TENTANG

STANDARISASI KODE KLASIFIKASI DIAGNOSIS DAN TERMINOLOGI YANG

DIGUNAKAN DI PUSKESMAS TEMPEH
KEPALA PUSKESMAS TEMPEH

Menimbang : a. bahwa dalam rangka menunjang diagnosis penyakit dan

peningkatan mutu pelayanan klinis di Pukesmas Tempeh,
maka perlu ada pembakuan klasifikasi diagnosis, kode
prosedur, symbol dan istilah yang dipakai;
b. bahwa sehubungan dengan pernyataan butir a di atas, maka
dalam melaksanakan diagnosis penyakit di Puskesmas
Tempeh, perlu ditentukan standarisasi dan klasifikasi diagnosis
dan terminologi yang di gunakan di Puskesmas Tempeh;
c. bahwa sehubungan dengan butir a dan b tersebut diatas, perlu
menetapkan Keputusan Kepala Puskesmas Tempeh tentang
Standarisasi dan Klasifikasi Diagnosis dan Terminologi yang
digunakan di Puskesmas Tempeh.

Mengingat : 1. Undang-undang Nomor 29 Tahun 2004 tentang Praktik

Kedokteran;
2. Peraturan Menteri Kesehatan No. 296/MENKES/PER/III/2008
tentang Rekam Medis;
3. Peraturan Menteri Kesehatan No. 46 tahun 2015 tentang
Akreditasi Puskesmas, Klinik Pratama, Tempat Praktik Mandiri
Dokter dan Dokter Gigi.
 MEMUTUSKAN

MENETAPKAN : KEPUTUSAN KEPALA PUSKESMAS TEMPEH TENTANG

STANDARISASI KODE KLASIFIKASI DIAGNOSIS DAN
TERMINOLOGI YANG DIGUNAKAN DI PUSKESMAS
TEMPEH.

Kesatu : Standarisasi kode klasifikasi diagnosis mengacu pada World

Healt Organization (WHO) yang secara internasional
digunakan buku ICD-X;

Kedua : Surat keputusan ini berlaku sejak tanggal ditetapkan dengan

ketentuan apabila dikemudian hari ternyata terdapat kekeliruan
akan diadakan perbaikan atau perubahan sebagaimana
mestinya.
