PENDAHULUAN

1.2 Tujuan
Untuk mempelajari definisi, epidemiologi, etiologi, jenis dan tipe
korosif, patofiologi, evaluasi klinis, serta tatalaksana dari lesi korosif pada
saluran pencernaan atas.
1.3 Manfaat
Mahasiswa mampu memahami definisi, epidemiologi, etiologi, jenis
dan tipe korosif, patofiologi, evaluasi klinis, serta tatalaksana dari lesi korosif
pada saluran pencernaan atas.

1.5 Abstrak
Zat korosif terus berdampak pada morbiditas anak-anak dan remaja di
negara berkembang. Zat korosif ini memiliki dampak terbesar pada pada anak-
anak antara usia 1 sampai 3 tahun. Lesi oral bukan merupakan prediktor luas
atau beratnya lesi korosif. Upper Endoskopi merupakan landasan evaluasi
untuk tingkat cedera gastrointestinal. Intervensi upper endoskopi, termasuk
evaluasi awal, pemasangan stent, dan dilatasi striktur dapat didiskusikan.

BAB II
PEMBAHASAN
2.1 Definisi
Lesi korosif saluran cerna bagian atas (corrosive injury of the upper
gastrointestinal tract) adalah kerusakan saluran cerna bagian atas terutama
esofagus yang terdiri dari kerusakan epitel mukosa saja sampai kerusakan
seluruh dinding esofagus karena tertelannya zat yang bersifat korosif misalnya
asam kuat, dan basa kuat,. Zat kimia yang tertelan dapat bersifat toksik atau
korosif. Zat kimia yang bersifat korosif ini akan menimbulkan kerusakan pada
saluran yang dilaluinya, sedangkan zat kimia yang bersifat toksik hanya
menimbulkan gejala keracunan bila diserap oleh darah.
2.2 Epidemiologi
Berdasarkan penelitian yang pernah dilakukan oleh American
Association of Poison Control Centers (AAPCC) pada tahun 2014 terdapat 2,1
juta yang terpapar cedera korosif di amerika serikat yang mana terdapat
sebanyak 48% terjadi pada anak-anak di bawah 5 tahun. Pada penelitian
tersebut tercatat kasusnya mencapai 112 hingga 518 dari 100.000 orang
pertahunnya. Cedera tersebut dikarenakan atas kelalaian dalam menggunakan
zat kaustik selama penggunaan di rumah. Anak laki-laki lebih rentan terkena
cedera korosif dibandingkan perempuan, terdapat 50-60% kasus cedera korosif.

Penyebab dari cedera korosif yang paling umum yaitu alkali (zat basa).
Alkali ini lebih berbahaya daripada asam, ini dikarenakan penetrasai
alkali lebih dalam sehingga kerusakan yang ditimbulkan lebih berat.
Dibawah ini merupakan tabel jenis-jenis zat korosif yang
menyebabkan lesi korosif.

2.3.2 Resiko berdasarkan jenis zat korosif
Cedera korosif ini sendiri juga dapat menyebabkan cedera
esofagus pada anak-anak yang mana sudah tercatat sebanyak 18-46%
yang mengalami cedera esofagus. Bahan pembersih rumah tangga yang
terdiri dari natrium dan kalium hidroksida merupakan bahan berbahaya
yang paling umum menyebabkan lesi korosif pada saluran pencernaan
atas. Kecuali dikonsumsi dalam jumlah besar, cairan pemutih atau
deterjen yang memiliki pH antara 9-11 tidak menyebabkan kerusakan
yang luar biasa. Produk tersebut dikemas dalam kemasan kecil dan
berwarna-warni yang terlihat seperti permen. Hal tersebut memiliki
risiko lebih besar untuk dikonsumsi secara tidak sengaja oleh anak-anak.
Namun cairan pemutih ini hanya memiliki pH kurang dari 11,5 dengan
konsentrasi natrium hipoklorit yang kurang atau sama dengan 5%, jarang
dapat menyebabkan cedera esofagus. Sementara itu, Produk rumah
tangga dengan bahan asam, seperti pembersih toilet, dan pembersih
kolam renang, menyebabkan kerusakan yang lebih serius pada
kerongkongan, lambung, dan duodenum.
2.4 Patofisiologi
2.4.1 Kimia
Perubahan keadaan ionisasi dan pemutusan ikatan kovalen dicapai
melalui pelepasan ion hidrogen dari zat asam dan ion hidroksil untuk zat
basa. Efek merugikan dari ion-ion ini ditingkatkan dalam larutan air.
2.4.2 Esofagus Injury
Zat Basa
Liquefaction necrosis adalah cedera patologis esofagus
disebabkan oleh zat basa. Proses patologis berkonotasi saponifikasi
lemak, kelarutan protein, dan pembengkakan jaringan kolagen. Sel

nekrosis dihasilkan dari membran sel dan emusifikasi. Thromboarthosis
juga terjadi.
Kontribusi produksi spesies oksigen reaktif juga disarankan,
menghasilkan peroksidasi lipid. Produk akhir peroksidasi lipid, seperti
malondialdehid, dideteksi pada konsentrasi yang sangat tinggi secara
signifikan pada 24 jam setelah pajanan sodium hidroksida. Di sisi lain,
penangkal radikal bebas, glutathione, sangat rendah. Temuan ini
mendukung peran radikal bebas pada esofagus injury.
Pengelupasan mukosa yang dapat berlanjut ke perforasi esofagus,
dengan patologi ini, tergantung pada konsentrasi sodium hidroksida dan
waktu kontak. Agen yang mengandung 10% sodium hidroksida hanya
perlu 1 menit untuk membakar esofagus secara mendalam. Alkali lemah
dengan kurang dari 2% sodium hidroksida tetapi dengan waktu kontak
yang lama menghasilkan nekrosis mukosa superfisial esofagus.
Striktur dan kontraktur dinding esofagus bergantung pada
kedalaman nekrosis. Nekrosis ketebalan penuh menghasilkan striktur
pada 100% kasus dan bahkan perforasi dalam 2 hingga 4 minggu.
Zat asam
Nekrosis koagulasi adalah lesi patologis yang disebabkan oleh zat
asam. Karena pembentukan koagulum, esofagus dilindungi dari
penetrasi yang lebih dalam, dan karenanya, perforasi lebih kecil
kemungkinannya. Selain itu, sifat lapisan epitel dan pH basa dinding
esofagus memberikan efek anti asam; namun, esofagus injury oleh agen
asam berkisar antara 6% hingga 20% kasus.
Perforasi esofagus kemudian terjadi ketika eskar atau koagulum
terlepas setelah 3 sampai 4 minggu, dengan pembentukan jaringan
koagulasi.
Delayed injury
Mulai dari minggu pertama, proses nekrotik berlanjut ke
inflamasi dan obliterasi vaskular. Kemudian, jaringan granulasi mulai

berkembang, dengan penipisan dinding esofagus, dan risiko preformasi
meningkat. Setelah selang waktu 3 minggu, proses fibrosis dimulai
dengan pembentukan striktur dan deformitas esofagus.

belakang toksik lain dari zat yang tertelan, selain sifat penyebabnya,
harus diperoleh dan dievaluasi secara menyeluruh.
Penghilang karat, misalnya, mengandung hidrogen fluorida yang
menyebabkan hipokalsemia sistemik, yang harus dikelola secara efektif
dan intensif untuk menyelamatkan nyawa korban. Gejala harus
dievaluasi secara hati-hati karena tidak berhubungan secara signifikan
dengan jenis, tingkat keparahan, atau luasnya lesi, termasuk luka bakar
oral. Intensionalitas lesi, apakah bunuh diri atau pelecehan anak, harus
dipertimbangkan dari aspek medikolegal.
2.5.2 Cedera GIT atas
Disfagia
Disfagia merupakan gejala yang paling umum dengan cedera
minimal. Disfagia awalnya disebabkan oleh hilangnya motilitas
esofagus dan waktu transit yang berkepanjangan yang dapat bertahan
lama. Fibrosis dengan atau tanpa deformitas striktur menjelaskan
penyebab akhir disfagia.
Nyeri (pain)
Lokasi perforasi esofagus ditandai dengan gejala nyeri pada 70%
hingga 90% kasus. Nyeri leher dan kekakuan menunjukkan perforasi
esofagus servikal, sedangkan nyeri subxiphoid mencirikan perforasi
esofagus anterior abdomen. Perforasi esofagus posterior abdomen
ditandai dengan nyeri epigastrium tumpul, dengan radiasi ke belakang
jika terjadi perforasi di kantung bawah. Perforasi esofagus toraks
menunjukkan nyeri retrosternal lateral pada sisi cedera. Ruptur
postemetik menyebabkan nyeri dada yang parah setelah regangan atau
hematemesis.
2.5.3 Cedera saluran pernapasan bagian atas

Gejala cedera saluran pernapasan atas (URT) pada anak-anak
adalah stridor, suara serak, retraksi, dan hidung melebar, yang
menunjukkan cedera epiglotis. Dispnea juga dapat terjadi.
2.5.4 Pemeriksaan fisik
Evaluasi cepat status mental, vital, dan pupil harus dilakukan pada
awalnya untuk evaluasi sistem saraf pusat. Pasien harus diperiksa untuk
tanda-tanda gangguan pernapasan, termasuk stridor, hidung melebar,
suara serak, retraksi, dan mengi. Cedera orofaringeal dan lesi esofagus
ditunjukkan oleh air liur, penolakan untuk makan, dan disfagia. Tanda-
tanda cedera berat adalah perubahan status mental, stridor, tanda-tanda
peritoneum, hipotensi, dan syok. Bukti perforasi kental harus dievaluasi
secara menyeluruh.
Perforasi viskus menunjukkan tanda-tanda kompensasi
hemodinamik, seperti takikardia dan takipnea. Sepsis diperumit oleh
hipotensi dan syok. Emfisema subkutan menunjukkan perforasi esofagus
serviks, sedangkan disfagia lanjut berhubungan dengan perforasi
esofagus toraks.
2.5.5 Prosedur pencitraan
Rontgen thoraks
Karena tidak diperlukan untuk pasien tanpa gejala, rontgen dada
(CXR) penting pada pasien dengan gejala pernapasan yang
berhubungan dengan perforasi viskus. Temuan melalui CXR
termasuk pneumomediastinum, efusi pleura, pneumoperitoneum,
dan pneumonitis aspirasi. Benda asing yang bersifat logam, seperti
baterai kancing, juga dapat dideteksi.
Radiografi dengan kontras
Prosedur ini hanya diindikasikan pada kasus tertentu yang secara
klinis diduga memiliki perforasi esofagus yang dikonfirmasi baik
dengan rontgen polos atau endoskopi. Studi kontras juga penting
dalam tindak lanjut pasien untuk mendeteksi deformitas striktur.

Kontras yang larut dalam air, seperti Hypague atau Gastrografin,
lebih sering digunakan untuk pemeriksaan ini.
Tomografi computer
Temuan pada CT sangat dapat diandalkan dalam pengambilan
keputusan untuk esofagostomi darurat. Sebuah sistem penilaian
menggunakan CT telah diusulkan untuk memprediksi striktur
esofagus berikut paparan korosif.
CT grade Features
Grade 1 Tidak ada pembengkakan yang pasti pada dinding
esofagus.
Grade 2 Penebalan dinding edema tanpa jaringan lunak peri-
esofagus.
Grade 3 Penebalan dinding edema dengan jaringan lunak peri-
esofagus infiltrasi, ditambah antarmuka jaringan yang
berbatas tegas.
Grade 4 Penebalan dinding edema dengan jaringan lunak peri-
esofagus infiltrasi ditambah pengaburan antarmuka
jaringan atau pengumpulan cairan lokal di sekitar
esofagus atau aorta desendens.
Pencitraan skintigrafi
Modalitas pencitraan terbaru, skintigrafi teknesium 99 m, telah
dicoba dalam mendeteksi luka bakar esofagus. Pencitraan skintigrafi
merupakan keuntungan pada kasus awal luka bakar esofagus,
terutama bila endoskopi tidak diindikasikan.
USG
Peran USG endoskopi dengan mini-probe adalah keuntungan
yang menjanjikan dalam evaluasi esofagus. Evaluasi lapisan otot
esofagus dapat memprediksi pembentukan striktur di masa depan dan
respons terhadap dilatasi.

2.6 Managemen
2.6.1 Manajemen awal
Manajemen awal pasien secara umum adalah dengan
menstabilkan keadaan pasien, memastikan jalan napas baik, terapi cairan,
menghilangkan nyeri, memberikan sedasi dan memperbaiki gangguan
elektrolit. Adanya distress pernapasan, perforasi viskus, dan gangguan
hemodinamik harus didiagnosis dan ditangani dengan secepat mungkin.
Cegah pemberian apapun yang menyebabkan muntah, karena muntah
dapat menyebabkan paparan ulang terhadap zat korosif. Bila muntah,
untuk melindungi selaput lendir esofagus dapat diberikan air atau susu.
Bila terjadi dalam 6 jam pertama, bila zat korosif bersifat basa kuat dapat
diberikan susu dan bila zat korosif bersifat asam kuat dapat diberi
antasida. Apabila ada gangguan keseimbangan elektrolit dapat diberikan
infuse aminofusin 600 2 botol, glukosa 10% 2 botol, dan Nacl 0,9% + KCl
5 meq/liter 1 botol.
Pasien lesi korosif dapat bersifat asimptomatis (tanpa gejala) dan
simptomatis (dengan gejala). Pasien dianggap asimptomatis bila tidak ada
lesi oral, tidak ada disfagia, tidak ada muntah, atau gejala parah lainnya.
pasien tanpa gejala harus diobservasi dan dipantau secara ketat selama
beberapa jam, karena beberapa zat korosif (terutama bubuk) baru
memiliki efek dalam beberapa waktu. Pemeriksaan Endoskopi dapat
dipertimbangkan pada kasus menelan zat dengan tingkat korosif yang
tinggi, kaustisitas yang tidak diketahui, atau konsumsi dalam jumlah yang
banyak. Sebelumnya, perforasi harus disingkirkan terlebih dahulu.
Pada pasien simptomatik atau dengan gejala, bila terdapat tanda
gangguan jalan napas, perforasi viskus, dan cedera parah harus segera
didiagnosis dan dipantau di bangsal. Sedangkan, apabila terjadi gangguan
hemodinamik dan distres jalan napas seperti stridor dan retraksi harus
segera ditangani di unit perawatan intensif (ICU) dengan melakukan
endotrakeal, trakeostomi, resusitasi jantung dan stabilisasi pasien secara

ekstensif. Kecuali dikontraindikasikan, pasien dengan gejala harus
dievaluasi secara endoskopi.
Upper flexibel endoskopi merupakan prosedur pilihan untuk
diagnosis dan evaluasi pasien simptomatik yang telah menelan zat korosif.
Endoskopi dilakukan pada pasien yang memiliki indeks korosif yang
tinggi. pasien yang diindikasikan endoskopi adalah pasien anak kecil, lesi
oral, pasien dengan gejala, pasien dengan status mental abnormal, pasien
yang mengkonsumsi zat korosif secara sengaja, serta konsumsi zat dalam
jumlah besar. Alasan dilakukannya endoskopi adalah untuk mengevaluasi
tingkat keparahan kerusakan jaringan, mendeteksi prognosis, dan
menunjukkan rencana tatalaksana. Pasien harus menjalani endoskopi
dalam waktu 24 jam setelah menelan. Endoskopi awal dalam waktu
kurang dari 6 jam setelah konsumsi tidak menunjukkan gambaran lengkap
dari cedera, sedangkan endoskopi akhir setelah 4 hari paparan dapat
meningkatkan risiko perforasi.
2.6.2 Intervensi
Intubasi nasogastrik
Pemasangan Nasogastric tube (NGT) diindikasikan pada pasien
dengan ulkus fokal atau sirkumferensial yang dalam serta pasien
dengan area ulserasi multipel dan menunjukan adanya tanda nekrosis.
Intubasi NGT harus dilakukan dibawah pengawasan selama prosedur
endoskopi untuk menghindari cedera jaringan lebih lanjut. NGT
berguna dalam pemberian makan pasien, pelestarian lumen selama
pembentukan striktur, dan sebagai guide untuk dilatasi esofagus.
Intubasi gastrostomi
Intubasi gastrostomi harus dipertimbangkan pada pasien dengan
luka bakar esofagus yang luas dan parah, baik selama atau segera
setelah evaluasi endoskopi, serta untuk pemberian makanan pada
pasien.

2.6.3 Terapi Medik
Glukokortikoid
Dijurnal ini dijelaskan bahwa glukokotikoid tidak dianjurkan
pada pasien dengan cedera korosif esofagus, karena tidak memiliki
efek deformitas striktur. Namun, pada penelitian acak menggunakan
metilprednisolon vs plasebo pada 83 anak dengan lesi esofagus grade
2B menunjukkan tingkat striktur yang rendah pada kelompok yang
diobati dengan metilprednisolon. Oleh karena itu, diperlukan studi
serta konfirmasi lebih lanjut mengenai efek glukokortikoid pada
pasien lesi korosif esofagus. Biasanya, jenis glukokortikoid yang
dapat diberikan pada pasien dengan lesi korosif adalah prednisone
dengan dosis 1-2 mg/kg hari dengan dosis maksimal 60 mg/hari.
Maksimum diberikan selama 21 hari termasuk tapering off. Diberikan
untuk mencegah terjadinya pembentukan fibrosis yang berlebihan.
Obat ini Sebaiknya diberikan sejak hari pertama hingga hari ketiga
dan diturunkan bertahap dalam 2 hari.
Antibiotik
Antiobiotik dapat diberikan selama 2-3 minggu terutama bila
terbukti terdapat perforasi dan infeksi sekunder. Antibiotik dapat
dilanjutkan selama 4-8. Antibiotik tidak akan mencegah pembentukan
striktur, tetapi akan membantu mengoptimalkan proses
penyembuhan. Antibiotik pilihannya berupa sefalosporin generasi
ketiga (cefotaxime, cefixime, ceftriaxone) atau penisilin/sulbaktam
diberikan untuk profilaksis. Biasanya diberikan penisilin dosis tinggi
1-1,2 juta unit/hari.

PENUTUP
3.1 Kesimpulan
Lesi korosif saluran cerna bagian atas (corrosive injury of the
upper gastrointestinal tract) adalah kerusakan saluran cerna bagian atas
terutama esofagus yang terdiri dari kerusakan epitel mukosa saja sampai
kerusakan seluruh dinding esofagus karena tertelannya zat yang bersifat
korosif misalnya asam kuat, basa kuat, dan zat organik. Penyebab dari
cedera korosif yang paling umum yaitu alkali (zat basa). Alkali ini lebih
berbahaya daripada asam, ini dikarenakan penetrasai alkali lebih dalam
sehingga kerusakan yang ditimbulkan lebih berat. Diagnosis pasien lesi
korosif didasarkan dengan adanya riwayat tertelan zat korosif serta
adanya keluhan utama berupa rasa nyeri hebat dan rasa terbakar di
daerah kerongkongan, serta disfagia. Apabila gejala-gejala tersebut
berat maka perlu dilakukan evaluasi endoskopi untuk mengevaluasi
tingkat keparahan kerusakan jaringan, mendeteksi prognosis, dan
menunjukkan rencana tatalaksana. Tatalaksana awal yang dapat
dilakukan pada pasien adalah dengan menstabilkan keadaan pasien,
memastikan jalan napas baik, terapi cairan, menghilangkan nyeri,
memberikan sedasi dan memperbaiki gangguan elektrolit.
