1. Tipe keluarga : Tipe keluarga adalah keluarga inti (Nuclear Family) dengan orang tua dan dua anak kandung tetapi
sekarang hanya tinggal berdua Tn H dan Ny
2. Suku : Keluarga ini memegang adat budaya Jawa dalam praktik kehidupan sehari-hari. Budaya tentang kesehatan
yaitu apabila sakit keluarga menyarankan untuk istirahat, tidak dibawa ke rumah sakit.
3. Agama : Keluarga memeluk agama Islam dan aktif dalam kegiatan keagamaan di lingkungan sekitar.
4. Status sosial ekonomi keluarga : Menurut Ny. I, penghasilan Tn. H sudah mencukupi kebutuhan sehari-hari terkadang
apabila membutuhkan sesuatu yang darurat keluarga biasanya dibantu oleh anaknya sudah berkerja.
5. Aktivitas rekreasi keluarga : Pada hari libur, biasanya keluarga Tn. H berkumpul di rumah untuk membersihkan kebun
kecil dibelakang rumah mereka dan menonton televisi bersama.
Riwayat dan Tahap Perkembangan Keluarga

1. Tahap perkembangan keluarga saat ini : Keluarga Tn. H dalam tahap keluarga dengan usia pertengahan. Tugas
perkembangan saat ini adalah mempunyai lebih banyak waktu dan kebebasan, mengolah minat social dan
waktu santai, memulihkan hubungan antara generasi muda dan tua, keakraban dengan pasangan, memelihara
hubungan atau kontak dengan anak dan keluarga, persiapan masa taua atau pensiun.
2. Tahap perkembangan keluarga yang belum terpenuhi : Tn. H tidak mempunyai banyak waktu dan kebebasan
untuk bersama keluarga sehingga komunikasi antar pasangan tidak terjalin dengan baik.
3. Riwayat keluarga inti : Saat ini kondisi Tn. H pusing dan berat pada tengkuk. Tn. H sudah di diagnosis
hipertensi sejak 3 tahun lalu saat berobat ke puskesmas.
4. Riwayat keluarga sebelumnya : Tn. H memiliki riwayat hipertensi dari kedua orang tuanya, ibu Tn. H sudah
meninggal dan Ny. I tidak memiliki riwayat kesehatan turunan.
Lingkungan

 Karakteristik rumah
Status kepemilikan rumah yang ditempati sekarang adalah milik keluarga sendiri. Luas rumah yang ditempati 70 m2 terdiri
dari 1 ruang tamu, 1 kamar tidur, 1 ruang dapur, 1 kamar mandi dan WC. Bangunan rumah berbentuk rumah permanen.
Lantai rumah terbuat dari ubin dengan keadaan bersih. Penataan alat/perabot rumah tangga rapi, pencahayaan dan
ventilasi cukup baik. Sumber air minum dan untuk keperluan cuci dan mandi menggunakan air sumur (sanyo). Keluarga
membuang sampah di tempat sampah yang ditaruh di depan rumah, lalu ada petugas yang mengangkut. Lingkungan sekitar
rumah tampak bersih.

 Karakteristik tetangga dan komunitas RW

Lingkungan di mana keluarga Tn. H tinggal merupakan tempat hunian yang padat. Antar tetangga sangat rukun, mereka
terkadang menghabiskan waktu untuk mengobrol di teras salah satu rumah.
Lingkungan

 Mobilitas geografis keluarga

Keluarga sudah menetap dan menempati rumah sejak tahun 1950. Sejak menikah, mereka sudah tinggal di lingkungan
yang saat ini mereka tempati dan tidak pernah pindah rumah.
 Perkumpulan keluarga dan interaksi dengan masyarakat
Anak-Anak Tn. H jarang berkumpul karena semua sudah meninggalkan rumah. . Menurut keluarga anak-anaknya
berkumpul pada saat lebaran saja. Keluarga terkadang berinteraksi dengan tetangganya dan aktif dalam kegiatan
masayarakat.
 Sistem pendukung keluarga
Ny. I sangat memperhatikan kesehatan Tn. H. semua kegiatan rumah tangga dikerjakan oleh Ny. I. Dukungan dari keluarga
besar terkadang sangat membantu keluarga Tn. H dan Ny. I. Tempat berobat yang sering dipakai keluarga adalah Puskesmas
tetapi jarak antar puskesmas cukup jauh sehingga kelurga tidak bisa memeriksakan kesehatan secara berkala.
 Struktur Keluarga

 Pola komunikasi keluarga

Ny. I mengatakan setelah Tn. H ditinggalkan anak keduanya Tn. H. sering tertutup dengan permasalahan. Apabila ada masalah
tidak dibicarakan secara terbuka.
 Struktur kekuatan keluarga
Di keluarga Tn. H, kekuasaan dibagi menurut peran masing- masing. Untuk masalah-masalah yang berhubungan dengan kepentingan
rumah tangga, Tn. H menyerahkan sepenuhnya pada Ny. I namun apabila tidak bisa diatasi, Ny. I selalu meminta bantuan dan
pertimbangan Tn. H.
 Struktur peran
Ny. I mengatakan baik Tn. H maupun anal-anaknya sudah menjalankan perannya dengan baik. Namun karena akhir-akhir ini terdapat
masalah, Tn. H menjadi sulit terbuka kepada Ny. I.
 Nilai dan norma budaya
Nilai yang mereka anut adalah nilai-nilai Jawa karena mereka berasal dari suku yang sama. Norma yang dianut adalah norma agama.
Apabila menurut agama tidak baik, maka mereka tidak akan melakukan hal tersebut.
 Fungsi Keluarga

 Fungsi afektif
Tn. H dan Ny. I selalu berusaha saling memperlihatkan kasih sayang baik antara mereka berdua untuk anak-anaknya. Tidak ada
perbedaan antara anak pertama dan kedua. Mereka selalu berusaha menerapkan komunikasi terbuka dalam segala hal sehingga jarang
terjadi perselisihan antara Tn. H dan Ny. I.
 Fungsi sosialisasi
Dalam hal pengasuhan anak, Tn. H menyerahkan sepenuhnya pada Ny. I namun apabila ada masalah yang mendesak biasanya mereka
membicarakan bersama teapi akhir-akhir ini jarang untuk membicarakan dengan terbuka.
 Fungsi perawatan keluarga
Pelayanan kesehatan yang diterima keluarga adalah pelayanan kesehatan dari Puskesmas dan pernah dirawat di rumah sakit swasta. Tn.
H tidak pernah kontrol ke Puskesmas baik untuk hipertensinya. Demikian juga dengan Ny. I, tidak pernah periksa ke Puskesmas atau tempat
lain. Keluarga tidak memiliki perencanaan khusus untuk kesehatan. Menurut keluarga, sakit itu datangnya dari Tuhan jadi tidak ada
perencanaan yang mengarah kesana. Transportasi yang digunakan oleh keluarga adalah sepeda dan transportasi umum.
Fungsi Keluarga

 Fungsi reproduksi
Keluarga Tn. H sudah mempunyai anak 2 yang sudah meninggalkan rumah dan keluarga sudah menjalani Keluarga
Berencana.
 Fungsi ekonomi
Keluarga Tn. H terolong keluarga Pra sejahtera jarena banyak hal yang harus terpenuhi tetapi belum terpenuhi.
Stres dan Koping

 Stressor jangka pendek dan panjang

Ny. I mengatakan bahwa Tn. H mulai kebingungan akan masa pensiun di tahun depan, Tn. H merasa cemas kegiatan apa
saja yang harus dilewati karena kegiatan sehari-harinya adalah sebagai guru SMP dan Tn. H takut tidak bisa
memenuhi kebutuhan istrinya lagi.
 Kemampuan keluarga berespon terhadap masalah
Keluarga memiliki sumber daya untuk berespon terhadap stressor yaitu:
1) Sistem dukungan sosial istri kuat akan tetapi Ny. I kesulitan untuk membantu Tn. H karena Tn. H
tidak mengungkapkan perasaannya.
2) Tempat tinggal yang memadai dengan sarana kesehatan yang tersedia
 Strategi koping yang digunakan
Strategi koping yang digunakan adalah berdasarkan pengalaman masa lalu dan berpusat pada Tn. H. untuk
menangani masalah kesehatan pada keluarga. Keluarga juga menggunakan sistem dukungan sosialnya yaitu dari keluarga
besar dalam membantu mereka saat membutuhkan pertolongan.
 Strategi adaptasi disfungsional
Keluarga terutama Tn. H. secara sadar telah melakukan adaptasi disfungsional yaitu apabila Tn. H mempunyai masalah
tidak bisa diungkapkan secara terbuka. Sehingga apabila ada masalah dipendam sendiri.
 Harapan Keluarga

Keluarga berharap dengan kedatangan perawat ke rumahnya adalah keluarga

dapat mengetahui status kesehatan keluarga. Dengan demikian keluarga
berharap akan selalu berada dalam kondisi sehat lahir dan batin. Mereka
juga berharap akan banyak mendapatkan banyak pengetahuan tentang berbagai
macam penyakit dan cara perawatannya.
 Pemeriksaan Fisik

Dari pemeriksaan fisik yang dilakukan, pada keluarga secara umum kondisi
kesehatan secara fisik, Ny. I tidak memiliki gangguan. Sedangkan Tn. H merasa
pusing dan berat pada tengkuk. Sdr. W dan Sdr. F belum terkaji karena mereka
tidak ada dirumah saat dilakukan pengkajian.
