• Sel-sel tubuh dapat hidup sehat dan aman di
lingkungan yang mempunyai pH antara 7,35 - 7,45.
• Bilamana terpaksa, sel-sel masih dapat hidup
dalam lingkungan yang mempunyai pH 6,8 sampai
7,8.
• Oleh karena sempitnya batas-batas tersebut, maka
usaha kompensasi dari tubuh sangat penting.
• Apabila tidak berhasil, akan terjadi dekompensasi
dan harus dilakukan tindakan koreksi.

Peter Sorensen pada tahun 1909 untuk menunjukkan log
negatif dari konsentrasi ion hidrogen
Simbol pH berarti ‘potenz’ (power) of Hydrogen.

Interpretasi Tradisional
• Pendekatan gangguan asam basa dengan menggunakan
persamaan Henderson-Hasselbach disebut juga Interpretasi
tradisional, yaitu tentang kelainan asam basa dimana pH
ditentukan oleh rasio bikarbonat menjadi karbon dioksida.
• Karya Henderson dan Hasselbalch ini sampai sekarang masih
luas digunakan.
• Metode ini relatif mudah dimengerti dan diterapkan dalam
berbagai situasi klinis.
• Implikasinya adalah bahwa keseimbangan disosiasi asam
karbonat dipandang sebagai patokan yang menentukan nilai
pH dan keseimbangan dari buffer tubuh lainnya. 2,3
• Berdasarkan pendekatan ini, asam didefinisikan sebagai
zat yang dapat mendonorkan ion hidrogen dan
sebaliknya basa adalah zat yang menerima hidrogen.
• Hubungan antara bikarbonat (HCO3-), CO2 dan pH
dijabarkan oleh persamaan Henderson-Hasselbalch.
• Seperti asam lainnya, asam karbonat (H2CO3)
berdisosiasi sebagian didalam larutan.
• Besarnya derajat disosiasi ini berhubungan
dengan konsentrasi asam karbonat dan 2 ion
yang menyusunnya (H+ and HCO3-).
• Besar disosiasi ini diwakili oleh konstanta
disosiasi (K') dalam sebuah persamaan:1,2,4
• Asam karbonat akan berdisosiasi menjadi CO2 dan H2O:

Metabolic Acidosis with normal anion gap
• This Delta Ratio is sometimes useful in the
assessment of metabolic acidosis.
• The Delta Ratio is defined as
Delta ratio =
(Increase in anion gap / Decrease in bicarbonate)
• The cations normally present in urine are Na+, K+, NH4+, Ca++ and
Mg++.
• The anions normally present are Cl-, HCO3-, sulphate, phosphate and
some organic anions.
• Only Na+, K+ and Cl- are commonly measured in urine so the other
charged species are the unmeasured anions (UA) and cations (UC).
• Because of the requirement for macroscopic electroneutrality,
total anion charge always equals total cation charge, so:
• Cl- + UA = Na+ + K+ + UC
• Rearranging:

• ( UA - UC ) = [Na+]+ [K+] - [Cl-]
The Urinary Anion Gap
• Clinical Use :
• The urinary anion gap can help to differentiate
between GIT and renal causes of a
hyperchloraemic metabolic acidosis
• In a patient with a hyperchloraemic metabolic
acidosis:
• A negative UAG suggests GIT loss of bicarbonate
(eg diarrhoea) A positive UAG suggests impaired
renal distal acidification (ie renal tubular acidosis).
• As a memory aid, remember ‘neGUTive’ - negative
UAG in bowel causes.
The osmolar gap is the difference between
the measured serum osmolality and the
calculated serum osmolarity. In normal
circumstances the osmolar gap is small.
The osmolar gap can be very useful in
assisting diagnosis in metabolic acidosis due
to toxic alcohols & glycols

• To concert pH to H+ ( in the pH range 7.25- 7.55
• Drop the 7 and decimal point
• Substract the value obtained from 40

• For example pH=7.25 drop 7 and decimal = 25 . 40-
25 = 15 . 15+40 = 55 H+ nmol /L
