaimana berdiri dibandingkan dengan HFNC. Memang, baru-baru ini, studi silang
acak fisiologis, menyimpulkan bahwa pada pasien dengan PaO 2 / FiO 2 < 200, helm
NIV PEEP tinggi lebih disukai daripada HFNC untuk mengoptimalkan oksigenasi dan
mengurangi upaya inspirasi, terutama pada sebagian besar pasien hipoksemia berat
dan pada mereka yang menunjukkan upaya inspirasi intens selama HFNC. Perhatian
diperlukan pada pasien dengan upaya inspirasi rendah selama HFNC, karena mereka
dapat mengalami peningkatan tekanan penggerak transpulmoner dinamis saat
menggunakan NIV dengan helm.
Menganalisis tren terkini dalam penggunaan NIV dan HFNC di semua seri
yang diterbitkan di jurnal utama menunjukkan hal berikut, rata-rata penggunaan NIV
pada pasien rawat inap di Cina adalah 20,1% (dari 4,9 menjadi 56%; seri yang lebih
tinggi termasuk hanya sakit kritis, sedang sampai berat dan dalam satu seri kasus
Pneumonia); di Italia 11%; dan di AS dari 0 hingga 19%. Rata-rata penggunaan
HFNC di Cina adalah 22,8% (berkisar antara 0 hingga 63,5% dan lebih tinggi secara
seri termasuk hanya dan sedang sampai berat ), di Italia 0%, dan di AS dari 4,8%
menjadi 42% (lebih tinggi di Sakit kritis di Seattle daripada di Washington).
Dalam studi dunia nyata di dua rumah sakit Chongqing di China dari pasien
yang mengalami GGA parah, 63% pasien diobati dengan HFNC sebagai terapi lini
pertama, 33% diobati dengan NIV dan 4% dirawat dengan ventilasi invasif. Dari
pasien HFNC, 41% mengalami kegagalan, angka kegagalan menjadi 0 pada pasien
dengan PaO 2 / FiO 2 > 200 dan 63% di antaranya dengan PaO 2 / FiO 2 ≤ 200. Salah
satu masalah penting tentang HFNC adalah jumlah kondensasi di sirkuit meningkat
ketika suhu lingkungan menurun. Saat ini, air yang terkondensasi dapat menjadi
sumber infeksi penting untuk COVID-19 sehingga, penghindaran atau pengurangan
kondensasi mungkin sangat penting ketika HFNC digunakan. Kontaminasi bakteri
pada permukaan bagian dalam sirkuit setelah penghentian HFNC telah ditunjukkan
pada 16,1% kasus, terutama terjadi di ujung antarmuka. Angka ini sama tingginya
dengan sirkuit pernapasan anestesi, tetapi dapat diturunkan dengan sirkuit yang
dilengkapi kabel pemanas, yang sangat mengurangi kondensasi. Meskipun bukti dari
seri terbaru kurang (dengan tidak ada satu pun penyebutan penularan kepada
profesional melalui teknik ini), ada penulis yang tidak merekomendasikan NIV atau
HFNC sampai pasien bebas dari COVID-19. Namun beberapa percobaan lain dari
China menunjukkan bahwa intervensi dini dengan HFNC dan NIV terkait atau tidak
dengan posisi rawan dapat menyebabkan kematian yang lebih rendah, kurang dari 1%
kasus yang membutuhkan intubasi (dibandingkan 2,3% dari rata-rata Nasional).
Dari penelitian ini, kami dapat menyimpulkan bahwa NIV melalui helm
dengan sirkuit tungkai ganda dan penutup yang baik pada antarmuka helm leher akan
menjadi pilihan yang aman untuk menangani pasien infeksius dengan hipoksemia
GGA. Sebagai alternatif, masker Udara Quattro (ResMed ®), atau masker ora-nasal
tanpa ventilasi dengan filter bakteriologis dikatup ekspirasi sirkuit bisa menjadi
alternatif yang lebih efisien.
HFNC juga dikenal dapat memberikan PEEP yang rendah, dimana dapat
memberikan efek menguntungkan bagi kondisi gagal napas ringan-sedang. Selain itu,
dengan memberikan gas yang hangat dan terhumidifikasi, HFNC mengurangi usaha
metabolik yang diperlukan untuk mengkondisikan udara. HFNC lebih dapat ditoleransi
dibandingkan dengan bantuan ventilasi lainnya dan mengurangi kejadian intubasi
sehingga memberikan prognosis klinis yang baik pada pasien dengan gagal napas akut
(Procopio, et al., 2020).
H. Analisa PICO
a. Population
Jumlah peserta dalam penelitian ini tidak dicantumkan dan tidak dijelaskan.
b. Intervention
Intervensi dalam penelitian ini adalah kombinasi NIV (Noninvasif Ventilation)
dan HFNC (High Flow Nasal Cannula). Pada pasien dengan PaO 2 / FiO 2 < 300,
mempelajari pengaruh penerapan sekuensial sesi HFNC dan NIV. Dengan Helm
StarMed CaStar R, kebocoran udara yang dihembuskan dapat diabaikan jika NIV
diterapkan dengan sirkuit tungkai ganda, filter dan segel yang baik pada
antarmuka helm leher, sedangkan kebocoran pada antarmuka leher dapat
mencapai jarak radial maksimum 0,27m melalui helm lain (model Sea-Long)
tanpa segel yang rapat di antarmuka. Sebagai alternatif, masker Udara Quattro
(ResMed ®), atau masker oro-nasal tanpa ventilasi dengan filter bakteriologis di
katup ekspirasi sirkuit bisa menjadi alternatif yang lebih efisien.
Dalam percobaan lain dengan boneka cebol dan tidak ada ruang tekanan
negatif, 10 menit HFNC pada aliran 60 L / menit, menyebabkan tidak ada
penyebaran jamur air di area> 60 cm dari wajah. Reposisi manual kanula sedikit
meningkatkan penyebaran. Pada pasien sakit kritis dengan Pneumonia Gram
negatif, di ruang tekanan negatif hunian tunggal, tidak ada perbedaan jumlah
bakteri antara HFNC dan Venturi Mask pada pelat 0,4m dan 1,5m. Dalam
percobaan dengan sukarelawan sehat untuk mensimulasikan pasien batuk saat
menggunakan HFNC untuk menilai jarak maksimum dispersi tetesan,
menunjukkan bahwa saat memakai kanula hidung yang pas pada 60 Lmin - 1
Aliran, tetesan yang dihasilkan batuk menyebar hingga jarak 4,50m. Untuk
menghindari beberapa risiko HFNC, pasien memakai masker bedah di atas kanula
hidung. Simulasi HFNC baru-baru ini bersama dengan sungkup wajah bedah yang
dipasang di atas kanula, menegaskan bahwa, pada 40 Lmin - 1, masker bedah
menangkap 83,2% partikel, dengan mengorbankan pengurangan sedang dalam
pembersihan CO2.

Tujuan dalam penelitian ini adalah untuk mengetahui efektivitas kombinasi antara
NIV dan HFNC dalam perlindungan bagi petugas kesehatan.
I. Implikasi Keperawatan
Penelitian ini berdampak pada pengembangan di bidang kesehatan, khususnya
dalam kasus perlindungan diri bagi petugas kesehatan di masa pandemic ini.
Penelitian ini membuktikan bahwa efektivitas kombinasi NIV dan HFNC penggunaan
ventilasi invasif secara dini benar-benar dapat meningkatkan prognosis. Dua fenotipe
berbeda pasien telah dihipotesiskan : lebih dari 50% pneumonia COVID-19 dengan
kriteria Berlin ARDS memiliki kepatuhan paru-paru normal. Diharapkan hasil
penelitian ini dapat menjadi bahan pertimbangan sebagai salah satu alat perlindungan
diri bagi petugas kesehatan.
J. Manfaat Jurnal

Sebagai mahasiswa kesehatan generasi penerus tenaga medis di rumah sakit
khususnya perawat, setelah membaca analisis jurnal ini bisa mengetahui alat
perlindungan diri bagi tenaga kesehatan

Dapat menjadi suatu pertimbangan atau solusi dalam perlindungan bagi petugas
kesehatan.
BAB III
PENUTUP
A. Kesimpulan
Semua terapis dan tenaga medis yang menangani masalah pernapasan
memiliki risiko terhadap prosedur yang menimbulkan aerosol selama perawatan
pasien dengan COVID-19. Alat Pelindung Diri dan Pengendalian / Rekayasa
Lingkungan harus menjadi perhatian dan pertimbangan awal saat menangani pasien
dengan COVID-19. Mengingat keadaan saat ini, tidak mungkin akan ada uji coba
terkontrol secara acak untuk memastikan bantuan pernapasan non invasif mana yang
lebih baik untuk mengurangi kebutuhan intubasi dalam konteks pandemi COVID-19.
Produsen harus didorong untuk membuat makser atau alat pelindung diri lainnya yang
lebih aman, sirkuit anti virus, dan ventilator non-invasif 'generasi baru' dengan
integrasi berbagai terapi, pemantauan khusus, dan fitur keselamatan yang diperlukan.
Petugas kesehatan harus tetap dalam keadaan yang lengkap dalam menangani pasien
gangguan pernapasan dengan COVID-19 dan tetap memperhatikan alat pelindung diri
yang akan digunakan. Penggunaan NIV dan HFNC pada pasien COVID-19 dengan
gangguan pernapasan harus tetap diperhatikan dengan baik agar tidak menimbulkan
penyebaran COVID-19 pada petugas kesehatan.
B. Saran
1. Bagi Mahasiswa
Agar mahasiswa tahu dan mampu menggunakan alat pelindung diri yang
benar sesuai standar yang telah ditentukan dan menggunakannya sesuai
dengan kebutuhan serta penggunaan alat bantu napas NIV dan HNFC.
2. Bagi Perawat
Agar perawat dan tenaga kesehatan lainnya dapat menggunakan alat pelindung
diri yang lengkap dan tepat dalam menangani pasien dengan COVID-19
sehingga tetap dalam kondisi aman meski harus berhadapan dengan pasien
COVID-19. Serta dapat menentukan pemilihan penggunaan alat bantu seperti
NIV dan HNFC pada pasien COVID-19 dengan gangguan pernapasan.
3. Bagi Rumah Sakit
Agar tetap memperhatikan keselamatan tenaga kesehatan dengan
menyediakan alat pelindung diri yang lengkap dan tepat sehingga tenaga
kesehatan yang bekerja dalam pandemic COVID-19 tetap merasa aman.
Rumah Sakit juga dapat menentukan standar penggunaan NIV ataupun HNFC
pada pasien COVID-19 dengan gangguan pernapasan.
